High levels of meat consumption are increasingly being criticised for ethical, environmental, and social reasons. Plant-based meat substitutes have been with reservations identified as healthy sources of protein in comparison to meat. This alternative offers several social, environmental, and probably health benefits, and it may play a role in reducing meat consumption. However, there has been a lack of research on how specific meat substitute attributes can influence consumers to replace or partially replace meat in their diets. Research has demonstrated that, in many countries, consumers are highly attached to meat. They consider it to be an essential and integral element of their daily diet. For the consumers that are not interested in vegan or vegetarian alternatives to meat, so-called meathybrids could be a low-threshold option for a more sustainable food consumption behaviour. In meathybrids, only a fraction of the meat product (e.g., 20% to 50%) is replaced with plant-based proteins. In this paper, the results of an online survey with 500 German consumers are presented with a focus on preferences and attitudes relating to meathyrids. The results show that more than fifty percent of consumers substitute meat at least occasionally. Thus, approximately half of the respondents reveal an eligible consumption behaviour with respect to sustainability and healthiness to a certain degree. Regarding the determinants of choosing either meathybrid or meat, it becomes evident that the highest effect is exerted by the health perception. The healthier meathybrids are perceived, the higher is the choice probability. Thus, this egoistic motive seems to outperform altruistic motives, like animal welfare or environmental concerns, when it comes to choice for this new product category.
Duckweed is a promising resource for future feed and food production as well as wastewater treatment. However, diseases and pests can critically limit the performance of the production systems. Patches of discolored and bleached duckweed (Lemna minor L.) appeared in hydroponic systems and spread rapidly through the crop. Pythium myriotylum was confirmed as the causing pathogen by microbiological and molecular biological analysis. This is the first report of P. myriotylum on duckweed in Germany. The result and possible countermeasures are discussed.
In order to produce protein-rich duckweed for human and animal consumption, a stable cultivation process, including an optimal nutrient supply for each species, must be implemented. Modified nutrient media, based on the N-medium for duckweed cultivation, were tested on the relative growth rate (RGR) and crude protein content (CPC) of Lemna minor and Wolffiella hyalina, as well as the decrease of nitrate-N and ammonium-N in the media. Five different nitrate-N to ammonium-N molar ratios were diluted to 10% and 50% of the original N-medium concentration. The media mainly consisted of agricultural fertilizers. A ratio of 75% nitrate-N and 25% ammonium-N, with a dilution of 50%, yielded the best results for both species. Based on the dry weight (DW), L. minor achieved a RGR of 0.23 ± 0.009 d−1 and a CPC of 37.8 ± 0.42%, while W. hyalina’s maximum RGR was 0.22 ± 0.017 d−1, with a CPC of 43.9 ± 0.34%. The relative protein yield per week and m2 was highest at this ratio and dilution, as well as the ammonium-N decrease in the corresponding medium. These results could be implemented in duckweed research and applications if a high protein content or protein yield is the aim.
Die Analyse der kommunalen Planungspraxis zur Auswahl und Bewertung von Siedlungsflächen in der Flächennutzungsplanung zeigt auf, welchen Einfluss Forschungsergebnisse auf die aktuelle Planungspraxis haben. Aufbauend auf einer Analyse des Forschungsstandes und der rechtlichen Rahmenbedingungen wird die Bewertung von Siedlungsflächen in 29 verschiedenen Flächennutzungsplänen untersucht. Als Ergebnis wurde ermittelt, dass in der Planungspraxis die Anzahl der Bewertungskriterien und die Komplexität der Bewertung in den letzten Jahren stark zugenommen hat. Außerdem kann gezeigt werden, dass und wie stark sich die Planungsmethodik und die verwendeten Kriterien zur Auswahl und Bewertung von Siedlungsflächen zwischen den einzelnen Gemeinden unterscheiden. Trotz umfangreicher anwendungsorientierter Forschung seit den 1990er Jahren zur Bewertung und Auswahl von Siedlungsflächen lassen sich jedoch nur indirekte Einflüsse der Forschungsergebnisse auf die Planungspraxis identifizieren. Teilweise wurde allerdings festgestellt, dass auch von den Gemeinden selbst neue Kriterien und Vorgehensweisen entwickelt werden.
Guided by cultural labor economics, the paper analyzes the career paths of former actors from popular television soap operas, and addresses in particular, if and under which conditions such serial engagements may function as a stepping stone for a subsequent professional acting career. A novel database of 396 German artists with detailed and long-term biographical information is used for the quantitative empirical analyses. The results indicate that soaps, contrary to popular opinion, function as a stepping stone, especially for younger actors. However, soap engagements should be rather short but long enough to allow artists to play multiple roles in other shows or films besides being in the cast of a soap. While formal acting education does not influence soap actors’ future filmographies, there is evidence that it helps artists to find jobs in arts-related occupations such as voice acting. Finally, publicity and media presence foster a later acting career. Practical implications for artists and their managers are outlined, along with a discussion on the meaning of serials for the creation and commercialization of stars.
Purpose
This paper aims to investigate the strategic consequences of manufacturing location decisions, with a focus on understanding the link between collocating manufacturing with other value chain activities, via reshoring or retaining and organizational agility.
Design/methodology/approach
The paper uses qualitative data from 115 interviews with executives from UK high value manufacturing companies to explore the recent phenomenon of reshoring and the strategic effects of manufacturing location.
Findings
The location of manufacturing is operationally and strategically important for multinational companies. The spatial dispersion of manufacturing is determined by firm-specific and external factors, both of which are subject to constant change. The analysis shows that concentrating on manufacturing in their home countries enables firms to increase organizational agility and stimulate innovation. Better integration with and more extensive collaboration between related value chain activities, such as research and development, sales and marketing, leads to higher flexibility, speed and responsiveness to customer requirements. However, under certain conditions, firms also continue to benefit from the known advantages of offshoring.
Originality/value
This research sheds light on possible strategic downsides of global value chains, characterized by dispersed activities and intermitted processes. The results provide evidence that retaining manufacturing or bringing back manufacturing operations to a company’s home country can increase organizational flexibility, speed, adaptability, innovativeness and responsiveness to customer requirements. As these capabilities are critical for long-term survival, especially in dynamic environments, firms need to review their global factory configurations and determine whether the short-term advantages of foreign locations continue to justify offshoring practices.
The ability of film stars to attract viewers is a widely accepted rationale for hiring expensive actors. However, the precise mechanisms behind this ‘drawing power’ remain unclear. By reviewing the existing literature, six possible antecedents of drawing power are identified. Artists with high acting abilities (i) increase a film’s artistic appeal, (ii) function as markers of quality and (iii) facilitate the matching process with the right audiences. Widely popular, commonly perceived as beautiful/handsome but often less talented actors, on the other hand, rather (iv) facilitate the marketing campaigns of films, (v) offer entertaining interactions with famous personalities and (vi) provide manifold opportunities for gossiping. Based on a discussion of the implications for theory and a preliminary analysis of a sample of Hollywood actors, managerial recommendations for those who have and those who depend on star drawing power are developed.
"The limits of my language are the limits of my mind. All I know is what I have words for" (Wittgenstein). When learning something completely new, we connect the unknown term to an already existing part of our knowledge. We can only build new ideas and insights upon an existing conceptual foundation. In the field of statistics, we educators frequently find ourselves met with great confusion when teaching novices. These students, entirely unfamiliar with even basic statistics, must connect the introduced statistical terms within their personal existing networks of largely non-statistical knowledge. Lecturers, on the other hand, who are well versed in statistics, have deeply internalized the content to be taught and its relevant context. The juxtaposition of the two roles may produce amusement in a lecturer upon gaining insight into the word associations made by the statistical novices. For example, a ‘logistic regression’ does not involve the ‘shipping of goods in economically difficult times,’ though this might seem entirely reasonable and intuitive to the statistics learner. Other times, these different perspectives can lead to headaches and frustration for both learners and their lecturers. In this article, we illustrate how simple statistical terms are initially connected to a student’s pre-exiting knowledge and how these associations change after completing an introductory course in applied statistics. Furthermore, we emphasize the important difference between “term”, “approach”, and “context”. Understanding this fundamental distinction may help improve the communication between the lecturer and the learner. We offer a collection of practical tools for instructors to help promote students’ conceptual understanding in a supportive, mutually-beneficial learning environment.
Die Landwirtschaft ist ein Wirtschaftszweig mit massiven Auswirkungen auf die biologische und agrobiodiverse Vielfalt. Nachhaltige Ernährung ist ein entscheidender politischer Hebel und eine realistische Chance, die Umweltauswirkungen des Agrar- und Ernährungssektors zu verringern und gleichzeitig die menschliche Gesundheit zu verbessern. Auswärts essen ist für viele Verbraucher eine immer häufiger anzutreffende Gewohnheit, und indem sie nachhaltige Gerichte anbieten, können Catering-Unternehmen eine zentrale Rolle spielen. Dazu müssen sie die Nachhaltigkeit ihres Lebensmittelangebots verstehen und richtig einschätzen können, aber es gibt noch keine gut etablierten Bewertungsinstrumente. Im Rahmen des NAHGAST-Projekts, zu dem diese Studie gehörte, wurde ein Instrument zur Bewertung der Nachhaltigkeit für Catering-Unternehmen entwickelt und getestet, das auf konkreten, pro Mahlzeit definierten Zielen basiert. Diese Studie befasst sich mit dem Mangel an Methoden zur Bewertung der Auswirkungen von Lebensmitteln auf die biologische Vielfalt, wobei der Schwerpunkt auf der Agrobiodiversität liegt. Die Arbeit veranschaulicht eine kontextspezifische Anwendung eines erweiterten DPSIR-Modells zur Strukturierung von Informationen und Auswahl von Indikatoren und schlägt eine transdisziplinäre Nutzung bestehender Messgrößen vor. Weitere Forschungsarbeiten sind erforderlich, um wissenschaftlich fundierte Zielwerte oder Nachhaltigkeitsspannen für jeden Indikator pro Mahlzeit zu definieren, damit diese berechnet werden können.

Quantitative Umweltbewertungen sind von entscheidender Bedeutung, wenn es darum geht, effektiv auf nachhaltige Produktions- und Verbrauchsmuster hinzuarbeiten. In den letzten Jahrzehnten haben sich Lebenszyklusanalysen (LCA) als praktikables Mittel zur Messung der Umweltauswirkungen von Produkten entlang der Lieferkette etabliert. In Bezug auf Nutzer- und Konsummuster wurden jedoch methodische Schwächen festgestellt, und es wurden mehrere Versuche unternommen, Ökobilanzen entsprechend zu verbessern, beispielsweise durch Einbeziehung von Effekten höherer Ordnung und verhaltenswissenschaftlicher Unterstützung. In einer Diskussion solcher Ansätze zeigen wir, dass den Konzepten des Verbrauchs keine explizite Aufmerksamkeit geschenkt wurde, was häufig zu produktzentrierten Bewertungen führt. Wir führen Theorien sozialer Praktiken ein, um Konsummuster für LCA zugänglich zu machen. Soziale Praktiken sind routinierte Handlungen, die aus miteinander verbundenen Elementen (Materialien, Kompetenzen und Bedeutungen) bestehen, die sie als eine Einheit denkbar machen (z. B. Kochen). Da die meisten sozialen Praktiken eine Art von Verbrauch (Material, Energie, Luft) beinhalten, konnten wir einen Rahmen entwickeln, der soziale Praktiken mit der Ökobilanz verbindet. Der vorgeschlagene Rahmen bietet eine neue Perspektive für quantitative Umweltbewertungen, indem er den Schwerpunkt von Produkten oder Nutzern auf soziale Praktiken verlagert. Dementsprechend sehen wir die Chance darin, die reduktionistische Sichtweise zu überwinden, dass Menschen nur Nutzer von Produkten sind, und sie stattdessen als Praktiker in sozialen Praktiken zu sehen. Dieser Wandel könnte neue Methoden der interdisziplinären Konsumforschung ermöglichen, die beabsichtigte Sozialwissenschaften und wirkungsorientierte Bewertungen einbeziehen. Allerdings bedarf der Rahmen einer weiteren Überarbeitung und vor allem einer empirischen Validierung.
