Geburtshilfe neu denken - Bericht zur Situation und Zukunft des Hebammenwesens in Deutschland

Schwangerschaft, Geburt, Wochenbett und Stillzeit sind vitale Ereignisse im Leben einer Frau und einer Familie und primär physiologische. Deshalb sollten in der Gesundheitsversorgung diejenigen Leistungen und Handlungsansätze stärker berücksichtigt werden, die auf die Unterstützung und den Erhalt von Gesundheit gerichtet sind. Die Lebensphase von Schwangerschaft, Geburt, Wochenbett und Stillzeit stellt für die Eltern und insbesondere für die Mutter eine biographische Übergangssituation dar, die Einfluss auf ihre zukünftige gesundheitliche Verfassung und ihr gesundheitsrelevantes Verhalten hat. Sie eröffnet Möglichkeiten, gesundheitsförderliche Ressourcen zu entdecken und im familiären Leben weiterzuentwickeln. Die gesundheitliche Versorgung sollte von einer individuellen und durchgängigen Betreuung von Frauen und ihren Kindern geprägt sein.
Der Bericht macht mit seiner Analyse darauf aufmerksam, dass der Berufsstand der Hebamme für die Bereiche Gesundheitsförderung und Prävention ein Potenzial bietet, welches gesellschaftlich wesentlich mehr als bisher genutzt und gefördert werden muss.
Als Fachfrauen für Geburtshilfe verfügen Hebammen über die Kompetenz und die rechtliche Befugnis, eigenverantwortlich Geburten zu betreuen. Die Verlagerung der Geburten in die Klinik führte zu der Entwicklung, dass auch physiologische Geburten unter ärztliche Leitung gestellt wurden. Dieses wurde begleitet durch eine zunehmende Medikalisierung und Pathologisierung der physiologischen Lebensphase von Schwangerschaft, Geburt, Wochenbett und Stillzeit.
Im europäischen Ausland wurde auf diese internationale Entwicklung bereits vor 15 Jahren mit gesundheitspolitischen Maßnahmen reagiert. Im klinischen Setting sollte die physiologische Geburt durch die Einrichtung von Midwife-Led-Units eigenverantwortlich von Hebammen geleitet werden. Studien, in denen dieses Betreuungskonzept untersucht wurde, belegen, dass gesunde Frauen im Hebammenkreißsaal genauso sicher gebären wie in den üblichen ärztlich
geleiteten Kreißsälen. Darüber hinaus sind die Frauen mit der Betreuung im Hebammenkreißsaal besonders zufrieden und die Rate der operativen Eingriffe ist niedrig. Zudem eröffnet es neue Wege der interdisziplinären Zusammenarbeit im geburtshilflichen Team. Angeregt von diesen positiven Aspekten des Konzeptes Hebammenkreißsaal will der Verbund Hebammenforschung das Modell wissenschaftlich gestützt auch in Deutschland in die Praxis umsetzen und hierzulande möglichst vielen Frauen zugänglich machen. Das vorliegende Handbuch „Der Hebammenkreißsaal – Von der Idee zur Umsetzung“ richtet sich an das interessierte Fachpublikum und Hebammenkolleginnen, die die Idee eines erweiterten geburtshilflichen Versorgungskonzeptes aufgreifen und in die klinische Praxis umsetzen möchten.
Continuity of care is a concept that is defined as the uninterrupted and coordinated care provided to a patient and that includes an informational dimension which describes the information exchange between the parties involved. In nursing, the nursing summary is the main instrument to ensure informational continuity of care. The aim of this paper is to present an HL7 Clinical Document Architecture based document standard for the eNursing Summary and to discuss the need for harmonizing these results at international level. The eNursing Summary proposed in this paper was developed on the basis of several internationally accepted concepts, primarily the nursing process, the ISO 18104 Reference Terminology Model for Nursing and various data sets. The standardisation process embraced several phases of involving nursing experts for validating its structure and content. It was finally evaluated by a network of 100 healthcare organizations. We argue that the eNursing Summary is a good starting point for standardising nursing discharge and transfer documents on a global level. However, further work is needed to bring together the different national and international strands in standardisation.
Background: IT adoption is a process that is influenced by different external and internal factors. This study aimed
1. to identify similarities and differences in the prevalence of medical and nursing IT systems in Austrian and German hospitals, and
2. to match these findings with characteristics of the two countries, in particular their healthcare system, and with features of the hospitals.
Methods: In 2007, all acute care hospitals in both countries received questionnaires with identical questions. 12.4% in Germany and 34.6% in Austria responded.
Results: The surveys revealed a consistent higher usage of nearly all clinical IT systems, especially nursing systems, but also PACS and electronic archiving systems, in Austrian than in German hospitals. These findings correspond with a significantly wider use of standardised nursing terminologies and a higher number of PC workstations on the wards (average 2.1 PCs in Germany, 3.2 PCs in Austria). Despite these differences, Austrian and German hospitals both reported a similar IT budget of 2.6% in Austria and 2.0% in Germany (median).
Conclusions: Despite the many similarities of the Austrian and German healthcare system there are distinct differences which may have led to a wider use of IT systems in Austrian hospitals. In nursing, the specific legal requirement to document nursing diagnoses in Austria may have stimulated the use of standardised terminologies for nursing diagnoses and the implementation of electronic nursing documentation systems. Other factors which correspond with the wider use of clinical IT systems in Austria are: good infrastructure of medical-technical devices, rigorous organisational changes which had led to leaner processes and to a lower length of stay, and finally a more IT friendly climate. As country size is the most pronounced difference between Germany and Austria it could be that smaller countries, such as Austria, are more ready to translate innovation into practice.
The last two decades have been characterized by a fundamental change in the approaches, tools and instruments in the quality management at Higher Education Institutions. Comparison of two Higher Education Institutions in the Slovak Republic and Germany highlights similarities and trends in quality assurance. Both case studies show how multifaceted the quality management is, and the needs to be approached if a meaningful progress is to be made. Complexity has to be explicitly recognized and built into the approach chosen. Higher Education Institutions have to develop internal quality assurance processes. Quality culture is key for addressing the challenges.
OBJETIVOS: Compreender o significado do cuidado de enfermagem como prática social empreendedora.
MÉTODOS: Foi baseada na Grounded Theory que, de forma sistemática, criativa e interativa possibilitou o desenvolvimento da teoria: "Vislumbrando o cuidado de enfermagem como prática social empreendedora". Amostra teórica constituiu-se de 35 sujeitos entrevistados, distribuídos em diferentes grupos amostrais. RESULTADOS: O cuidado de enfermagem como prática social empreendedora está associado ao sistema de relações e interações, à capacidade de interagir com os diferentes atores sociais, na capacidade de criar novos canais de comunicação e ações pró-ativas.
CONCLUSÃO: A partir do cuidado como prática social empreendedora é possível atuar de forma pró-ativa, inovadora e participativa, sem desconsiderar as contradições sociais emergentes.
Estudo qualitativo que partiu da questão: como vêm sendo construídas as teorias e modelos de cuidado de enfermagem, focalizando o processo de construção da teoria substantiva, referente à Tese que teve como objetivos compreender o significado do ambiente de cuidados em Unidade de Terapia Intensiva e construir um modelo teórico sobre ele. O método utilizado foi a Grounded Theory. Realizaram-se 39 entrevistas com 47 sujeitos diferenciados de três Unidades de Terapia Intensiva Adulto, em Florianópolis/SC, Santa Maria/RS e Pelotas/RS, entre junho de 2009 a setembro de 2010. A teoria "Sustentando a vida no ambiente complexo de cuidados em Unidade de Terapia Intensiva" foi delimitada por oito categorias. Conclui-se que sustentar a vida no ambiente de Unidade de Terapia Intensiva significa investir intensivamente no cuidado de pacientes instáveis, com auxílio de tecnologias diferenciadas e profissionais capacitados, trabalhando em equipe, onde se convive com estresse/conflitos e dificuldades para lidar com a morte.
Objectives: This paper addresses recent steps for reforming the eligibility criteria of the German long-term care insurance that have been initiated to overcome shortcomings in the current system.
Methods: Based on findings of a survey of international long-term care systems, assessment tools and the relevant literature on care needs a new tool for determining eligibility in the German long-term care insurance was developed.
Results: The new tool for determining long-term care eligibility broadens the understanding of what ‚dependency on nursing care' implies for the person affected. The assessment results in a degree of dependency from personal help provided by formal or informal caregivers. This degree of dependency can be used for determining eligibility for and the amount of long-term care benefits.
Discussion: The broader understanding of "dependency on nursing care' and the new tool are important steps to adapt the German long-term care insurance to the challenges of the demographic and societal changes in the future
Characteristics of German Hospitals Adopting Health IT Systems : Results from an Empirical Study

Hospital characteristics that facilitate IT adoption have been described by the literature extensively, however with controversial results. The aim of this study therefore is to draw a set of the most important variables from previous studies and include them in a combined analysis for testing their contribution as single factors and their interactions. Total number of IT systems installed and number of clinical IT systems in the hospital were used as criterion variables. Data from a national survey of German hospitals served as basis. Based on a stepwise multiple regression analysis four variables were identified to significantly explain the degree of IT adoption (60% explained variance): 1) hospital size, 2) IT department, 3) reference customer and 4) ownership (private vs. public). Our results replicate previous findings with regard to hospital size and ownership. In addition our study emphasizes the importance of a reliable internal structure for IT projects (existence of an IT department) and the culture of testing and installing most recent IT products (being a reference customer). None of the interactions between factors was significant.
