The Internet of Things (IoT) is the enabler for new innovations in several domains. It allows the connection of digital services with physical entities in the real world. These entities are devices of different categories and sizes range from large machinery to tiny sensors. In the latter case, devices are typically characterized by limited resources in terms of computational power, available memory and sometimes limited power supply. As a consequence, the use of security algorithms requires of them to work within the limited resources. This means to find a suitable implementation and configuration for a security algorithm, that performs properly on the device, which may become a challenging task. On the other side, there is the desire to protect valuable assets as strong as possible. Usually, security goals are recorded in security policies, but they do not consider resource availability on the involved device and its power consumption while executing security algorithms. This paper presents an IoT security configuration tool that helps the designer of an IoT environment to experiment with the trade-off between maximizing security and extending the lifetime of a resource constrained IoT device. The tool is controlled with high-level description of security goals in the form of policies. It allows the designer to validate various (security) configurations for a single IoT device up to a large sensor network.
Hintergrund
Die Anämie hat eine hohe Prävalenz bei Patienten vor Hüftgelenkrevisionsoperation und ist mit einer erhöhten Komplikationsrate assoziiert. Die vorliegende Arbeit untersucht erstmals den Zusammenhang von Kosten, realen DRG-Erlösen und Falldeckung der präoperativen Anämie bei elektiven Hüftgelenkrevisionsoperationen.
Methoden
Für alle Patienten, die sich von 2010 bis 2017 an 2 Campi der Charité – Universitätsmedizin Berlin einer Hüftgelenkrevisionsoperation unterzogen, wurden Daten zu Patienten sowie Transfusionen, Kosten und Erlösen gesammelt. Subgruppen- und lineare Regressionsanalysen untersuchten die Falldeckung anämischer und nichtanämischer Patienten.
Ergebnisse
Von 1187 eingeschlossenen Patienten waren 354 (29,8 %) präoperativ anämisch. Insgesamt wurden 565 (47,6 %) Patienten, mit einem deutlichen Überwiegen anämischer Patienten (72,6 % vs. 37,0 %, p < 0,001), transfundiert. Kosten (12.318 € [9027;20.044 €] vs. 8948 € [7501;11.339 €], p < 0,001) und Erlöse (11.788 € [8992;16.298 €] vs. 9611 € [8332;10.719 €], p < 0,001) waren für anämische Patienten höher, die Fallkostendeckung defizitär (−1170 € [−4467;1238 €] vs. 591 € [−1441;2103 €] €, p < 0,001). Bei anämischen Patienten nahm die Falldeckung mit zunehmender Transfusionsrate ab (p ≤ 0,001). Komorbiditäten hatten keinen signifikanten ökonomischen Einfluss.
Schlussfolgerung
Die präoperative Anämie und perioperative Transfusionen bei Hüftgelenkrevisionsoperationen sind mit erhöhten Behandlungskosten und einer finanziellen Unterdeckung für Kostenträger im Gesundheitswesen verbunden. Konzepte zur Behandlung der präoperativen Anämie (z. B. Patient Blood Management) könnten mittelfristig Behandlungskosten senken.
Easy and inexpensive methods for measuring ammonia emissions in multi-plot field trials allow the comparison of several treatments with liquid manure application. One approach that might be suitable under these conditions is the dynamic tube method (DTM). Applying the DTM, a mobile chamber system is placed on the soil surface, and the air volume within is exchanged at a constant rate for approx. 90 s. with an automated pump. This procedure is assumed to achieve an equilibrium ammonia concentration within the system. Subsequently, a measurement is performed using an ammonia-sensitive detector tube. Ammonia fluxes are calculated based on an empirical model that also takes into account the background ammonia concentration measured on unfertilized control plots. Between measurements on different plots, the chamber system is flushed with ambient air and cleaned with paper towels to minimize contamination with ammonia. The aim of this study was to determine important prerequisites and boundary conditions for the application of the DTM.
We conducted a laboratory experiment to test if the ammonia concentration remains stable while performing a measurement. Furthermore, we investigated the cleaning procedure and the effect of potential ammonia carryover on cumulated emissions under field conditions following liquid manure application. The laboratory experiment indicated that the premeasurement phase to ensure a constant ammonia concentration is not sufficient. The concentration only stabilized after performing more than 100 pump strokes, with 20 pump strokes (lasting approximately 90 s) being the recommendation.
However, the duration of performing a measurement can vary substantially, and linear conversion accounts for those differences, so a stable concentration is mandatory. Further experiments showed that the cleaning procedure is not sufficient under field conditions. Thirty minutes after performing measurements on high emitting plots, which resulted in an ammonia concentration of approx.
10 ppm in the chamber, we detected a residual concentration of 2 ppm. This contamination may affect measurements on plots with liquid manure application as well as on untreated control plots. In a field experiment with trailing hose application of liquid manure, we subsequently demonstrated that the calculation of cumulative ammonia emissions can vary by a factor of three, depending on the degree of chamber system contamination when measuring control plots. When the ammoni background values were determined by an uncontaminated chamber system that was used to measure only control plots, cumulative ammonia emissions were approximately 9 kg NH3-N ha1.
However, when ammonia background values were determined using the contaminated chamber system that was also used to measure on plots with liquid manure application, the calculation of cumulative ammonia losses indicated approximately 3 kg NH3-N ha1. Based on these results, it can be concluded that a new empirical DTM calibration is needed for multi-plot field experiments with high-emitting treatments.
The BBI is a first step toward putting biodiversity conservation into practice in the OHC context. The results are consistent with studies related to nutrition However, the results also show that there is room for improvement and that there are further areas to be addressed. It is also clear that commercial kitchens currently have only limited room for maneuver. If OHC is to become more biodiversity-friendly, greater transparency is needed in terms of origin labels and species/variety identifiers, and a wide range of options will also be required in terms of procurement. That being the case, it is essential to focus on the entire value chain. Furthermore, in addition to the initial recommendations, much more knowledge is required about the impacts of farming methods and heritage varieties and species, as well as about the use of fish, other marine animals and game meat. In principle, however, the BBI can already be implemented in commercial kitchens by identifying recipe optimizations that kitchens can feasibly implement, that align with their budgets, and that maintain acceptance among patrons. In addition, this approach has the potential to be integrated into the assessment framework of the NAHGAST calculator, making it readily accessible and free for OHC facilities to use. In the OHC context in particular, this could be leveraged to drive sustainable change in the food system.
The influence of moderate electric fields (MEF) on thermally induced gelation and network structures of patatin enriched potato protein (PPI) was investigated. PPI solutions with 9 wt% protein (pH 7) and 25 mM NaCl were heated from 25 to 65 °C via OH (3–24 V/cm) or conventional heating (COV) at various come-up (240 s and 1200 s) and holding times (30 s and 600 s). Self-standing gels were produced but less proteins denatured when heated via OH. Further, SDS-PAGE and GPC measurements revealed more native patatin remaining after OH treatment. Scanning electron microscopy showed OH gels to have more gap-like structures and frayed areas than COV treated gels which resulted in lower water holding capacity. On molecular scale, less hydrophobic interactions were measured within the protein network and FTIR trials showed the MEF to affect beta-sheet structures. OH gels further showed lower rigidity and higher flexibility, thus, gelling functionality was affected via OH.

Plant-based proteins are rapidly emerging, while novel technologies are explored to offer more efficient extraction processes. The current study aimed to evaluate the effects of pulsed electric fields (PEFs) and temperature on the extraction of soluble proteins from nettle leaves (Urtica dioica L.) and identify an optimal operational range for the highest yield of soluble proteins. Extractions and kinetic modeling were conducted with whole and ground dried leaves at different temperatures (30–70 °C) and specific energy of PEF (0–30 kJ kg−1) with extraction times of up to 60 min. The influence of temperature and specific energy on the soluble protein extraction yields was investigated and modeled using composite central design and response surface methodology. The experimental results were fitted to Peleg's kinetic model, which satisfactorily described the extraction process (R2 > 0.902), and PEF treated samples resulted in a higher soluble protein yield and shortened processing time. Response surface methodology showed that the linear effect of temperature and quadratic effect of PEF (p < 0.01) were highly significant for protein yield. In the optimized PEF-extraction region (specific energy between 10 and 24 kJ kg−1, and 70–78 °C), soluble protein yield was higher than 60% after 5 minutes of extraction. The achieved results are relevant for developing processes for PEF assisted extraction of soluble proteins from leaves. Understanding the effects of PEFs and process parameters is crucial to obtain high protein yields, while requiring low energy and short processing time.
The kiwifruit processing industry is focused on product yield maximization and keeping energy costs and waste effluents to a minimum while maintaining high product quality. In our study, pulsed electric field (PEF) pretreatment enhanced kiwifruit processing to facilitate peelability and specific peeling process and enhanced valorization of kiwifruit waste. PEF optimization was applied to obtain the best treatment parameters. A 32 factorial design of response surface methodology was applied to find the effect of time elapsed after PEF treatment and the PEF-specific energy input on specific peeling force and kiwifruit firmness as response criteria. Under the optimized condition, the specific peeling force decreased by 100, and peelability increased by 2 times. The phenolic content and antioxidant capacity of PEF-treated kiwifruit bagasse were 5.1% and 260% richer than the control sample. Overall, the optimized PEF pretreatments incorporated into kiwifruit processing led to decreased energy demand and increased productivity.
