In einem Online-Experiment mit 776 Probanden wird untersucht, inwieweit ein Adelsprädikat im Namen der Bewerber, ihr Geschlecht sowie die Auswahlerfahrung der Beurteiler einen Einfluss auf die Bewertung der Kandidaten nimmt. Als Stimulusmaterial wurde den Beurteilern der Lebenslauf eines fiktiven Bewerbers vorgelegt und in systematischer Weise sowohl das Adelsprädikat als auch das Geschlecht der Bewerber variiert. Im Ergebnis zeigt sich, dass Bewerber mit Adelsprädikat signifikant anders bewertet werden als solche, die über kein Adelsprädikat verfügen (bezogen auf ihre Offensivität sowie die Wahrscheinlichkeit einer späteren Einstellung). Dies gilt in besonderer Weise für weibliche Bewerber mit Adelsprädikat im Hinblick auf die Wahrscheinlichkeit einer Einstellung. Männliche Bewerber werden unabhängig vom Adelsprädikat hinsichtlich ihrer Professionalität negativer bewertet als Frauen. Die Erfahrung der Probanden in der Personalauswahl schützt nicht vor entsprechenden Urteilsverzerrungen.
Zusammenhang zwischen Lücken im Lebenslauf und Berufserfolg : ein Mythos der Personalauswahlpraxis

Mit dieser explorativ angelegten Studie (N = 2225) wird kritisch hinterfragt, ob und inwieweit Lücken im Lebenslauf Indikatoren für den Berufserfolg darstellen. Dabei bilden die Lückendauer im Allgemeinen sowie separiert nach neun Gründen für deren Entstehen die Prädiktoren. Als Kriterien für den Berufserfolg dienen Entwicklung und Höhe des Einkommens, beruflicher Status sowie subjektive Erfolgseinschätzungen. Effekte des Geschlechts, Alters, der Ausbildung sowie sozial erwünschten Antwortverhaltens wurden herauspartialisiert. Für die Gesamtstichprobe ergeben sich signifikante negative Zusammenhänge zwischen der Lückendauer und allen Berufserfolgskriterien, die jedoch als gering anzusehen sind. Bei Berücksichtigung der Ursachen für die Lücken zeigt sich, dass die Lückendauer aufgrund von Reisen mit keinem der Kriterien einen signifikanten Zusammenhang aufweist. Das Gegenteil zeigt sich bei einer unfreiwilligen Arbeitslosigkeit. Aufgrund der geringen Varianzaufklärung des Berufserfolgs durch Lücken im Lebenslauf und den in sich differenziert zu betrachtenden Ergebnissen, erscheint eine pauschale Nutzung von Lücken im Lebenslauf als Kriterium in der Personalauswahl nicht empfehlenswert.
While the importance of supervisors’ communication for the successful implementation of organisational change is often emphasised, evidence regarding the actual dimensions of change communication and the reaction of change recipients is scarce. As communication is highly context and culture specific, this study included participants from Germany and India. To meet these research aims, we conducted focus group discussions (FGD) with overall 50 graduate students with work experiences from Germany and India. We identified key dimensions of informative, empathic and participatory change communication that are of high importance for change recipients. The German and Indian participants had similar expectations regarding the informative and empathic dimensions of change communication. However, interestingly, their assessment of the actual participation differed substantially, with consequences for their emotional responses. The findings emphasise the crucial role of supervisors’ communication for employees’ emotions in times of change. Supervisors thereby need to consider the diverse dimensions of their communication and stimulate employees’ expectations regarding information, empathy and participation with different communicative activities and approaches. This study provides important insights into organisational change communication by analysing its key dimensions, linking it to employees’ emotions and considering the cultural context.
Der zunehmende Einsatz von Informations- und Kommunikationstechnologie im Gesundheitswesen verlangt auch von Angehörigen der Pflegeberufe Kompetenzen zur Nutzung der entsprechenden Systeme und Verfahren. Vor diesem Hintergrund haben sich die AG „Informationsverarbeitung in der Pflege“ der Deutschen Gesellschaft für Medizinische Informatik, Biometrie und Epidemiologie (GMDS), die Österreichische Gesellschaft für Pflegeinformatik (ÖGPI) und die Schweizerische Interessensgruppe Pflegeinformatik (IGPI) innerhalb des Schweizer Berufsverband der Pflegefachfrauen und Pflegefachmänner (SBK) entschlossen, gemeinsame Empfehlungen für benötigte Kernkompetenzfelder in Pflegeinformatik zu erarbeiten. Auf Basis einer iterativen multimethodischen Vorgehensweise unter Einbeziehung von einer großen Anzahl von Fachexperten aus Deutschland, Österreich und der Schweiz (D-A-CH) wurden 24 notwendige Felder von Kernkompetenzen definiert und deren Relevanz für fünf typische Berufsfelder in der Pflege bewertet. Damit liegt erstmalig eine wissenschaftlich fundierte Empfehlung für zu vermittelnde Kernkompetenzfelder in der Pflegeinformatik für verschiedene pflegerische Berufsfelder vor. Sie richtet sich an alle Personen mit Verantwortung für die Planung von Studium, Lehre, Aus- und Weiterbildung in der Pflege.