In der öffentlichen Wahrnehmung gilt die Universität Duisburg-Essen als Best-practice-Beispiel für das Deutschlandstipendien-Programm, das seit dem Jahr 2011 bundesweit leistungsstarke Studierende fördert und auf Basis des nahezu baugleichen NRW-Stipendiums entwickelt wurde. Zur Finanzierung der Stipendienprogramme dienen Reverse Matching Funds-Konstruktionen, innerhalb derer die durch private Förderer zur Verfügung gestellten Spenden durch Staatsmittel bis zu einer festgelegten Obergrenze verdoppelt werden. Die Hochschulen werden dadurch motiviert, stärker in das in Deutschland gegenüber dem internationalen Vergleich bisher stark unterentwickelte Hochschulfundraising zu investieren, um die benötigten privaten Förderer zu akquirieren und an die Hochschule zu binden. Als Finanzierungs- und Steuerungsinstrument ist das Matching Funds-Stipendienprogramm seiner Architektur nach ein Hybridwesen aus Elementen des Hochschulfundraisings und von Public Private Partnerships, zu denen beiden es bereits zahlreiche wissenschaftliche Untersuchungen gibt - anders als zum Phänomen der voraussetzungsreichen Matching Funds.
Die vorliegende Studie erörtert zunächst die Finanzierung staatlicher Hochschulen in Deutschland, um die drei Finanzierungsinstrumente Fundraising, Public Private Partnership und Matching Funds darin einordnen zu können. Die drei Instrumente werden umfangreich beschrieben und analysiert, um aufgrund des bestehenden Forschungsdesiderats zu Matching Funds einen selbstentwickelten, geschlossenen Kriterien-Katalog aufzustellen, der als Bewertungsraster der Untersuchung dienen soll. Der Darstellung der Matching Funds-Architektur des Stipendienprogramms und seiner konkreten Umsetzung an der Universität Duisburg-Essen als Fallbeispiel folgt eine kritische Analyse und Bewertung sowohl der Matching Funds-Konstruktion als solcher, als auch des vermeintlichen Best-practice-Beispiels.
Objective: To understand the significance of healthy living for users, professionals and managers of the Family Health Strategy (FHS) team.
Methods: Research of a qualitative nature, based on grounded theory. For data collection, interviews were conducted with 25 participants, including users, professionals and managers of a FHS team, during the period between March and December, 2009. Results: The collection and analysis of data was conducted in a systematic and comparative manner, demonstrating that healthy living can be characterized as a selforganizing process, mediated by the action of the FHS team professionals, especially by the community health agent, through creation of bonds of trust and stimulation of interactions and community associations. Conclusion: We concluded that healthy living is a singular phenomenon, complex, interactive, associative, political and social, coupled with the active involvement and participation of the users and by the engagement of effective and socially responsible professionals, managers and established political authorities.
O presente estudo objetivou compreender o significado da atuação profissional de uma equipe ESF em uma comunidade socialmente vulnerável. „Kompetenz und Kompetenzentwicklung jenseits des curricularen Tellerrandes – Möglichkeiten von Supervision und Coaching für Studierende“ Lernen in Hochschule führt häufig zu trägem Wissen. Darunter wird theoretisches Wissen verstanden, das für die Lösung komplexer, realitätsnaher Probleme nicht genutzt werden kann. Somit entsteht eine Kluft zwischen Wissen und Handeln im Rahmen der hochschulischen Studienproramme. Gruber et al. resümieren: „Das gewissermaßen ‚in vitro’ erworbene Wissen kann zwar im universitätsanalogen [und hochschulanalogen, Anm. d. Verf.] Kontext, in dem es erworben wurde, genutzt werden, etwa bei Prüfungen: in komplexen analogen Problemsituationen gelingt die Wissensanwendung jedoch nur unvollständig oder überhaupt nicht.“ (vgl. Gruber et al., 2000). Supervision und Coaching als Beratungsformate können möglicherweise sinnvolle Instrumente im studienbegleitenden Kontext sein, um ergänzend zu den curriclar angebotenen Lehrveranstaltungen Studierende in Fragen der Studieniorganisation, der Karrieplanung und Rollenfindung zu unterstützen. Nach einer differenzierten Einführung in die Beratungsformaten Supervision und Coaching und einer Diskussion des Kompetenzbegriffes schließt sich die Darstellung einer Forschungsarbeit an, die die Kompetenzerwerbserwartung von Studierenden verschiedenener Studiengänge in unterschiedlichen Fakultäten der Hochschule Osnabrück durch studienbegleitende Supervion und Coaching beleuchtet und breit angelegt diskuttiert. Fazitierend endet die Arbeit mit Schlussfolgerungen und Handungsempfehlungen zum Einsatz von studienbegleitenden Beratungsangeboten für den hochschulischen Kontext.
Für die meisten Studierenden ist ein erfolgreiches Studium Motivation und Ziel zugleich. Erfolgreiche Absolventen und Absolventinnen sind nicht nur für die Hochschule und Universitäten ein „Aushängeschild“. Ebenso profitiert die Gesellschaft von den Investitionen in die Bildung junger Erwachsener, wenn die ausgebildeten Fachkräfte in den Arbeitsmarkt und das Wirtschaftssystem eingebunden werden. In der geplanten Studie „Bedingungen von Studienerfolg“ steht die Erfassung der zentralen Einflussfaktoren auf den Studienerfolg im Fokus. Erst wenn die zentralen Einflüsse bekannt sind, kann der Studienerfolg effektiv gefördert, gesteuert und Studierende unterstützt werden. Dies scheint aktuell von Relevanz, da in Deutschland niedrige Studienanfängerquoten und hohe Studienabbruchraten zu verzeichnen sind. Kritische Stimmen stellen den Bologna Prozess schon längst in Frage, unter anderem weil sich die Studierenden mit den neuen Anforderungen, wie z. B. zu wenig optionale Lehrveranstaltungen, hoher Prüfungsdruck durch die Menge und studienbegleitenden Prüfungen, zu dichte Stundenpläne oder ungenügende Möglichkeiten für Selbststudium, überfordert fühlen (vgl. Banscherus et al., 2009).In der Langzeitstudie „Bedingungen von Studienerfolg“ besteht die allgemeine Forschungsfrage darin, was die die zentralen Einflussfaktoren auf den Studienerfolg sind. Zur Beantwortung der aufgeführten Forschungsfrage werden zwei aufeinander folgende empirische Studien geplant und umgesetzt: Interviewstudie und Fragebogenstudie. Die im Vorfeld der schriftlichen Befragung durchgeführte Interviewstudie dient dazu, einen explorativen Einblick in die Thematik an der Hochschule Osnabrück zu erhalten.
O presente estudo, de caráter teórico, objetivou analisar e discutir um possível código binário para o sistema de Enfermagem, no sentido de identificar o seu próprio saber, na perspectiva dos pressupostos teóricos de Niklas Luhmann. Apostar em uma comunicação funcionalmente diferenciada e socialmente relevante para o sistema de enfermagem implica em transcender o tradicional código saúde-doença, predominante no sistema de saúde e cuja comunicação socialmente relevante é a doença. Implica, ainda, em investir proativamente na promoção e proteção do viver saudável de indivíduos, famílias e comunidades, para que a saúde seja o ponto central das discussões e intervenções.
Die verfasste Bachelorarbeit verknüpft in Form der "Offenen Kinder- und Jugendarbeit" sowie dem Aspekt der "Migration", zwei relevante Bereiche, die in der Sozialen Arbeit wesentlich thematisiert werden. Beide Hauptthemenschwerpunkte dieser Arbeit basieren auf einem Facettenreichtum ihrer Spezifika. Auf Grundlage der Tatsache, dass das Jugendzentrum des "Stadtteiltreff Haste" in Osnabrück, einen Besucheranteil von ca. 90% von Kindern und Jugendlichen mit Migrationshintergrund aufweist, befasst sich diese Arbeit mit der Frage, wie so ein großer prozentualer Anteil an Jugendlichen mit Migrationsstatus als Besucherklientel eines Jugendzentrums zustande kommt. Mittels einer im Stadtteiltreff Haste durchgeführten, quantitativen Forschung (Befragung per Fragebogen) von insgesamt 55 befragten Kindern und Jugendlichen mit Migrationshintergrund, sollte herausgefunden werden, wovon aus der Perspektive der Besucher/-innen, eine erfolgreiche offene Kinder- und Jugendarbeit mit dem Migrationsklientel abhängig ist.
Die Nutzung des sozialen Online-Netzwerkes „Facebook“ ist ein wichtiger Bestandteil in der
Lebenswelt von Jugendlichen und jungen Erwachsenen, welcher viele Vorteile, jedoch auch
Nachteile mit sich bringt. Der theoretische Teil dieser Arbeit setzt sich mit dem Begriff der
Jugend auseinander, stellt besondere Merkmale dieser Lebensphase dar und erläutert das
Konzept der Entwicklungsaufgaben, um in diesem Zusammenhang den Bezug zum sozialen
Online-Netzwerk „Facebook“ herzustellen. Anschließend wird der Begriff des sozialen Online
Netzwerkes bestimmt, die Geschichte beziehungsweise die Erfolge von Facebook genauer
thematisiert und eine Auswahl an Funktionen vorgestellt, um den Leser in das Themenfeld
einzuführen. Der zweite Teil der Arbeit beinhaltet die empirische Untersuchung, um
herauszufinden in welcher Alterskategorie der Hilfebedarf zur Unterstützung und Begleitung
seitens der Sozialen Arbeit am größten ist. Anhand der empirischen Methode des
Fragebogens wurden Daten erhoben, um die Forschungsfrage zu beantworten, wie
Jugendliche und junge Erwachsene im Alter von 13 bis 25 Jahren Facebook nutzen.