Der Text behandelt die Bedeutung nachhaltiger Ernährung und die Herausforderungen bei der Integration von Nachhaltigkeitsaspekten in die berufliche Bildung, insbesondere in den Korn-handhabenden Berufen. Tägliche Ernährungsentscheidungen werden als maßgeblich für Umwelt, Gesellschaft und Gesundheit hervorgehoben. Die EAT-Lancet-Kommission schlägt eine Planetarische Gesundheitsdiät vor, die klimafreundlich und gesund ist. Die Förderung von Nachhaltigkeitskompetenzen in der beruflichen Bildung wird als Schlüssel für langfristige Veränderungen in Betrieben betont. Der Modellversuch Korn-Scout zielt darauf ab, Lehr-/Lernmaterialien zu entwickeln, die die Nachhaltigkeitsorientierung von Auszubildenden in Korn-handhabenden Berufen fördern. Dieser besteht aus vier Elementen: Status-quo-Analyse, Entwicklung von Lehr-/Lernmaterialien, Erprobung und Transfer in die Praxis. Kommunikation wird als Schlüsselfaktor für die systematische Integration von Bildung für nachhaltige Entwicklung in die Ausbildung betont. Die Evaluation der Materialien während der Covid-19-Pandemie führte zu sinnvollen Optimierungsvorschlägen.Erunterstreicht die Bedeutung von Kommunikationskompetenzen für nachhaltiges Handeln in den Korn-handhabenden Berufen. Effektive Kommunikation entlang der Wertschöpfungskette ist entscheidend, um Nachhaltigkeitsaspekte von der landwirtschaftlichen Produktion bis zum Verzehr zu integrieren. Neben der Kommunikationskompetenz werden auch andere Fähigkeiten wie die Suche, Verarbeitung und Integration von nachhaltigkeitsrelevanten Informationen betont. Der Modellversuch Korn-Scout identifizierte berufsübergreifende Nachhaltigkeitskompetenzen entlang der Korn-Food-Supply-Chain, wobei einige Berufe spezifische Kompetenzen aufweisen. Die systematische Weiterentwicklung dieser Kompetenzen ist entscheidend für die strukturelle Integration von Nachhaltigkeit in der beruflichen Praxis. Lehr-/Lernmaterialien aus dem Modellversuch werden durch Zusammenarbeit mit Praxispartnern in die Praxis übertragen. Der Ausblick betont die Notwendigkeit weiterer Projekte und Anpassungen in den Ausbildungsordnungen für eine umfassende Verankerung von Nachhaltigkeit in der beruflichen Ausbildung.
Ohmic heating (OH) is an alternative sustainable heating technology that has demonstrated its potential to modify protein structures and aggregates. Furthermore, certain protein aggregates, namely amyloid fibrils (AF), are associated with an enhanced protein functionality, such as gelation. This study evaluates how Ohmic heating (OH) influences the formation of AF structures from ovalbumin source under two electric field strength levels, 8.5 to 10.5 and 24.0–31.0 V/cm, respectively. Hence, AF aggregate formation was assessed over holding times ranging from 30 to 1200 sunder various environmental conditions (3.45 and 67.95 mM NaCl, 80, 85 and 90 °C, pH = 7). AF were formed under all conditions. SDS-PAGE revealed that OH had a higher tendency to preserve native ovalbumin molecules. Furthermore, Congo Red and Thioflavin T stainings indicated that OH reduces the amount of AF structures. This finding was supported by FTIR measurements, which showed OH samples to contain lower amounts of beta-sheets. Field flow fractioning revealed smaller-sized aggregates or aggregate clusters occurred after OH treatment. In contrast, prolonged holding time or higher treatment temperatures increased ThT fluorescence, beta-sheet structures and aggregate as well as cluster sizes. Ionic strength was found to dominate the effects of electric field strength under different environmental conditions.
Olive oil holds significant importance in the European diet and is renowned globally for its sensory attributes and health benefits. The effectiveness of producing olive oil is greatly influenced by factors like the maturity and type of olives used, as well as the milling techniques employed. Generally, mechanical methods can extract approximately 80% of the oil contained in the olives. The rest 20% of the oil remains in the olive waste generated at the end of the process. Additionally, significant amounts of bioactive compounds like polyphenols are also lost in the olive pomace. Traditionally, heat treatment, enzymes, and other chemicals are used for the enhancement of oil extraction; however, this approach may impact the quality of olive oil. Therefore, new technology, such as pulsed electric field (PEF), is of great benefit for nonthermal yield and quality improvements.
Die Autoren präsentieren einen innovativen Ansatz für Circular Economy (CE)-Strategien mit hohem Umweltpotenzial. Dieser Ansatz betont Verweigerung, Umdenken und Reduzieren, indem die Perspektive vom Verbraucher auf soziale Praktiken verschoben wird. Dabei wird Suffizienz als Schlüsselkonzept eingeführt, das entscheidend ist, um Umweltbelastungen durch CE zu reduzieren und die Transformation des Produktions- und Konsumsystems zu fördern. Die Autoren betonen, dass nachhaltige Kreislaufwirtschaft ohne Suffizienz nicht möglich ist.

The present study aims to compare the effects of maternal and offspring treatment with 25-hydroxycholecalciferol (25-OHD3) and vitamin D3 on vitamin D status, mineral homoeostasis, bone metabolism and locomotion in the offspring. Either vitamin D3 (50 μg/kg diet) or 25-OHD3 (50 μg/kg diet) was supplemented to the gestation and lactation diets of 49 multiparous sows and/or to the diets of their growing offspring. Treatment of the sows did not affect plasma concentrations of 25-OHD3 of the offspring. Pigs fed 25-OHD3 had higher plasma concentrations of 25-OHD3 than pigs that received vitamin D3 during rearing and fattening. However, neither plasma concentrations of calcium, phosphate and bone markers during the observation period nor bone ash and bone mineral density at slaughter were clearly affected by the treatment. Maternal and offspring treatment with 25-OHD3 instead of vitamin D3 resulted in a slight reduction in the prevalence of leg swelling. In addition, more pigs walked with even steps and normal stride length. Further studies are needed to test whether the slight effects observed in the present experiment are reproducible and of relevance for animal health and welfare. In that case, the underlying mechanisms should be revealed in order to take advantage of potentially beneficial influences especially under certain feeding regimes.
Novel foods by process are a special case in the catalogue of the ten novel food categories according to Article 3 (2) point (a) of the Novel Food Regulation (EU) 2015/2283, since the other nine categories derive their assessment as possible novel foods from their purely substantial properties. In the case of novel foods by process, the problem of dealing with the reference date of 15 May 1997, which is in the end a random reference date, is particularly significant. It would make more sense to have a dynamic reference date that ‘moves along the timeline’ or at least is reset from time to time and is more up-to-date. The characteristic that a process causes ‘significant changes in the composition or structure of the food, affecting its nutritional value, metabolism or level of undesirable substances’ must be understood in such a way that it is only a question of the generation of undesirable substances through the application of the process, but not their reduction, e.g. the reduction of undesirable microorganisms. Finally, the question also arises as to how the assessment of the process technology relates to the assessment of a food in the context of a novel food by composition category. This concerns the exemption for foods that have a history of use as safe foods, which, according to the view taken here, must also be interpreted into the category of novel foods by process.
Leftovers lovers vs. haters: A latent class analysis on dinner leftover management behaviours

Die Studie untersucht die Heterogenität im Umgang mit Speiseresten in Haushalten, insbesondere unter Verwendung von Mahlzeitensets mit angepassten Portions- und Zutatengrößen. Durch eine latenten Klassenanalyse werden Verbrauchersegmente identifiziert, wobei die positive Einstellung zu Essensresten und bewusster Umgang damit untersucht werden. Die Studie analysiert auch die Auswirkungen von Emotionen, persönlichen Normen, Intentionen und Essensbeschaffungsroutinen auf die Zugehörigkeit zu einem Resteverwertungssegment. Daten von 868 Haushalten aus sechs Ländern zeigen fünf Verbrauchersegmente mit Unterschieden in der Menge der Essensreste. Mahlzeitensets haben heterogene Auswirkungen, indem sie in einigen Segmenten die Menge der Essensreste verringern, in anderen jedoch nicht. Die Ergebnisse bieten neue Erkenntnisse über die Vielfalt im Umgang mit Speiseresten und liefern Implikationen für Theorie und Politik zur Reduzierung von Lebensmittelverschwendung im Haushalt.
Auf der dritten Strategietagung Nachhaltigkeit wurde deutlich, dass Nachhaltigkeit breit angelegte Veränderungen in Staat, Gesellschaft und Unternehmen erfordert. Diakonische Unternehmen sind hochmotiviert und bestens geeignet, diese Änderungen zu vollziehen. Sie haben sich längst auf den Weg gemacht, neben der ökologischen auch die soziale Nachhaltigkeit zu befördern. Es hat für Frustration gesorgt, dass Themen besprochen werden mussten, die bereits bei der ersten Tagung als dringend identifiziert worden waren. Dem gegenüber standen aber die Beispiele aus den Unternehmen, die zeigten, dass bereits im bestehenden Rahmen viele entscheidende Verbesserungen möglich sind. Immer mehr diakonische Unternehmen entwickeln Nachhaltigkeitsstrategien und installieren ein Nachhaltigkeitsmanagement. Die Berichterstattung, z.B. in Form von CO2-Bilanzen oder Nachhaltigkeitsberichten nach anerkannten Standards wie EMAS-plus, DNK oder GWÖ, wird zügig ausgebaut. Soweit Daten vorhanden sind, konnten die CO2-Emissionen bereits signifikant reduziert werden. Zudem, das wurde an vielen Punkten der Tagung klar, stellten die Beteiligten eine stark gewachsene Bereitschaft in der Politik fest, die Nachhaltigkeits- und Klimaziele zu konkretisieren und Maßnahmen gegen den Klimawandel ressortübergreifend und engagiert auf den Weg zu bringen.
Lahmheiten bilden bei Sauen und wachsenden Schweinen ein ernstzunehmendes Gesundheits- und Tierwohlproblem. Die Studie zeigte, dass der diätetische Einsatz von 25-Hydroxycholecalciferol im Austausch zum Vitamin D3 zu einer Reduktion von Bewegungsproblemen bei Sauen und wachsenden Schweinen geführt hat.

This paper investigates four different mobile robots with respect to their drivingcharacteristics and soil preservation properties in an agricultural environment.Thereby, robots of classical design from agriculture as well as systems from spacerobotics with advanced locomotion concepts are considered to determine theindividual advantages of each rover concept with respect to the application domain.Locomotion experiments were conducted to analyze the general driving behavior,tensile force, and obstacle‐surmounting capability and ground interaction of eachrobot. Various soil conditions typical for the area of application are taken intoaccount, which are varied in terms of moisture and density. The presented workcovers the specification of the conducted experiments, documentation of theimplementation as well as analysis and evaluation of the collected data. In theevaluation, particular attention is paid to the change in driving characteristics underdifferent soil conditions, as well as to the soil stress caused by driving, since soilquality is of critical importance for agricultural applications. The analysis shows thatthe advanced locomotion concepts, as used in space robotics, also have positiveimplications for certain requirements in agricultural applications, such as maneuver-ability in wet conditions and soil conservation. The results show potential for designinnovations in agricultural robotics that can be used, to open up new fields ofapplication for instance in the context of precision farming.