Climate change is expected to exacerbate the current threats to freshwater ecosystems, yet multifaceted studies on the
potential impacts of climate change on freshwater biodiversity at scales that inform management planning are lacking. The aim of this study was to fill this void through the development of a novel framework for assessing climate
change vulnerability tailored to freshwater ecosystems. The three dimensions of climate change vulnerability are as
follows: (i) exposure to climate change, (ii) sensitivity to altered environmental conditions and (iii) resilience potential.
Our vulnerability framework includes 1685 freshwater species of plants, fishes, molluscs, odonates, amphibians, crayfish and turtles alongside key features within and between catchments, such as topography and connectivity. Several
methodologies were used to combine these dimensions across a variety of future climate change models and scenarios. The resulting indices were overlaid to assess the vulnerability of European freshwater ecosystems at the catchment scale (18 783 catchments). The Balkan Lakes Ohrid and Prespa and Mediterranean islands emerge as most
vulnerable to climate change. For the 2030s, we showed a consensus among the applied methods whereby up to 573
lake and river catchments are highly vulnerable to climate change. The anthropogenic disruption of hydrological
habitat connectivity by dams is the major factor reducing climate change resilience. A gap analysis demonstrated that
the current European protected area network covers <25% of the most vulnerable catchments. Practical steps need to
be taken to ensure the persistence of freshwater biodiversity under climate change. Priority should be placed on
enhancing stakeholder cooperation at the major basin scale towards preventing further degradation of freshwater
ecosystems and maintaining connectivity among catchments. The catchments identified as most vulnerable to climate
change provide preliminary targets for development of climate change conservation management and mitigation
strategies.
Biomechanische Analysen sind in der Lage, menschliche Bewegungen valide und umfassend zu erfassen und auszuwerten. Neben den beiden großen Bereichen Kinetik und Kinematik bietet die Elektromyografie (EMG) eine zuverlässige Möglichkeit, die neuromuskuläre Aktivität zu analysieren. Mithilfe des EMG können neuromuskuläre Parameter erhoben werden, die präzise Aussagen beispielsweise zur inter- und intramuskulären Koordination, der Muskelfaserverteilung, des Ermüdungsverhaltens oder des Timings zulassen.
Für verlässliche Daten sind im klinischen Setting jedoch einige wichtige Faktoren zu berücksichtigen. Diese sind von großer Bedeutung und sollten vor einer Analyse beachtet werden. Daneben hängt ein effektiver Einsatz des EMG im klinischen Setting von der Integration in den Clinical-Reasoning-Prozess ab. Die jeweilige individuelle Patientensituation benötigt eine klare Fragestellung. Dazu kann auf ein Ebenenmodell aufgebaut werden, welches die biomechanischen Steuerungsgrößen in der klinischen Anwendung berücksichtigt.
Der Artikel stellt die physiologischen Grundlagen der Elektromyografie, die Einflüsse von Verletzungen auf die Muskelfaserzusammensetzung, die grundlegende Signalverarbeitung und Dateninterpretation, ein Ebenenmodell für die klinische Anwendung sowie Einsatzfelder in der Physiotherapie vor.
Research into positive aspects of the psyche is growing as psychologists learn more about the protective role of positive processes in the development and course of mental disorders, and about their substantial role in promoting mental health. With increasing globalization, there is strong interest in studies examining positive constructs across cultures. To obtain valid cross-cultural comparisons, measurement invariance for the scales assessing positive constructs has to be established. The current study aims to assess the cross-cultural measurement invariance of questionnaires for 6 positive constructs: Social Support (Fydrich, Sommer, Tydecks, & Brähler, 2009), Happiness (Subjective Happiness Scale; Lyubomirsky & Lepper, 1999), Life Satisfaction (Diener, Emmons, Larsen, & Griffin, 1985), Positive Mental Health Scale (Lukat, Margraf, Lutz, van der Veld, & Becker, 2016), Optimism (revised Life Orientation Test [LOT-R]; Scheier, Carver, & Bridges, 1994) and Resilience (Schumacher, Leppert, Gunzelmann, Strauss, & Brähler, 2004). Participants included German (n = 4,453), Russian (n = 3,806), and Chinese (n = 12,524) university students. Confirmatory factor analyses and measurement invariance testing demonstrated at least partial strong measurement invariance for all scales except the LOT-R and Subjective Happiness Scale. The latent mean comparisons of the constructs indicated differences between national groups. Potential methodological and cultural explanations for the intergroup differences are discussed. Background and purpose:
Clinical information logistics is a construct that aims to describe and explain various phenomena of information provision to drive clinical processes. It can be measured by the workflow composite score, an aggregated indicator of the degree of IT support in clinical processes. This study primarily aimed to investigate the yet unknown empirical patterns constituting this construct. The second goal was to derive a data-driven weighting scheme for the constituents of the workflow composite score and to contrast this scheme with a literature based, top-down procedure. This approach should finally test the validity and robustness of the workflow composite score.