Bezüglich der Konkretisierung der Fragestellung wurden sechs Hypothesen hergeleitet, um
verschiedene Teilbereiche der Facebok-Nutzung, wie zum Beispiel das Datenschutzthema
oder die täglich verbrachte Zeit, genauer untersuchen zu können. Die Stichprobe von N=
108, die im Schneeballsystem erhoben wurde, setzt sich aus Teilnehmern jeglichen
Geschlechts, Alters oder sozialer Schicht zusammen. Damit die Teilnehmer miteinander
verglichen werden können, wurden sie in drei verschiedene Kategorien von 13 bis 15
Jahren, 16 bis 18 Jahren und 19 bis 25 Jahren, unterteilt.
Als Resultat der empirischen Untersuchung wird deutlich, dass sich die drei verschiedenen
Alterskategorien nur in einigen Themenbereichen unterschieden und ansonsten ähnliche
Ansichten teilen. Lediglich die Alterskategorie der 16-18jährigen nimmt eine besondere Rolle
ein, da sie sich am stärksten von den anderen beiden Kategorien unterscheidet, da zum
Beispiel alle Teilnehmer dieser Kategorie bei Facebook einen Account besitzen und täglich
auch am längsten online sind.
Dadurch ergibt sich für die Soziale Arbeit ein Handlungsbedarf, der an kein bestimmtes Alter
gebunden ist, sondern sich an alle Alterskategorien richten sollte.
Handovers need a common ground on the clinical cases between the members of the successive shifts to establish continuity of care. Conventional electronic patient record systems (EHR) proved to be only insufficiently suitable for supporting the grounding process. Against this background we proposed a basic concept for a handover EHR that extends general EHRs in particular openEHR based systems. The resulting handover information model was implemented in a database and evaluated based on 120 clinical cases. The information items of these cases could be mapped successfully to the model, however, the new class “anticipatory guidance” needed to be introduced. The evaluation also demonstrated the importance of highly aggregated information on the clinical case, opinions and meta-information such as the relevance of an item during handovers. Based on these findings, in particular the handover database, handover EHR applications are currently developed to support the grounding process.
Due to the emerging evidence of health IT as opportunity and risk for clinical workflows, health IT must undergo a continuous measurement of its efficacy and efficiency. IT-benchmarks are a proven means for providing this information. The aim of this study was to enhance the methodology of an existing benchmarking procedure by including, in particular, new indicators of clinical workflows and by proposing new types of visualisation. Drawing on the concept of information logistics, we propose four workflow descriptors that were applied to four clinical processes. General and specific indicators were derived from these descriptors and processes. 199 chief information officers (CIOs) took part in the benchmarking. These hospitals were assigned to reference groups of a similar size and ownership from a total of 259 hospitals. Stepwise and comprehensive feedback was given to the CIOs. Most participants who evaluated the benchmark rated the procedure as very good, good, or rather good (98.4%). Benchmark information was used by CIOs for getting a general overview, advancing IT, preparing negotiations with board members, and arguing for a new IT project.
Die Frage, ob Fachhochschulen ein Promotionsrecht erhalten sollen, hat weitreichende Folgen für die Hochschullandschaft und das Wissenschaftssystem in Deutschland.Prof. Dr. Nicolai Müller-Bromley, Präsident des Hochschullehrerbundes, nimmt zu dieser wissenschaftspolitischen Herausforderung Stellung und erläutert wichtige Argumente, die für ein Promotionsrecht von Fachhochschulen sprechen.
Background:
One of the main problems of Internet-delivered interventions for a range of disorders is the high dropout rate, yet little is known about the factors associated with this. We recently developed and tested a Web-based 6-session program to enhance motivation to change for women with anorexia nervosa, bulimia nervosa, or related subthreshold eating pathology.
Objective:
The aim of the present study was to identify predictors of dropout from this Web program.
Methods:
A total of 179 women took part in the study. We used survival analyses (Cox regression) to investigate the predictive effect of eating disorder pathology (assessed by the Eating Disorders Examination-Questionnaire; EDE-Q), depressive mood (Hopkins Symptom Checklist), motivation to change (University of Rhode Island Change Assessment Scale; URICA), and participants’ age at dropout. To identify predictors, we used the least absolute shrinkage and selection operator (LASSO) method.
Results:
The dropout rate was 50.8% (91/179) and was equally distributed across the 6 treatment sessions. The LASSO analysis revealed that higher scores on the Shape Concerns subscale of the EDE-Q, a higher frequency of binge eating episodes and vomiting, as well as higher depression scores significantly increased the probability of dropout. However, we did not find any effect of the URICA or age on dropout.
Conclusions:
Women with more severe eating disorder pathology and depressive mood had a higher likelihood of dropping out from a Web-based motivational enhancement program. Interventions such as ours need to address the specific needs of women with more severe eating disorder pathology and depressive mood and offer them additional support to prevent them from prematurely discontinuing treatment.
Multiple-group confirmatory factor analysis (MG-CFA) is among the most productive extensions of.structural equation modeling. Many researchers conducting cross-cultural or longitudinal studies are interested in testing for measurement and structural invariance. The aim of the present paper is to provide a tutorial in MG-CFA using the freely available R-packages lavaan, semTools, and semPlot. The combination of these packages enable a highly efficient analysis of the measurement models both for normally distributed as well as ordinal data. Data from two freely available datasets – the first with continuous the second with ordered indicators - will be used to provide a walk-through the individual steps.
The article describes an analysis of the use of e-learning to improve the learning transfer to practice in continuing education. Therefore an e-learning offer has been developed as a part between two attendance periods of a training course in the field of Ambient Assisted Living (AAL). All participants of the course were free to use the e-learning offer. After the end of the e-learning part we compared the e-learning users to the other participants. Using an online questionnaire we explored if there are differences in the activities in the field AAL after the training course. The results show that e-learning is beneficial especially for communication processes. Due to the fact that the possibility to talk about the learning content is an essential factor for the learning transfer, e-learning can improve the learning success.
Angesichts der wachsenden Zahl chronisch erkrankter Menschen auch in der reproduktiven Lebensphase gewinnt das Thema der Versorgung betroffener Frauen in Schwangerschaft, Geburt und Wochenbett an Aktualität. Frauen mit chronischen Erkrankungen müssen in der Zeit rund ums Mutterwerden sowohl die grundsätzlichen Herausforderungen dieser Lebensphase als auch die Besonderheiten ihrer Erkrankung bewältigen. Ihr Erleben und Bewältigungshandeln in diesem Spannungsfeld ist Gegenstand der vorliegenden Dissertation. Die Ergebnisse sollen auch der Entwicklung angepasster Versorgungs-konzepte rund um die Geburt dienen.
Die Untersuchung wurde entsprechend der Grounded Theory Methodologie durchgeführt. Datengrundlage sind 27 Interviews mit Frauen, die ein Kind geboren haben und mindestens zwei Jahre vor der Schwangerschaft chronisch erkrankten. Die Auswertung der Daten und Identifizierung der Kategorien folgte den Prinzipien des Theoretischen Samplings, der komparativen Analyse und des begleitenden Schreibens von Memos.
In den Ergebnissen kann gezeigt werden, dass betroffene Frauen in der Schwangerschaft die Sicherstellung größtmöglicher Gesundheit in gleichzeitiger Sorge um zwei bedürftige Körper, des eigenen und den des Ungeborenen, zum Ziel haben. Allgemeine Bedingungen der Erkrankung wirken auf dieses Geschehen ein. Der Einsatz verschiedener Strategien wie die Beobachtung der körperlichen Veränderungen und des kindlichen Wachstums, des Generierens von Wissen und die Aushandlungsprozesse mit den Professionen des Gesundheitssystems führen zu einer positiven oder negativen Bewertung und Bewältigung der Situation. Diese münden entsprechend in einem Gefühl neuer Potenz und Selbstermächtigung oder in einer Irritation gegenüber dem Krankheitsmanagement und potenziellen Handlungsoptionen.
Für die Frauen mit chronischen Erkrankungen hat das Gesundheitssystem in der Phase des Mutterwerdens eine relevante Bedeutung. Dabei wird deutlich, dass die Anforderungen an die Krankheits-, Biografie- und Alltagsarbeit bei ihnen verstärkt erlebt werden. Auch unterscheidet sich ihr Erleben, trotz einer für fast alle Frauen geltenden risikoorientierten Schwangerenbetreuung, in der Ausprägung, Dringlichkeit und Komplexität von dem der Frauen ohne Krankheitsdiagnose oder mit akuten Schwangerschaftskomplikationen. Dies hat auch Auswirkungen auf die Beziehung zum Kind und die Wahrnehmung gesellschaft-licher Teilhabe und Normalität.
Angesichts mangelnder Wahlbeteiligung und zunehmender Politikverdrossenheit stellt sich die Frage nach alternativen Methoden zur Revitalisierung der deutschen Demokratie. Eine der hierzu entwickelten Theorien ist die der Liquid Democracy. Wo sich bisherige Forschung größtenteils mit politikwissenschaftlichen Fragen beschäftigt hat, soll diese Ausarbeitung einen Einblick in die rechtlichen – insbesondere die verfassungsrechtlichen -Fragen bieten, die Liquid Democracy aufwirft.