BACKGROUND: Postoperative delirium (POD) is an acute and common complication after surgery that can increase morbidity and mortality. Few previous studies with inconsistent findings have examined the association of preoperative pain and POD. Our purpose is to investigate the association of preoperative chronic pain and POD.
METHODS: This prospective observational cohort study included 200 patients ≥ 18 years scheduled for elective surgery under general anaesthesia in a tertiary care hospital. POD was defined as meeting diagnostic criteria during the study visits (according to delirium screening tests and the Diagnostic and Statistical Manual of Mental Disorders, Fifth Edition), or by diagnosis of the responsible physicians. Chronic pain was defined as pain lasting six months or longer. Features of chronic pain were assessed with the German Pain Questionnaire, including the Depression Anxiety and Stress Scale-21 (DASS-21). Associations with POD were assessed using logistic regression analysis adjusting for confounding factors.
RESULTS: Thirty-nine (22%) out of 176 patients developed POD. Chronic pain was not associated with POD after adjustment for ASA physical status, duration of anesthesia and DASS-21 Anxiety score (Odds ratio [OR], 95%-Confidence Interval [CI], 2.216 [0.968;5.070], P=0.060). A subgroup analysis of chronic pain patients revealed that current pain intensity was higher in patients with POD.
CONCLUSIONS: Preoperative chronic pain was no independent predictor for POD. Current pain intensity was higher in chronic pain patients with POD. This indicates that certain features of pain might be influential. Further research is needed to examine different forms of preoperative pain and their possible influence on POD.
Introduction Postoperative delirium (POD) is seen in approximately 15% of elderly patients and is related to poorer outcomes. In 2017, the Federal Joint Committee (Gemeinsamer Bundesausschuss) introduced a ‘quality contract’ (QC) as a new instrument to improve healthcare in Germany. One of the four areas for improvement of in-patient care is the ‘Prevention of POD in the care of elderly patients’ (QC-POD), as a means to reduce the risk of developing POD and its complications.
The Institute for Quality Assurance and Transparency in Health Care identified gaps in the in-patient care of elderly patients related to the prevention, screening and treatment of POD, as required by consensus-based and evidence-based delirium guidelines. This paper introduces the QC-POD protocol, which aims to implement these guidelines into the clinical routine. There is an urgent need for well-structured, standardised and interdisciplinary pathways that enable the reliable screening and treatment of POD. Along with effective preventive measures, these concepts have a considerable potential to improve the care of elderly patients.
Methods and analysis The QC-POD study is a non-randomised, pre–post, monocentric, prospective trial with an interventional concept following a baseline control period. The QC-POD trial was initiated on 1 April 2020 between Charité-Universitätsmedizin Berlin and the German health insurance company BARMER and will end on 30 June 2023. Inclusion criteria: patients 70 years of age or older that are scheduled for a surgical procedure requiring anaesthesia and insurance with the QC partner (BARMER). Exclusion criteria included patients with a language barrier, moribund patients and those unwilling or unable to provide informed consent. The QC-POD protocol provides perioperative intervention at least two times per day, with delirium screening and non-pharmacological preventive measures.
Ethics and dissemination This protocol was approved by the ethics committee of the Charité-Universitätsmedizin, Berlin, Germany (EA1/054/20). The results will be published in a peer-reviewed scientific journal and presented at national and international conferences.
In recent years, ISO, IFS, BRC and FSSC 22000 standards in the areas of quality, environment and occupational health and safety have been increas-ingly implemented in companies in various industries. The main focus of these developed standards are the processes. But the past shows that the factor human is another very important factor, which should be much more in the middle in organization. The new developed guideline has the human factor in the foreground. In particular, the attitude and awareness of occupational safe-ty and health protection in the behaviour of employees at all levels of the or-ganisation are at the centre of consideration.
Inspired by this approach, a group of experts from the fields of quality scienc-es, standardisation and certification as well as consulting for system-relevant companies in the agricultural and food industry came together to form a committee during the Corona crisis in spring 2020. The common goal is to develop a new standard. The first step is to establish criteria for a guideline.
and establish an evaluation system for several pillars of a House of Total Safety Culture (HSC) tailored to the entire value chains of the agri-food sec-tor. In addition, the essential building blocks of the guideline must be defined. The attitude of managers and employees, their behaviour and their compe-tence form the roof of the standard. The integrated management system with a continuous improvement process forms the foundation of the House of To-tal Safety Culture across the company in value chains. Qualification and communication are the main pillars and thus further elements of the HSC. Particular attention is paid to the fields of action of occupational safety and health protection, food safety, animal and environmental protection as well as sustainability and digitisation. They form the four inner pillars of the House of Total Safety Culture. The design of the respective certification levels is con-sidered as a “construction phase”. As part of the standardisation process, the coordination of the respective requirements for safety culture has not yet been completed. This article first provides an overview of the state of knowledge in relation to the established standards and norms of the agri-food industry with special consideration of the Safety Culture Ladder model. The procedure for developing and establishing the new guideline is then ex-plained. For this purpose, the composition of the expert forum is presented before the concrete steps to establish the guideline are presented. The model of the House of Total Culture is presented below. Building on this, the proce-dure for assessing the degree of maturity and possible concepts of continuing training are explained before the conclusion of this article.
Die vorliegende Bachelorarbeit behandelt das Thema das betriebliche Ideenmanagement. Das Ziel ist den Prozess zur Einreichung, Evaluierung und Umsetzung von Ideen weiterzuentwickeln und zu digitalisieren.
Um das Ziel zu erreichen, werden Primärdaten mit Hilfe von fünf qualitativen Interviews und einer quantitativen Umfrage mit 50 Mitarbeitern erhoben. Abgesehen davon wurden auch Daten aus der Sekundärforschung erfasst, deren Bestandteile die internen Unterlagen zum bestehenden Ideenmanagement dem Status quo, und wissenschaftliche Quellen sind.
A brief questionnaire for measuring alarm fatigue in nurses and physicians in intensive care units

When exposed to hundreds of medical device alarms per day, intensive care unit (ICU) staff can develop “alarm fatigue” (i.e., desensitisation to alarms). However, no standardised way of quantifying alarm fatigue exists. We aimed to develop a brief questionnaire for measuring alarm fatigue in nurses and physicians. After developing a list of initial items based on a literature review, we conducted 15 cognitive interviews with the target group (13 nurses and two physicians) to ensure that the items are face valid and comprehensible. We then asked 32 experts on alarm fatigue to judge whether the items are suited for measuring alarm fatigue. The resulting 27 items were sent to nurses and physicians from 15 ICUs of a large German hospital. We used exploratory factor analysis to further reduce the number of items and to identify scales. A total of 585 submissions from 707 participants could be analysed (of which 14% were physicians and 64% were nurses). The simple structure of a two-factor model was achieved within three rounds. The final questionnaire (called Charité Alarm Fatigue Questionnaire; CAFQa) consists of nine items along two scales (i.e., the “alarm stress scale” and the “alarm coping scale”). The CAFQa is a brief questionnaire that allows clinical alarm researchers to quantify the alarm fatigue of nurses and physicians. It should not take more than five minutes to administer.
Dynamic Controlled Atmosphere-Chlorophyll Fluorescence storage (DCA-CF) uses a fluorescence-based measurement method to detect fermentation in apples (Malus × domestica BORKH.) caused by low-oxygen levels at an early stage. In recent years, it has been observed that individual apples of the same variety and origin can exhibit different fermentation behavior when stored under completely identical conditions. The causes of the different fermentation behavior must be found in order to be able to use DCA storage optimally. This study aimed to find the causes of the different fermentation behaviors of individual apples. Our results show that fruit ripeness can affect the lower oxygen limit (LOL), especially immediately after harvest, when the starch degradation in the fruit is not yet complete. A significant increase in the LOL was observed in ‘Elstar’ (2020: 0.3 kPa, 0.6 kPa, 0.9 kPa; 2021: 0.3 kPa, 0.4 kPa, 0.6 kPa). ‘Braeburn’ also exhibited this behavior regarding the LOL at a lower level. The LOL could not be identified for some of the fruit (varying from 12.5% to 41.7% of the examined apples) previously stored in Ultra Low Oxygen (ULO) storage for 4 months. Also, the chlorophyll content in the apple skin influences the fluorescence measurement method. Within 2 weeks, the chlorophyll content in the apple skin was halved. If the chlorophyll content drops, the reliability of the fluorescence measurement also decreases. It turned out that apples with an Fv/Fm < 0.7 were unsuitable for valid LOL identification.

Long-term storage of apples (Malus x domestica, Borkh.) is increasingly taking place under Dynamic Controlled Atmosphere (DCA). The oxygen level is lowered to ≤ 1 kPa O2 and the apples are stored just above the Lower Oxygen Limit (LOL). Low oxygen stress during controlled atmosphere storage can lead to fermentation in apples if oxygen levels are too low. Chlorophyll fluorescence can be used to detect low-oxygen stress at an early stage during storage. The currently available non-imaging fluorescence systems often use the minimal fluorescence (Fo) parameter. In contrast, the use of chlorophyll fluorescence kinetics is insufficiently described. Therefore, this study aimed to gain more knowledge about the response of chlorophyll fluorescence kinetics to low oxygen stress in apples using a fluorescence imaging system. The results show that the kinetic fluorescence curves differ under aerobic and fermentation conditions. The fermentative conditions initiated a decrease in fluorescence intensity upon application of the saturation pulses during exposure to actinic light. This result was made at 18 °C and 2 °C ambient temperatures. Interestingly, the kinetic curve changed at 2 °C before fermentation products accumulated in the apples. Non-photochemical quenching (NPQ) decreased under fermentation conditions in the dark phase after relaxation. Upon entering the dark relaxation phase after Kautsky induction, ɸPSII began to increase. Under atmospheric oxygen conditions, ɸPSII reached values of 0.81 to 0.76, while under fermentation, ɸPSII values ranged from 0.57 to 0.44.