Methods:
Based on secondary data from 183 German hospitals, a tiered factor analytic approach (confirmatory and subsequent exploratory factor analysis) was pursued. A weighting scheme, which was based on factor loadings obtained in the analyses, was put into practice.
Results:
We were able to identify five statistically significant factors of clinical information logistics that accounted for 63% of the overall variance. These factors were “flow of data and information”, “mobility”, “clinical decision support and patient safety”, “electronic patient record” and “integration and distribution”. The system of weights derived from the factor loadings resulted in values for the workflow composite score that differed only slightly from the score values that had been previously published based on a top-down approach.
Conclusion:
Our findings give insight into the internal composition of clinical information logistics both in terms of factors and weights. They also allowed us to propose a coherent model of clinical information logistics from a technical perspective that joins empirical findings with theoretical knowledge. Despite the new scheme of weights applied to the calculation of the workflow composite score, the score behaved robustly, which is yet another hint of its validity and therefore its usefulness.
Information Technology (IT) continues to evolve and develop with electronic devices and systems becoming integral to healthcare in every country. This has led to an urgent need for all professions working in healthcare to be knowledgeable and skilled in informatics. The Technology Informatics Guiding Education Reform (TIGER) Initiative was established in 2006 in the United States to develop key areas of informatics in nursing. One of these was to integrate informatics competencies into nursing curricula and life-long learning. In 2009, TIGER developed an informatics competency framework which outlines numerous IT competencies required for professional practice and this work helped increase the emphasis of informatics in nursing education standards in the United States. In 2012, TIGER expanded to the international community to help synthesise informatics competencies for nurses and pool educational resources in health IT. This transition led to a new interprofessional, interdisciplinary approach, as health informatics education needs to expand to other clinical fields and beyond.
In tandem, a European Union (EU) - United States (US) Collaboration on eHealth began a strand of work which focuses on developing the IT skills of the health workforce to ensure technology can be adopted and applied in healthcare. One initiative within this is the EU*US eHealth Work Project, which started in 2016 and is mapping the current structure and gaps in health IT skills and training needs globally. It aims to increase educational opportunities by developing a model for open and scalable access to eHealth training programmes. With this renewed initiative to incorporate informatics into the education and training of nurses and other health professionals globally, it is time for educators, researchers, practitioners and policy makers to join in and ROAR with TIGER.
Objectives
The aims of the present study were to provide back pain (BP) point prevalence data from inpatients at an Australian tertiary hospital on one day, and compare this with Australian non-hospitalized population prevalence data; to collect data around the development of BP throughout hospital admission; and to analyse the association between BP and past history of BP, gender, age, admission specialty and hospital length of stay (LOS).
Methods
This was a single-site, prospective, observational study of hospitalized inpatients on one day during 2016, with a subsequent survey over the following 11 days (unless discharge or death occurred sooner).
Results
Data were collected from 343 patients (75% of the hospitalized cohort). A third of patients (n = 108) reported BP on admission, and almost a fifth (n = 63) developed new BP during their hospitalization. Patients who described BP at any time during their hospital stay had a higher chance of having had a history of BP, with odds increasing after adjustment for age and gender (odds ratio 5.89; 95% confidence interval (CI) 3.0 to 11.6; p < 0.001). After adjusting for age and gender, those experiencing BP had a significantly longer LOS (median 13 days; CI 10.8 to 15.3) than those who did not (median 10 days; CI 8.4 to 11.6; p = 0.034).
Conclusions
Hospital LOS for patients who complained of BP at any time during their admission was 3 days longer than those who had no BP, and a history of BP predicted a higher likelihood of BP during admission. Screening of patients on admission to identify any history of BP, and application of a package of care including early mobilization and analgesia may prevent the onset of BP and reduce LOS.
The acceptance of mobile payments has been limited. This paper, therefore, attempts to investigate the determinants of mobile payment adoption. To this end, it examines the relationships between the personality trait dimensions of the Technology Readiness Index 2.0 and the system-specific dimensions of the Technology Acceptance Model in Germany and South Africa. The model was tested using structural equation modeling. Results show that some, but not all, of the Technology Readiness Index 2.0 variables had a significant influence on the dimensions of the Technology Acceptance Model. Perceived Usefulness was the strongest predictor of the intention to use mobile payments. The factor “country” did not moderate the structural relationships.