Der Konsumbereich „Ernährung“ verantwortet in Deutschland rund 15 % der Treibhausgasemissionen (THG). Aufgrund der hohen Nachfrage hat die Außer-Haus-Verpflegung eine starke Hebelwirkung, um eine klimaeffiziente Ernährung voranzutreiben – so auch Schulküchen. Aktuell sind der Verpflegung jedes Schulkindes, welches ganzjährig an der Mittagsverpflegung teilnimmt, pro Jahr etwa 264 kg THG-Emissionen zuzuschreiben.
Im Projekt KEEKS „Klima- und energieeffiziente Küche in Schulen” wurden deshalb in 22 Schulküchen von Ganztagsschulen mit täglich insgesamt 5 000 Mittagessen der Status Quo ermittelt. Dies geschah mittels Energiemessungen, Analyse von Ausstattung, Technik und Prozessen der Küchen sowie leitfadengestützten Interviews mit den Küchenleitungen. Treibhausgasemissionen von Menüs und Küchenprozessen wurden bilanziert, Einsparpotenziale identifiziert sowie Handlungsempfehlungen entwickelt und erprobt. Die effektivsten Maßnahmen – die Reduktion und Substitution von Fleisch und Fleischprodukten sowie der Aufbau eines effizienten Abfallmanagements – sparen rund 10 % des Treibhausgasausstoßes einer Schulküche ein. Die erarbeiteten Handlungsempfehlungen können Küchenpersonal dabei unterstützen, eine klimafreundliche, kindgerechte, gesunde und bezahlbare Schulküche zu gestalten.
Red pepper (Capsicum annuum L.) is one of the major spices consumed globally, recognized for its aroma and nutrient properties, and it has a major economic value for high producing countries. However, characterization of its techno-functional properties and in-depth understanding of oxidative stability is needed to produce food of high quality and stability. Thus, this work focused on the chemical, functional, thermal, oxidative stability and rheological properties of red pepper powder and paste. Experiment was designed in a Completely Randomized Design (CRD) fashion. The red pepper powder contained 14.50 g/100 g, 44.00 g/100 g and 7.57 g/100 g of crude fat, crude fiber and ash, respectively. The concentration of total phenols, carotenoids and antioxidants activity of the powder were 1.04 g GAE/100 g, 374 mg βc/100 g and 38.61 μmol TE/g, respectively. Functional properties showed lower bulk density (395.1 kg/m3) and higher tapped density (583.4 kg/m3) indicating the higher compressibility of the powder. In contrast, Hausner ratio (1.48), Carr’s index (32%) and angle of repose (45°) indicated poor flowability of the powder. Particle size distribution also indicated that the volume weighted mean values D[4,3] of the powder and paste were 262.20 and 201.46, respectively. Emulsifying capacity of the powder was 47.5%. Oil and water absorption capacities varied from 1.41 to 1.73 and 0.86 to 2.29 g/g of initial weight, respectively. Higher glass transition temperature was observed for the powder (62.54°C) than the paste (45.64°C). The induction period indicated that red pepper was more stable against oxidation in powder (5.2 h) than in the paste form (3.2 h). Rheological analysis revealed that the paste exhibited shear-thinning behavior. Overall, understanding of the properties of red pepper could contribute to enhance quality.
The increased consumption of reduced-fat or non-fat products leads to a reduced intake of fat-soluble bioactive substances, such as fat-soluble vitamins. Due to their natural role as transport systems for hydrophobic substances, casein micelles (CM) might depict a viable system. The structure of CM is characterized by a lipophilic core stabilized by an electric double layer-like structure. Modification allows accessibility of the core and, therefore, the inclusion of fat-soluble bioactive substances. Well-known modifications are pH reduction and use of rennet enzyme. A completely new procedure to modify CM structure is offered by pulsed electrical fields (PEF). The principle behind PEF is called electroporation and affects the electric double layer of CM so that it is interrupted. In this way, lipophilic substances can be incorporated into CM. In this work, we evaluated integration of β-carotene into native CM by an industry-compatible process to overcome disadvantages associated with the use of Na-caseinate and avoid great technical effort, e.g., due to treatment with high hydrostatic pressure. Our research has shown that PEF can be used for disintegration of CM and that significant amounts of β-carotene can be incorporated in CM. Furthermore, after disintegration using PEF, a combination of another PEF and thermal treatment was applied to restructure CM and trap significant amounts of β-carotene, permanently, ending up with an encapsulation efficiency of 78%.
The influence of oil content and droplet size of oil-in-water emulsions on the heat development in an ohmic heating system was investigated. The setup was run with constant power or voltage. Emulsions consisted of sunflower oil (10–50 wt%), aqua dest. (90–50 wt%) and whey protein isolate (1.25/ 2.5/ 3.75/ 5.0 and 6.25 wt%) Two different droplet size distributions were produced, large (d0.5 ≈ 2.0 μm) and small (d0.5 ≈ 0.3 μm), for each oil mass fraction. The emulsions were ohmically heated from 10 to 80 °C at a constant power of 3.0 kW and constant voltage of 15 V/cm. The electrical conductivity decreased with an increasing oil content, resulting in longer or shorter heating time for constant voltage or constant power input, respectively. The droplet size only affected the heating process at the highest oil content.
Industrial relevance
Emulsions occur in a wide range of food products (e.g. sauces, dressings, desserts) and have properties giving structure to the food system. Ohmic heating is an emerging thermal process with improved (e.g. faster or less energy required) heating characteristics. The influence of physical changes due to different droplet sizes are of interest because these might also affect the heating characteristic. In addition, the direct comparison of two different process regulations (constant power and constant voltage) indicate which set up is expedient to a successful heating process. This study aims to identify the influence of emulsion-induced structural changes and process changes on the heating rates, which is of interest for the food industry and the related machine building industry.
This study reported the impact of electron beam (e-beam) treatment on microbiota and mycotoxins naturally present in red pepper powder and physicochemical quality changes. Treatment at 6 kGy indicated significant (p < 0.05) decontamination of yeasts and molds by 3.0 and 4.4 log CFU/g, respectively. A reduction of 4.5 log CFU/g of total plate counts (TPC) was observed at 10 kGy for 23 s. Fungal inactivation followed first-order kinetics while TPC better fitted with Gompertz function (R2 = 0.9912). E-beam treatment was not efficient for the degradation of aflatoxins but indirectly controlled their production by inactivation of mycotoxigenic molds. Indeed, reduction of 25% ochratoxin A was recorded at 30 kGy retaining >85% of total phenols, carotenoids and antioxidants activity. Moreover, treatment impact on total color difference (ΔE*) indicated ‘slight differences’. Overall, e-beam treatments up to 10 kGy were efficient in decontaminating the natural microbiota without detrimental effects on the physicochemical qualities of red pepper powder.
The study aimed for the analysis of the impact of pulsed electric field (PEF) pre-treatment on convection (CD) and microwave (MW-CD) assisted air drying. Drying kinetics acceleration and retention of bioactive compounds of PEF pre-treated carrots and apples has been demonstrated. Moreover, the direct and indirect environmental energy impacts of CD and MW-CD technologies with consideration of bioactive compounds preservation has been evaluated. PEF assisted CD and MW-CD demonstrated lower energy use, especially for indirect energy consumption, in the case of carotenoids preservation in dried carrots.
Background
In mucosal barrier interfaces, flexible responses of gene expression to long-term environmental changes allow adaptation and fine-tuning for the balance of host defense and uncontrolled not-resolving inflammation. Epigenetic modifications of the chromatin confer plasticity to the genetic information and give insight into how tissues use the genetic information to adapt to environmental factors. The oral mucosa is particularly exposed to environmental stressors such as a variable microbiota. Likewise, persistent oral inflammation is the most important intrinsic risk factor for the oral inflammatory disease periodontitis and has strong potential to alter DNA-methylation patterns. The aim of the current study was to identify epigenetic changes of the oral masticatory mucosa in response to long-term inflammation that resulted in periodontitis.
Methods and results
Genome-wide CpG methylation of both inflamed and clinically uninflamed solid gingival tissue biopsies of 60 periodontitis cases was analyzed using the Infinium MethylationEPIC BeadChip. We validated and performed cell-type deconvolution for infiltrated immune cells using the EpiDish algorithm. Effect sizes of DMPs in gingival epithelial and fibroblast cells were estimated and adjusted for confounding factors using our recently developed “intercept-method”. In the current EWAS, we identified various genes that showed significantly different methylation between periodontitis-inflamed and uninflamed oral mucosa in periodontitis patients. The strongest differences were observed for genes with roles in wound healing (ROBO2, PTP4A3), cell adhesion (LPXN) and innate immune response (CCL26, DNAJC1, BPI). Enrichment analyses implied a role of epigenetic changes for vesicle trafficking gene sets.
Conclusions
Our results imply specific adaptations of the oral mucosa to a persistent inflammatory environment that involve wound repair, barrier integrity, and innate immune defense.
Background
In DNA methylation analyses like epigenome-wide association studies, effects in differentially methylated CpG sites are assessed. Two kinds of outcomes can be used for statistical analysis: Beta-values and M-values. M-values follow a normal distribution and help to detect differentially methylated CpG sites. As biological effect measures, differences of M-values are more or less meaningless. Beta-values are of more interest since they can be interpreted directly as differences in percentage of DNA methylation at a given CpG site, but they have poor statistical properties. Different frameworks are proposed for reporting estimands in DNA methylation analysis, relying on Beta-values, M-values, or both.
Results
We present and discuss four possible approaches of achieving estimands in DNA methylation analysis. In addition, we present the usage of M-values or Beta-values in the context of bioinformatical pipelines, which often demand a predefined outcome. We show the dependencies between the differences in M-values to differences in Beta-values in two data simulations: a analysis with and without confounder effect. Without present confounder effects, M-values can be used for the statistical analysis and Beta-values statistics for the reporting. If confounder effects exist, we demonstrate the deviations and correct the effects by the intercept method. Finally, we demonstrate the theoretical problem on two large human genome-wide DNA methylation datasets to verify the results.
Conclusions
The usage of M-values in the analysis of DNA methylation data will produce effect estimates, which cannot be biologically interpreted. The parallel usage of Beta-value statistics ignores possible confounder effects and can therefore not be recommended. Hence, if the differences in Beta-values are the focus of the study, the intercept method is recommendable. Hyper- or hypomethylated CpG sites must then be carefully evaluated. If an exploratory analysis of possible CpG sites is the aim of the study, M-values can be used for inference.
Background
Postoperative delirium (POD) and postoperative cognitive dysfunction (POCD) are frequent and serious complications after surgery. We aim to investigate the association between genetic variants in cholinergic candidate genes according to the Kyoto encyclopedia of genes and genomes - pathway: cholinergic neurotransmission with the development of POD or POCD in elderly patients.
Methods
This analysis is part of the European BioCog project (www.biocog.eu), a prospective multicenter observational study with elderly surgical patients. Patients with a Mini-Mental-State-Examination score ≤ 23 points were excluded. POD was assessed up to seven days after surgery using the Nursing Delirium Screening Scale, Confusion Assessment Method and a patient chart review. POCD was assessed three months after surgery with a neuropsychological test battery. Genotyping was performed on the Illumina Infinium Global Screening Array. Associations with POD and POCD were analyzed using logistic regression analysis, adjusted for age, comorbidities and duration of anesthesia (for POCD analysis additionally for education). Odds ratios (OR) refer to minor allele counts (0, 1, 2).