Die Autorinnen widmen sich der Frage, welche Motive, Ziele, Herausforderungen und Chancen damit verbunden sind, dass Migrant*innen eigene Organisationen gründen und wie sie damit den Integrationsprozess unterstützen. Mayra Martin Espinosa gibt in ihrem Best-Practice-Artikel zu „Good leadership practices of NGOs managers throughout the COVID-19 pandemic in Colombia” einen Einblick in Führungspraktiken von NGOs in Kolumbien. Dabei kommt sie zu differenzierten Erkenntnissen, wie sich diese Praktiken in Krisenzeiten entwickeln und identifiziert insbesondere ein Führungsdilemma, für das sie verschiedene Lösungsoptionen skizziert. Mit diesen diversen Perspektiven auf das Nonprofit Management startet das JNMP und eröffnet die Debatte, die in den kommenden Ausgaben dann in unterschiedlichen Facetten aus den verschiedenen Disziplinen und Praxisfeldern weiter bereichert werden soll. Wir wünschen allen Leser*innen eine anregende Lektüre!
This chapter examines the integration of Sustainable Development Goal 5 (SDG 5) into identity-based brand management by focusing on Dove’s brand management as a case study. The Dove “Real Beauty” campaign highlights the potential for brands to address gender equality and female empowerment by aligning with pro-female and feminist principles. A narrative literature review shows how the components of identity-based brand management have been observed in scholarly discussions. Despite mixed responses and criticisms, Dove initiated important conversations around beauty standards and gender equality. The chapter emphasizes the need for authenticity, sensitivity, and continuous improvement in integrating SDG 5 into brand management while acknowledging the potential risks and limitations of consumerist therapy and false hopes. Future research could therefore explore diverse brands, industries, and cultural contexts, as well as the role of intersectionality in identity-based brand management.
Diabetic foot ulcer (DFU) is a chronic wound and a common diabetic complication as 2% – 6% of diabetic patients witness the onset thereof. The DFU can lead to severe health threats such as infection and lower leg amputations, Coordination of interdisciplinary wound care requires well-written but time-consuming wound documentation. Artificial intelligence (AI) systems lend themselves to be tested to extract information from wound images, e.g. maceration, to fill the wound documentation. A convolutional neural network was therefore trained on 326 augmented DFU images to distinguish macerated from unmacerated wounds. The system was validated on 108 unaugmented images. The classification system achieved a recall of 0.69 and a precision of 0.67. The overall accuracy was 0.69. The results show that AI systems can classify DFU images for macerations and that those systems could support clinicians with data entry. However, the validation statistics should be further improved for use in real clinical settings. In summary, this paper can contribute to the development of methods to automatic wound documentation.
The diabetic foot ulcer, which 2% – 6% of diabetes patients experience, is a severe health threat. It is closely linked to the risk of lower extremity amputation (LEA). When a DFU is present, the chief imperative is to initiate tertiary preventive actions to avoid amputation. In this light, clinical decision support systems (CDSS) can guide clinicians to identify DFU patients early. In this study, the PEDIS classification and a Bayesian logistic regression model are utilised to develop and evaluate a decision method for patient stratification. Therefore, we conducted a Bayesian cutpoint analysis. The CDSS revealed an optimal cutpoint for the amputation risk of 0.28. Sensitivity and specificity were 0.83 and 0.66. These results show that although the specificity is low, the decision method includes most actual patients at risk, which is a desirable feature in monitoring patients at risk for major amputation. This study shows that the PEDIS classification promises to provide a valid basis for a DFU risk stratification in CDSS.
Rationale:
Biomechanical analyses are capable of capturing and evaluating human motions. In addition to the major biomechanical fields of kinetics and kinematics, electromyography (EMG) provides a reliable way to analyse neuromuscular activities, e.g. inter- and intramuscular coordination or fatigue behavior. Based on these parameters it is possible to conclude to clinically relevant parameters such as motor control, muscular coordination or compensation strategies with different loads. In addition to this, EMG can be used in treatment itself, e.g. biofeedback-training with an EMG is an effective and evidenced based tool to improve neuromuscular control.
Purpose:
To show the advantages of implementing EMG in performing artists´ health and to demonstrate additional therapy and diagnostic options.

This workshop briefly introduces the theoretical principles of EMG and the clinical applications in the context of performing artists´ health. It explains why EMG provides an additional value in the clinical reasoning process and supports the therapist, but decision making in the clinical reasoning process should never be based on EMG solely.
In the further course of the workshop the use of EMG in diagnostics and therapy (biofeedback) with performing artists is practically demonstrated and discussed with the participants.

Clinical Significance:
EMG based functional neuromuscular diagnostics and biofeedback-training provides both the therapist as well as the performing artist with additional value in their clinical work.
Introduction: Hamstring injuries are among the most common injuries in soccer players. Especially the danger of hamstring injuries increases in the final third of each half, with the biceps femoris muscle being highly susceptible, but currently there is no supporting evidence in soccer players. It is important to investigate to provide a basis for further clinical investigations in order to reduce the risk of hamstring injuries.
Objective: To investigate a soccer-specific muscle fatigue protocol has an effect on the activation patterns of the biceps and semitendinosus muscles and correlates with the subjectively perceived exertion of the soccer players.
Methods: 19 healthy soccer players aged 19 - 35 years were included and underwent a standardized procedure that included: (1) performance of the nordic hamstring curls (NHC) and a 10m sprint (pre-test), (2) a soccer-specific muscle fatigue loading protocol for 30 minutes with subsequent rating of perceived exertion (RPE), (3) re-assessed like pre-assessment (post- test). Electromyography (EMG) data were recorded the biceps and
semitendinosus muscles of both legs. Two parameters were analyzed: the peak amplitude during the NHC and the mean amplitude during the 10m sprint.
Results: The RPE score (9.5, p<0.001) and 10m sprint shows significant differences between pre- and post- test for the biceps (standing leg: -82.45, kicking leg: -81.77; p<0.01) and semitendinosus muscle (standing leg: -60.08, p=0.001); kicking leg: -65.30, p=0.03). Significant correlation exists between RPE score and biceps muscles at posttest (standing leg: r=-0.54, kicking leg: r=-0.51; p<0.05).
Conclusion: Muscle fatigue leads to significant changes in the activation behavior of the hamstring muscles. In particular, the biceps muscle shows a correlation with perceived exertion, the higher the exertion, the lower the muscle activity. This may help explain the higher prevalence of the biceps muscle in hamstring injuries and offers a basis for further clinical investigations
Führungskräfte in Nonprofit-Organisationen befinden sich in sich stetig wandelnden Handlungsfeldern. Damit sie in dieser Komplexität konstruktiv agieren können, sind Kompetenzen erforderlich: Ganz zentral ist die Fähigkeit, sich initiativ und eigenverantwortlich mit den Herausforderungen unserer Zeit auseinanderzusetzen und im besten Fall geeignete Lösungen dafür zu finden sowie diese zielführend kommunizieren zu können. Aus diesem Grund wird genau diese Fähigkeit bei Studierenden aus den Masterstudiengängen Management in Nonprofit-Organisationen und Soziale Arbeit der Hochschule Osnabrück gefördert.
Im Rahmen des Moduls Handlungsfelder II entwickelten rund 30 Studierende im Wintersemester 2021/2022 in einer Denkwerkstatt Forschungsergebnisse und Lösungsideen zum Thema „Osnabrück 2030 – Nachhaltigkeit“. Die Studierenden wählten in einem partizipativen Prozess ihre eigenen Schwerpunktthemen wie zum Beispiel Mobilität oder Bildung aus und arbeiteten dann ein Semester lang an den Inhalten. Bei der Erarbeitung wurden sie durch Marlene Eimterbäumer begleitet, die Modelle, Methoden und Coaching zur Unterstützung bereit stellte. Die Modelle und Methoden finden sich teilweise in den Beiträgen der Studierenden wieder (unter anderem das Socio Ecological Model und der Business Model Canvas).
Am Ende des Semesters stand eine Präsentation vor den Kommiliton*innen, der Lehrenden und auch vor externen Gästen, die aus unterschiedlichen Expertisegebieten im Themenbereich Nachhaltigkeit kamen und dementsprechend Feedback aus der Praxis geben konnten. Die Entwicklung des Moduls erfolgte durch Gesa Birnkraut und Marlene Eimterbäumer und wurde von der Hochschule im Rahmen der Innovativen Lehre an der Fakultät Wirtschafts- und Sozialwissenschaften gefördert. Für die Studierenden stellte das Modul durchaus eine Herausforderung dar, denn in der Denkwerkstatt musste unter hoher Komplexität stark prozessbezogen gearbeitet werden im Gegensatz zu der sonstigen hohen Ergebnisorientierung. Die durchweg sehr guten Ergebnisse zeigen, dass der Einsatz und die kontinuierliche Kommunikation und Zusammenarbeit gelohnt haben.
Aufgeteilt ist das vorliegende Buch in die fünf Themen: Mobilität, nachhaltige Beschaffung, nachhaltige Bildung, Stadtplanung und Wirtschaft. Zu diesen Themen finden Sie jeweils erst einen Beitrag der Forscher*innen und darauffolgend einen Beitrag zur Darstellung einer Lösungsidee.
