Many people across the world suffer from iodine (I) deficiency and related diseases. The I content in plant-based foods is particularly low, but can be enhanced by agronomic biofortification. Therefore, in this study two field experiments were conducted under orchard conditions to assess the potential of I biofortification of apples and pears by foliar fertilization. Fruit trees were sprayed at various times during the growing season with solutions containing I in different concentrations and forms. In addition, tests were carried out to establish whether the effect of I sprays can be improved by co-application of potassium nitrate (KNO3) and sodium selenate (Na2SeO4). Iodine accumulation in apple and pear fruits was dose-dependent, with a stronger response to potassium iodide (KI) than potassium iodate (KIO3). In freshly harvested apple and pear fruits, 51% and 75% of the biofortified iodine was localized in the fruit peel, respectively. The remaining I was translocated into the fruit flesh, with a maximum of 3% reaching the core. Washing apples and pears with running deionized water reduced their I content by 14%. To achieve the targeted accumulation level of 50–100 μg I per 100 g fresh mass in washed and unpeeled fruits, foliar fertilization of 1.5 kg I per hectare and meter canopy height was required when KIO3 was applied. The addition of KNO3 and Na2SeO4 to I-containing spray solutions did not affect the I content in fruits. However, the application of KNO3 increased the total soluble solids content of the fruits by up to 1.0 °Brix compared to the control, and Na2SeO4 in the spray solution increased the fruit selenium (Se) content. Iodine sprays caused leaf necrosis, but without affecting the development and marketing quality of the fruits. Even after three months of cold storage, no adverse effects of I fertilization on general fruit characteristics were observed, however, I content of apples decreased by 20%.
Background and Aims: Agronomic biofortification of food crops with iodine may improve the dietary intake of this trace element, which is essential for human development and health. So far, little is known about the suitability of this technique in pome fruits. The objectives of this study were (1) to investigate uptake and translocation of exogenously applied iodine in apple trees, (2) to identify possible strategies of iodine biofortification for this type of fruit, and (3) to evaluate interactions between foliar applied iodine and selenium.
Methods: Apple trees were cultivated in a plastic tunnel for two growing seasons. Iodine was applied via leaves or substrate. During the 2nd year, simultaneous foliar application of iodine and selenium were tested as well. At harvest time, iodine and selenium content in leaves and fruits were determined. The phytoavailable iodine concentration in the growing medium was analyzed following an extraction with calcium chloride. In addition, the dynamics of iodine applied as potassium iodide and iodate in a peat‐based substrate was investigated in an incubation experiment without plants.
Results: The iodine concentration in washed apples increased more than 100‐fold, valuing around 50 µg (100 g FM)−1 by foliar application of iodine as compared to the control treatment. However, this level was only achieved in fruits which were directly wetted by the spray solution. The translocation of leaf‐absorbed iodine to fruits was negligible. Following a substrate fertilization, the fruit iodine content remained rather low due to a strong retention of iodine in the growing medium. When using foliar sprays, the addition of selenium did not affect the iodine enrichment of the apple fruits.
Conclusions: Foliar fertilization of iodine seems to be a promising method to biofortify apples with iodine. The level of I achieved in apple fruits by means of foliar fertilization can significantly contribute to the daily I intake requirement of humans.
Various overoxidized poly(1H-pyrrole) (PPy), poly(N-methylpyrrole) (PMePy) or poly(3,4-ethylenedioxythiophene) (PEDOT) membranes incorporated into an acrylate-based solid polymer electrolyte matrix (SPE) were directly electrosynthesized by a two-step in situ procedure. The aim was to extend and improve fundamental properties of pure SPE materials. The polymer matrix is based on the cross-linking of glycerol propoxylate (1PO/OH) triacrylate (GPTA) with poly(ethylene glycol) diacrylate (PEGDA) and lithium bis(trifluoromethanesulfonyl)imide (LiTFSI) as a conducting salt. A self-standing and flexible polymer electrolyte film is formed during the UV-induced photopolymerization of the acrylate precursors, followed by an electrochemical polymerization of the conducting polymers to form a 3D-IPN. The electrical conductivity of the conducting polymer is destroyed by electrochemical overoxidation in order to convert the conducting polymer into an ion-exchange membrane by introduction of electron-rich groups onto polymer units. The resulting polymer films were characterized by scanning electron microscopy, cyclic voltammetry, electrochemical impedance spectroscopy, differential scanning calorimetry, thermal analysis and infrared spectroscopy. The results of this study show that the combination of a polyacrylate-matrix with ion selective properties of overoxidized CPs leads to new 3D materials with higher ionic conductivity than SPEs and separator or selective ion-exchange membrane properties with good stability by facile fabrication.