Dazu wird aus der sehr allgemeinen Liquid-Democracy-Theorie eine eigenständige Demokratietheorie, die Cooperative Liquid Democracy, entwickelt, in der neben einem Liquid-Democracy-Organ die kommunale Vertretung bestehen bleibt.
Es werden zudem auch Fragen des NKomVG, der NV und des GG beantwortet. Diese Ausarbeitung setzt seinen Fokus auf die Beantwortungder Frage nach der Abschaffbarkeit der Vertretung, der Machtverschiebung von der Vertretung auf die Bürger und nach der Möglichkeit der Durchführung von Online-Wahlen. Dabei werden aktuelle Urteile des Bundesverfassungsgerichtes berücksichtigt.
Der Autor setzt sich, zum Teil sehr kritisch, mit den Ausführungen der Gerichte und der Literatur auseinander und kommt zum Ergebnis der generellen Vereinbarkeit der Cooperative Liquid Democracy mit der Verfassung.
Zudem werden praxisnahe Empfehlungen und Hinweise gegeben, um die Theorie der Cooperative Liquid Democracy anschaulicher darzustellen und eine Umsetzung in der Praxis anzuregen.
Einleitung: Aktuelle Erkenntnisse der Faszienforschung zeigen, dass Faszien weitaus mehr Einfluss auf den menschlichen Bewegungsapparat haben, als bisher angenommen. Besonders die Fascia Thoracolumbalis (FTL) stellt eine große Faszie dar, deren patholo-gische Veränderungen zu Beschwerden im Bereich des unteren Rückens führen können. Auf der Grundlage dieser Erkenntnisse wurde die Behandlungsmethode mit der Foam Roll entwickelt, um Muskeln und Faszien gezielt zu behandeln und Beschwerden vorzubeugen. Beobachtungen zeigen, dass das Foam Rolling die Dehnfähigkeit der Muskulatur verbes-sert. Jedoch wurde bisher nicht untersucht, wie sich Übungen mit der Foam Roll auf die Beweglichkeit der Faszien auswirken. Ziel dieser Studie ist es daher, aufzuzeigen, ob das Foam Rolling Einfluss auf die Beweglichkeit der FTL hat. Zu diesem Zweck wurden sowohl die kurzfristigen, als auch die langfristigen Auswirkungen untersucht.
Methode: Es nahmen 38 Probanden an der Studie teil. Diese wurden in eine Foam Roll Gruppe, eine Placebo- und eine Kontrollgruppe eingeteilt. Das Bewegungsausmaß einer Rumpfflexion von 30° wurde mit Videos eines Ultraschallgerätes, als Grundlage der Unter-suchungen über die Beweglichkeit der FTL, aufgezeichnet. Anschließend erfolgte die Ana-lyse mit der Cross Correlation Software nach Dr. Andrew Dilley. Hierdurch konnte das Bewegungsausmaß der Faszie ermittelt werden. Neben der Faszienbeweglichkeit wurden Messungen bezüglich der lumbalen, sowie thoracolumbalen Flexion und der Mechanosen-sivität einzelner Muskeln durchgeführt. Zur Ermittlung der Werte gab es eine Messung vorab und zehn Minuten nach der Intervention. Die Messwerte wurden sowohl in einen Vorher-Nachher-Vergleich gestellt, sowie in einem Vergleich zwischen den drei Gruppen. Zusätzlich nahmen zwei Probanden an einer Fallstudie teil, deren Ziel es war die langfristi-gen Wirkungen auf die Beweglichkeit der Faszie zu analysieren.
Ergebnisse: Nach der Behandlung mit der Foam Roll verbesserte sich die Beweglichkeit der FTL durchschnittlich um 1,7915 mm (p<0,001). Dagegen konnte in der Placebogruppe (p=0,397) und in der Kontrollgruppe (p=0,861) keine signifikante Verbesserung der Beweg-lichkeit verzeichnet werden. Messungen mit weiteren Messinstrumenten zeigten keine Verbesserungen. Bei den zwei Probanden der Fallstudie konnte, über einen Zeitraum von vier Wochen, eine Verbesserung der Beweglichkeit der FTL festgestellt werden.
Schlussfolgerung: Durch die Anwendung der Foam Roll konnte die Beweglichkeit der Fascia Thoracolumbalis signifikant verbessert werden.

Restricted Versus Unrestricted Search Space : Experience from Mining a Large Japanese Database

The aim of this study was to investigate whether standard Big Data mining methods lead to clinically useful results. An association analysis was performed using the apriori algorithm to discover associations among co-morbidities of diabetes patients. Selected data were further analyzed by using k-means clustering with age, long-term blood sugar and cholesterol values. The association analysis led to a multitude of trivial rules. Cluster analysis detected clusters of well and badly managed diabetes patients both belonging to different age groups. The study suggests the usage of cluster analysis on a restricted space to come to meaningful results.

ist und dass die Persönlichkeit der Kitaleiterin Ausgangspunkt der Teamentwicklung ist. Anhand von praktischen Beispielen aus dem Kita-Alltag werden

The assessment of somatosensory function is a cornerstone of research and clinical practice in neurology. Recent initiatives have developed novel protocols for quantitative sensory testing (QST). Application of these methods led to intriguing findings, such as the presence lower pain-thresholds in healthy children compared to healthy adolescents. In this article, we (re-) introduce the basic concepts of signal detection theory (SDT) as a method to investigate such differences in somatosensory function in detail. SDT describes participants’ responses according to two parameters, sensitivity and response-bias. Sensitivity refers to individuals’ ability to discriminate between painful and non-painful stimulations. Response-bias refers to individuals’ criterion for giving a “painful” response. We describe how multilevel models can be used to estimate these parameters and to overcome central critiques of these methods. To provide an example we apply these methods to data from the mechanical pain sensitivity test of the QST protocol. The results show that adolescents are more sensitive to mechanical pain and contradict the idea that younger children simply use more lenient criteria to report pain. Overall, we hope that the wider use of multilevel modeling to describe somatosensory functioning may advance neurology research and practice.
BACKGROUND:
There is little knowledge regarding the association between psychological factors and complex regional pain syndrome (CRPS) in children. Specifically, it is not known which factors precipitate CRPS and which result from the ongoing painful disease.
OBJECTIVES:
To examine symptoms of depression and anxiety as well as the experience of stressful life events in children with CRPS compared with children with chronic primary headaches and functional abdominal pain.METHODS: A retrospective chart study examined children with CRPS (n=37) who received intensive inpatient pain treatment between 2004 and 2010. They were compared with two control groups (chronic primary headaches and functional abdominal pain; each n=37), who also received intensive inpatient pain treatment. Control groups were matched with the CRPS group with regard to admission date, age and sex. Groups were compared on symptoms of depression and anxiety as well as stressful life events.
RESULTS:
Children with CRPS reported lower anxiety and depression scores compared with children with abdominal pain. A higher number of stressful life events before and after the onset of the pain condition was observed for children with CRPS.
CONCLUSIONS:
Children with CRPS are not particularly prone to symptoms of anxiety or depression. Importantly, children with CRPS experienced more stressful life events than children with chronic headaches or abdominal pain. Prospective long-term studies are needed to further explore the potential role of stressful life events in the etiology of CRPS.
Attitudes Concerning Postmortem Organ Donation : A Multicenter Survey in Various German Cohorts

BACKGROUND
The aim of this study was to characterize postmortem organ donation attitudes in various German cohorts.
MATERIAL AND METHODS
Employees of 2 German cities and 2 German university hospitals, employees of a German automobile enterprise, and members of a German Medical Society were administered a questionnaire about postmortem organ and tissue donation attitudes. Demographic data and general attitudes were questioned and focused on: I) willingness to donate organs, II) holding a donor card, and III) having discussed the topic with the family.
RESULTS
Of 5291 participants, 65.2% reported favoring postmortem organ donation. Missing negative experiences, the idea that donation is helpful, a non-medical professional environment, excellent general health, gender, agreement with the brain-death paradigm, and age significantly influenced the participants’ attitudes. Participants were more likely to possess donor cards and had discussed more often with family members if they agreed with the brain-death paradigm and considered donation to be helpful. Males and older participants were the most likely to neglect donor cards, and Catholics, Protestants, and participants with poor health were the least likely to donate organs. Interest in receiving more information was expressed by 38.1% and 50.6% of participants refusing donation of all or of specific organs, respectively, and suggested the internet (60.0%) and family doctors (35.0%) as preferred sources of information.
CONCLUSIONS
Public campaigns in Germany should focus on males and older people as regards donor cards, and females, younger, and religiously affiliated persons as regards the general willingness to donate organs postmortem.
Usability is a core construct of website evaluation and inherently defined as interactive. Yet, when analysing first impressions of websites, expected usability, i.e., before use, is of interest. Here we investigate to what extend ratings of expected usability are related to (a) experienced usability, i.e., ratings after use, and (b) objective usability measures, i.e., task performance. Furthermore, we try to elucidate how ratings of expected usability are correlated to aesthetic judgments. In an experiment, 57 participants submitted expected usability ratings after the presentation of website screenshots in three viewing-time conditions (50, 500, and 10,000 ms) and after an interactive task (experienced usability). Additionally, objective usability measures (task completion and duration) and subjective aesthetics evaluations were recorded for each website. The results at both the group and individual level show that expected usability ratings are not significantly related either to experienced usability or objective usability measures. Instead, they are highly correlated with aesthetics ratings. Taken together, our results highlight the need for interaction in empirical website usability testing, even when exploring very early usability impressions. In our study, user ratings of expected usability were no valid proxy neither for objective usability nor for experienced website usability.