Der vorliegende Aufsatz untersucht den Einfluss körperorientierter Influencer-Kommunikation auf die Rezipienten. Hierfür wurde im Rahmen einer Bachelorarbeit ergründet, inwiefern die Inhalte zu Verhaltensänderungen führen und wie sie sich auf die Körperwahrnehmung der Rezipienten auswirken. Die Forschung fußt auf der Theorie des sozialen Vergleichs. In einer quantitativen Inhaltsanalyse wurde kategorisiert, welche körperorientierten Influencer-Inhalte die Probandinnen sehen. Auf dieser Grundlage bauen die anschließenden Leitfadeninterviews auf. Sie wurden mithilfe einer qualitativen Inhaltsanalyse ausgewertet. Die Erkenntnisse belegen Teile der Theorie des sozialen Vergleichs. Auch stimmen sie mit aktuellen Forschungsergebnissen überein, die den Einfluss körperorientierter Influencer-Kommunikation untersuchen. Die Probandinnen erfahren überwiegend aufwärtsgerichtete, soziale Vergleiche mit den Influencer-Inhalten. Sowohl bei Inhalten, die das geltende Schönheitsideal unterstreichen, als auch bei solchen, die Selbstliebe und Akzeptanz äußerlicher Makel kommunizieren. Die Inhalte beeinflussen Verhaltensweisen der Rezipienten, besonders hinsichtlich sportlicher Aktivitäten sowie dem Umgang mit dem eigenen Körper. Der Einfluss variiert von einer positiven bis zu einer deutlich negativen Wirkung. Abhängig ist dies von Inhaltsform und zugehöriger Vergleichsrichtung: Stark aufwärtsgerichtete Vergleiche haben einen deutlich negativeren Einfluss auf die Rezipienten, als nur leicht aufwärtsgerichtete oder gleichgestellte soziale Vergleiche. Diese extremen Aufwärtsvergleiche werden besonders von Influencer-Inhalten hervorgerufen, die das geltende Schönheitsideal verkörpern. Für die Praxis bedeuten die Ergebnisse, dass körperorientierte Kommunikation deutlich bewusster und mit Blick auf das Wohlbefinden der Rezipienten geschehen muss. Auch geben sie Anlass zu weiterer Forschung: Besonders interessant wäre es, den Abwärtsvergleich zu Influencern eingehender zu untersuchen.
In der digitalisierten Arbeitswelt ermöglichen neue und flexible Informations- und Kommunikationstechnologien (IKT) eine raum- und zeitunabhängige Erreichbarkeit (Albers, 2018). Während die „ständige“ Erreichbarkeit öffentlich kontrovers diskutiert wird, zeigt sich im Bereich des Kommunikationsmanagements, insbesondere hinsichtlich der Untersuchung von Agenturen, eine Forschungslücke (Röttger & Zielmann, 2009). Dieser Beitrag knüpft daran an und liefert empirische sowie praxisnahe Erkenntnisse zur arbeitsbezogenen erweiterten Erreichbarkeit. Mit dem Fokus auf Mitarbeitende in Kommunikationsagenturen wurde untersucht, wie ihre Erreichbarkeit ausfällt und welche Gestaltungsansätze sich im Arbeitskontext daraus ableiten. Zu ihrer Beantwortung wurden qualitative, leitfadengestützte Interviews mit Vollzeitbeschäftigten (n= 10) aus mittelgroßen Agenturen (Größe: 20 bis 50 Beschäftigte, n= 5) durchgeführt und ausgewertet. Die Interviewstudie analysiert Ausmaß, Ursachen, individuelle und betriebliche Umgangsweisen sowie darauf aufbauende Handlungsansätze. Die Ergebnisse verdeutlichen zwei Aspekte der erweiterten Erreichbarkeit in Agenturen: Individualität und Vielfalt. Innerhalb der Untersuchung kann verdeutlicht werden, dass die Erreichbarkeit auf einer Kombination vielfältiger Ursachen beruht. Für die Gestaltung der Erreichbarkeit leiten sich daraus relevante Implikationen ab: Statt verbindlicher Vorschriften sollte ein selbstbestimmtes, flexibles Arbeiten im Vordergrund stehen.
Einige simple Fragen an das kommunikative Handeln in und von Organisationen sind geradezu unverwüstlich. Eine davon lautet: Und was hat das alles mit mir zu tun? Diese rhetorische Frage wird oft gestellt, um anschließend die Auswirkungen einer Veränderung auf die individuelle Arbeits- oder Lebenssituation zu analysieren. Was die digitalen Medien ganz konkret mit der Art und Weise zu tun haben, wie wir arbeiten, uns orientieren und erinnern, das zeigen die Beiträge der aktuellen Ausgabe.

Der vorliegende Aufsatz setzt sich mit dem Hostile-Media-Phänomen auseinander, nach welchem Personen, die eine starke Einstellung bei einem Thema aufweisen, die mediale Berichterstattung dazu als tendenziell konträr zu ihrer eigenen Meinung wahrnehmen (Vallone et al., 1985). Der Effekt wird durch Merkmale der Quellenangabe verstärkt, die Gruppenzugehörigkeiten des Kommunikators suggerieren (Ariyanto et al., 2007; Arpan & Raney, 2003; Gunther & Liebhart, 2006; Gunther et al., 2016; Kim & Billings, 2017; Reid, 2012). Nun wird für diese Arbeit vermutet, dass das Alter – als natürliches Merkmal – in bestimmten Kontexten als Heuristik für die ideologische Perspektive des Kommunikators fungiert. Entsprechend, so die Annahme, werden junge Befürworter bei dem Thema „Fridays for Future“ einen jungen Kommunikator mit der In-Group und einen alten Kommunikator mit der Out-Group assoziieren. Die Zuordnung des Kommunikators zu der In-Group resultiert demnach in einer Assimilation mit den Inhalten. Wird der Artikel hingegen der Out-Group zugerechnet, sollten die Inhalte abgelehnt beziehungsweise als feindlich verzerrt wahrgenommen werden. In einer experimentellen Studie wurden 108 Studierende um die Einschätzung zu einem Kommentar über „Fridays for Future“ gebeten. Der ausgewogene Text wurde entweder einem vermeintlich jungen oder alten Autor zugeschrieben. Dem Kommentar in der Version ALT wurde von den jungen Befürwortern erwartungsgemäß weniger zugestimmt als dem identischen Inhalt in der Version JUNG. Allerdings konnten keine signifikanten Unterschiede zwischen den beiden Versionen bei der Einschätzung der Ausgewogenheit des Artikels festgestellt werden. Diese Arbeit wirft einen neuen Blickwinkel auf potenziell relevante Merkmale der Quellenangabe bei der Entstehung von Hostile-Media-Effekten und gibt damit Anreize für zukünftige Forschung in diesem Bereich.
Die vorliegende Studie beschäftigt sich mit dem Einfluss der Corona-Pandemie auf die Interne Unternehmenskommunikation in Kliniken und deren interne Positionierung. Dafür wurden sechs Expert:inneninterviews mit Verantwortlichen der Internen Unternehmenskommunikation in Kliniken geführt, die anschließend durch eine qualitative Inhaltsanalyse ausgewertet wurden. Die Untersuchung zeigt, dass die Corona-Pandemie aufgrund ihrer Kriseneigenschaften eine Chance für die Bedeutung der Internen Unternehmenskommunikation in den befragten Kliniken darstellt. Dazu stellt sich heraus, dass die Krise die hierarchische Einbindung, die finanziellen Ressourcen und die interne Akzeptanz der internen Kommunikationsverantwortlichen positiv beeinflusst. Auch wenn die Pandemie nicht auf alle Aspekte der internen Positionierung einwirkt, lässt sie sich als eine Chance für die Neupositionierung der Internen Unternehmenskommunikation identifizieren. Gerade mit ihrem Einfluss auf die interne Bedeutungswahrnehmung gelingt es der Krise, an einer wichtigen Stellschraube für die Verbesserung der internen Positionierung zu drehen.
Dieser Artikel setzt sich anhand eines Beispiels mit der Frage auseinander, welchen Beitrag soziale Medien für die Pflege des kollektiven Gedächtnisses leisten und welche Risiken mit dem Einsatz von sozialen Medien in der Erinnerungskultur einhergehen. Anhand des Instagram-Projekts @ichbinsophiescholl vom SWR und BR wurden die Vor- und Nachteile von sozialen Medien unter Berücksichtigung von wissenschaftlichen Publikationen diskutiert. Zehn Monate lang, von Mai 2021 bis Februar 2022, konnten Instagram-Nutzer das Leben einer fiktiven Sophie Scholl verfolgen. Durch Storytelling und die Kombination aus fiktiven und realen Inhalten versuchten die Projektverantwortlichen, die Geschichte der Widerstandskämpferin auf dem Instagram-Kanal @ichbinsophiescholl zu erzählen. Aufgrund von fehlenden Quellenangaben und dem Verzicht darauf, fiktive Inhalte kenntlich zu machen, wurde das Projekt kritisiert. Der Vorwurf, dass das Projekt ein falsches Bild von Sophie Scholl erschafft, wurde laut. Damit trifft die Sorge, dass in sozialen Medien nicht immer zwischen anerkannter Geschichtsdarstellung, Fiktion oder Unwahrheit unterschieden werden kann, auch auf @ichbinsophiescholl zu. Es ist noch ein langer Weg, bis solche Projekte einen wertvollen Beitrag zum Erhalt der Erinnerungskultur beitragen können, ohne Gefahr zu laufen, Geschichte falsch oder unangemessen darzustellen. Um herauszufinden, wie Erinnerungsprojekte aufgebaut sein müssen, um als sinnvoller Beitrag zum kollektiven Gedächtnis wahrgenommen zu werden und eine Balance zwischen dem aktuellen Zeitgeist und einer trivialisierenden Darstellung zu finden, werden weitere wissenschaftliche Untersuchungen benötigt.
ZUKUNFTSBÄUME FÜR MÜNSTER – ERARBEITUNG EINES BAUMENTWICKLUNGSKONZEPTES MIT STANDORTSPEZIFISCHEN ARTENEMPFEHLUNGEN UNTER ZUKÜNFTIGEN KLIMABEDINGUNGEN UNTER BERÜCKSICHTIGUNG DES KLIMAHÜLLENKONZEPTES
Durch den voranschreitenden Klimawandel werden die ohnehin ungünstigen Standortbedingungen von Bäumen im urbanen Raum zunehmend schwieriger. Gleichzeitig steigt die Bedeutung von Stadtbäumen, denn im Rahmen der Klimaanpassung haben ihre Ökosystemleistungen einen besonders hohen Stellenwert. Viele der gängigen Arten leiden allerdings stark unter den bereits spürbaren Auswirkungen des Klimawandels – auch im klimatisch vergleichsweise milden Münster. In der vorliegenden Arbeit wird eine vereinfachte Typologisierung der urbanen Baumstandorte in Münster vorgenommen. Unter Berücksichtigung der stadtklimatischen Besonderheiten und spezifischen Einschränkungen wird das Klimahüllenkonzept von Kölling auf die jeweiligen Standorttypen übertragen und in Bezug zu den zukünftig erwarteten klimatischen Verhältnissen gesetzt. Hierbei zeigt sich, dass zum Ende des aktuellen Jahrhunderts nicht mehr viele der klassischen Arten als Stadtbäume in Münster geeignet sein werden. Daher werden standortbezogene Zukunftsbaum-Empfehlungen in Form eines vereinfachten Baumentwicklungskonzeptes erarbeitet. Die Standortgerechtigkeit und die Diversität des urbanen Baumbestandes werden im Hinblick auf die geänderten klimatischen Bedingungen in Zukunft enorm an Bedeutung gewinnen. Die Erweiterung und Optimierung des zukünftigen Stadtbaumsortiments ist insofern ein wichtiger Baustein zur Anpassung des urbanen Baumbestandes an den voranschreitenden Klimawandel.