Results
745 patients could be included in the POD analysis, and 452 in the POCD analysis. The rate of POD within this group was 20.8% (155 patients), and the rate of POCD was 10.2% (46 patients). In a candidate gene approach three genetic variants of the cholinergic genes CHRM2 and CHRM4 were associated with POD (OR [95% confidence interval], rs8191992: 0.61[0.46; 0.80]; rs8191992: 1.60[1.22; 2.09]; rs2067482: 1.64[1.10; 2.44]). No associations were found for POCD.
Conclusions
We found an association between genetic variants of CHRM2 and CHRM4 and POD. Further studies are needed to investigate whether disturbances in acetylcholine release and synaptic plasticity are involved in the development of POD.

Data collected in various scientific fields are count data. One way to analyze such data is to compare the individual levels of the factor treatment using multiple comparisons. However, the measured individuals are often clustered – e.g. according to litter or rearing. This must be considered when estimating the parameters by a repeated measurement model. In addition, ignoring the overdispersion to which count data is prone leads to an increase of the type one error rate. We carry out simulation studies using several different data settings and compare different multiple contrast tests with parameter estimates from generalized estimation equations and generalized linear mixed models in order to observe coverage and rejection probabilities. We generate overdispersed, clustered count data in small samples as can be observed in many biological settings. We have found that the generalized estimation equations outperform generalized linear mixed models if the variance-sandwich estimator is correctly specified. Furthermore, generalized linear mixed models show problems with the convergence rate under certain data settings, but there are model implementations with lower implications exists. Finally, we use an example of genetic data to demonstrate the application of the multiple contrast test and the problems of ignoring strong overdispersion.
BACKGROUND:
Intraoperative electroencephalography (EEG) signatures related to the development of postoperative delirium (POD) in older patients are frequently studied. However, a broad analysis of the EEG dynamics including preoperative, postinduction, intraoperative and postoperative scenarios and its correlation to POD development is still lacking. We explored the relationship between perioperative EEG spectra-derived parameters and POD development, aiming to ascertain the diagnostic utility of these parameters to detect patients developing POD.
METHODS:
Patients aged ≥65 years undergoing elective surgeries that were expected to last more than 60 minutes were included in this prospective, observational single center study (Biomarker Development for Postoperative Cognitive Impairment [BioCog] study). Frontal EEGs were recorded, starting before induction of anesthesia and lasting until recovery of consciousness. EEG data were analyzed based on raw EEG files and downloaded excel data files. We performed multitaper spectral analyses of relevant EEG epochs and further used multitaper spectral estimate to calculate a corresponding spectral parameter. POD assessments were performed twice daily up to the seventh postoperative day. Our primary aim was to analyze the relation between the perioperative spectral edge frequency (SEF) and the development of POD.
RESULTS:
Of the 237 included patients, 41 (17%) patients developed POD. The preoperative EEG in POD patients was associated with lower values in both SEF (POD 13.1 ± 4.6 Hz versus no postoperative delirium [NoPOD] 17.4 ± 6.9 Hz; P = .002) and corresponding γ-band power (POD −24.33 ± 2.8 dB versus NoPOD −17.9 ± 4.81 dB), as well as reduced postinduction absolute α-band power (POD −7.37 ± 4.52 dB versus NoPOD −5 ± 5.03 dB). The ratio of SEF from the preoperative to postinduction state (SEF ratio) was ~1 in POD patients, whereas NoPOD patients showed a SEF ratio >1, thus indicating a slowing of EEG with loss of unconscious. Preoperative SEF, preoperative γ-band power, and SEF ratio were independently associated with POD (P = .025; odds ratio [OR] = 0.892, 95% confidence interval [CI], 0.808–0.986; P = .029; OR = 0.568, 95% CI, 0.342–0.944; and P = .009; OR = 0.108, 95% CI, 0.021–0.568, respectively).
CONCLUSIONS:
Lower preoperative SEF, absence of slowing in EEG while transitioning from preoperative state to unconscious state, and lower EEG power in relevant frequency bands in both these states are related to POD development. These findings may suggest an underlying pathophysiology and might be used as EEG-based marker for early identification of patients at risk to develop POD.
Objective:
The cervical mucus plugs are enriched with proteins of known immunological functions. We aimed to characterize the anti-HIV-1 activity of the cervical mucus plugs against a panel of different HIV-1 strains in the contexts of cell-free and cell-associated virus.
Design:
A cohort of consenting HIV-1-negative and HIV-1-positive pregnant women in labour was recruited from Mthatha General Hospital in the Eastern Cape province of South Africa, from whom the cervical mucus plugs were collected in 6 M guanidinium chloride with protease inhibitors and transported to our laboratories at −80 °C.
Methods:
Samples were centrifuged to remove insoluble material and dialysed before freeze--drying and subjecting them to the cell viability assays. The antiviral activities of the samples were studied using luminometric reporter assays and flow cytometry. Time-of-addition and BlaM-Vpr virus-cell fusion assays were used to pin-point the antiviral mechanisms of the cervical mucus plugs, before proteomic profiling using liquid chromatography-tandem mass spectrometry.
Results:
The proteinaceous fraction of the cervical mucus plugs exhibited anti-HIV-1 activity with inter-individual variations and some degree of specificity among different HIV-1 strains. Cell-associated HIV-1 was less susceptible to inhibition by the potent samples whenever compared with the cell-free HIV-1. The samples with high antiviral potency exhibited a distinct proteomic profile when compared with the less potent samples.
Conclusion:
The crude cervical mucus plugs exhibit anti-HIV-1 activity, which is defined by a specific proteomic profile.
Background:
Cardiac surgery patients represent a high-risk cohort in intensive care units (ICUs). Central venous pressure (CVP) measurement seems to remain an integral part in hemodynamic monitoring, especially in cardio-surgical ICUs. However, its value as a prognostic marker for organ failure is still unclear. Therefore, we analyzed postoperative CVP values after adult cardiac surgery in a large cohort with regard to its prognostic value for morbidity and mortality.
Methods:
All adult patients admitted to our ICUs between 2006 and 2019 after cardiac surgery were eligible for inclusion in the study (n = 11,198). We calculated the median initial CVP (miCVP) after admission to the ICU, which returned valid values for 9802 patients. An ROC curve analysis for optimal cut-off miCVP to predict ICU mortality was conducted with consecutive patient allocation into a (a) low miCVP (LCVP) group (≤11 mmHg) and (b) high miCVP (HCVP) group (>11 mmHg). We analyzed the impact of high miCVP on morbidity and mortality by propensity score matching (PSM) and logistic regression.
Results:
ICU mortality was increased in HCVP patients. In addition, patients in the HCVP group required longer mechanical ventilation, had a higher incidence of acute kidney injury, were more frequently treated with renal replacement therapy, and showed a higher risk for postoperative liver dysfunction, parametrized by a postoperative rise of ≥ 10 in MELD Score. Multiple regression analysis confirmed HCVP has an effect on postoperative ICU-mortality and intrahospital mortality, which seems to be independent.
Conclusions:
A high initial CVP in the early postoperative ICU course after cardiac surgery is associated with worse patient outcome. Whether or not CVP, as a readily and constantly available hemodynamic parameter, should promote clinical efforts regarding diagnostics and/or treatment, warrants further investigations.
Preoperative medication use and development of postoperative delirium and cognitive dysfunction

Postoperative delirium (POD) and postoperative (neuro-)cognitive disorder (POCD) are frequent and serious complications after operations. We aim to investigate the association between pre-operative polypharmacy and potentially inappropriate medications and the development of POD/POCD in elderly patients. This investigation is part of the European BioCog project (www.biocog.eu), a prospective multicenter observational study with elderly surgical patients. Patients with a Mini-Mental State Examination score less than or equal to 23 points were excluded. POD was assessed up to 7 days after surgery using the Nursing Delirium Screening Scale, Confusion Assessment Method (for the intensive care unit [ICU]), and a patient chart review. POCD was assessed 3 months after surgery with a neuropsychological test battery. Pre-operative long-term medication was evaluated in terms of polypharmacy (≥5 agents) and potentially inappropriate medication (defined by the PRISCUS and European list of potentially inappropriate medications [EU(7)-PIM] lists), and associations with POD and POCD were analyzed using logistic regression analysis. Eight hundred thirty-seven participants were included for analysis of POD and 562 participants for POCD. Of these, 165 patients (19.7%) fulfilled the criteria of POD and 60 (10.7%) for POCD. After adjusting for confounders, pre-operative polypharmacy and intake of potentially inappropriate medications could not be shown to be associated with the development of POD nor POCD. We found no associations between pre-operative polypharmacy and potentially inappropriate medications and development of POD and POCD. Future studies should focus on the evaluation of drug interactions to determine whether patients benefit from a pre-operative adjustment.
Background:
Etomidate is typically used as an induction agent in cardiac surgery because it has little impact on hemodynamics. It is a known suppressor of adrenocortical function and may increase the risk for post-operative infections, sepsis, and mortality. The aim of this study was to evaluate whether etomidate increases the risk of postoperative sepsis (primary outcome) and infections (secondary outcome) compared to propofol.
Methods:
This was a retrospective before–after trial (IRB EA1/143/20) performed at a tertiary medical center in Berlin, Germany, between 10/2012 and 01/2015. Patients undergoing cardiac surgery were investigated within two observation intervals, during which etomidate and propofol were the sole induction agents.
Results:
One-thousand, four-hundred, and sixty-two patients, and 622 matched pairs, after caliper propensity-score matching, were included in the final analysis. Sepsis rates did not differ in the matched cohort (etomidate: 11.5% vs. propofol: 8.2%, p = 0.052). Patients in the etomidate interval were more likely to develop hospital-acquired pneumonia (etomidate: 18.6% vs. propofol: 14.0%, p = 0.031).
Conclusion:
Our study showed that a single-dose of etomidate is not statistically associated with higher postoperative sepsis rates after cardiac surgery, but is associated with a higher incidence of hospital-acquired pneumonia. However, there is a notable trend towards a higher sepsis rate.
The benzodiazepine, midazolam, is one of the most frequently used sedatives in intensive care medicine, but it has an unfavorable pharmacokinetic profile when continuously applied. As a consequence, patients are frequently prolonged and more deeply sedated than intended. Due to its distinct pharmacological features, including a cytochrome P450-independent metabolization, intravenous lormetazepam might be clinically advantageous compared to midazolam. In this retrospective cohort study, we compared patients who received either intravenous lormetazepam or midazolam with respect to their survival and sedation characteristics. The cohort included 3314 mechanically ventilated, critically ill patients that received one of the two drugs in a tertiary medical center in Germany between 2006 and 2018. A Cox proportional hazards model with mortality as outcome and APACHE II, age, gender, and admission mode as covariates revealed a hazard ratio of 1.75 [95% CI 1.46–2.09; p < 0.001] for in-hospital mortality associated with the use of midazolam. After additionally adjusting for sedation intensity, the HR became 1.04 [95% CI 0.83–1.31; p = 0.97]. Thus, we concluded that excessive sedation occurs more frequently in critically ill patients treated with midazolam than in patients treated with lormetazepam. These findings require further investigation in prospective trials to assess if lormetazepam, due to its ability to maintain light sedation, might be favorable over other benzodiazepines for sedation in the ICU.