Background:
Recurrent pain is a common experience in childhood, but only few children with recurrent pain attend a physician. Previous studies yielded conflicting findings with regard to predictors of health care utilization in children with recurrent pain.
Methods:
The present study analyzes data from the German Health Interview and Examination Survey for Children and Adolescents (KiGGS) study comprising n = 2,149 children (3–10 years old) with recurrent pain to find robust predictors.We used multiple logistic regressions to investigate age, gender, socio-economic status(SES), migration background, pain intensity, pain frequency, pain-related disability, mental health problems, and health-related quality of life (HRQL) as predictors for visiting a doctor due to pain.
Results:
Overall, young girls with high pain-related disability, intensity, frequency, and migration background were more likely to attend a physician. Pain-related disability had the largest impact. Socioeconomic status, health-related quality of life anmental health problems were not systematically related to health care utilization. An analysis of the variability of these results indicated that several hundred participants
are needed until the results stabilize.
Conclusions:
Our findings highlight the importance of pain-related disability and frequency in assessing the severity of recurrent pain. Generic predictors and demographic variables are of lesser relevance to children with recurrent pain. On a methodological level, our results show that large-scale studies are need to reliably
identify predictors of health care utilization.
Although national eHealth strategies have existed now for more than a decade in many countries, they have been implemented with varying success. In Germany, the eHealth strategy so far has resulted in a roll out of electronic health cards for all citizens in the statutory health insurance, but in no clinically meaningful IT-applications. The aim of this study was to test the technical and organisation feasibility, usability, and utility of an eDischarge application embedded into a laboratory Health Telematics Infrastructure (TI). The tests embraced the exchange of eDischarge summaries based on the multiprofessional HL7 eNursing Summary standard between a municipal hospital and a nursing home. All in all, 36 transmissions of electronic discharge documents took place. They demonstrated the technical-organisation feasibility and resulted in moderate usability ratings. A comparison between eDischarge and paper-based summaries hinted at higher ratings of utility and information completeness for eDischarges. Despite problems with handling the electronic health card, the proof-of-concept for the first clinically meaningful IT-application in the German Health TI could be regarded as successful.

Die anhaltend hohen Interventionsraten in der geburtshilflichen Versorgung und ihre Ursachen werden weltweit aber auch national thematisiert. Hierbei wird auch auf die Betreuungsverhältnisse im Kreißsaal fokussiert: Eine erhöhte Arbeitsbelastung hat das Potential, die Betreuungsdichte der Hebammen einzuschränken. Dies kann die Betreuungsqualität negativ beeinflussen. Auf Grund einer insuffizienten psychosozialen Unterstützung verstärken sich Ängste und Unsicherheiten der Gebärenden. Dies wirkt sich möglicherweise negativ auf den physiologischen Geburtsverlauf aus. Eine Zunahme der Arbeitsbelastung und eine verminderte Betreuungsdichte der Hebamme begünstigen somit vermutlich Interventionen, wie operative Geburtsmodi oder Analgesie- und Anästhesieverfahren.
Zielsetzung
Im Rahmen der vorliegenden Sekundärdatenanalyse wurde untersucht, ob ein Zusammenhang zwischen der Arbeitsbelastung der Hebammen und der Betreuungsdichte im Kreißsaal besteht. Zudem wurde analysiert, ob diese Betreuungsmerkmale mit der Rate operativer Geburtsmodi sowie der Inanspruchnahme von Analgesie- und Anästhesieverfahren assoziiert sind.
Methodik
Die Hypothesen wurden im Rahmen von multivariaten linearen und logistischen Regressionsverfahren explorativ getestet. Basis der Analyse ist eine Stichprobe von 999 Low-Risk Gebärenden.
Ergebnisse
Eine geringere Arbeitsbelastung der Hebamme steht in einem signifikanten Zusammenhang mit einer Zunahme der Betreuungsdichte. Die Betreuungsdichte und die Arbeitsbelastung sind jedoch nicht signifikant mit dem operativen Geburtsmodus oder der Inanspruchnahme von Analgesie- und Anästhesieverfahren assozi-iert. Andere Variablen, welche die Betreuungssituation im Kreißsaal operationalisieren, sowie frau- oder geburtsspezifische Parameter tragen hingegen signifikant zur Varianzaufklärung bei.
Schlussfolgerung
Mit den Ergebnissen kann ein Zusammenhang zwischen den strukturellen Arbeitsbedingungen wie der Arbeitsbelastung im Kreißsaal und der Betreuungsdichte aufgezeigt werden. Gleichwohl sind möglicherweise insbesondere die Handlungskultur im Kreißsaal sowie die Aufenthaltsdauer der Gebärenden in Hinblick auf Interventionskaskaden von besonderer Bedeutung. Für prospektive Untersuchungen ist daher eine umfassendere Operationalisierung der Hebammenbetreuung und des Geburtsprozesses sowie darüber hinaus die Berücksichtigung der individuellen Perspektive der Gebärenden von Bedeutung. Eine adäquate Personalausstattung ist eine Voraussetzung, um in der Praxis die Betreuungsqualität zu verbessern. Zusätzlich sollten jedoch auch Betreuungskonzepte von Hebammen reflektiert und strukturiert in der beruflichen Praxis implementiert werden.
Background: The majority of health IT adoption research focuses on the later stages of the IT adoption process: namely on the implementation phase. The first stage, however, which is defined as the knowledge-stage, remains widely unobserved. Following Rogers’ Diffusion of Innovation Theory (DOI) this paper presents a research framework to examine the possible lack of shared IT awareness-knowledge, i.e. an information gradient, of two crucial stakeholders, the Chief Information Officer (CIO) and the Director of Nursing (DoN). This study shall answer the following research questions: (1.) Does this gradient exist? (2.) Which direction does it have? (3.) Are certain health IT (HIT) attributes associated with a potential gradient? (4.) Which determinants of diffusion go along with this gradient?
Method: Results of two surveys that focused on the topic “IT support of clinical workflows” from the viewpoint of CIOs and DoNs with corresponding datasets from 75 hospitals were used in a secondary data analysis. The gradient was operationalised by measuring the disagreement of CIOs and DoNs on the availability and implementation status of 29 IT functions. HIT attributes tested were relevance and market penetration of the IT functions, determinants of diffusion were inter-professional leadership and IT service density.
Results: The analysis revealed a significant disagreement on the availability of 9 out of 29 HIT functions. In 23 HIT functions, the CIOs reported a higher implementation status than the DoNs, which pointed to a trend for a unidirectional gradient. The disagreement was significantly lower when the relevance of the IT function was high. Both determinants of diffusion correlated significantly negative with the degree of disagreement.
Conclusion: This is the first study to empirically examine shared awareness-knowledge of two IT-stakeholders that are crucial for triggering IT adoption on the frontline level in hospitals. It could be shown that a gradient and thus a lack of shared awareness-knowledge existed and was associated with certain factors. In conclusion, hospitals should implement improved cooperation between IT staff and clinicians and IT service density when establishing the prerequisites for successful IT adoption processes.
Die Kooperation von Hebammen im Kontext der ambulanten Versorgung – Ergebnisse einer Literaturstudie

The central objective of early prevention in Germany is an improved cooperation between professional groups of the health services and child and youth welfare in interprofessional networks. This objective derives from the realisation that proper care for families with infants can only be achieved if the various groups act in close integration. The ‘Federal Initiative early prevention’ explicitly calls for freelance midwives to be integrated in this context. However, only a few scientific findings on midwives’ cooperation in networks of early prevention have been published to date. This integrative review aims to identify the central themes of interprofessional cooperation of midwives in out-of-hospital obstetrical care from national and international research literature. A systematic search of five research databases for publications between 2005 and 2015 was performed, complemented by a manual search. 25 studies were identified describing various contexts where midwives in out-of-hospital obstetrical care cooperate with other professional groups. Four key themes were analysed: contexts of cooperation, benefits of cooperation, facilitating and restrictive factors of cooperation, and competencies of cooperation. The studies show that there is only limited research coverage of the midwives’ perspective regarding interprofessional cooperation. The existing studies examine the cooperation of midwives primarily with health care professionals, and secondarily with professionals in the social services. In order to expand knowledge on the cooperation of freelance midwives in the networks of early prevention, future research should focus on the perspective of midwives regarding cooperation with other professional groups, both in the health care sector and in the field of social services.
Innovations are typically characterised by their relative newness for the user. In order for new eHealth applications to be accepted as innovations more criteria were proposed including “use” and “usability”. The handoverEHR is a new approach that allows the user to translate the essentials of a clinical case into a graphical representation, the so-called cognitive map of the patient. This study aimed at testing the software usability. A convenience sample of 23 experienced nurses from different healthcare organisations across the country rated the usability of the handoverEHR after performing typical handover tasks. All usability scales of the IsoMetricsL questionnaire showed positive values (4 “I agree”) with the exception of “error tolerance” (3 “neutral statement”). A significant improvement was found in self-descriptiveness as compared to an initial usability testing prior to this study. Different subgroups of users tended to rate the usability of the system differently. This study demonstrated the benefits of formative evaluations in terms of improving the usability of an entirely new approach. It thus helps to transform a novel piece of software towards becoming a real innovation. Our findings also hint at the importance of user characteristics that could affect the usability ratings.