Pediatric headache is an increasingly reported phenomenon. Cervicogenic headache (CGH) is a subgroup of headache, but there is limited information about cervical spine physical examination signs in children with CGH. Therefore, a cross-sectional study was designed to investigate cervical spine physical examination signs including active range of motion (ROM), posture determined by the craniovertebral angle (CVA), and upper cervical ROM determined by the flexion–rotation test (FRT) in children aged between 6 and 12 years. An additional purpose was to determine the degree of pain provoked by the FRT. Thirty children (mean age 120.70 months [SD 15.14]) with features of CGH and 34 (mean age 125.38 months [13.14]) age-matched asymptomatic controls participated in the study. When compared to asymptomatic controls, symptomatic children had a significantly smaller CVA (p < 0.001), significantly less active ROM in all cardinal planes (p < 0.001), and significantly less ROM during the FRT (p < 0.001), especially towards the dominant headache side (p < 0.001). In addition, symptomatic subjects reported more pain during the FRT (p < 0.001) and there was a significant negative correlation (r = −0.758, p < 0.001) between the range recorded during the FRT towards the dominant headache side and FRT pain intensity score. This study found evidence of impaired function of the upper cervical spine in children with CGH and provides evidence of the clinical utility of the FRT when examining children with CGH.
Aim
Manual cranial bone tissue techniques (CBTT) are used by physiotherapists as a tool for assessment and treatment of complaints in the craniocervical, face, and head regions. The goal of this study was to determine whether CBTT were able to discriminate between subjects with cervical and/or temporomandibular disorders (TMD) and a healthy group. In addition, the inter-rater reliability when applying CBTT was also investigated.
Methods
A cross-sectional study was conducted and included 60 participants. Six standardized passive techniques were applied and judged for resistance, compliance, and sensory answer. In order to evaluate the inter-rater reliability of these techniques a cohort of participants was measured twice (by two evaluators) prospectively. A logistic regression model and Receiver Operating Curve (ROC) analyses were used to determining the discriminative validity of these techniques.
Results
Logistic regression identified a significant difference for five techniques for resistance and/or compliance and/or the sensory answer between the groups. Based on the Area Under the Curve (AUC) analysis, the discriminative ability of the temporal rotation to distinguish between the groups was fair to good (for resistance AUC = 0.7775 and for compliance AUC = 0.8065). The highest agreement between the two assessors was for the resistance with occipital compression (73%) technique.
Conclusion
This study highlights that some of the CBTT could be potentially useful in distinguishing subjects with cervical and/or TMD from healthy subjects. Inter-rater reliability was moderate. CBTT could be potentially integrated in the examination of participants with complaints in the craniofacial region.

Inhalt: Mithilfe des ISK-360° werden grundlegende soziale Kompetenzen in Selbst- und Fremdbild erfasst. Das Instrument besteht im Kern aus einem Fragebogen zur Erfassung des Selbstbildes (ISK-360°-S) und einem Fragebogen zur Erfassung des Fremdbildes (ISK-360°-F), welche beide je 32 Items beinhalten. Untersucht werden vier soziale Kompetenzen: Soziale Orientierung, Offensivität, Selbststeuerung, Reflexibilität. Den Ausgangspunkt der Entwicklung des ISK-360° bildet die Kurzversion des Inventars sozialer Kompetenzen (ISK, Kanning, 2009), bei dem es sich um ein Selbstbeurteilungsinstrument handelt. Bei der Formulierung der Items des ISK-360° wurde darauf geachtet, dass sie sich gleichermaßen zur Selbst- wie zur Fremdbeschreibung eignen. Items, die ausschließlich innerpsychische Prozesse und Bewertungen beschreiben, wurden eliminiert. Das primäre Ziele des ISK-360° ist ein Vergleich zwischen dem Selbstbild einer Person sowie einem oder mehreren Fremdbildern derselben Person, die von anderen Menschen generiert werden. Zusätzlich ist ein Vergleich des Selbstbildes mit Normwerten möglich.