Many people across the world suffer from iodine (I) deficiency and related diseases. The I content in plant-based foods is particularly low, but can be enhanced by agronomic biofortification. Therefore, in this study two field experiments were conducted under orchard conditions to assess the potential of I biofortification of apples and pears by foliar fertilization. Fruit trees were sprayed at various times during the growing season with solutions containing I in different concentrations and forms. In addition, tests were carried out to establish whether the effect of I sprays can be improved by co-application of potassium nitrate (KNO3) and sodium selenate (Na2SeO4). Iodine accumulation in apple and pear fruits was dose-dependent, with a stronger response to potassium iodide (KI) than potassium iodate (KIO3). In freshly harvested apple and pear fruits, 51% and 75% of the biofortified iodine was localized in the fruit peel, respectively. The remaining I was translocated into the fruit flesh, with a maximum of 3% reaching the core. Washing apples and pears with running deionized water reduced their I content by 14%. To achieve the targeted accumulation level of 50–100 μg I per 100 g fresh mass in washed and unpeeled fruits, foliar fertilization of 1.5 kg I per hectare and meter canopy height was required when KIO3 was applied. The addition of KNO3 and Na2SeO4 to I-containing spray solutions did not affect the I content in fruits. However, the application of KNO3 increased the total soluble solids content of the fruits by up to 1.0 °Brix compared to the control, and Na2SeO4 in the spray solution increased the fruit selenium (Se) content. Iodine sprays caused leaf necrosis, but without affecting the development and marketing quality of the fruits. Even after three months of cold storage, no adverse effects of I fertilization on general fruit characteristics were observed, however, I content of apples decreased by 20%.

Mindestens 3,5 Mio. von ihnen entwickeln die teilweise erheblichen allergischen Symptome als Ausdruck eines Oralen Allergie-Syndroms. Es gibt bisher keine medikamentöse Therapie gegen diese Allergie.
Apfelallergiker können daher nur auf Äpfel ganz verzichten, oder vorher erhitzte Äpfel essen oder Sorten suchen, die wenig Allergene enthalten und deshalb als allergikerfreundliche Apfelsorten bezeichnet werden können.
Alleinige Bestimmungen von Allergenen im Labor können nicht voraussagen, ob ein Apfel ohne allergische Symptome von Apfelallergikern gegessen werden kann; dazu bedarf es klinischer Prüfungen.

Background: In longitudinal studies, observations are made over time. Hence, the single observations at each time point are dependent, making them a repeated measurement. In this work, we explore a different, counterintuitive setting: At each developmental time point, a lethal observation is performed on the pregnant or nursing mother. Therefore, the single time points are independent. Furthermore, the observation in the offspring at each time point is correlated with each other because each litter consists of several (genetically linked) littermates. In addition, the observed time series is short from a statistical perspective as animal ethics prevent killing more mother mice than absolutely necessary, and murine development is short anyway. We solve these challenges by using multiple contrast tests and visualizing the change point by the use of confidence intervals.
Results: We used linear mixed models to model the variability of the mother. The estimates from the linear mixed model are then used in multiple contrast tests.There are a variety of contrasts and intuitively, we would use the Changepoint method. However, it does not deliver satisfying results. Interestingly, we found two other contrasts, both capable of answering different research questions in change point detection: i) Should a single point with change direction be found, or ii) Should the overall progression be determined? The Sequen contrast answers the first, the McDermott the second. Confidence intervals deliver effect estimates for the strength of the potential change point. Therefore, the scientist can define a biologically relevant limit of change depending on the research question.
Conclusion: We present a solution with effect estimates for short independent time series with observations nested at a given time point. Multiple contrast tests produce confidence intervals, which allow determining the position of change points or to visualize the expression course over time. We suggest to use McDermott’s method to determine if there is an overall significant change within the time frame, while Sequen is better in determining specific change points. In addition, we offer a short formula for the estimation of the maximal length of the time series.
Background
There is no consensus on the instruments for diagnosis of post-intensive care syndrome (PICS). We present a proposal for a set of outcome measurement instruments of PICS in outpatient care.
Methods
We conducted a three-round, semi-structured consensus-seeking process with medical experts, followed each by exploratory feasibility investigations with intensive care unit survivors (n1 = 5; n2 = 5; n3 = 7). Fourteen participants from nine stakeholder groups participated in the first and second consensus meeting. In the third consensus meeting, a core group of six clinical researchers refined the final outcome measurement instrument set proposal.
Results
We suggest an outcome measurement instrument set used in a two-step process. First step: Screening with brief tests covering PICS domains of (1) mental health (Patient Health Questionnaire-4 (PHQ-4)), (2) cognition (MiniCog, Animal Naming), (3) physical function (Timed Up-and-Go (TUG), handgrip strength), and (4) health-related quality of life (HRQoL) (EQ-5D-5L). Single items measure subjective health before and after the intensive care unit stay. If patients report new or worsened health problems after intensive care unit discharge and show relevant impairment in at least one of the screening tests, a second extended assessment follows: (1) Mental health (Patient Health Questionnaire-8 (PHQ-8), Generalized Anxiety Disorder Scale-7 (GAD-7), Impact of Event Scale – revised (IES-R)); (2) cognition (Repeatable Battery for the Assessment of Neuropsychological Status (RBANS), Trail Making Test (TMT) A and B); (3) physical function (2-Minute Walk Test (2-MWT), handgrip strength, Short Physical Performance Battery (SPPB)); and (4) HRQoL (EQ-5D-5L, 12-Item WHO Disability Assessment Schedule (WHODAS 2.0)).
Conclusions
We propose an outcome measurement instrument set used in a two-step measurement of PICS, combining performance-based and patient-reported outcome measures. First-step screening is brief, free-of-charge, and easily applicable by health care professionals across different sectors. If indicated, specialized healthcare providers can perform the extended, second-step assessment. Usage of the first-step screening of our suggested outcome measurement instrument set in outpatient clinics with subsequent transfer to specialists is recommended for all intensive care unit survivors. This may increase awareness and reduce the burden of PICS.
Background
A pre-existing neurocognitive disorder (NCD) is a relevant factor for the outcome of surgical patients. To improve understanding of these conditions, we investigated the association between parameters of the cholinergic system and NCD.
Method
This investigation is part of the BioCog project (www.biocog.eu), which is a prospective multicenter observational study including patients aged 65 years and older scheduled for elective surgery. Patients with a Mini-Mental State Examination (MMSE) score ≤23 points were excluded. Neurocognitive disorder was assessed according to the fifth Diagnostic and Statistical Manual of Mental Disorders criteria. The basal forebrain cholinergic system volume (BFCSV) was assessed with magnetic resonance imaging, the peripheral cholinesterase (ChE) activities with point-of-care measurements, and anticholinergic load by analyzing the long-term medication with anticholinergic scales (Anticholinergic Drug Scale [ADS], Anticholinergic Risk Scale [ARS], Anticholinergic Cognitive Burden Scale [ACBS]). The associations of BFCSV, ChE activities, and anticholinergic scales with NCD were studied with logistic regression analysis, adjusting for confounding factors.
Results
A total of 797 participants (mean age 72 years, 42% females) were included. One hundred and eleven patients (13.9%) fulfilled criteria for mild NCD and 82 patients (10.3%) for major NCD criteria. We found that AcetylChE activity was associated with major NCD (odds ratio [95% confidence interval]: [U/gHB] 1.061 [1.010, 1.115]), as well as ADS score ([points] 1.353 [1.063, 1.723]) or ARS score, respectively ([points] 1.623 [1.100, 2.397]) with major NCD. However, we found no association between BFCSV or ButyrylChE activity with mild or major NCD.
Conclusions
AcetylChE activity and anticholinergic load were associated with major NCD. Future research should focus on the association of the cholinergic system and the development of postoperative delirium and postoperative NCD.
Acute post-operative delirium (POD) and long-term post-operative cognitive dysfunction (POCD) are frequent and associated with increased mortality, dependency on care giving and institutionalization rates. The POCD-related cost burden on the German long-term care insurance provides an indication for the savings potential from risk-adapted treatment schemes. Comprehensive estimates have not been assessed or published so far.
A model-based cost-analysis was designed to estimate POCD-related costs in the long-term care insurance. Comprehensive analysis of inpatient operations and procedures (OPS-codes) served as the base for case number calculations, which were then used as input to the actual cost model. POCD-incidence rates were obtained from the BioCog study. Various sensitivity analyses were performed to assess uncertainty of the model results.
Total POCD related annual costs in the German long-term care insurance account for approximately 1.6 billion EUR according to the base case of our analysis. Total annual costs for all POCD cases depend on surgery numbers, incidence rates, other assumptions, and uncertain input parameters.
The financial burden to the long-term care insurance is substantial, even in a conservative scenario of the cost model. Variability of results stems from uncertain assumptions, POCD-incidence rates and from uncertain patient numbers who are undergoing surgery and are therefore at risk to develop POCD.
The effect of pulsed electric field (PEF) and ultrasound (US) on the frying behavior of potato chips was investigated. For this purpose, a special fryer with a window was designed to enable the investigation of water evaporation by the characterization of bubble formation during frying. The number of water vapor bubbles and the bubble volume distribution were analyzed in order to gain an insight into heat and mass transfer affected by PEF and US treatment. Quality parameters of the potato chips such as moisture, fat and acrylamide content were measured. Overall, the results of this study show for the first time impacting effects on the frying process that can be achieved by combining PEF as a volumetric cell disintegration technology and ultrasound as a mean to affect interface phenomena. The obtained results can be used to further optimize frying processes used for the production of chips and other products.
Background and Objectives
Despite the long-standing consensus on the importance of tumor size, tumor number and carcinoembryonic antigen (CEA) levels as predictors of long-term outcomes among patients with colorectal liver metastases (CRLM), optimal prognostic cut-offs for these variables have not been established.
Methods
Patients who underwent curative-intent resection of CRLM and had available data on at least one of the three variables of interest above were selected from a multi-institutional dataset of patients with known KRAS mutational status. The resulting cohort was randomly split into training and testing datasets and recursive partitioning analysis was employed to determine optimal cut-offs. The concordance probability estimates (CPEs) for these optimal cut offs were calculated and compared to CPEs for the most widely used cut-offs in the surgical literature.
Results
A total of 1643 patients who met eligibility criteria were identified. Following recursive partitioning analysis in the training dataset, the following cut-offs were identified: 2.95 cm for tumor size, 1.5 for tumor number and 6.15 ng/ml for CEA levels. In the entire dataset, the calculated CPEs for the new tumor size (0.52), tumor number (0.56) and CEA (0.53) cut offs exceeded CPEs for other commonly employed cut-offs.
Conclusion
The current study was able to identify optimal cut-offs for the three most commonly employed prognostic factors in CRLM. While the per variable gains in discriminatory power are modest, these novel cut-offs may help produce appreciable increases in prognostic performance when combined in the context of future risk scores.
Background
Postoperative delirium (POD) is a frequent and serious complication after surgery. Evidence of a relationship between anticholinergic medication and the development of delirium is inconclusive, but studies on POD are rare.
Objectives
The objective of this study was to evaluate the anticholinergic load of preoperative medication in older adult patients and its association with the development of POD.
Methods
This investigation was part of the European BioCog project (http://www.biocog.eu), a prospective multicenter observational study in older adult surgical patients (ClinicalTrials.gov identifier: NCT02265263, 15 October 2014). Patients with a Mini–Mental State Examination score ≤ 23 points were excluded. POD was assessed up to 7 days after surgery using the Nursing Delirium Screening Scale, Confusion Assessment Method and a patient chart review. The preoperative anticholinergic load was calculated using the Anticholinergic Drug Scale (ADS), the Anticholinergic Risk Scale (ARS) and the Anticholinergic Cognitive Burden Scale (ACBS), and associations with POD were analyzed using logistic regression analysis adjusting for age, comorbidities, duration of anesthesia and number of drugs used.
Results
In total, 837 participants were included for analysis, and 165 patients (19.7%) fulfilled the criteria of POD. After adjusting for confounders, we found no association between preoperative anticholinergic load and the development of POD (ADS [points] odds ratio [OR] 0.928; 95% confidence interval [CI] 0.749–1.150; ARS [points] OR 0.832; 95% CI 0.564–1.227; ACBS [points] OR 1.045; 95% CI 0.842–1.296).