Andere sehen hingegen bereits die Herrschaft des Computers, über den Menschen, am Horizont aufziehen. Wieder andere wittern ganz einfach nur das große Geschäft. Doch wie sieht die Realität aus? Wo liegen die Chancen und wo die Risiken der neuen Technologie. Kann eine Software auf der Basis von Internetdaten abgesicherte Persönlichkeitsprofile von Menschen erstellen? Diese und weitere Fragen sollen im Folgenden beantwortet werden.

Explizit negative Charakterisierungen sind in Arbeitszeugnissen rechtlich nicht erlaubt. Daher versuchen manche Arbeitgeber mit Verschleierungstechniken versteckt Botschaften zu transportieren. Ist es sinnvoll nach solchen Verschleierungstechniken zu suchen oder sie zu interpretieren?
Die alltägliche pädagogische Praxis von Grundschulen hat sich durch die Implementierung unterschiedlicher multiprofessioneller Kooperationsprozesse maßgeblich geändert (vgl. Beck & Maykus 2016, S. 148f.). So kooperieren laut Thieme (2019) seit dem Ausbau von ganztägig-inklusiven Schulsettings zunehmend auch Lehrkräfte und sozialpädagogische Fachkräfte unterschiedlicher Fachbereiche und Institutionen (vgl. Thieme, 2019, S. 247). Die Schulsozialarbeit an Grundschulen ist dabei ein sozialpädagogisches Arbeitsfeld, welches als Schnittstelle zwischen verschiedenen pädagogischen Berufsgruppen gilt (vgl. Thimm 2017, S. 108) und dem der Vernetzungs- und Kooperationsgedanken als Leitmotiv (sozial-)pädagogischer Praxis inhärent ist. Vor allem die Kooperation zwischen den Lehrkräften und der Schulsozialarbeit ist dabei an bestimmte Voraussetzungen geknüpft. Das BMBF-geförderte Forschungs- und Entwicklungsprojekt „Qualifizierungsmodule für die interprofessionelle Kooperation in inklusiven Grundschulen – Entwicklung und Evaluation des Manuals InproKiG“ (kurz: InproKiG) hat mit unterschiedlichen Erhebungsinstrumenten u.a. diese Voraussetzungen und Herausforderungen in der alltäglichen Kooperationspraxis näher analysiert. Der Beitrag stellt Ergebnisse einer quantitativen Erhebung zum Stand der Kooperation und zu Kooperationsbedarfen von Grundschulteams vor, welche in den Jahren 2018 und 2019 an Grundschulen in Hannover, Osnabrück und Gießen durchgeführt wurde. In Einzelanalysen werden vor diesem Hintergrund vertiefend die Perspektiven von Fachkräften der Schulsozialarbeit und ihre Erfahrungen interprofessioneller Kooperation untersucht.
Der primäre Einsatzzweck von Reifegradmodellen besteht zumeist in der reinen Inventarisierung der vorhandenen IT-Komponenten. Das vorliegende Kapitel gibt IT-Entscheider*innen in Krankenhäusern Empfehlungen, wie Reifegradmodelle für eine kontinuierliche Weiterentwicklung, Umsetzung und Evaluation von Digitalisierungsstrategien eingesetzt werden können. Als Prüfschema für die Auswahl geeigneter Verfahren werden neun Anforderungen an die Entwicklung und den Einsatz von Reifegradmodellen formuliert. Entlang von drei strategischen Handlungsfeldern – dem klinischen Anwendungsfeld, dem Informationsmanagement und dem organisatorischen Umfeld – werden dem Leser generische Digitalisierungsziele und dazugehörige Beispielindikatoren zur Erfolgskontrolle bereitgestellt.
Digitale Technologien lassen Daten im Berufsalltag von Beschäftigten im Gesundheitswesen allgegenwärtig werden. Diese Entwicklung stellt neue Herausforderungen an das Kompetenzprofil dieser Berufsgruppen, die sich unter dem Dachbegriff Datenkompetenz zusammenfassen lassen. Im vorliegenden Beitrag wird der Begriff vor dem Hintergrund der digitalen Transformation motiviert und definiert. Es wird eine zusammenfassende Sicht auf die Teilaspekte von Datenkompetenz gegeben und auf den derzeitigen Stand der systematischen Messung von Datenkompetenz eingegangen. Abschließend gibt der Beitrag eine Reihe von Handlungsempfehlungen, auf welche Weise Datenkompetenz im Rahmen der Weiterqualifikation von Beschäftigten vermittelt werden sollte.

Aber es gibt viele weitere Beispiele dafür, dass Gruppen, die mit hochkarätigen Personen besetzt sind, zu folgenschweren Fehlentscheidungen gelangen. Woran liegt das? Das sozialpsychologische Phänomen des Groupthink liefert eine Erklärung.
Today's development of client-side web applications is based on one of the JavaScript-frameworks, such as Angular or React. The excessive dependencies that arise in the ecosystem from the Node-Package-Manager increase the security risk and the dependency of your own web application on third-party packages. Moreover, the frameworkless approach proposes a renaissance of classic web development, because it strives to avoid external dependencies as far as possible and to fall back on the standards. Whether the implementation achieves maintainability and security of frameworks is questionable. Therefore, it makes sense to research which core concepts of the frameworks meet the requirements for maintainability and security and how these are implemented. The novelty is that the concepts to be explored are moved to a standard in order to ensure the developer efficiency, security, performance and maintainability in the long term. This allows existing approaches to focus on other essential features.
In the context of the ongoing digitization of interdisciplinary subjects, the need for digital literacy is increasing in all areas of everyday life. Furthermore, communication between science and society is facing new challenges, not least since the COVID-19 pandemic. In order to deal with these challenges and to provide target-oriented online teaching, new educational concepts for the transfer of knowledge to society are necessary. In the transfer project “Zukunftslabor Gesundheit” (ZLG), a didactic concept for the creation of E-Learning classes was developed. A key factor for the didactic concept is addressing heterogeneous target groups to reach the broadest possible spectrum of participants. The concept has already been used for the creation of the first ZLG E-Learning courses. This article outlines the central elements of the developed didactic concept and addresses the creation of the ZLG courses. The courses created so far appeal to different target groups and convey diverse types of knowledge at different levels of difficulty.
Das Unternehmen "100 Worte Sprachanalyse" hat 2021 den "HR Startup Award" erhalten, der federführend vom Bundesverband der Personalmanager (BPM) verliehen wird. Überzeugt hat 100 Worte "mit dem Versprechen, den unbewussten Gender-Bias in Stellenanzeigen zu reduzieren und unterschiedliche Grundmotive von Bewerbergruppen zu erfassen", so die Initiatoren. Doch es bestehen berechtigte Zweifel daran, dass die Software dieses Versprechen einlösen kann.
Apps have been attested to empower patients regarding disease self-management through numerous studies. However, it is still unclear what factors determine the perception of patients whether an app is a useful tool for this purpose. A multiple regression model that was informed by the Technology Acceptance Model (TAM 2) was tested based on the answers of 235 app users with Diabetes type 1 or 2. The model accounted for 59.2% of the variance of the perceived degree of self-management. Factors belonging to the relevance-usefulness-quality complex as well as factors reflecting the patient’s self-control were found to be significant in the model. Patient demographics, i.e. age, gender, app experience and type of Diabetes did not play any significant role. In conclusion, this study raises the question whether apps should be designed to strengthen self-management in the sense of self-control (e.g. own measurements, diary) as opposed to guiding and advice giving.
With the start of the 21st century, patient safety as a topic of special interest has attracted increasing attention in both academia and clinical practice. As technology has continued to develop since then, questions and focal points surrounding the topic have also shifted. In particular, questions regarding the impact of digitalization on patient safety and its measurement are now of high interest. This work aims to develop a maturity assessment instrument in the form of a criteria set for measuring structural requirements for digital patient safety in hospitals. Based on the results of a literature review and a derivation of maturity objects (MO) from known maturity models, 64 criteria across 11 categories were developed. Written comments of two digital patient safety experts as well as subsequent interviews were used to evaluate and refine the criteria catalog. The resulting catalog offers hospitals guidance for detecting possible areas of structural improvements in their information systems with regard to patient safety and represents a unique instrument for assessing digital maturity in this particular area.
Venous leg ulcers and diabetic foot ulcers are the most common chronic wounds. Their prevalence has been increasing significantly over the last years, consuming scarce care resources. This study aimed to explore the performance of detection and classification algorithms for these types of wounds in images. To this end, algorithms of the YoloV5 family of pre-trained models were applied to 885 images containing at least one of the two wound types. The YoloV5m6 model provided the highest precision (0.942) and a high recall value (0.837). Its mAP_0.5:0.95 was 0.642. While the latter value is comparable to the ones reported in the literature, precision and recall were considerably higher. In conclusion, our results on good wound detection and classification may reveal a path towards (semi-) automated entry of wound information in patient records. To strengthen the trust of clinicians, we are currently incorporating a dashboard where clinicians can check the validity of the predictions against their expertise.