Zuverlässigkeit: Die Konsistenzkoeffizienten variieren in der Normierungsstichprobe zwischen .70 und .79 für den Selbstbildfragebogen bzw. zwischen .77 und .87 für den Fremdbildfragebogen. Die Retest-Koeffizienten wurden an einer Stichprobe von 53 Studierenden im zeitlichen Abstand von etwa 6 Monaten ermittelt. Sie variieren zwischen .75 und .84 für den Selbstbildfragebogen bzw. zwischen .77 und .93 für den Fremdbildfragebogen.
Testgültigkeit: Die Überprüfung der faktoriellen Validität bestätigt die vierfaktorielle Struktur. Umfangreiche Validitätsstudien belegen die konvergente Validität in Bezug auf ausgewählte Persönlichkeitsmerkmale wie etwa die »Big 5«. Es liegen Befunde zur Stützung der kriterienbezogenen Validität in Bezug auf Arbeitszufriedenheit, Commitment und Arbeitsleistung vor.
Normen: Der Selbstbildfragebogen wurde an 3.600 Personen normiert. Es liegen Prozentrangwerte, Stanine-Werte und T-Werte für die Gesamtstichprobe, Frauen, Männer, Nichtberufstätige, Berufstätige ohne Führungsaufgaben, Führungskräfte und Selbstständige vor.
Bearbeitungsdauer: Die Bearbeitungszeit eines Fragebogens beträgt maximal 10 Minuten.

Ist Coaching wirksam?

Kaum eine Methode der Personalentwickung hat in den letzten Jahren so viel Aufmerksamkeit auf sich gezogen, wie das Coaching. Viele tausend Menschen bieten Dienstleistungen als Coach an und noch mehr Menschen lassen sich coachen. Doch wie wirksam ist Coaching wirklich?
Passive manuelle Bewegungen von Gelenk- und Neuralstrukturen werden häufig mit einem ersten (R 1 = erster fühlbarer Widerstand innerhalb einer passiven Bewegung) und zweiten Widerstand (R2 = fühlbarer Widerstand am Bewegungsende) gemessen.
Die vorliegende Arbeit gibt einen Überblick über die Zuverlässigkeit der Messung von R1 und R2. In den Datenbanken PubMed (Medline), The Cochrane Database, CINAHL und Web of Science wurde in einer systematischen Literaturrecherche nach Diagnostikstudien zu R1 und R2 gesucht und die eingeschlossenen Studien mit dem Quality Appraisal of Reliability Studies Tool (QAREL) bewertet.
Die Ergebnisse zeigten widersprüchliche Reliabilität bei neurodynamischen Tests. Passive Außenrotationen bei Schulterpathologien erzielten eine gute, passive akzessorische intervertebrale Bewegungen (PAIVM) eher eine geringere Reliabilität.
R2 scheint eine höhere Reliabilität als R1 aufzuweisen, und die Intrarater-Reliabilität bei neurodynamischen Tests besser zu sein als die Interrater-Reliabilität. Für die geringe Reliabilität werden verschiedene Ursachen diskutiert. Trotz der methodischen Mängel in einigen Studien scheinen Tests mit größerem Hebel eine höhere Reliabilität zu erreichen als PAIVM.

Hintergrund
Ziel dieser Studie ist es, die Auswirkungen einer Kinesiophobie auf die Emotions- sowie Lateralitätserkennung zu untersuchen.
Material und Methoden
67 Probanden mit chronischen muskuloskeletalen Schmerzen wurden untersucht. Hierbei erzielten 24 Probanden einen Wert von >37 auf der Tampa-Skala für Kinesiophobie und wurden in die Querschnittstudie eingeschlossen. Die Fähigkeit zur Erkennung mimisch codierter Basisemotionen wurde mittels des Facially-Expressed-Emotion-Labeling(FEEL)-Tests und die Lateralitätserkennung anhand eines speziellen Face-Mirroring-Assessment-and-Treatment-Programms ermittelt. Die Toronto-Alexithymie-Skala (TAS) 26 diente zur Erfassung einer Alexithymie.
Ergebnisse
Der FEEL-Score der Probanden mit Kinesiophobie war signifikant niedriger (p = 0,019). Die Basisemotionen Angst (p = 0,026), Ärger (p = 0,027) und Überraschung (p = 0,014) verdeutlichten einen signifikanten Unterschied zwischen den Ergebnissen der Querschnittstudie und gesunden Probanden. Der Alexithymiefragebogen TAS-26 zeigte lediglich in der TAS 1 (Schwierigkeiten bei der Identifikation von Gefühlen; p = 0,008) einen signifikanten Unterschied zwischen Probanden mit Kinesiophobie.