This workshop will review the history of the TIGER initiative in order to set the framework for an understanding of international informatics competencies. We will include a description of clinical nursing informatics programs in 37 countries as well as the results of a recent survey of nursing competencies in order to further discussions of internationally agreed-upon competency definitions. These two surveys will provide the basis for developing a consensus regarding the integration of core competencies into informatics curriculum developments. Expected outcomes include building consensus on core competencies and developing plans toward implementing intra- and inter-professional informatics competencies across disciplines globally.
Household income determines access to specialized pediatric chronic pain treatment in Germany

Background
Families with lower socioeconomic status (SES) often face problems with gaining access to health care services. Information is scarce on the relationship between SES and health care delivery for children suffering from chronic pain.
Methods
Families presenting to a specialized pain center (N = 1,001) provided information on ‘household income, ‘parental education’ and ‘occupation’ to aid the evaluation of their SES. To assess whether the SES of the clinical sample is representative of the general population, it was compared to data from a community sample (N = 14,558). For the clinical sample, travel distance to the clinic was described in relation to the 75 % catchment area. Multiple logistic regression was used to analyze the association between SES and the journey from outside the catchment area.
Results
The SES was significantly higher in the clinical sample than in the community sample. Within the clinical sample, the distance traveled to the pain center increased with increasing SES. The 75 % catchment area was 143 miles for families with the highest SES and 78 miles for the lowest SES. ‘Household income’ predicted travel distance (OR 1.32 (1.12–1.56)). Education and occupational status were not significant predictors of travel from outside the catchment area.
Conclusions
In Germany, specialized care for children with chronic pain is subject to disparities in access. Future activities should focus on identifying barriers to access and seeking to prevent inequalities in specialized pediatric health care delivery. Increasing the number of specialized treatment facilities could improve access to specialized pediatric pain treatment, regardless of socioeconomic determinants.
Background: Singers belong to the group of professional voice users with the highest demands regarding voice quality and vocal load. Thus, they also have a high risk of developing a voice disorder, which in return has major impact on their ability to work. Besides voice disorders caused by organic changes, there are functional voice disorders caused by, e.g., a hypertonia of the larynx, shoulder and neck muscles or insufficient breathing patterns. In these cases, physiotherapy can be one component of a multidisciplinary approach to treatment.
The purpose of this presentation is, based on anatomical considerations and current evidence, to inform about and demonstrate physiotherapy techniques for treating singers with functional voice disorders.
Approach of Presentation: A case from a special physiotherapy outpatient clinic for vocalists will be described. Based on this example, information on the evidence of physiotherapy approaches for functional voice disorders will be provided. Afterwards, some practical hands-on techniques will be demonstrated for participants to try.
Content of Presentation: This workshop will focus on the physiotherapy treatment for a vocalist with functional voice disorders. The vocalist experienced changed pitch and hypertonia in both the muscles of the shoulder-neck region and the extrinsic laryngeal muscles. Paralaryngeal manual techniques, in addition to posture and breathing exercises, will be demonstrated with the purpose of mobilizing the larynx and relaxing the hypertonic muscles.
Conclusions and Practical Relevance: This workshop highlights the special potential of physical therapy in the treatment of functional voice disorders in singers.
Hochschule und Universität Osnabrück haben mit regionalen Partnern (Stadt, Landkreis, Bistum, evangelisch-lutherischem Kirchenkreis, Kompetenzzentrum Gesundheitswirtschaft) die Grundsatzvereinbarung unterzeichnet, in Osnabrück einen Gesundheitscampus zu etablieren. Das Ziel ist, einen Ort zu schaffen, an dem Wissenschaft, Unternehmen der Gesundheitsversorgung, Träger von Gesundheitseinrichtungen und Politik zusammenkommen, um innovative Versorgungskonzepte für die Region zu erproben. „ROSE – das Lernende Gesundheitssystem in der Region Osnabrück-Emsland“ ist ein Großprojekt im Rahmen des Gesundheitscampus Osnabrück, das von dem Niedersächsischen Ministerium für Wissenschaft und Kultur (MWK) über 5 Jahre gefördert wird. Dabei wird das Prinzip des Lernens durch Feedback angewendet. Das bedeutet, dass durch Forschung in und mit der Praxis Evidenz im Sinne von practice-based evidence erzeugt wird, d.h. Evidenz für eine bessere Versorgungspraxis unter Berücksichtigung städtischer und ländlicher Strukturen. Dies hat zur Konsequenz, dass der Transferprozess zwischen Hochschule und Versorgungspraxis nicht am Ende sondern bereits am Anfang steht. Mit dem Ansatz einer wiederkehrenden Abfolge von Forschungsfragen und Analysen von Daten aus der Versorgungspraxis rekurriert ROSE auf das Prinzip des „Learning Health Care System“ (IOM, 2007). Im Rahmen von ROSE stimmen sich Hochschule und Universität Osnabrück ab, um die Ziele des Gesundheitscampus zu erreichen. Die geplante Umsetzung wird anhand eines Modells mit fünf Maßnahmen vorgestellt. Diese bauen auf der Vielfalt von bereits bestehenden Gesundheitsstudiengängen in Osnabrück auf und bringen Forschung, Nachwuchsförderung und Translation von Forschungsergebnissen zusammen.
An Iterative Methodology for Developing National Recommendations for Nursing Informatics Curricula

The increasing importance of IT in nursing requires educational measures to support its meaningful application. However, many countries do not yet have national recommendations for nursing informatics competencies. We thus developed an iterative triple methodology to yield validated and country specific recommendations for informatics core competencies in nursing. We identified relevant competencies from national sources (step 1), matched and enriched these with input from the international literature (step 2) and fed the resulting 24 core competencies into a survey (120 invited experts from which 87 responded) and two focus group sessions with a total of 48 experts (steps 3a/3b). The subsequent focus group sessions confirmed and expanded the findings. As a result, we were able to define role specific informatics core competencies for three countries.
Informatics competencies of the health care workforce must meet the requirements of inter-professional process and outcome oriented provision of care. In order to help nursing education transform accordingly, the TIGER Initiative deployed an international survey, with participation from 21 countries, to evaluate and prioritise a broad list of core competencies for nurses in five domains: 1) nursing management, 2) information technology (IT) management in nursing, 3) interprofessional coordination of care, 4) quality management, and 5) clinical nursing. Informatics core competencies were found highly important for all domains. In addition, this project compiled eight national cases studies from Austria, Finland, Germany, Ireland, New Zealand, the Philippines, Portugal, and Switzerland that reflected the country specific perspective. These findings will lead us to an international framework of informatics recommendations.
Das Gesundheitswesen steht vor zahlreichen Veränderungen. Dazu zählen auch sich wandelnde Kompetenzprofile. Auf Seiten der Hochschulen stellt sich neben den fachlichen Aspekten zukünftiger Kompetenzprofile verstärkt die Frage nach einer höheren systemischen Durchlässigkeit und damit auch nach einer optimalen Vernetzung hochschulischer und außerhochschulisch erworbener Kompetenzen. Insbesondere in den Gesundheitsfachberufen nehmen non-formal und informell erworbene Kompetenzen einen hohen Stellenwert ein. Somit stellt sich die Herausforderung, wie die Ausgestaltung eines hochschulischen Zertifikatsprogrammes erfolgen kann, das zum einen eine Qualifizierung auf akademischem Niveau bietet und gleichzeitig die bestehenden non-formal bzw. informell im Berufsfeld erworbenen Kompetenzen der Lernenden erfassen, bewerten und entsprechend zertifizieren kann. Zielstellung ist es letztendlich, die Gesamtkompetenz einer Person in einem klar umgrenzten thematischen Umfeld zu erfassen, wertzuschätzen“ bzw. zu bewerten und gezielt weiter zu entwickeln, wobei außerhochschulische Kompetenzen eine wichtige Rolle spielen. Die in diesem Rahmen zertifizierten Kompetenzen stehen in einem möglichen Folgeprozess für eine entsprechende Anrechnung zur Verfügung. Vorgestellt wird ein erster Prototyp, der sich insbesondere als eine Erweiterung bestehender Denkmodelle im Bereich der Anrechnung versteht.
Mit der Akademisierung von praktischen Gesundheitsberufen stehen die Hochschulen und Universitäten gleichermaßen vor der Herausforderung, Pflege- und Therapiekräfte für ein noch nicht definiertes Berufsfeld zu qualifizieren. Die Wissenschaft hat die Fragen nach den genauen Inhalten und der Abgrenzung akademisierter Gesundheitsberufe zu anderen, seit langem etablierten Tätigkeitsbereichen in der Patientenversorgung zu beantworten und weiterführend zukünftigen Entwicklungen aufzuzeigen. Hier erwächst der Bedarf nach einer weitergehenden Arbeitsforschung in den akademisierten, praktischen Gesundheitsberufen. Der in diesem Beitrag vorgestellte Ansatz kombiniert die Weiterqualifizierung mit der Arbeitsforschung im Berufsfeld. Ziel ist es, Weiterbildungsmodule zu konzipieren, bei denen das praktische Arbeitsfeld mit den jeweiligen Inhalten der Module systematisch in Bezug gesetzt wird. Grundlage hierzu sind didaktische Ansätze der Theorie-Praxis-Relation aus dualen Studiengängen. Zentrales Instrument ist eine reflexionsorientierte Fallstudie, die von den Teilnehmern der Weiterbildung für jedes Modul erstellt wird, in der die Teilnehmer theoriebasierte Inhalte des Moduls aufgreifen und dazu nutzen, ihr jeweiliges praktisches Arbeitsfeld mit der fachlichen Perspektive des Moduls zu reflektieren. Die Teilnehmer werden so zu Praxisforschern und die Weiterbildung gleichzeitig zu einer wissenschaftlichen Berufsfeldreflexion, aus der die Praxis Impulse zur Weiterentwicklung erhält.