Die vorliegende Studie untersucht erstmals differenziert konkrete Strategien, die Bewerber in der Phase der Vorbereitung sowie der Durchführung von Auswahlverfahren einsetzen. In einer Befragung von 999 Personen wird sowohl nach der Einstellung bezüglich derartiger Strategien als auch nach deren Umsetzung bezogen auf Bewerbungsunterlagen, Einstellungsinterview, Testverfahren und Assessment Center gefragt. Die Ergebnisse zeigen, dass eine Mehrheit der Bewerber in starkem Maße strategisch agiert. In der Vorbereitungsphase bezieht sich dies insbesondere auf die Sichtung von Ratgeberliteratur und den Austausch mit Freunden / Bekannten. Bewerbungstrainings haben demgegenüber eine untergeordnete Bedeutung. Bei der Verfassung von Bewerbungsunterlagen greift die Mehrheit unter anderem auf Vorlagen zurück, die nur noch angepasst werden. Im Einstellungsinterview wird unter anderem die Darstellung der eigenen Stärken an die Stellenanforderungen angepasst. Fast alle Befragten wenden wenigstens eine Strategie an. Bewerber, die sich in den letzten fünf Jahren beworben haben, agieren insgesamt aktiver als Personen, deren letzte Bewerbung länger zurückliegt.
Bei der Sichtung von Bewerbungsunterlagen werden in der Praxis u. a. Freizeitaktivitäten wie etwa sportliche Betätigungen zur Bewertung der Kandidaten herangezogen. Im Rahmen einer Studie mit 291 Probanden wird erstmals differenziert untersucht, inwieweit sportliche Aktivitäten einen Indikator der Leistungsmotivation darstellen und sich somit als Kriterium zur Sichtung von Bewerbungsunterlagen eignen. Die Probanden füllen mehrere Skalen zur Messung ihrer beruflichen Leistungsmotivation aus und machen zusätzlich Angaben zu Art und Intensität sportlicher Aktivitäten inklusive der Teilnahme an sportlichen Wettkämpfen. Im Ergebnis zeigt sich, dass die Intensität der sportlichen Betätigung mit erhöhten Werten auf den meisten Skalen der beruflichen Leistungsmotivation einhergeht. Die Wettkampfintensität zeigt positive Zusammenhänge zu Wettbewerbsorientierung, Leistungsstolz und Beharrlichkeit. Weder die Sportart (Individual- vs. Mannschaftssport) noch die Dauer der Jahre, über die hinweg Sport betrieben wurde, ermöglichen jedoch eine Aussage über die berufliche Leistungsmotivation. Aus den Befunden werden Empfehlungen für die Praxis der Sichtung von Bewerbungsunterlagen abgeleitet.
Recent theoretical models suggest that repetitive negative thinking might be a key mechanism explaining the negative effects of maternal psychopathology on mother–infant relations. While an emerging body of research largely supports this idea, the relative importance of differences in the trajectory of repetitive negative thinking during and after pregnancy for mother–infant bonding as well as maternal depressive symptoms is currently unknown. Therefore, we investigated associations between the course of maternal repetitive negative thinking during pregnancy and after birth and mother–infant-bonding and maternal depressive symptoms in a longitudinal study. The overall level of repetitive negative thinking was a significant predictor of mother–infant bonding, maternal anxiety and rejection in dealing with her infant four months after birth. Furthermore, differences in the trajectory of repetitive negative thinking predicted bonding, but not anxiety or rejection. The overall levels of repetitive negative thinking as well as the differences in the trajectory of repetitive negative thinking were significant predictors of maternal depressive symptoms. These findings indicate that changes of repetitive negative thinking during and after pregnancy can increase the risk of postpartum depressive symptoms.
Background: For more than 30 years, there has been close cooperation between Japanese and German scientists with regard to information systems in health care. Collaboration has been formalized by an agreement between the respective scientific associations. Following this agreement, two joint workshops took place to explore the similarities and differences of electronic health record systems (EHRS) against the background of the two national healthcare systems that share many commonalities.
Objectives: To establish a framework and requirements for the quality of EHRS that may also serve as a basis for comparing different EHRS.
Methods: Donabedian's three dimensions of quality of medical care were adapted to the outcome, process, and structural quality of EHRS and their management. These quality dimensions were proposed before the first workshop of EHRS experts and enriched during the discussions.
Results: The Quality Requirements Framework of EHRS (QRF-EHRS) was defined and complemented by requirements for high quality EHRS. The framework integrates three quality dimensions (outcome, process, and structural quality), three layers of information systems (processes and data, applications, and physical tools) and three dimensions of information management (strategic, tactical, and operational information management).