Conclusion
This study found no association between the anticholinergic load of drugs used preoperatively and the development of POD in older adult patients without severe preexisting cognitive impairment. Future analyses should examine the influence of intra- and postoperative administration of anticholinergic drugs as well as dosages of and interactions between medications.
Background
Diabetes mellitus is a major global health issue with a growing prevalence. In this context, the number of diabetic complications is also on the rise, such as diabetic foot ulcers (DFU), which are closely linked to the risk of lower extremity amputation (LEA). Statistical prediction tools may support clinicians to initiate early tertiary LEA prevention for DFU patients. Thus, we designed Bayesian prediction models, as they produce transparent decision rules, quantify uncertainty intuitively and acknowledge prior available scientific knowledge.
Method
A logistic regression using observational collected according to the standardised PEDIS classification was utilised to compute the six-month amputation risk of DFU patients for two types of LEA: 1.) any-amputation and 2.) major-amputation. Being able to incorporate information which is available before the analysis, the Bayesian models were fitted following a twofold strategy. First, the designed prediction models waive the available information and, second, we incorporated the a priori available scientific knowledge into our models. Then, we evaluated each model with respect to the effect of the predictors and validity of the models. Next, we compared the performance of both models with respect to the incorporation of prior knowledge.
Results
This study included 237 patients. The mean age was 65.9 (SD 12.3), and 83.5% were male. Concerning the outcome, 31.6% underwent any- and 12.2% underwent a major-amputation procedure. The risk factors of perfusion, ulcer extent and depth revealed an impact on the outcomes, whereas the infection status and sensation did not. The major-amputation model using prior information outperformed the uninformed counterpart (AUC 0.765 vs AUC 0.790, Cohen’s d 2.21). In contrast, the models predicting any-amputation performed similarly (0.793 vs 0.790, Cohen’s d 0.22).
Conclusions
Both of the Bayesian amputation risk models showed acceptable prognostic values, and the major-amputation model benefitted from incorporating a priori information from a previous study. Thus, PEDIS serves as a valid foundation for a clinical decision support tool for the prediction of the amputation risk in DFU patients. Furthermore, we demonstrated the use of the available prior scientific information within a Bayesian framework to establish chains of knowledge.
Die meisten börsennotierten Gesellschaften haben ein Risikofrüherkennungssystem, das vom Abschlussprüfer ohne den Verweis auf schwerwiegende Mängel testiert wird. Tatsächlich zeigen Studien, dass die in der Praxis implementierten Systeme häufig ungeeignet sind, um an sich erkennbare bestandsgefährdende Entwicklungen zu identifizieren. Dies liegt an methodischen Schwächen, z. B. an einer fehlenden Betrachtung der Liquiditätswirkung von Risiken, einer vernachlässigten Analyse von Extremrisiken und insb. am Fehlen einer Risikoaggregation, die auch Kombinationseffekte berücksichtigt. Bei Insolvenzen, wie hier im Beispiel von Gerry Weber, lässt sich bei Betrachtung des dokumentierten Risikomanagementsystems erkennen, dass die gesetzlichen Anforderungen nicht erfüllt wurden. Um solche „überraschenden Insolvenzen“ zu vermeiden, ist es erforderlich, bessere Verfahren zu implementieren und durch Prüfer testieren zu lassen, was bisher nicht konsequent geschieht. Der Beitrag schlägt ein einfaches aber umfassendes Prüfschema für Risikofrüherkennungssysteme vor, das sowohl intern als auch von externen Prüfern verwendet werden kann.
Die folgende Studie wurde vor dem Hintergrund der zunehmenden Relevanz von ökologisch nachhaltigem Handeln in unserer Gesellschaft aufgrund von wachsenden erlebbaren Auswirkungen durch unsere Lebensweise und den Klimawandel durchgeführt. Für die folgende qualitative Studie wurden fünf Studierende und fünf Berufstätige zu den Hintergründen ihres ökologisch nachhaltigen Handelns befragt. Es wurden zehn Einflussfaktoren identifiziert, welche ökologisch nachhaltiges Handeln von Studierenden und Berufstätigen beeinflussen: Wissen & Informationen; Persönliche Relevanz; empfundene Wichtigkeit & Interesse; Erlebnisse & Erfahrungen (auch ehrenamtliches Engagement); Soziale Umfelder (persönliches, studentisches und berufliches Umfeld); Beeinflussung durch Andere (auch Beeinflussung durch Medien); Positiver Nutzen; Abwägung; Gewohnheiten; Gefühle; Möglichkeiten zur Umsetzung. Als theoretische Basis zur Untersuchung von beeinflussenden Faktoren wurde das psychologische Modell zur Erklärung nachhaltigen Handelns von Hamann, Baumann und Löschinger (2016) hinzugezogen. Des Weiteren wurden drei Aspekte zur positiven Auswirkung von ökologisch nachhaltigem Handeln zur Förderung ökologisch nachhaltigen Handelns in der Gesellschaft gefunden: fortlaufender Prozess; Beeinflussung von Anderen; Ideen zur Förderung von ökologisch nachhaltigem Handeln. Weitere Maßnahmenvorschläge und Handlungsempfehlungen wurden v. a. für die Kontexte Beruf, Studium, Schule und Medien aus den Erkenntnissen der Studie abgeleitet.
Various overoxidized poly(1H-pyrrole) (PPy), poly(N-methylpyrrole) (PMePy) or poly(3,4-ethylenedioxythiophene) (PEDOT) membranes incorporated into an acrylate-based solid polymer electrolyte matrix (SPE) were directly electrosynthesized by a two-step in situ procedure. The aim was to extend and improve fundamental properties of pure SPE materials. The polymer matrix is based on the cross-linking of glycerol propoxylate (1PO/OH) triacrylate (GPTA) with poly(ethylene glycol) diacrylate (PEGDA) and lithium bis(trifluoromethanesulfonyl)imide (LiTFSI) as a conducting salt. A self-standing and flexible polymer electrolyte film is formed during the UV-induced photopolymerization of the acrylate precursors, followed by an electrochemical polymerization of the conducting polymers to form a 3D-IPN. The electrical conductivity of the conducting polymer is destroyed by electrochemical overoxidation in order to convert the conducting polymer into an ion-exchange membrane by introduction of electron-rich groups onto polymer units. The resulting polymer films were characterized by scanning electron microscopy, cyclic voltammetry, electrochemical impedance spectroscopy, differential scanning calorimetry, thermal analysis and infrared spectroscopy. The results of this study show that the combination of a polyacrylate-matrix with ion selective properties of overoxidized CPs leads to new 3D materials with higher ionic conductivity than SPEs and separator or selective ion-exchange membrane properties with good stability by facile fabrication.
Im Rahmen des Forschungsschwerpunktes „Inklusive Bildung“ der Hochschule Osnabrück wurde eine quantitative Erhebung mittels eines schriftlichen Fragebogens durchgeführt.Die Ergebnisse verweisen in vergleichender Perspektive auf unterschiedliche Handlungs- und Sichtweisen der Berufsgruppen hin. Damit ergeben sich verschiedene Chancen sowie Herausforderungen in der Kooperation der Bildungsbereiche – und die Notwendigkeit einer Systematisierung dieser Prozesse.
Career Decisions of Indian Female Talent: Implications for Gender-sensitive Talent Management

Purpose: Talent scarcity in emerging economies such as India poses challenges for companies,and limited labour market participation among well-educated women has been observed. The reasons that professionals decide not to pursue a further corporate career remain unclear. By investigating career decision making, this article aims to highlight (1) the contextual factors that impact those decisions, (2) individuals’ agency to handle them, and (3) the implications for talent management (TM).
Design/methodology/approach: Following a qualitative research design, computer-aided analysis was conducted on interviews with 24 internationally experienced Indian business professionals. A novel application of neo-institutionalism in the Indian context was combined with the family-relatedness of work decisions (FRWD) model.
Findings: Career decisions indicate that rebellion against Indian societal and family expectations is essential to following a career path, especially for women. TM as part of the current institutional framework serves as a legitimising façade veiling traditional practices that hinder females’ careers.
Research limitations: Interviewees adopted a retrospective perspective when describing their career decisions; therefore, different views might have existed at the moment of decision making.
Practical implications: Design and implementation of gender-sensitive TM adjusted to fit the specific Indian context can contribute to retaining female talent in companies and the labour market.
Originality/value: The importance of gender-sensitive TM can be concluded from an empirical study of the context-based career decision making of experienced business professionals from India. The synthesis of neo-institutionalism, the FRWD model and the research results provides assistance in mapping talent experiences and implications for overcoming the challenges of talent scarcity in India.
Background
Against the background of a steadily increasing degree of digitalization in health care, a professional information management (IM) is required to successfully plan, implement, and evaluate information technology (IT). At its core, IM has to ensure a high quality of health data and health information systems to support patient care.
Objectives
The goal of the present study was to define what constitutes professional IM as a construct as well as to propose a reliable and valid measurement instrument.
Methods
To develop and validate the construct of professionalism of information management (PIM) and itsmeasurement, a stepwise approach followed an established procedure from information systems and behavioral research. The procedure included an analysis of the pertaining literature and expert rounds on the construct and the
instrument, two consecutive and comprehensive surveys at the national and international level, exploratory and confirmatory factor analyses as well as reliability and validity testing.
Results
Professionalism of information management was developed as a construct consisting of the three dimensions of strategic, tactical, and operational IMas well as of the regularity and cyclical phases of IM procedures as the two elements of professionalism.
The PIM instrument operationalized the construct providing items that incorporated IM procedures along the three dimensions and cyclical phases. These procedures had to be evaluated against their degree of regularity in the instrument. The instrument proved to be reliable and valid in two consecutive measurement phases
and across three countries.
Conclusion
It can be concluded that professionalism of information management is a meaningful construct that can be operationalized in a scientifically rigorous manner. Both science and practice can benefit from these developments in terms of improved self-assessment, benchmarking capabilities, and eventually, obtaining a better understanding of health IT maturity.
Vanadium carbide (VC) reinforced FeCrVC hardfacings have become important to improving the lifetime of tools suffering abrasive and impressive loads. This is because the microstructural properties of such hardfacings enable the primary VCs to act as obstacles against the penetrating abrasive. Because dilution is supposed to be the key issue influencing the precipitation behaviour of primary carbides during surfacing, the development of deposit welding processes exhibiting a reduced thermal impact, and hence lower dilution to the base material, is the primary focus of the current research. By inserting an additional hot wire in the melt, an approach was developed to separate the material and energy input during gas metal arc welding (GMAW) and hence realised low dilution claddings. The carbide content could be increased, and a grain refinement was observed compared with conventional GMAW. These effects could be attributed to both the reduced dilution and heterogeneous nucleation.

Der vorliegende Beitrag thematisiert den Umgang mit digitalen Medien im Berufsalltag hinsichtlich Strategien zur reduzierten oder reflektierten Mediennutzung. Konkret wurde folgender Forschungsfrage nachgegangen: Inwiefern erachten Beschäftigte eine Begrenzung der Nutzung von digitalen Medien im Arbeitskontext als sinnvoll? Der bisherige Forschungsstand ist von einer quantitativen Herangehensweise und dem Fokus auf die Unternehmensperspektive geprägt. Zur Annäherung an die Beantwortung der Forschungsfrage wurden im Rahmen einer Masterarbeit sieben qualitative, telefonische Leitfadeninterviews geführt. Befragt wurden Beschäftigte aus verschiedenen Großunternehmen in Deutschland, in denen die genannten Strategien zur Anwendung kommen. Den Antworten ist zu entnehmen, dass unter Arbeitnehmenden grundsätzlich Bedarf und Interesse an Strategien der reduzierten oder reflektierten Nutzung digitaler Medien besteht. Die meisten Befragten kritisieren jedoch insbesondere solche unternehmerischen Vorhaben, durch die ihnen die Autonomie genommen wird, flexibel zu entscheiden, wann und in welcher Form sie ihre Mediennutzung einschränken. Eine klare Handlungsempfehlung für Unternehmen ist deshalb, vorrangig Strategien einzusetzen, welche die Beschäftigten in ihrer individuellen und selbstbestimmten Arbeitsweise nicht behindern und mit denen sie sich daher identifizieren können.