Schlussfolgerung
Die Ergebnisse zeigen, dass kinesiophobische Patienten Veränderungen in der Emotions- und Lateralitätserkennung sowie alexithyme Merkmale aufweisen.
Ist Liebe im Büro verboten?

Einleitung
Lymphödeme können als direkte Erkrankung oder als Symptome bei vielen Krankheiten auftreten. Die gängigste Methode zur Behandlung von Lymphödemen ist die Komplexe Physikalische Entstauungstherapie (KPE), die sich aus Manueller Lymphdrainage (MLD), Kompression, Bewegungsübungen und Hautpflege zusammensetzt. In Deutschland wird meist nur MLD verschrieben. Das Ziel dieses systematischen Reviews ist es, die Effektivität der MLD zu überprüfen.
Methodik
Es wurde eine systematische Literaturrecherche durchgeführt und zusätzlich Kontakt zur Deutschen Gesellschaft für Lymphologie (DGL) aufgenommen. Die Studien wurden anhand von Kriterien der Cochrane Collaboration auf ihr Evidenzlevel hin überprüft und die methodische Qualität mithilfe der PEDro-Skala beurteilt.
Ergebnisse
37 in diese Übersichtsarbeit eingeschlossene Studien umfassen die Behandlungsmethoden MLD alleine, MLD in Kombination mit Kompression und MLD mit zusätzlicher Physiotherapie sowie mechanische Druckkompression und KPE. Die Evidenzlevel waren unterschiedlich und die methodische Qualität der Studien mäßig. Alle Studien zum Thema Lymphödembehandlung bei Brustkrebspatienten zeigten gute Erfolge, unabhängig von der Art der Intervention.
Schlussfolgerungen
Während zur Behandlung von Lymphödemen infolge von Brustkrebs einige Studien vorhanden sind, gibt es zu allen anderen Krankheitsbildern, die im Heilmittelkatalog angegeben sind, kaum Studien. Hier wird dringend weitere Forschung benötigt.
Background: IT adoption is a process that is influenced by different external and internal factors. This study aimed
1. to identify similarities and differences in the prevalence of medical and nursing IT systems in Austrian and German hospitals, and
2. to match these findings with characteristics of the two countries, in particular their healthcare system, and with features of the hospitals.
Methods: In 2007, all acute care hospitals in both countries received questionnaires with identical questions. 12.4% in Germany and 34.6% in Austria responded.
Results: The surveys revealed a consistent higher usage of nearly all clinical IT systems, especially nursing systems, but also PACS and electronic archiving systems, in Austrian than in German hospitals. These findings correspond with a significantly wider use of standardised nursing terminologies and a higher number of PC workstations on the wards (average 2.1 PCs in Germany, 3.2 PCs in Austria). Despite these differences, Austrian and German hospitals both reported a similar IT budget of 2.6% in Austria and 2.0% in Germany (median).
Conclusions: Despite the many similarities of the Austrian and German healthcare system there are distinct differences which may have led to a wider use of IT systems in Austrian hospitals. In nursing, the specific legal requirement to document nursing diagnoses in Austria may have stimulated the use of standardised terminologies for nursing diagnoses and the implementation of electronic nursing documentation systems. Other factors which correspond with the wider use of clinical IT systems in Austria are: good infrastructure of medical-technical devices, rigorous organisational changes which had led to leaner processes and to a lower length of stay, and finally a more IT friendly climate. As country size is the most pronounced difference between Germany and Austria it could be that smaller countries, such as Austria, are more ready to translate innovation into practice.

Die Ergebnisse des IT-Reports Gesundheitswesen zeigen, dass der Pro-zess der Visitenvorbereitung, -durch-führung und -nachbereitung am besten durch IT unterstützt wurde, gefolgt von der OP- Vorbereitung, der OP-Nachbereitung und schließlich der Entlassung (Abbildung l). Von möglichen zehn Punkten in dem jeweiligen Prozess-Score erreichte im Mittel nur die Visite einen Wert über 6,0. Mit 5,3 erzielte der Entlassungsprozess einen deutlich niedrigeren Wert.