Conclusions: Describing and comparing the quality of EHRS is in fact a multidimensional problem as given by the QRF-EHRS framework. This framework will be utilized to compare Japanese and German EHRS, notably those that were presented at the second workshop.
The primary objective of this study was to determine the structural and known-group validity as well as the inter-rater reliability of a test battery to evaluate the motor control of the craniofacial region. Seventy volunteers without TMD and 25 subjects with TMD (Axes I) per the DC/TMD were asked to execute a test battery consisting of eight tests. The tests were video-taped in the same sequence in a standardised manner. Two experienced physical therapists participated in this study as blinded assessors. We used exploratory factor analysis to identify the underlying component structure of the eight tests. Internal consistency (Cronbach's α), inter-rater reliability (intra-class correlation coefficient) and construct validity (ie, hypothesis testing-known-group validity) (receiver operating curves) were also explored for the test battery. The structural validity showed the presence of one factor underlying the construct of the test battery. The internal consistency was excellent (0.90) as well as the inter-rater reliability. All values of reliability were close to 0.9 or above indicating very high inter-rater reliability. The area under the curve (AUC) was 0.93 for rater 1 and 0.94 for rater two, respectively, indicating excellent discrimination between subjects with TMD and healthy controls. The results of the present study support the psychometric properties of test battery to measure motor control of the craniofacial region when evaluated through videotaping. This test battery could be used to differentiate between healthy subjects and subjects with musculoskeletal impairments in the cervical and oro-facial regions. In addition, this test battery could be used to assess the effectiveness of management strategies in the craniofacial region.
Viele „Overhead Athletes“ leiden am Glenohumeral Internal Rotation Deficit Syndrome (GIRD; glenohumerales Innenrotationsdefizit). Die persistierenden Schulterbeschwerden werden durch Überbelastung im oberen Quadranten verursacht. Bislang ist der Einfluss der orofazialen Region und der dentalen Okklusion auf die (motorische) Funktion bei den betroffenen Patienten nicht systematisch erforscht.
Diese Fallserie untersuchte bei (semi)professionellen Overhead Athletes die Einschränkung ihrer Innen- und Außenrotation sowie der statischen Kraft in 4 Bewegunsrichtungen. Aufgrund der Resultate wurde aus der 1. Gruppe eine 2. Gruppe gebildet, die 3 manualtherapeutische Behandlungen und anschließend eine Dentalschiene erhielt.
Die multidisziplinäre Behandlung durch den Zahnarzt und einen (spezialisierten) Manualtherapeuten reduzierte signifikant die Beschwerden und Dysfunktionen. Eine orofaziale klinische Diagnose nach den Richtlinien der Diagnostic Criteria/Temporomandibular Dysfunctions (DC/TDM) und der okklusale kinästhetische Sensibilisierungstest (OKST) scheinen prognostische Indikatoren für die Behandlung des GIRD-Syndroms zu sein.
Die funktionelle Dysphonie (FD) präsentiert sich klinisch mit ventralen Halsschmerzen, Heiserkeit und Stimmstörungen. Organisch liegt kein pathologischer Befund vor, aber möglicherweise spielen laryngeale Strukturen, die Muskulatur und neurale Ansteuerung eine ursächliche Rolle. Die FD wird zwar primär von Logopäden und Sprachtherapeuten behandelt, aber spezialisierte Manualtherapeuten können wesentlich dazu beitragen, dass die Therapie erfolgreich verläuft.
Forschungsfrage: In welchem Umfang werden Persönlichkeitseigenschaften in Stellenanzeigen verwendet, die eher als maskulin wahrgenommen werden?
Methodik: Wir nutzen einen Pool von deutschsprachigen Stellenanzeigen von Dax-30-Unternehmen, mittelständischen Unternehmen und des öffentlichen Sektors in den Bereichen Personalwesen (N=2.086) und Softwareentwicklung (N=1.811).
Praktische Implikationen: Unternehmen, die eine diverse Belegschaft anstreben, sollten nicht einseitig vor allem maskuline Persönlichkeitseigenschaften fordern. Die Eigenschaften signalisieren auch die Unternehmenskultur.
Mutterschaft ab 35 Jahren: Das Altersrisiko in der Wahrnehmung von Frauen – eine Literaturstudie

Hintergrund Die Anzahl derjenigen Frauen, die im Alter von 35 Jahren oder später Mutter werden, steigt in den Industrienationen weltweit. Mit dem höheren Alter ist eine Zunahme von Risiken für die Fertilität, für die Schwangerschaft, für die Geburt und für die Neugeborenen verbunden. Risikozuschreibungen in der Schwangerschaft beeinflussen die psychosoziale Situation von Frauen negativ. Daher stellt sich die Frage, wie Frauen das Altersrisiko erleben.