Postbürokratische Organisationsformen spielen im Zuge der Digitalisierung, Globalisierung sowie des demographischen und gesellschaftlichen Wandels eine aktuelle und zunehmend wichtige Rolle. Der Büroraum kann in dieser Hinsicht einen wesentlichen Beitrag zur Förderung interpersoneller Kommunikation, als Rahmenbedingung neuer Organisationsformen, leisten. Dennoch wird dem Büroraum in Wissenschaft und Praxis bislang ein geringer Stellenwert zugeschrieben. In Folge einer Literaturstudie zeigt dieser Beitrag, inwiefern Kommunikation und Büroarchitektur zusammenhängen und was daher bei der Einführung von Büroformen zu beachten ist. Der Beitrag liefert dadurch Anregungen und Orientierung für das Kommunikationsmanagement.
Die öffentlich-rechtlichen Medienanstalten spielen rechtlich wie wirtschaftlich eine besondere Rolle im deutschen Mediensystem. Verhandlungen um Sport-Übertragungsrechte stehen vor diesem Hintergrund und in Anbetracht der Preise für die Lizenzpakete stets unter medialer Beobachtung.
Von kommunikationswissenschaftlichem Interesse ist deshalb, wie sich die Öffentlich-Rechtlichen in Deutschland in einer solchen Diskussion präsentieren und welchen Einfluss sie tatsächlich auf die Medienberichterstattung ausüben können. Als Fallbeispiel dienen in diesem Beitrag die Verhandlungen um die Olympia-Übertragungsrechte 2018 bis 2024. Auf der theoretischen Grundlage des Framings nach Robert Entman wurde ein inhaltsanalytisches Instrumentarium angewendet, mit dem sich strategische wie mediale Deutungsmuster offenlegen lassen. Es zeigte sich, dass ARD und ZDF über alle Phasen der Verhandlungen hinweg in der Lage waren, die Aussagen ihrer Sprecher in den Berichten zu positionieren.
Aus diesen Erkenntnissen ließen sich im Kern drei Ableitungen für die Kommunikationsarbeit der Öffentlich-Rechtlichen treffen. Erstens sollten sie sich ihrer außergewöhnlichen Kommunikatorposition bewusst sein. Außerdem sollten sie diese Position nutzen, um medial noch mehr zu agieren statt zu reagieren. Im Kontext von Sportrechteverhandlungen sollten sie zudem den Aspekt des verantwortungsvollen Umgangs mit Gebührengeldern stärker betonen.
Mithilfe von Social Bots werden in sozialen Netzwerken massenhaft Inhalte verbreitet. Welche Auswirkungen dies mit sich bringt, ist umstritten. Verbreiten die Computerprogramme Falschnachrichten und stellen eine Gefahr für die Gesellschaft und die Demokratie dar? Um zu untersuchen, welche Aussagen es zu diesem Thema gibt, werden in der Bachelorarbeit der journalistische und der kommunikationswissenschaftliche Diskurs über Social Bots verglichen. Da bislang keine Studien zu diesem Thema existieren, soll mithilfe der Arbeit diese Theorielücke geschlossen und ein Diskurs zwischen den Fachrichtungen angeregt werden. Die zu untersuchenden Fragestellungen beziehen sich darauf, welche Frames sich im journalistischen Diskurs über Social Bots identifizieren lassen, welche Ergebnisse die einschlägigen kommunikationswissenschaftlichen Studien liefern und welche Gemeinsamkeiten und Unterschiede die Diskurse aufweisen. Herangezogen werden vorrangig Literatur und Studien aus den Bereichen Framing und Risikokommunikation sowie über Social Bots. Als Methode wird eine qualitative Frame-Analyse gewählt, mit der die Berichterstattung über Social Bots in ausgewählten deutschen Qualitätsmedien, genauer in der ZEIT sowie im SPIEGEL und in den zugehörigen Online-Angeboten, analysiert wird. Der Vergleich der Ergebnisse aus den beiden Diskursen ergibt geringere Unterschiede als angenommen. Die Unterschiede bestehen vor allem hinsichtlich thematischer Schwerpunkte der untersuchten Studien und Artikel. Die Ergebnisse werden in die Theorie eingeordnet. Zudem werden Schlussfolgerungen für die weitere Forschung zum Thema Social Bots und den Umgang mit den Computerprogrammen gezogen. Die Ergebnisse zeigen die Notwendigkeit der weiteren Erforschung von Social Bots sowie eines Diskurses zwischen Kommunikationswissenschaften, Journalismus und Politik auf, um den Entwicklungen begegnen zu können.
In einem gemeinsamen Fachaufsatz verbinden die Autoren Determann und Ostheim die Erkenntnisse aus ihren Abschlussarbeiten am Institut für Kommunikationsmanagement. Eingangs findet sich eine theoretische Einordnung der Litigation-PR, in der die zunehmende Medialisierung der Gesellschaft und entsprechend auch die der Justiz als Ursprung einer strategischen Rechtskommunikation in der Öffentlichkeit ausgemacht werden. Im weiteren Verlauf gehen die Autoren insbesondere auf die Praxis spezialisierter PR-Agenturen ein. Der Aufsatz schließt mit einem Ausblick auf die Entwicklung der strategischen Rechtskommunikation in der nahen Zukunft und die Berufsperspektiven für das Kommunikationsmanagement.
Menschen erzählen sich Geschichten, um sich etwas vorzustellen, sich eine Vision auszumalen und daraus etwas Neues, Besseres zu erbauen. Im Rahmen des Aufsatzes werden Geschichten über eine nachhaltige bzw. zukunftsfähige Lebensweise betrachtet. Diese realen (faktualen) Geschichten erzählen von Menschen und Organisationen, denen der Wandel zu einer nachhaltigen Lebens- und Wirtschaftsweise in ihrem Handlungsfeld gelingt und die andere zum Wandel ermutigen (als sogenannte Change Agents). Sie etablieren innerhalb der bestehenden Rahmenbedingungen neue Werte und Handlungsweisen und schaffen damit Reallabore, in denen „das Neue“ ausprobiert, angeschaut und weitererzählt werden kann. Der Aufsatz nimmt erzähltheoretische Grundlagen zur Hilfe und zeigt anhand eines Fallbeispiels, wie Geschichten das zunächst abstrakte Konzept Nachhaltigkeit konkret und anschaulich kommunizieren können. Die entsprechende narrativ-analytische Perspektive öffnet den Blick für die Werte und Prinzipien, die in Form von Narrativen (Erzählstrukturen) in einzelnen Geschichten und auch in Diskursen vermittelt werden. Für professionelle Kommunikatoren kann diese Perspektive ein Methodenansatz sein, um Veränderungsprozesse besser zu verstehen und das eigene Storytelling auf eine fundierte Basis zu heben.
Seit Jahren ist die steuerliche Behandlung von Aufwendungen für ein häusliches Arbeitszimmer ein immer wiederkehrendes Thema in der Fachliteratur. Auch zahlreiche Auseinandersetzungen mit dem Finanzamt resultieren aus der (Nicht-)Berücksichtigung dieser Aufwendungen. In der Vergangenheit wurden bereits etliche Gerichtsurteile zu unterschiedlichen Fallkonstruktionen aus diesem Bereich gesprochen. Momentan hat das Thema erneut an Aktualität gewonnen. So arbeitet aufgrund der derzeit herrschenden Pandemie eine große Anzahl an Steuerpflichtigen (erstmals) im Homeoffice und wendet sich ratsuchend mit Fragen zu den entsprechenden steuerlichen Folgen an ihre steuerlichen Berater. Der vorliegende Beitrag beschäftigt sich unter Heranziehung einschlägiger Urteile mit der steuerlichen Behandlung von Aufwendungen für ein häusliches Arbeitszimmer, nimmt Stellung zu den Möglichkeiten der steuerlichen Abzugsfähigkeit in der aktuellen besonderen Situation, analysiert diese kritisch und formuliert konkrete Handlungsempfehlungen.
Hintergrund
International stellen präventive Hausbesuche (PHB) ein implementiertes Konzept dar. Aufgrund des demografischen Wandels und andauernden Fachkräftemangels werden alternative Versorgungsformen wie PHB benötigt. Ziel der PHB ist, älteren Menschen den Verbleib in ihrem Zuhause und somit ihre präferierte Lebensform möglichst lange zu ermöglichen.
Methodik
Im Rahmen des vorliegenden Scoping-Reviews wurde eine datenbankgestützte, systematische Literaturreche durchgeführt, um einen Überblick der Literatur über PHB darzustellen. Der Fokus der Literaturanalyse lag auf den Effekten PHB, die aus internationalen und nationalen Reviews abgeleitet werden konnten, nationale Studien dienten der Analyse des Konzepts PHB. Es wurden 9 internationale und ein nationales Review sowie 7 nationale Studien eingeschlossen.
Ergebnisse
Die PHB erzielen heterogene Effekte auf die Mortalität, den stationären Krankenhausaufenthalt, die Pflegeheimaufnahme, das Sturzereignis, den funktionellen Status und die Lebensqualität. Die Ergebnisse sprechen tendenziell dafür, dass durch die PHB Pflegebedürftigkeit hinausgezögert oder gar vermieden werden kann.
Schlussfolgerungen
Weitere Forschungsarbeiten zu PHB sind notwendig, um das bisherige Wissen zu ergänzen und mögliche Konzeptanpassungen für eine größere Akzeptanz der Zielgruppe sowie die deutschlandweite Implementierung zu erreichen. Zudem ist die Politik gefordert, die Finanzierung innerhalb Deutschlands sicherzustellen. In Deutschland sind PHB noch nicht flächendeckend implementiert, obwohl verschiedene Projekte in unterschiedlichen Kontexten auf einen anhaltenden gesellschaftlichen Bedarf für und Interesse an PHB hindeuten.
The location of manufacturing is operationally and strategically important for multinational companies. The spatial dispersion of manufacturing is determined by firm-specific as well as external factors, both of which are subject to constant change. After decades of offshoring production, the paper uses interview data from UK high value manufacturing companies to explore the recent phenomenon of reshoring and the strategic and operational effects of manufacturing location. The analyses show that concentrating manufacturing in home countries enables firms to increase organizational agility and stimulate innovation. However, under certain conditions, firms also continue benefiting from the known advantages of offshoring. This underlines that there is no universal recommendation for improving competitiveness by a certain spatial configuration of manufacturing. Instead, firms need to consider their specific markets, competitive context, and capabilities when strategizing manufacturing location."
Studiengänge zur Versorgungsforschung in Deutschland : aktueller Stand und zukünftige Perspektiven

Ziel der Studie: Ziel dieser Untersuchung war es, die derzeit akkreditierten Versorgungsforschungsstudiengänge in Deutschland im Hinblick auf ihre Strukturdaten und Inhalte zu analysieren.
Methodik: Im Rahmen eines deskriptiven qualitativen Designs wurden die aktuellen Studienverlaufspläne, die Modulhandbücher und die Steckbriefe akkreditierter Masterstudiengänge der Versorgungsforschung inhaltsanalytisch ausgewertet.