Im Rahmen der vorliegenden Masterarbeit im Studiengang Landschaftsarchitektur an der Hochschule Osnabrück wurden Untersuchungen zur Winteraktivität der Zwergfledermaus (Pipistrellus pipistrellus) in Osnabrück (Niedersachsen) durchgeführt und Methoden zum ultraschallakustischen Nachweis von Gebäudequartieren erprobt und weiterentwickelt. Vorausgegangen war eine Online-Umfrage bei 17 Mitarbeitenden Unterer Naturschutzbehörden in Niedersachsen zur Verfahrenspraxis bei fledermauskritischen Gebäudesanierungen und Abrissen. Defizite bestehen demnach möglicherweise nicht nur in der Qualität fledermauskundlicher Erfassungen, sondern auch in einer gravierenden Kenntnislücke der zuständigen Behörden über Vorhaben, bei denen gebäudebewohnende Fledermäuse und ihre Quartiere beeinträchtigt werden. Das Feststellen von schwärmenden Zwergfledermäusen kann dazu dienen, noch während der Überwinterung eine Quartiersnutzung festzustellen. Winterschwärmkontrollen sollten daher in allgemeine Methodenempfehlungen Eingang finden, um bei Artenschutzprüfungen eine bessere Wissensgrundlage über die phänologische Quartiersfunktion und -nutzung zu ermöglichen.
Akustische Quartiersnachweise können sowohl über aktive Untersuchungen als auch unter bestimmten Voraussetzungen mithilfe passiver automatischer Ultraschallerfassungen erfolgen. Optische Hilfsmittel, vorzugsweise moderne Wärmebildkameras, sollten darüber hinaus unbedingt hinzugezogen werden. In der Stadt Osnabrück konnten in einer mehrtägigen Frostperiode Ende Dezember 2021 vier neue Winterquartiere der Zwergfledermaus bei Begehungen mit Detektor gefunden werden. Ein parallel durchgeführtes Dauermonitoring an einem großen Winterquartier über den Zeitraum 29.10.2021 bis 01.03.2022 offenbarte erhöhte Schwärmaktivität nicht nur bei Frost, sondern ebenfalls nach kräftigen Temperaturanstiegen mit Nachttemperaturen bis über 10°C. Die genauen Zusammenhänge zwischen allgemeiner Aktivität und Witterungsbedingungen sind komplex und benötigen weitergehender Untersuchungen. Auch auf Grundlage des derzeitigen Kenntnisstandes können großräumige Quartierssuchen in Form von winterlichen Schwärmkontrollen bereits jetzt dazu beitragen, essentielle Quartiere der Zwergfledermaus zu identifizieren. Den Unteren Naturschutzbehörden wird u.a. vorgeschlagen, ein Kataster über diese Quartiere in ihrem Zuständigkeitsbereich zu führen und beständig zu aktualisieren.
Das Pfeifregister, auch als flageolet-, flute-, bell- oder whistle register bezeichnet, wird als der höchste tonale Bereich der menschlichen Stimme begriffen und zählt zu den am wenigsten erforschten Funktionen der Singstimme. Der Fokus dieser Forschungsarbeit liegt in der Erarbeitung stimmtechnischer Vorgehensweisen für die gesunde Tonproduktion beim Singen in diesem Register, ausgehend von den zugrunde liegenden anatomisch-phonetischen Prozessen. Die unter dem Begriff ‚Pfeifregister‘ zusammengefassten stimmlichen Höhen lassen sich in zwei verschiedene Register unterteilen, fortlaufend als M3 und M4 bezeichnet. M4 hingegen hat sich als ein von anderen Stimmfunktionen distinkt zu unterscheidendes Register herausgestellt, das eher indirekt, spielerisch und über auf dieses Register ausgelegte Gesangsübungen, in erster Linie mittels großer Intervallsprünge sowie der drei Katalysatoren Vokal, Tonhöhe und Lautstärke angesteuert werden kann.
Power consumption has become a major design constraint, especially for battery-powered embedded systems. However, the impact of software applications is typically considered in later phases, where both software and hardware parts are close to their finalization. Power-related issues must be detected in early stages to keep the development costs low, satisfy time-to-market, and avoid cost-intensive redesign loops. Moreover, the variety of hardware components, architectures, and communication interfaces make the development of embedded software more challenging. To manage the complexity of software applications, approaches such as model-driven development (MDD) may be used. This article proposes a power-estimation approach in MDD for software application models in early development phases. A unified modeling language (UML) profile is introduced to model power-related properties of hardware components. To determine the impact of software applications, we defined two analysis methods using simulation data and a novel in-the-loop concept. Both methods may be applied at different development stages to determine an energy trace, describing the energy-related behavior of the system. A novel definition of energy bugs is provided to describe power-related misbehavior. We apply our approach to a sensor node example, demonstrate an energy bug detection, and compare the runtime and accuracy of the analysis methods.
Die UN-Mitgliedsstaaten beschlossen im Jahr 2015 mit der Agenda 2030 insgesamt 17 Ziele für eine nachhaltige Entwicklung. Dem Verkehr sowie der Verkehrsinfrastruktur wird hierbei eine besondere Bedeutung beigemessen. Anhand ökologischer und sozioökonomischer Indikatoren wird die Verkehrsinfrastruktur in Bezug auf die nachhaltige Entwicklung von drei ausgewählten Region in Deutschland analysiert. Unter Beachtung der Ergebnisse sind Maßnahmen dargelegt, mit denen die Verkehrsinfrastruktur einen Beitrag zur nachhaltigen Entwicklung einer Region leisten kann.
Der vorliegende Band "Theater, Kampf und Kollektive" geht der Frage nach, welche Diskurse das Arbeitertheater des 20. Jahrhunderts auszeichnen. Im Rahmen des Forschungsprogrammszur "Archäologie der Theaterpädagogik" legt Eva Renvert damit einen Beitrag zur Geschichte des nicht-professionellen Theaters in Deutschland vor.Anhand zweier Beispiele - dem Arbeitertheater Erwin Piscators 1920 bis 1924 und der "Horizonte"-Inszenierung des Arbeitertheaters Schwedt 1968 - werden Arbeitsweisen und Wirkungen dieser Versuche rekonstruiert und als frühe Formen eines interventionistischen Theaters kenntlich gemacht. Ausgehend vom russischen Proletkult wertet die Autorin dazu zahlreiche Quellen des deutschen Arbeitertheaters aus und kontextualisiert sie für die Gegenwart.Es zeigt sich, dass historische Diskurse sich in Diskurssträngen verdichten, die bis in die heutige Theater- und theaterpädagogische Praxis reichen. So bildet diese Publikation auch eine wertvolle Grundlage für das Forschungsfeld "Theater als Intervention" sowie für eine politisch motivierte Theaterpädagogik.
Die deutschen Kommunen stehen aktuell vor schweren Herausforderungen: der durch die Covid-19-Pandemie bedingte wirtschaftliche Abschwung hat zu massiven Einbrüchen in den kommunalen Steuereinnahmen geführt und gleichzeitig hohe außerplanmäßige Ausgaben bewirkt. Weitere zeitaktuelle Herausforderungen für den kommunalen Finanzhaushalt sind die drohende Energiekrise und die hohe Inflation. Daraus resultierend befinden sich die Kommunen in einer angespannten Haushaltslage und sind gezwungen, unpopuläre Sparmaßnahmen zwecks Haushaltsausgleich einzuleiten. Auch der Landkreis Osnabrück unterliegt einem enormen Konsolidierungsdruck. Die Verwaltungsführung und die politischen Gremien haben beschlossen, ein umfassendes Haushaltskonsolidierungskonzept durchführen zu lassen, im Rahmen dessen u. a. Aufgabenkritik ausgeübt werden soll. Hierbei unterliegen insb. freiwillige Aufgaben einer strengen Aufgabenkritik. Zu den freiwilligen Aufgaben gehört auch die Europaarbeit des ED. Das ED ist eine zentrale europäische Anlaufstelle für die Bevölkerung der Region Osnabrück, bei der sich die Bürger über die EU informieren und zu allerlei europabezogenen Anliegen beraten lassen können. Vor dem Hintergrund der geschilderten Situation steht der ED-Leiter nun vor der Herausforderung, die Europaarbeit des ED vor der Politik und Verwaltungsführung zu rechtfertigen. Aus diesem Anlass untersucht die Verfasserin in der vorliegenden Arbeit, welche Bedeutung dem ED des Landkreises Osnabrück aus Bürgersicht zukommt. Hierzu nimmt sie eine Betrachtung des ED aus aufgabenkritischer Perspektive vor. Im Rahmen der Betrachtung zieht sie die ihr zugrundeliegenden Informationen über das ED sowie Ergebnisse einer stichprobenartigen Umfrage heran.
Im Ergebnis stellt sich heraus, dass die ED-Europaarbeit beibehalten und weiterhin durch den Landkreis Osnabrück wahrgenommen werden sollte. Im Hinblick auf den Geschäftsprozess ergibt sich die Notwendigkeit zu einer Reduktion des Aufgabenumfangs und einer Umpriorisierung der Aufgaben. Insofern bestehen Optimierungspotentiale, die im Rahmen einer GPO näher analysiert werden müssten.
Im Rahmen dieser Untersuchung sollen folgende Fragen beantwortet werden:
- Was sind die entscheidenden Aspekte in der zukünftigen Entwicklung der Energieversorgung und des -verbrauchs im Landkreis Emsland?
- Welche Annahmen können für den Energieverbrauch im Zieljahr 2040 getroffen werden?
- Wie kann ein angemessener Beitrag des Landkreises Emslands zur klimaneutralen Energieversorgung Deutschlands quantifiziert werden?
- Welche Energieszenarien sind für den Landkreis Emsland im Jahr 2040 denkbar?
- Welche Empfehlungen lassen sich aus diesen Szenarien für Unternehmen, Politik und Verwaltung ableiten?
In diesem Weißbuch stellen wir Methoden und Maßnahmen vor, die Unternehmen dabei unterstützen, ihre Energieversorgung auf erneuerbare Energien umzustellen, sich von den Preisschwankungen an den fossilen Weltmärkten unabhängiger zu machen und Entscheidungen für eine Klimastrategie für die nächsten Jahrzehnte zu treffen. Es werden drei mögliche Energiewelten der Zukunft skizziert. Informationen zum Thema Eigenstromerzeugung und zu Effizienzmaßnahmen werden anschaulich dargestellt sowie Links zu Fördermöglichkeiten genannt. Wir zeigen drei verschiedene Szenarien für die Energieversorgung von Unternehmen in der Zukunft auf. Sie geben Unternehmen eine Entscheidungshilfe für die eigene Klimastrategie. Die drei Szenarien haben Unterschiede, aber auch Gemeinsamkeiten.