Methodik Die internationale systematische Literaturrecherche wurde zwischen Dezember 2015 und März 2016 in den elektronischen Datenbanken PubMed, CINAHL, PsycINFO und Sowiport durchgeführt. Eingeschlossen wurden Studien, welche die Wahrnehmung der reproduktiven Altersrisiken der Mutterschaft in fortgeschrittenem Alter, definiert als ≥ 35 Jahre, fokussieren.
Ergebnisse Es wurden elf Studien unterschiedlichen Designs ausgewertet. Mit Ausnahme von Kenntnissen über die durch das Alter abnehmende Fertilität und die Zunahme bei Trisomie 21 zeigen sich insgesamt geringe Kenntnisse der Teilnehmenden über altersassoziierte Risiken. In der Konfrontation mit den Risiken werden diese als bedrohlich erlebt. Strategien im Umgang mit der Besorgnis sind vielfältig: Informationsmanagement mit Problemen von Über- und Fehlinformation und Informationsvermeidung, Ablenkung von Gedanken an Risiken, Bedürfnis nach Kontrolle und guter Vorbereitung oder eine emotionale Distanz zur Schwangerschaft. Die persönliche Risikobewertung durch die Frauen ist komplex und bezieht ihre sozialen Lebensumstände und ihren Lebensstil mit ein. Dabei werden die medizinischen Zuschreibungen relativiert. Durch die Fokussierung ein gesundes Kind zu gebären unterbleibt häufig die Vorbereitung auf das Leben mit dem Neugeborenen. Auswirkungen auf das Erleben von Mutterschaft zeigen sich insbesondere in einer postpartalen Überforderung in der Versorgung des Neugeborenen.
Diskussion Das Altersrisiko kann von den Betroffenen nur unzureichend eingeschätzt werden. Es zeigen sich verschiedene aus der Risikoforschung bekannte Phänomene. Die medizinische Risikoeinschätzung unterscheidet sich von der Risikowahrnehmung der Frauen. Die Folgen für das Erleben der Mutterschaft machen das Konzept des Altersrisikos diskussionswürdig. Diese Ergebnisse legen für die Gesundheitsberufe nahe, die Versorgung von Schwangeren in fortgeschrittenem Alter stärker auf gesundheitsfördernde Aspekte, Partnerschaft und das Neugeborene auszurichten.
The aim of this study was to provide a systematic overview of available pressure ulcer prevention quality indicators and to evaluate the underlying empirical evidence. A systematic mapping review was conducted with combined searches in Embase and Medline, and websites of relevant institutions and organisations. The eligibility criteria were clear use of the term “quality indicator” regarding pressure ulcer prevention; English or German language; and all settings, populations, and types of resources, including articles, brochures, and online material. In total, n = 146 quality indicators were identified. Most indicators were published in the United States (n = 50). The majority of indicators was developed for the hospital setting (n = 102). Process indicators were the most common (n = 71), followed by outcome indicators (n = 49). Less than half of identified indicators appeared to be practically used. Evidence supporting the validity and reliability were reported for n = 25 and n = 30 indicators respectively. The high number of indicators demonstrate the importance of measuring pressure ulcer prevention quality. This is not an indicator of our ability to accurately measure and evaluate this construct. There is an urgent need to develop evidence-based and internationally comparable indicators to help improve patient care and safety worldwide.
Für die Versorgungsforschung ist wichtig, dass verteilte und heterogene Daten so integriert werden, dass sie offen für neue Analyse-Anforderungen und leicht um neue Datenquellen erweiterbar sind. Für die Integration von Versorgungsdaten werden bislang hauptsächlich Data-Warehouses eingesetzt, die Daten dimensional oder als Entity-Attribute-Value-Struktur (EAV) modellieren. Diese Datenmodelle sind jedoch entweder unflexibel oder weisen ein zu geringes Maß an Datenorganisation auf, was longitudinale Analysen erschwert. Wir haben den EAV-Ansatz um die Data-Vault-Modellierung ergänzt und damit die Datenstrukturen der Krankenhaus-Qualitätsberichte des Gemeinsamen Bundesausschusses (G-BA) modelliert sowie die Daten der Jahre 2011 bis 2015 integriert. Dies ermöglicht eine Historisierung der Metadaten für Merkmale, insbesondere der Qualitätsindikatoren, sowie ein hohes Maß an Erweiterbarkeit gegenüber neuen heterogenen Datenquellen. Der vorgeschlagene Ansatz erlaubt es, den Abstraktionsgrad für die zu modellierenden Entitäten frei zu wählen, so dass auch ein vollständig generisches EAV-Modell mit historisierten Metadaten erstellt werden kann.