Ergebnisse: Die 6 analysierten Studiengänge zeigen sich hinsichtlich ihrer strukturellen Merkmale und lehr-lerninhaltlichen Ausrichtung in den wesentlichen Punkten als homogen. Ziel aller Studiengänge ist es, fach- und disziplinübergreifende personale und soziale Kompetenzen zu fördern, um Versorgungsbedarfe im Gesundheitswesen zu identifizieren, Innovationen der Versorgung zu entwickeln, zu initiieren, zu evaluieren und kritisch zu reflektieren.
Schlussfolgerungen: Trotz ähnlicher Lehr-Lerninhalte bestehen spezifische Ausrichtungen, für die sich die Studierenden vor dem Hintergrund ihrer individuellen Präferenzen entscheiden können. Diese Ergebnisse könnten unter Berücksichtigung aktueller Diskurse und weiterer Erkenntnisse als Grundlage für Empfehlungen zur Entwicklung eines Kerncurriculums dienen.
Background: Pancreatic cancer is one of the malignant diseases with the highest cancer-specific mortality. At the time of diagnosis, life expectancy is often already very limited, as it is usually discovered late and in an advanced stage. Coping with cancer is a complex process. Coping strategies of patients with pancreatic cancer probably differ from those of other malignancies. Yet to date, there exists no pancreatic cancer-specific coping model.
Objective: The objective of this scoping review is to explore and characterize the academic literature related to coping processes in patients with pancreatic cancer.
Methods/Design: The JBI's three-step search strategy, combined with the Arksey and O'Malley framework, will be used to identify articles via PubMed/MEDLINE, CINAHL, Cochrane Library, Google Scholar, CAMbase, CareLit, CC Med, Scopus, and PsycARTICLES (Arksey & O'Malley, 2005; Peters et al., 2017). It follows the PRISMA guidelines for scoping reviews (Tricco et al., 2018). Primary and secondary studies and reviews which report on coping with pancreatic cancer (adenocarcinoma) in adults in English or German language will be included in this scoping review, regardless of publication date or study design.

Injection of slurry or digestate below maize seeds is a relatively new technique developed to improve nitrogen use efficiency. However, this practice has the major drawback of increasing nitrous oxide (N2O) emissions. The application of a nitrification inhibitor (NI) is an effective method to reduce these emissions. To evaluate the effect of the NI 3,4‐dimethypyrazole phosphate (DMPP) on N2O emissions and the stabilization of ammonium, a two‐factorial soil‐column experiment was conducted. PVC pipes (20 cm diameter and 30 cm length) were used as incubation vessels for the soil‐columns. The trial consisted of four treatments in a randomized block design with four replications: slurry injection, slurry injection + DMPP, digestate injection, and digestate injection + DMPP. During the 47‐day incubation period, N2O fluxes were measured twice a week and cumulated by linear interpolation of the gas‐fluxes of consecutive measurement dates. After completion of the gas flux measurement, concentration of ammonium and nitrate within the soil‐columns was determined. DMPP delayed the conversion of ammonium within the manure injection zone significantly. This effect was considerably more pronounced in treatment digestate + NI than in treatment slurry + NI. Regarding the cumulated N2O emissions, no difference between slurry and digestate treatments was determined. DMPP reduced the release of N2O significantly. Transferring the results into practice, the use of DMPP is a promising way to reduce greenhouse gas emissions and nitrate leaching, following the injection of slurry or digestate.
The objective of this review is a global assessment of the economics of second‐generation biorefineries, with a focus on the use of food waste and agricultural residues for chemical production by applying biotechnological processes. Analyses are conducted on feedstock and product distribution, applied economic models, and profitability figures for the period 2013–2018. In a study of 163 articles on different biorefinery systems, the production of chemicals is identified as the second major product class, after bioenergy. Bagasse and straw are frequently analyzed second‐generation feedstocks. Based on the evaluation of 22 articles, second‐generation biorefineries producing chemicals by applying biotechnological processes proves to be economically feasible. On average, both the internal rate of return (IRR) and the return on investment (ROI) are 20% and the payback period (PP) is 6 years. The cost share of feedstock in biorefineries is between 0–50%. The price of the end product and the fermentation yields have the most impact on profitability. The processing of food waste that has industrial and municipal origins appears more economical than the processing of agricultural residues. Scientists, policy makers and entrepreneurs with an appropriate risk tolerance are advised to pay particular attention to municipal food waste and the potential economic production of carboxylic acids. For various economic issues related to biorefineries, dynamic‐deterministic models are recommended, which can be extended by a stochastic model. This review provides an initial overview of the economic feasibility of second‐generation biorefineries. Further techno‐economic analyses are required to produce statistically significant statements on key profitability figures. © 2020 The Authors. In diesem erfahrungsbasierten Artikel geht es um die praktische Anwendung von Recovery-Strukturen-Setting und den Versuch, das Potenzial von Recovery für den rehabilitativen Bereich der Psychiatrie herauszustellen. Darüber hinaus wird weiterer Forschungsbedarf dargestellt. Hintergrund ist ein zehnwöchiges Praktikum in einer Tagesklinik in der Schweiz.
Im Rahmen eines Unterauftrags durch die Universität Bremen, die vom Verein Geschäftsstelle Qualitätsausschuss Pflege e.V. mit der Entwicklung eines wissenschaftlich fundierten Verfahrens zur einheitlichen Bemessung des Personalbedarfs in Pflegeeinrichtungen nach qualitativen und quantitativen Maßstäben nach § 113c SGB XI beauftragt wurde, hat die Hochschule Osnabrück Personalfragen in der ambulanten Pflege untersucht.
Background: In aged nursing care receivers, the prevalence of adverse skin conditions such as xerosis cutis, intertrigo, pressure ulcers or skin tears is high. Adequate skin care strategies are an effective method for maintaining and enhancing skin health and integrity in this population.
Objectives: The objective was to summarize the empirical evidence about the effects and effectiveness of non-drug topical skin care interventions to promote and to maintain skin integrity and skin barrier function in the aged, to identify outcome domains and outcome measurement instruments in this field.
Design: An update of a previous systematic review published in 2013 was conducted.
Data sources: Databases MEDLINE and EMBASE via OvidSP and CINAHL (original search January 1990 to August 2012, update September 2012 to May 2018) and reference lists were searched. Forward searches in Web of Science were conducted.
Methods: A review protocol was registered in Prospero (CRD42018100792). Main inclusion criteria were primary intervention studies reporting treatment effects of basic skin care strategies in aged people with a lower limit of age range of 50 years and published between 1990 and 2018. Primary empirical studies were included with experimental study designs including randomized controlled trials and quasi-experimental designs. Methodological quality of included randomized controlled trials was evaluated using the Cochrane Collaboration's Tool for assessing risk of bias. Levels of evidence were assigned to all included studies.
Results: Sixty-three articles were included in the final analysis reporting effects of interventions to treat and/or to prevent skin dryness, pruritus, general skin barrier improvement, incontinence-associated dermatitis, skin tears and pressure ulcers. Skin cleansers containing syndets or amphotheric surfactants compared with standard soap and water improved skin dryness. Lipophilic leave-on products containing humectants decreased skin dryness and reduced pruritus. Products with pH 4 improved the skin barrier. Application of skin protectants and structured skin care protocols decreased the severity of incontinence-associated dermatitis. Formulations containing glycerin and petrolatum reduced the incidence of skin tears. Thirty-five outcome domains were identified with nearly 100 different outcome measurement instruments.
Conclusion: Included studies showed substantial heterogeneity regarding design, interventions and outcomes. Basic skin care strategies including low-irritating cleansers and lipophilic humectant-containing leave-on products are helpful for treating dry skin and improving skin barrier in the aged. Lower pH of leave-on products improves the skin barrier. The number of different outcome domains was unexpectedly high. We recommend to identify critical outcome domains in the field of skin care to make trial results more comparable in the future and to measure possible performance differences between different skin care strategies and products.

IT-Standard für das pflegerische Entlassmanagement.
Der von der Hochschule Osnabrück entwickelte „ePflegebericht“ kann die bisherigen unterschiedlichen papierbasierten Überleitungen ablösen, indem die entsprechenden IT-Systeme interoperabel Dokumente austauschen. Die Pflege erhält mit diesem IT-Standard erstmals einen möglichen Zugang zur Telematikinfrastruktur, um zwischen Einrichtungen und über Sektorengrenzen hinweg pflegerisch relevante Informationen schnell und sicher zu übermitteln.
Planung, Strukturierung und Umsetzung der Entlassung aus dem Krankenhaus gestalten sich vielfach schwierig. Diese Schnittstellenproblematik bezieht sich auf die Überleitung in nachfolgende Versorgungsbereiche, dadurch gefährdete Versorgungskontinuität sowie Informationsverluste und Unstimmigkeiten. „Passing the Baton“ ist ein Leitfaden für ein effektives Entlassungsmanagement in einer Rehabilitationseinrichtung in Wales.
Eine strukturierte Triage in der Notaufnahme durch Pflegefachpersonen leistet einen wesentlichen Beitrag für die schnelle und sichere Identifizierung lebensbedrohlich erkrankter Patienten. Es stehen unterschiedliche Triagesysteme zur Auswahl. Manchester Triage System und Emergency Severity Index sind die hierzulande am häufigsten verwendeten Instrumente. Zu beachten sind jedoch ihre spezifischen Vor- und Nachteile.
Die häusliche Pflege hat eine kontinuierlich hohe gesundheitswirtschaftliche, gesamtgesellschaftliche wie auch individuelle Bedeutung. Angehörige pflegebedürftiger Menschen benötigen eine bedürfnisorientierte und qualitätsgesicherte Unterstützung bei der häuslichen Pflege. Vor diesem Hintergrund ist zu fragen, welche Evidenz zu Unterstützungsangeboten im Sinne von "Respite Care" für Angehörige vorzufinden ist. Die vorliegende Literaturanalyse bezieht deutsche Studien ein und liefert ausgewählte Ansatzpunkte für den Aufbau, die Evaluation und Weiterentwicklung von Angeboten. Es konnten Bedingungen der Inanspruchnahme, Formen, mögliche Qualitätskriterien und Hinweise zur Wirkung von "Respite Care" bzw. Unterstützung identifiziert werden. Auf Basis der Ergebnisse sollten die Unterstützungsangebote mit Angehörigen gezielt abgestimmt und zukünftig anhand unterschiedlicher Forschungsdesigns überprüft werden.
Das Delir ist eine Komplikation, die Patienten während ihrer stationären Behandlung erleiden können und trotz Screeninginstrumenten oftmals nicht erkannt werden. Das vorliegende Survey hatte zum Ziel, die Gütekriterien Handhabbarkeit, Akzeptanz und Relevanz aus Sicht der Anwender*innen zu erfragen. Basierend auf einer Literaturrecherche wurde ein standardisierter Fragebogen entwickelt. 45 Pflegenden von drei Pilotstationen haben teilgenommen. Die Auswertung erfolgte deskriptiv. Die Mehrheit (n=25) der Pflegenden hat die Nu-DESC als ein einfaches und kurzes Screeninginstrument wahrgenommen. Als problematisch stellte sich die Herleitung klinischer Konsequenzen aus dem Screening für die Pflegenden dar. 19 Pflegende sahen einen Mehrwert. Die Zielsetzung war jedoch nur der Hälfte der Befragten bekannt. Es wurde deutlich, dass die Nu-DESC von mehr als der Hälfte der Befragten nicht zur Steuerung der pflegerischen Versorgung wahrgenommen wird. Die Nu-DESC ist ein Screeninginstrument, welches – im Sinne der Handhabbarkeit - zumindest im Kontext der drei Stationen als geeignet gelten kann. In Bezug auf die Akzeptanz und Relevanz gibt es in den Daten eindeutige Hinweise für einen erweiterten Schulungsbedarf.