Für die Versorgungsforschung ist wichtig, dass verteilte und heterogene Daten so integriert werden, dass sie offen für neue Analyse-Anforderungen und leicht um neue Datenquellen erweiterbar sind. Für die Integration von Versorgungsdaten werden bislang hauptsächlich Data-Warehouses eingesetzt, die Daten dimensional oder als Entity-Attribute-Value-Struktur (EAV) modellieren. Diese Datenmodelle sind jedoch entweder unflexibel oder weisen ein zu geringes Maß an Datenorganisation auf, was longitudinale Analysen erschwert. Wir haben den EAV-Ansatz um die Data-Vault-Modellierung ergänzt und damit die Datenstrukturen der Krankenhaus-Qualitätsberichte des Gemeinsamen Bundesausschusses (G-BA) modelliert sowie die Daten der Jahre 2011 bis 2015 integriert. Dies ermöglicht eine Historisierung der Metadaten für Merkmale, insbesondere der Qualitätsindikatoren, sowie ein hohes Maß an Erweiterbarkeit gegenüber neuen heterogenen Datenquellen. Der vorgeschlagene Ansatz erlaubt es, den Abstraktionsgrad für die zu modellierenden Entitäten frei zu wählen, so dass auch ein vollständig generisches EAV-Modell mit historisierten Metadaten erstellt werden kann.
Background: For more than 30 years, there has been close cooperation between Japanese and German scientists with regard to information systems in health care. Collaboration has been formalized by an agreement between the respective scientific associations. Following this agreement, two joint workshops took place to explore the similarities and differences of electronic health record systems (EHRS) against the background of the two national healthcare systems that share many commonalities.
Objectives: To establish a framework and requirements for the quality of EHRS that may also serve as a basis for comparing different EHRS.
Methods: Donabedian's three dimensions of quality of medical care were adapted to the outcome, process, and structural quality of EHRS and their management. These quality dimensions were proposed before the first workshop of EHRS experts and enriched during the discussions.
Results: The Quality Requirements Framework of EHRS (QRF-EHRS) was defined and complemented by requirements for high quality EHRS. The framework integrates three quality dimensions (outcome, process, and structural quality), three layers of information systems (processes and data, applications, and physical tools) and three dimensions of information management (strategic, tactical, and operational information management).
Conclusions: Describing and comparing the quality of EHRS is in fact a multidimensional problem as given by the QRF-EHRS framework. This framework will be utilized to compare Japanese and German EHRS, notably those that were presented at the second workshop.
Objective: To pilot benchmark measures of health information and communication technology (ICT) availability and use to facilitate cross-country learning.
Materials and Methods: A prior Organization for Economic Cooperation and Development–led effort involving 30 countries selected and defined functionality-based measures for availability and use of electronic health records, health information exchange, personal health records, and telehealth. In this pilot, an Organization for Economic Cooperation and Development Working Group compiled results for 38 countries for a subset of measures with broad coverage using new and/or adapted country-specific or multinational surveys and other sources from 2012 to 2015. We also synthesized country learnings to inform future benchmarking.
Results: While electronic records are widely used to store and manage patient information at the point of care—all but 2 pilot countries reported use by at least half of primary care physicians; many had rates above 75%—patient information exchange across organizations/settings is less common. Large variations in the availability and use of telehealth and personal health records also exist.
Discussion: Pilot participation demonstrated interest in cross-national benchmarking. Using the most comparable measures available to date, it showed substantial diversity in health ICT availability and use in all domains. The project also identified methodological considerations (e.g., structural and health systems issues that can affect measurement) important for future comparisons.
Conclusion: While health policies and priorities differ, many nations aim to increase access, quality, and/or efficiency of care through effective ICT use. By identifying variations and describing key contextual factors, benchmarking offers the potential to facilitate cross-national learning and accelerate the progress of individual countries.
Die Hochschule Osnabrück zeichnet sich durch einen Forschungsschwerpunkt Familienorientierte geburtshilfliche Versorgung aus, welcher auf der Forschungslandkarte der Hochschulrektorenkonferenz (HRK) verzeichnet ist. Seit 2000 werden Studien zur geburtshilflichen Versorgung durchgeführt. Im Forschungsschwerpunkt IsQua wurden in vier Teilprojekten A bis D, verschiedene Instrumente zur sektorenübergreifenden Qualitätssicherung in der Lebensphase von Schwangerschaft, Geburt und Wochenbett entwickelt, erprobt und evaluiert. Mit der Entwicklung von diesen Instrumenten wird das Ziel verfolgt, Prozesse, die die physiologische Geburt befördern, zu unterstützen. Damit soll der derzeitigen Entwicklung in der Geburtshilfe gegengesteuert werden, die u. a. von hohen Interventionsraten, wie z.B. von einer anhaltend hohen Kaiserschnittrate von 32% (IQTIG [Hg.], 2017) gekennzeichnet ist.
Alle genannten Aspekte werden im Kontext eines Theorie Praxis Transfers gedacht. Instrumente wurden unter Beteiligung der Berufsgruppe der Hebammen heraus entwickelt. Sie wurden auf Anwenderebene – i. d. Regel durch Hebammen – und auf Ebene der Nutzerinnen bzw. Klientinnen, den schwangeren Frauen bzw. Müttern nach der Geburt, erprobt. Hierzu wurden vielfältige Kooperationen mit Einrichtungen des Gesundheitswesens geknüpft und verschiedene Zugänge eines Wissenstransfers hergestellt. Dieses war die Aufgabe von Teilprojekt E, welches horizontal zu den Teilprojekten A bis D angelegt war. Dabei wurde teilweise interdisziplinär mit Kollegen und Kolleginnen aus der Pflegewissenschaft und Gesundheitsökonomie gearbeitet. Inhaltlich stellt der Forschungsschwerpunkt ein nachgefragtes Thema zur Verbesserung der geburtshilflichen Versorgungsqualität regional und überregional dar.
Die dem Forschungsschwerpunkt angegliederte Graduiertenförderung im Umfang von drei Stipendien eröffnete erstmalig die Möglichkeit zur strukturierten Förderung von wissenschaftlichem Nachwuchs an Fachhochschulen in der jungen Disziplin Hebammenwissenschaft. Seit 2011 kann über eine Kooperation zwischen der Hochschule Osnabrück und der privaten Universität Witten Herdecke, Fakultät Gesundheit, die Promotion verfolgt werden. Eine weitere Dissertation konnte mit Teilprojekt A assoziiert werden. Alle Promotionen waren inhaltlich mit den einzelnen Teilprojekten verbunden.
Hintergrund: Für etwa 13% der Familien mit Säuglingen in Deutschland besteht Unterstützungsbedarf im Rahmen Früher Hilfen. Um die Entwicklungsbedingungen für Kinder zu fördern, gewinnt die Kooperation zwischen den Berufsgruppen des Gesundheits- und Sozialwesens an Relevanz. Freiberufliche Hebammen werden als Partnerinnen in den Netzwerken Früher Hilfen angesehen, ihre Perspektive zur Kooperation ist kaum erforscht.
Ziel: Die Studie analysiert zentrale Konzepte des Hebammenhandelns Fakultät Wirtschafts- und im Kontext der interprofessionellen Kooperation in den Frühen Hilfen. Sozialwissenschaften,
Methodik: 27 problemzentrierte Interviews mit freiberuflichen Hebammen in Deutschland bilden die Basis der qualitativen Untersuchung. Der Feldzugang erfolgte über lokale Mailverteiler, Anzeigen in Hebammenzeitschriften, Besuche von Kongressen und durch Unterstützung von Gatekeepern. Die Interviews wurdenmittels Leitfaden geführt, digital aufgezeichnet und transkribiert. Die Auswertung erfolgte mittels qualitativer Inhaltsanalyse nach Kuckartz.
Ergebnisse: Als zentrales Konzept der Kooperation von Hebammen in den Frühen Hilfen wurde die Vertrauensbildung zwischen Hebammen und Klientinnen identifiziert. Diese prägt die Arbeitsbeziehung der beiden, sowie die Zusammenarbeit der Hebammen mit den Akteurinnen und Akteuren der Kinder- und Jugendhilfe. Einige Hebammen bauen auf dem Vertrauen der Klientinnen auf, um sie an Angebote des Jugendamtes zu vermitteln, andere nutzen es zur Abgrenzung vom Jugendamt.
Schlussfolgerung: Die Herausforderung für die freiberuflichen Hebammen besteht darin, ihre Rolle als Kooperationspartnerin in den Netzwerke Früher Hilfen zu finden, ohne dabei das Vertrauen der Frauen zu verlieren. Dies erfordert klare und verständliche Rahmenbedingungen für alle in die Netzwerke eingebundenen Akteurinnen und Akteure.