Hintergrund
Ziel dieser Studie ist es, die Auswirkungen einer Kinesiophobie auf die Emotions- sowie Lateralitätserkennung zu untersuchen.
Material und Methoden
67 Probanden mit chronischen muskuloskeletalen Schmerzen wurden untersucht. Hierbei erzielten 24 Probanden einen Wert von >37 auf der Tampa-Skala für Kinesiophobie und wurden in die Querschnittstudie eingeschlossen. Die Fähigkeit zur Erkennung mimisch codierter Basisemotionen wurde mittels des Facially-Expressed-Emotion-Labeling(FEEL)-Tests und die Lateralitätserkennung anhand eines speziellen Face-Mirroring-Assessment-and-Treatment-Programms ermittelt. Die Toronto-Alexithymie-Skala (TAS) 26 diente zur Erfassung einer Alexithymie.
Ergebnisse
Der FEEL-Score der Probanden mit Kinesiophobie war signifikant niedriger (p = 0,019). Die Basisemotionen Angst (p = 0,026), Ärger (p = 0,027) und Überraschung (p = 0,014) verdeutlichten einen signifikanten Unterschied zwischen den Ergebnissen der Querschnittstudie und gesunden Probanden. Der Alexithymiefragebogen TAS-26 zeigte lediglich in der TAS 1 (Schwierigkeiten bei der Identifikation von Gefühlen; p = 0,008) einen signifikanten Unterschied zwischen Probanden mit Kinesiophobie.
Schlussfolgerung
Die Ergebnisse zeigen, dass kinesiophobische Patienten Veränderungen in der Emotions- und Lateralitätserkennung sowie alexithyme Merkmale aufweisen.
Objective: To pilot benchmark measures of health information and communication technology (ICT) availability and use to facilitate cross-country learning.
Materials and Methods: A prior Organization for Economic Cooperation and Development–led effort involving 30 countries selected and defined functionality-based measures for availability and use of electronic health records, health information exchange, personal health records, and telehealth. In this pilot, an Organization for Economic Cooperation and Development Working Group compiled results for 38 countries for a subset of measures with broad coverage using new and/or adapted country-specific or multinational surveys and other sources from 2012 to 2015. We also synthesized country learnings to inform future benchmarking.
Results: While electronic records are widely used to store and manage patient information at the point of care—all but 2 pilot countries reported use by at least half of primary care physicians; many had rates above 75%—patient information exchange across organizations/settings is less common. Large variations in the availability and use of telehealth and personal health records also exist.
Discussion: Pilot participation demonstrated interest in cross-national benchmarking. Using the most comparable measures available to date, it showed substantial diversity in health ICT availability and use in all domains. The project also identified methodological considerations (e.g., structural and health systems issues that can affect measurement) important for future comparisons.
Conclusion: While health policies and priorities differ, many nations aim to increase access, quality, and/or efficiency of care through effective ICT use. By identifying variations and describing key contextual factors, benchmarking offers the potential to facilitate cross-national learning and accelerate the progress of individual countries.
Multinational health IT benchmarks foster cross-country learning and have been employed at various levels, e.g. OECD and Nordic countries. A bi-national benchmark study conducted in 2007 revealed a significantly higher adoption of health IT in Austria compared to Germany, two countries with comparable healthcare systems. We now investigated whether these differences still persisted. We further studied whether these differences were associated with hospital intrinsic factors, i.e. the innovative power of the organisation and hospital demographics. We thus performed a survey to measure the “perceived IT availability” and the “innovative power of the hospital” of 464 German and 70 Austrian hospitals. The survey was based on a questionnaire with 52 items and was given to the directors of nursing in 2013/2014. Our findings confirmed a significantly greater IT availability in Austria than in Germany. This was visible in the aggregated IT adoption composite score “IT function” as well as in the IT adoption for the individual functions “nursing documentation” (OR = 5.98), “intensive care unit (ICU) documentation” (OR = 2.49), “medication administration documentation” (OR = 2.48), “electronic archive” (OR = 2.27) and “medication” (OR = 2.16). “Innovative power” was the strongest factor to explain the variance of the composite score “IT function”. It was effective in hospitals of both countries but significantly more effective in Austria than in Germany. “Hospital size” and “hospital system affiliation” were also significantly associated with the composite score “IT function”, but they did not differ between the countries. These findings can be partly associated with the national characteristics. Indicators point to a more favourable financial situation in Austrian hospitals; we thus argue that Austrian hospitals may possess a larger degree of financial freedom to be innovative and to act accordingly. This study is the first to empirically demonstrate the effect of “innovative power” in hospitals on health IT adoption in a bi-national health IT benchmark. We recommend directly including the financial situation into future regression models. On a political level, measures to stimulate the “innovative power” of hospitals should be considered to increase the digitalisation of healthcare.