Health IT and communication systems are indispensable in German hospitals for clinical as well as administrative process support. However, IT is often regarded as a “black box” for hospital CEOs. Thus, the question arises how can CEOs decide if they do not know what is in the box? In order to answer this question, half-structured interviews with 14 German hospital CEOs were conducted. They revealed three principle decision processes: the supported decision, the joint decision and the corporate level decision. In all cases, the hospital CEO and the CIO interacted to reach the final decision, most strongly in the joint decision mode and least strongly in the corporate decision mode. Only the joint decision mode definitely forced the CEO to open the “black box” of IT. In the era of digitalisation, however, CEOs must develop better competencies to decide over complex matters.
Although user participation may facilitate the realisation of IT innovations, various literature analyses show only minimal to moderate evidence for such effects possibly due to disregard of mediating factors. Against this background, this study examines the extent to which joint intrapreneurship of clinical leaders and IT leaders as well as a distinct innovation culture mediate the effect of user participation on hospitals’ IT innovativeness. IT innovativeness was measured by the availability and usability of IT functions and by the perceived ‘innovative power’ of a hospital. An empirical model was developed and tested with data from 168 clinical leaders and IT leaders who participated pairwise in a survey representing 84 German hospitals. Three parallel mediation analyses indicated that the participation of users could only lead to IT innovativeness if they were accompanied by intrapreneurial leadership on the part of clinical directors and IT leaders and if a pronounced innovation culture prevailed.

Benchmarking, sprich die Vergleichsanalyse von Prozessen mit festgelegtem Bezugswert, findet zunehmend Einzug in die Welt der Gesundheits-IT. Dabei spielen jedoch viele Faktoren zusammen, die einen einfachen Vergleich von IT-Kosten bei Weitem übersteigen. Eine Forschungsgruppe der Hochschule Osnabrück hat mit dem IT-Benchmark Gesundheitswesen ein Analysetool vorgelegt, das auch einen Länder- vergleich ermöglicht.
Die Verbreitung von Informationstechnologien (IT) im Gesundheitswesen sowie deren Einflussgrößen sind Betrachtungsobjekt der Adoptions- und Diffusionsforschung. Neues Wissen aus diesen Studien wird dabei häufig als summative Umfrageergebnisse disseminiert. Mit dem in diesem Beitrag vorgestellten Web-Portal werden die individuellen Umfrageergebnisse im Vergleich zu einer Referenzgruppe präsentiert. Das erfolgt in flexibler Form unter Verwendung von reliablen und validen Kennzahlen der IT-Prozessunterstützung, die in einer hierarchischen Struktur angeordnet sind. Es werden die Entwicklung des Web-Portals als Benchmarking Instrument, seine Anwendung und eine initiale Evaluation vorgestellt. Es zeigte sich, dass das Web-Portal anhand aktueller Benchmarking-Ergebnisse von 197 Krankenhäusern einsetzbar ist, seine Anwendung als nützlich und die Indikatoren als verständlich eingeschätzt werden.
Due to the emerging evidence of health IT as opportunity and risk for clinical workflows, health IT must undergo a continuous measurement of its efficacy and efficiency. IT-benchmarks are a proven means for providing this information. The aim of this study was to enhance the methodology of an existing benchmarking procedure by including, in particular, new indicators of clinical workflows and by proposing new types of visualisation. Drawing on the concept of information logistics, we propose four workflow descriptors that were applied to four clinical processes. General and specific indicators were derived from these descriptors and processes. 199 chief information officers (CIOs) took part in the benchmarking. These hospitals were assigned to reference groups of a similar size and ownership from a total of 259 hospitals. Stepwise and comprehensive feedback was given to the CIOs. Most participants who evaluated the benchmark rated the procedure as very good, good, or rather good (98.4%). Benchmark information was used by CIOs for getting a general overview, advancing IT, preparing negotiations with board members, and arguing for a new IT project.
A structured hospital discharge management process can lead to a smoother transition to aftercare. In practice, providing continuity of nursing care after a stationary hospital stay is accompanied by numerous challenges. The presented study aims to point out the use cases and requirements for an IT system supporting the diverse tasks of the participating actors. Within the scope of a multi-method research design, the authors conducted and analysed stakeholder interviews, a shadowing, a systematic literature search and statutes in order to gain the presented results. This publication presents 37 requirements, grouped to 14 use cases. A process model in BPMN visualises the discharge management process. Further, the authors derived implications for practice and research. These can be used for the development, classification and assessment of IT systems. Therefore, this publication provides a significant contribution to the development of socio-technical systems within the health care domain.