Der Beitrag geht der Frage nach, wie eine vertrauensvolle Zusammenarbeit mit der Ausgestaltung interprofessioneller Kooperationen von Lehr- und Fachkräften in Grundschulteams zusammenhängt. Zunächst wird die Bedeutung von Vertrauen in der Zusammenarbeit als Voraussetzung für interprofessionelle Kooperation behandelt und anschließend durch die Darstellung und Diskussion quantitativer Ergebnisse eines Forschungs- und Entwicklungsprojektes zur interprofessionellen Kooperation in inklusiven Grundschulen konkretisiert. Die Studienergebnisse zeigen, dass die befragten Lehr- und Fachkräfte (n=208) enger und intensiver zusammenarbeiten, je stärker das Vertrauensniveau im Kollegium ausgeprägt ist. Schlussfolgernd sollten Fort- und Weiterbildungsangebote für Lehr- und Fachkräfte inklusiver Grundschulen auch die Entwicklung, Aufrechterhaltung und Intensivierung einer vertrauensvollen interprofessionellen Kooperation unterstützen.
In der inklusiven Grundschule arbeiten zunehmend verschiedene Professionen, wie bspw. Lehrer*innen, Sonderpädagog*innen oder Schulsozialarbeiter*innen, interprofessionell zusammen. Dieser Wandel in schulischen Arbeitskontexten geht mit veränderten Professionalisierungsanforderungen für die Lehr- und Fachkräfte einher (vgl. Kleina & Tan, 2019, S. 63). Die Praxis interprofessioneller Kooperation in der inklusiven Grundschule ist dabei bisher nur in Teilfragen erforscht (vgl. Moldenhauer, 2019), kaum aber mit Blick auf die Erfahrungen und Erwartungen der pädagogischen Lehr- und Fachkräfte selbst. Hierzu werden Ergebnisse einer quantitativen Bedarfserhebung (N=271), welche im Rahmen des vom BMBF geförderten Forschungs- und Entwicklungsprojektes InproKiG bei Lehrkräften und pädagogischen Fachkräften von Schulteams in Niedersachsen und Hessen durchgeführt wurde, vorgestellt. Die Ergebnisse geben Aufschluss über die Entwicklung und Gestaltung von Qualifizierungsmaßnahmen in diesem Bereich. Neben der Qualifizierung der bereits tätigen Lehr- und Fachkräfte, kann das InproKiG-Manual auch in der Lehrer*innenausbildung eingesetzt werden.
This article is based on an international comparative social policy module held annually at Malmö University (Sweden) with partner universities from several countries. Our study examines the results of intra- and interpersonal dynamics among participants and lecturers, which we call "productive tension", in relation to overarching questions about knowledge acquisition and the importance of reflexivity and reflectivity in the learning process. Students and faculty create a "community of practice" (CoP) (Lave & Wenger 1991) that benefits from a continuous interactive environment and direct engagement. Our study uses a mixed method approach. The analysis considers qualitative data from interviews with the participating students and quantitative data from questionnaires. In this article, we focus on the productive tension inherent in the reflective and reflexive processes. Reflective and reflexive processes are identified that influence the students' experience of cross-border cooperation and their professional identity. Our study also demonstrates how the knowledge about "Social Work" as a profession can be broadened through international comparative teaching and learning. The main conclusion of this work is that reflective and reflexive learning processes in social work education enable participants to see and understand themselves from a broader perspective and strengthen their own professional identity.
Die Geschichte von »Wellness« lässt sich lange zurückverfolgen. Der Artikel stellt einige Eckpfeiler dieser bewegten Geschichte dar. Aufgezeigt wird vor allem, wie »Wellness« gerade im deutschsprachigen Raum auf offene Ohren stieß und wie der Begriff durch eine hemmungslose Kommerzialisierung seinen ursprünglichen Bedeutungsgehalt veränderte.
Entrapment neuropathies such as carpal tunnel syndrome, radiculopathies, or radicular pain are the most common peripheral neuropathies and also the most common cause for neuropathic pain. Despite their high prevalence, they often remain challenging to diagnose and manage in a clinical setting. Summarising the evidence from both preclinical and clinical studies, this review provides an update on the aetiology and pathophysiology of entrapment neuropathies. Potential mechanisms are put in perspective with clinical findings. The contemporary assessment is discussed and diagnostic pitfalls highlighted. The evidence for the noninvasive and surgical management of common entrapment neuropathies is summarised and future areas of research are identified.
Entrapment neuropathies are a heterogenous condition reflecting distinct underlying pathomechanisms. A contemporary assessment aimed at identifying potential mechanisms may help target management for these patients.
Wir kennen neurodynamische Tests und Behandlungsmethoden, nutzen diese alltäglich in der Praxis und gehen auf Kurse, um mehr darüber zu lernen. Aber was verstehen wir tatsächlich darunter? Kommen wir in unserem Verständnis darüber auf einen gemeinsamen Nenner? Dieser Artikel gibt einen Einblick in die Thematik Neurodynamik. Bisherige Überzeugungen stehen dabei auf dem Prüfstand.
Nervenschmerz ist nicht gleich Nervenschmerz. Um Patienten mit ausstrahlenden Schmerzen, bei denen die Nerven eine Rolle spielen könnten, adäquat zu therapieren, sind eine gründliche Untersuchung und ein fundiertes Clinical Reasoning unerlässlich. Nur dadurch entpuppen sich die beiden Patientinnen mit fast identischen Symptomen als sehr unterschiedlich.
Background/Aim
This study aimed to establish the somatosensory profile of patients with lumbar radiculopathy at pre-and post-microdiscectomy and to explore any association between pre-surgical quantitative sensory test (QST) parameters and post-surgical clinical outcomes.
Methods
A standardized QST protocol was performed in 53 patients (mean age 38 ± 11 years, 26 females) with unilateral L5/S1 radiculopathy in the main pain area (MPA), affected dermatome and contralateral mirror sites and in age- and gender-,and body site-matched healthy controls. Repeat measures at 3 months included QST, the Oswestry Disability Index (ODI) and numerous other clinical measures; at 12 months, only clinical measures were repeated. A change <30% on the ODI was defined as ‘no clinically meaningful improvement’.
Results
Patients showed a significant loss of function in their symptomatic leg both in the dermatome (thermal, mechanical, vibration detection p < .002), and MPA (thermal, mechanical, vibration detection, mechanical pain threshold, mechanical pain sensitivity p < .041) and increased cold sensitivity in the MPA (p < .001). Pre-surgical altered QST parameters improved significantly post-surgery in the dermatome (p < .018) in the symptomatic leg and in the MPA (p < .010), except for thermal detection thresholds and cold sensitivity. Clinical outcomes improved at 3 and 12 months (p < .001). Seven patients demonstrated <30% change on the ODI at 12 months. Baseline loss of function in mechanical detection in the MPA was associated with <30% change on the ODI at 12 months (OR 2.63, 95% CI 1.09–6.37, p = .032).
Conclusion
Microdiscectomy resulted in improvements in affected somatosensory parameters and clinical outcomes. Pre-surgical mechanical detection thresholds may be predictive of clinical outcome.
Significance
This study documented quantitative sensory testing (QST) profiles in patients with lumbar radiculopathy in their main pain area (MPA) and dermatome pre- and post-microdiscectomy and explored associations between QST parameters and clinical outcome. Lumbar radiculopathy was associated with loss of function in modalities mediated by large and small sensory fibres. Microdiscectomy resulted in significant improvements in loss of function and clinical outcomes in 85% of our cohort. Pre-surgical mechanical detection thresholds in the MPA may be predictive of clinical outcome.
Abstract
Background
The clinical presentation of neck-arm pain is heterogeneous with varying underlying pain types (nociceptive/neuropathic/mixed) and pain mechanisms (peripheral/central sensitization). A mechanism-based clinical framework for spinally referred pain has been proposed, which classifies into (1) somatic pain, (2) neural mechanosensitivity, (3) radicular pain, (4) radiculopathy and mixed pain presentations. This study aims to (i) investigate the application of the clinical framework in patients with neck-arm pain, (ii) determine their somatosensory, clinical and psychosocial profile and (iii) observe their clinical course over time.
Method
We describe a study protocol. Patients with unilateral neck-arm pain (n = 180) will undergo a clinical examination, after which they will be classified into subgroups according to the proposed clinical framework. Standardized quantitative sensory testing (QST) measurements will be taken in their main pain area and contralateral side. Participants will have to complete questionnaires to assess function (Neck Disability Index), psychosocial factors (Tampa Scale of Kinesiophobia, Pain Catastrophizing Scale, Depression, anxiety and stress scale), neuropathic pain (Douleur Neuropathique 4 Questions, PainDETECT Questionnaire) and central sensitization features (Central Sensitization Inventory). Follow-ups at three, six and 12 months include the baseline questionnaires. The differences of QST data and questionnaire outcomes between and within groups will be analyzed using (M)AN(C)OVA and/or regression models. Repeated measurement analysis of variance or a linear mixed model will be used to calculate the differences between three, six, and 12 months outcomes. Multiple regression models will be used to analyze potential predictors for the clinical course.
Conclusion
The rationale for this study is to assess the usability and utility of the proposed clinical framework as well as to identify possible differing somatosensory and psychosocial phenotypes between the subgroups. This could increase our knowledge of the underlying pain mechanisms. The longitudinal analysis may help to assess possible predictors for pain persistency.
In der Corona-Pandemie ist es für nahezu alle Organisationen – von der Behörde, über die Unternehmung, bis hin zur Hochschule – zur Aufgabe geworden, den „richtigen“ Umgang mit der Krise zu finden. Was effektives Krisenmanagement ausmacht, soll deshalb vor dem Hintergrund des aktuellen Standes der Fachdiskussion beleuchtet und für eine Selbstreflexion aufbereitet werden, wie sie im Rahmen eines dualen Studiengangs eingeübt und im Sinne einer persönlichen Kompetenzentwicklung vertieft werden kann.
Hintergrund Die physiotherapeutische Dokumentation spielt im Therapieprozess eine wichtige Rolle, erfolgt jedoch auf unterschiedlichste Weise. Es existieren derzeit eine Vielzahl von Softwarelösungen für die physiotherapeutische Dokumentation, welche sich jedoch in ihren Funktionalitäten stark unterscheiden. Ziel Mithilfe einer Befragung soll ein Konsens von Expertenaus der Physiotherapie im Themengebiet der softwarebasierten Dokumentation ermittelt werden. Anhand der Ergebnisse wird ein Anforderungskatalog für die Entwicklung einer neuartigen und benutzerorientierten Dokumentationssoftware erstellt. Methode Online-Delphi-Befragung mit neun Experten über drei Befragungswellen.
Ergebnisse Hinsichtlich der Anforderungen an die Anamnese konnte ein Konsens erzielt werden. Bei der Gliederung des Befundes kam es zu keiner Übereinstimmung der Experten. Das Ergebnis lässt sich durch unterschiedliche manualtherapeutische Konzepte erklären, die für die Befunderhebung verwendet wurden. Schlussfolgerung Eine softwarebasierte Dokumentation sollte standardisierter als bisher erfolgen, um den ClinicalReasoning-Prozess zu unterstützten. Gleichzeitig ist dabei eine gewisse Flexibilität geboten. Die gesammelten Anforderungen können für die Entwicklung einer neuartigen und benutzerorientierten mobilen Anwendung zur Effizienzsteigerung in der physiotherapeutischen Dokumentation verwendet werden.
In den meisten Unternehmen stellt das Einstellungsinterview nach der Sichtung der Bewerbungsunterlagen die einzige Methode der Personalauswahl dar. Umso wichtiger ist es, dass diese Methode möglichst valide die Eignung der Kandidaten erfassen kann. Unbewusste Beurteilungen funken hier jedoch oft dazwischen. Eine Studie hat die Bedeutung der Ähnlichkeit zwischen Interviewer und Bewerber untersucht.