Die Kieferregion und die HWS stehen neuroanatomisch und biomechanisch in enger Verbindung zueinander. Darüber hinaus hat die kraniomandibuläre Region durch ihren funktionellen Aufbau, ihre anatomische Positionierung und ihre neurologische Wertigkeit eine Bedeutung für den gesamten menschlichen Organismus.
Das Ziel der Studie war es zu beobachten, ob eine durch eine veränderte dentale Okklusion hervorgerufene Veränderung im Mandibulabereich einen direkten Einfluss auf die aktive und passive physiologische HWS-Beweglichkeit ausübt.
Neben der Gruppenauswertung wurden die 61 Probanden zusätzlich von einem Zahnarzt in die verschiedenen Angle-Klassifikationen (Klasse 1, 2a, 2b und 3) eingeteilt und auf Unterschiede untersucht. Bei allen Probanden wurde die physiologische Beweglichkeit der HWS mit dem CROM Device gemessen und der Flexions-Rotations-Test (FRT) für die passive physiologische Bewegungsüberprüfung mit einem digitalen Goniometer durchgeführt. Die Messungen erfolgten jeweils 2-mal innerhalb eines kurzen Zeitabstands, und zwar einmal ohne und einmal mit Veränderung der Okklusion durch einen Papierstreifen.
Beim Vergleich der beiden Messungen der gesamten Stichprobe zeigte sich eine signifikant vergrößerte Beweglichkeit in jeder gemessenen Bewegungsrichtung. Zwischen den Angle-Klassifikationen waren keine signifikanten Unterschiede zu beobachten.
Kreuzbandrisse (Rupturen des vorderen Kreuzbands, VKB) ziehen oft eine langwierige Rehabilitation und Wiedereingliederung nach sich. Die physiotherapeutische Nachbehandlung beinhaltet aktive und passive Maßnahmen. Bislang existieren keine Studien zu neurodynamischen Aspekten der Rehabilitation von Kreuzbandrissen. Das Ziel der vorliegenden Studie bestand darin, die Veränderung der Bewegung des N. tibialis des betroffenen und nicht betroffenen Beins bei Probanden nach einer VKB-Ruptur zu untersuchen.
Bestandteil der verblindeten Untersuchung der 19 Probanden waren der Knee Outcome Survey für alltägliche Aktivitäten (KOS) und verschiedene Funktionsuntersuchungen (Slump-Test mit passiver Knieextension, Long Sitting Slump Test mit passiver Dorsalextension des oberen Sprunggelenks, Straight Leg Raise Test, belastete Dorsalextension im Stand). Die longitudinale Nervenexkursion des N. tibialis wurde mit bildgebendem Ultraschall in sitzender Slump-Position untersucht. Die Auswertung der Ultraschallvideos erfolgte mit einer Frame-by-frame Cross Correlation Software.
Die Ergebnisse zeigten eine signifikant geringere Nervenexkursion des N. tibialis am betroffenen Knie. Allerdings fand sich kein klinisch signifikanter Zusammenhang zu den Funktionsuntersuchungen.
Stephan Maykus, ebenfalls Mitglied der Expertenkommission, kommentiert in diesem Beitrag den Teil des 15. Kinder- und Jugendberichts, der eine der zentralen Lebenswelten Jugendlicher in den Blick nimmt: die zuletzt sehr expansive Schule in Form der Ganztagsschule. Die Sachverständigenkommission kommt bei ihrer Bestandsaufnahme zur Ganztagsschule zu einer relativ bescheidenen Bilanz. Allerdings hatte die zunehmende Verschulung des Jugendalters weitreichende Auswirkungen auf die generationale Lage der Jugend insgesamt. Der Autor verweist darauf, dass mit dem neuesten Kinder- und Jugendbericht erstmalig eine dezidiert alters- und lebensphasenbezogene Bilanz der Ganztagsschulentwicklung vorgenommen wurde, die gerade auch für die Jugendarbeit vielfältige Herausforderungen offenbart. Die Ganztagsschule sei bisher kaum hinsichtlich ihrer Zielgruppe Jugend diskutiert und konzipiert worden, stellt Maykus fest. Er fragt, wie die Ganztagsschule so ausgestaltet werden könne, dass sie den Entwicklungsprozessen im Jugendalter pädagogisch gerecht werde. Der Autor belegt das Auseinanderklaffen von Wunsch und Wirklichkeit der Ganztagsschulen und betont, dass auch in der Forschung das Verhältnis von Ganztagsschule und Jugend kaum genauer untersucht worden sei. Er entwickelt schließlich vier Leitfragen zur Bewertung der Ganztagsschulentwicklung aus jugendpädagogischer Sicht.