Die EU hat im „Green Deal“ Maßnahmen festgelegt, damit Europa im Jahr 2050 klimaneutral ist. Deutschland hat seine Klimaziele zuletzt noch einmal verschärft und sich als Ziel die Klimaneutralität bis 2045 gesetzt. Um diese Ziele zu erreichen, stehen Industrie und Gewerbe vor der Herausforderung, ihre Energieversorgung auf erneuerbare Energien umzustellen und in Zukunft fossile Brenn- und Kraftstoffe zu ersetzen. Dies gilt für kleine und mittlere Unternehmen (KMU) ebenso wie für die Großindustrie. Es stellt sich also die Frage, wie Unternehmen sich mit Strom, Wärme, Dampf, Kälte und weiteren Energieformen versorgen. Welches ist dabei aus heutiger Sicht die günstigste Technologie? Zentrale Rollen hierbei spielen Energieeffizienzmaßnahmen und die Substitution fossiler durch regenerative Energieträger. Lösungen müssen sowohl technisch als auch ökonomisch realisiert werden können. Im Vorteil ist, wer heute schon eine unternehmensinterne Klimaschutzstrategie verfolgt und die ersten Schritte in Richtung Klimaneutralität unternimmt.
Automobilunternehmen müssen im Übergang zur Elektromobilität häufig Geschäftsmodelle für die Elektromobilität sowie die konventionelle Mobilität parallel managen. Verschiedene Arbeiten, insbesondere von Proff und anderen, haben das Management dieser Geschäftsmodelle untersucht. Der Schwerpunkt dieser Arbeiten liegt vorwiegend auf dem Management der neuen Geschäftsmodelle für die Elektromobilität. Diese Arbeit rückt das Management der Geschäftsmodelle für die konventionelle Mobilität in den Mittelpunkt und betrachtet die Entscheidungen zur Wettbewerbsstrategie als einer wesentlichen Geschäftsmodellkomponente. In den Geschäftsmodellen für die konventionelle Mobilität findet in der Regel die Ressourcenexploitation statt, um die Ressourcenexploration für die Elektromobilität zu finanzieren. Unter Rückgriff auf theoriebasierte Portfoliomodelle und Proff (2019) werden die Entscheidungen zu Wettbewerbsvorteilen in der konventionellen Technologie diskutiert. Daran anknüpfend findet die Prüfung der abgeleiteten Annahmen statt. Hierzu dient eine Inhaltsanalyse der Geschäftsberichte führender deutscher Automobilzulieferer. Diese Inhaltsanalyse liefert Hinweise für eine verstärkte Bedeutung von Kostenführerschaft bei Geschäftsmodellen für die konventionelle Mobilität. Der Beitrag untersucht auf diese Weise wie gefordert die Entscheidungen zu Geschäftsmodellen im Übergang zur Elektromobilität retroperspektiv und beleuchtet im Sinne der Ambidextrie das Management der Geschäftsmodelle der konventionellen Mobilität und der dort notwendigen Ressourcenexploitation.

Hypothesis/Aims of study
Many results related to the effectiveness of surgical and non-surgical proce- dures for treating urinary incontinence (UI) are reported in the literature. Following the principles of evidence based-practice, besides the interpreta- tion of study results based on statistical significance, authors should consid- er evaluating the clinical relevance of treatment effects in this field.
The minimal important difference (MID) of clinical outcomes could be used to assess the clinical relevance of interventions. MID is defined as “the small- est difference in score in the domain of interest that patients perceive as important, either beneficial or harmful, and which would lead the clinician to consider a change in the patient’s management’’[1]. One common way to obtain MID for outcomes of interest is by using anchor-based methods. These methods apply one anchor that analyzes the change in the patient's health status according to the patient’s perception.
However, MIDs should be provided according to appropriate calculations and methods and based on the definition of a MID. However, there are a lot of misconceptions and misunderstandings related to the MID. These mis- understandings have led to incorrect reports of these values. Moreover, it is still not known which criteria the authors considered during the analysis of the MID in the Women’s Health area. Therefore, with this preliminary re- port, we aimed to identify and report all anchor-based methods to estimate MIDs for outcomes measures related to UI available in the literature; and analyze which concepts and levels of improvement in the health status of the patient have been considered by the authors to calculate the MID.
Study design, materials and methods
This systematic review was conducted according to PRISMA guide- lines. The study protocol was registered in the PROSPERO database (CRD42022299686). A systematic search was performed using Ovid Med- line, Embase, Web of Science, and Scopus from May to June 2021. Any study generating MIDs for UI that included women with more than 18 years, stress urinary incontinence (SUI), urgency urinary incontinence (UUI) and/ or miXed urinary incontinence (MUI) was included. The primary outcome was the MID for outcomes related to UI. No limits were applied on the data- bases for the date, language or publication range.
Studies were classified into three categories according to the level of im- provement in health status assessed by the anchor and considered by the authors during the MID calculation: 1) slight improvement: if authors in- cluded participants that evaluated their health status as “a little better” in their analysis; 2) moderate improvement: if authors considered women that reported a “better” or a “much better” status of the condition; or 3) strong imporvement: if all patients that improved (“very much better” or if authors grouped all the patients that improved in one single category) were consid- ered in one group against other group that did not report any improvement. After classifying the papers, we counted and reported how many studies were considering only the minimal level of improvement to reported the MID, according to previous definition and recommendation.
The initial electronic search resulted in a total of 1,662. After removing du- plicates (n=719), 943 were screened, and at the end of the selection stages, nine papers that reported anchor-analysis were included in this preliminary report. Seven studies included women with SUI (total sample size= 2,436), while one study included only women with UUI (n=307), and the other one evaluated women with SUI and MUI (n=288). SiX studies analyzed data and provided the MID after a non-surgical treatment of UI, while three analyzed the results after surgery to correct UI. Eleven different questionnaires to measure the patient-related outcomes related to UI with their MIDs were identified. All the tools were related to measuring the impact, distress, or quality of life of women with UI.
Different anchors were used to analyze MID, including scales that evaluated the improvement and satisfaction of the patient, and the visual analogue scale, measures of urinary leakage and questionnaires that measure the se- verity and impact of UI. The MID of siX tools was determined according to the smallest difference detected by the patients, using the Patient Global Im- pression of Improvement questionnaire and the self-reported satisfaction to assess the change of the condition. Most of the MIDs (n=28, 80%) were mis- calculated considering a moderate or a strong improvement of the patients, and not a minimal improvement as suggested by the literature (Table 1).
Interpretation of results
Although previous systematic reviews have reported the psychometric prop- erties of different questionnaires to measure UI outcomes, this is the first study to analyze methods of obtaining MIDs for UI outcomes from the pa- tients perspective (anchor based methods). All the tools with their respec- tives MIDs were related to the impact, distress, and/or quality of life of women with UI. The use of these outcomes measures is in line with the as- sociated impairments of social, psychological, financial, and sexual aspects of a women’s life produced by UI.
Most of the authors in this field did not consider the smallest difference identified by the participants to calculate the MID, which does not follow the original definition of MID proposed by Jaeschke et al.,1 This could gen- erate underestimation or over-estimation of MID, which may directly im- pact the interpretation of the findings from the clinical trials[2] and biased interpretation of the results of the clinical significance from the interven- tions used to manage female UI. Therefore, the interpretation of the clinical significance related to UI outcomes should be done with caution.
Concluding message
Few studies that aimed to calculate the MID using anchor-based methods for outcomes related to female UI were found in the literature. Eleven different questionnaires to measure the outcomes related to UI with their MIDs were identified. However, most studies had not considered the smallest change of improvement (as perceived by the patients) in their analysis, which does not follow the definition of the MID. This could impact decision making. Future research should provide clear guidelines on how to calculate, report, and interpret MIDs in this field.
Rationale:
Neck pain is a large health problem worldwide and often seen in musicians [1, 2]. Neck pain can radiate into the arm due to various underlying pain types and pain mechanisms making it heterogeneous in clinical signs and symptoms [3-5]. On the one hand, patients may present with dominant nociceptive neck-arm pain caused by activation of the nociceptors in muscles, joints, ligaments, fascia, tendons and the connective tissues of a nerve [6, 7]. Activation of nociceptors in nerve connective tissues may cause clinical signs of heightened nerve mechanosensitivity what is per definition categorized as nociceptive pain [6, 8, 9]. On the other hand, patients may present with dominant neuropathic pain, defined as pain as a direct consequence of a lesion or disease affecting the somatosensory system [10, 11]. The clinical profile of these different pain types is sometimes difficult to disentangle based on the localization and pain character [12]. Moreover, non-specific neck-arm pain patients shown a neuropathic pain component based on somatosensory changes detected via Quantitative Sensory Testing (QST) [3, 13]. Classifications with a defined physical examination pathway can be helpful to define subgroups to guide the clinical decision making [14]. This workshop updates the background about the pathophysiology of neck-arm pain and mediates an evidence-based examination to classify patients.
Purpose:
The aim of this workshop is to give a current insight into the background and evidence of neck-arm pain and to plan and practice a physical examination.
Content of Presentation:
This workshop will summarize evidence of neck-arm pain. Thereupon, current evidencebased diagnostic options will be presented and practiced together. Finally, a short insight in the management of neck-arm pain will be given.
Approach of Presentation:
QST testing and current cost-effective evidence-based methods will be presented to identify neuropathic components in neck-arm pain. Selected methods will be performed practically together, e.g. bedside sensory testing and neurodynamic tests.
Clinical Significance:
After the workshop, participants will have improved skills to diagnose in the spectrum of neck-arm pain for musicians with neck-arm pain.
At the end of the presentation, the participants will be able to:
- understand the heterogeneity of neck-arm pain,
- plan an appropriate diagnostic physical examination,
- have an insight in possible management strategies.