Aim: The aim of this study was to give an overview of family caregiver satisfaction within the home palliative care situation in North Rhine-Westphalia, Germany.
Design: A cross-sectional study.
Methods: An anonymous questionnaire, with seven validated scales, and comprising of 71 items, was used. The items investigated perceived needs and burdens of families within a home-based palliative care situation.
The satisfaction of the family caregivers with the services delivered by palliative care teams was measured by the FAMCARE-2 Scale. Descriptive statistics and analysis of covariance (ANCOVA) were performed.
Results: A convenience sample of 106 family members agreed to participate in the study. Overall, we found high satisfaction within our sample. There was high satisfaction with how the services respected the dignity of families, and how they provided comfort to patients. Satisfaction was lower with regard to information about patients.
Conclusion: High or low satisfaction with palliative care, tells us little about the quality of services. The high satisfaction within this study could be interpreted as a sign that palliative care was important to families at the time of availability. Regular and continuous assessment can serve to inform the continuous quality of care provision for patients and their families.
CIOs' innovation capability is regarded as a precondition of successful HIT adoption in hospitals. Based on the data of 142 CIOs, this study aimed at identifying antecedents of perceived innovation capability. Eight features describing the status quo of the hospital IT management (e.g. use of IT governance frameworks), four features of the hospital structure (e.g. functional diversification) and four CIO characteristics (e.g. duration of employment) were tested as potential antecedents in an exploratory stepwise regression approach. Perceived innovation capability in its entirety and its three sub-dimensions served as criterion. The results show that CIOs' perceived innovation capability could be explained significantly (R2=0.34) and exclusively by facts that described the degree of formalism and structure of IT management in a hospital, e.g. intensive and formalised strategic communication, the existence of an IT strategy and the use of IT governance frameworks. Breaking down innovation capability into its constituents revealed that “innovative organisational culture” contributed to a large extent (R2=0.26) to the overall result sharing several predictors. In contrast, “intrapreneurial personality” (R2=0.11) and “openness towards users” (R2=0.18) could be predicted less well. These results hint at the relationship between working in a well-structured, formalised and strategy oriented environment and the overall feeling of being capable to promote IT innovation.
Patient handovers are cognitively demanding, crucial for information continuity and patient safety, but error prone. This study investigated the effect of an electronic handover tool, i.e. the handoverEHR, on the memory and care planning performance of nurse students (n=32) in a randomised, controlled cross-over design with the factors handover task and handover role. On a descriptive level, handover recipients could improve their memory performance with electronic support, handover givers their performance of writing care plans. Statistically meaningful differences occurred, however, only when the participants were givers. Without handover experience and with low fluency to word problems, givers performed badly in the most demanding of the handover tasks. Final recommendations, however, can only be made after replicating this study in a clinical setting with mixed groups.
Health IT and communication systems are indispensable in German hospitals for clinical as well as administrative process support. However, IT is often regarded as a “black box” for hospital CEOs. Thus, the question arises how can CEOs decide if they do not know what is in the box? In order to answer this question, half-structured interviews with 14 German hospital CEOs were conducted. They revealed three principle decision processes: the supported decision, the joint decision and the corporate level decision. In all cases, the hospital CEO and the CIO interacted to reach the final decision, most strongly in the joint decision mode and least strongly in the corporate decision mode. Only the joint decision mode definitely forced the CEO to open the “black box” of IT. In the era of digitalisation, however, CEOs must develop better competencies to decide over complex matters.
Health IT adoption research is rooted in Rogers' Diffusion of Innovation theory, which is based on longitudinal analyses. However, many studies in this field use cross-sectional designs. The aim of this study therefore was to design and implement a system to (i) consolidate survey data sets originating from different years (ii) integrate additional secondary data and (iii) query and statistically analyse these longitudinal data. Our system design comprises a 5-tier-architecture that embraces tiers for data capture, data representation, logics, presentation and integration. In order to historicize data properly and to separate data storage from data analytics a data vault schema was implemented. This approach allows the flexible integration of heterogeneous data sets and the selection of comparable items. Data analysis is prepared by compiling data in data marts and performed by R and related tools. IT Report Healthcare data from 2011, 2013 and 2017 could be loaded, analysed and combined with secondary longitudinal data.
Hospital CIOs play a central role in the adoption of innovative health IT. Until now, it remained unclear which particular conditions constitute their capability to innovate in terms of intrapersonal as well as organisational factors. An inventory of 20 items was developed to capture these conditions and examined by analysing data obtained from 164 German hospital CIOs. Principal component analysis resulted in three internally consistent components that constitute large portions of the CIOs innovation capability: organisational innovation culture, entrepreneurship personality and openness towards users. Results were used to build composite indicators that allow further evaluations.
We investigated whether design experts or laypersons evaluate webpages differently. Twenty participants, 10 experts and 10 laypersons, judged the aesthetic value of a webpage in an EEG-experiment. Screenshots of 150 webpages, judged as aesthetic or as unaesthetic by another 136 participants, served as stimulus material. Behaviorally, experts and laypersons evaluated unaesthetic webpages similarly, but they differed in their evaluation of aesthetic ones: experts evaluated aesthetic webpages as unaesthetic more often than laypersons did. The ERP-data show main effects of level of expertise and of aesthetic value only. There was no interaction of expertise and aesthetics. In a time-window of 110–130 ms after stimulus onset, aesthetic webpages elicited a more negative EEG-amplitude than unaesthetic webpages. In the same time window, experts had more negative EEG-amplitudes than laypersons. This patterning of results continued until a time window of 600–800 ms in which group and aesthetic differences diminished. An interaction of perceiver characteristics and object properties that several interactionist theories postulate was absent in the EEG-data. Experts seem to process the stimuli in a more thorough manner than laypersons. The early activation differences between aesthetic and unaesthetic webpages is in contrast with some theories of aesthetic processing and has not been reported before.
Sängerinnen und Sänger gehören zu der Gruppe der sogenannten „Professional Voice User“ bzw. der BerufssprecherInnen. Diese Gruppe ist den höchsten Anforderungen hinsichtlich der Stimmqualität und der Stimmbelastung ausgesetzt. Deshalb haben SängerInnen ein hohes Risiko eine Stimmstörung zu entwickeln, die wiederum ihre Arbeitsfähigkeit und Lebensqualität beeinträchtigen kann. Neben Stimmstörungen mit organischen Ursachen gibt es funktionelle Stimmstörungen, die z.B. durch ein ungünstiges Atemmuster oder Hypertonus der Schulter-, Nacken- und Halsmuskulatur verursacht werden können. In diesem Vortrag werden die unterschiedlichen Risikofaktoren für die Entwicklung einer Stimmstörung bei SängerInnen aufgezeigt. Anschließend werden physiotherapeutische Behandlungsansätze vorgestellt, die als Teil eines multidisziplinären Managements von funktionellen Stimmstörungen bei SängerInnen eingesetzt werden können.
Ziel des Workshops ist es, das besondere Potential der Physiotherapie im multidisziplinären Management von Sängerinnen und Sängern mit funktionellen Stimmstörungen aufzuzeigen. Anhand eines Fallbeispiels aus der physiotherapeutischen Musikersprechstunde wird der physiotherapeutische Behandlungsansatz vorgestellt. Die Sängerin mit ärztlich diagnostizierter hyperfunktioneller Stimmstörung zeigte Symptome wie ein sternales Atemmuster und Hypertonus der Nacken-, Kiefer- und der äußeren Kehlkopf-Muskulatur. Basierend auf der aktuellen Evidenzlage und der Anatomie werden die theoretischen Überlegungen der klinischen Urteilsbildung erläutert. Das Vorgehen im Behandlungsprozess wird beschrieben und einige praktische Beispiele von Behandlungstechniken, z.B. manualtherapeutische Techniken am Kehlkopf, werden demonstriert.
Hintergrund: Mangelhafte Informationsübermittlung führt bei der Überleitung Pflegebedürftiger zu einer Gefährdung von Sicherheit und Lebensqualität. Elektronische Instrumente können diese Situation grundsätzlich verbessern, werden jedoch bislang kaum eingesetzt. Ein Grund ist die fehlende Umsetzung von IT Standards.
Ziel der Arbeit: Ziel dieser Studie ist daher die technisch-organisatorische Machbarkeit, die Gebrauchstauglichkeit, Nützlichkeit und Vollständigkeit einer elektronisch unterstützten standardisierten Pflegeüberleitung zu untersuchen.
Material und Methoden: Zu diesem Zweck wurde eine Test-Telematikinfrastruktur aufgebaut, die die elektronische Gesundheitskarte einbezog, sowie ein HL7 CDA basiertes Überleitungsinstrument entwickelt, das den Informationsaustausch zwischen einem Krankenhaus und Pflegeheimen ermöglichte. Die Anwender bewerteten die Gebrauchstauglichkeit des Überleitungsinstruments, sowie die Nützlichkeit und Vollständigkeit von elektronischen und papierbasierten Überleitungen.
Ergebnisse: Es zeigte sich, dass das elektronische Überleitungsinstrument auf Basis des HL7 CDA Standards im Rahmen der Test-Telematikinfrastruktur technisch umgesetzt und von den Anwendern als gebrauchstauglich eingeschätzt werden konnte. Im Vergleich lieferte die elektronische Überleitung vollständigere und nützlichere Daten als die Papierform. Die Anwendung der elektronischen Gesundheitskarte (eGK) wurde von den Anwendern als hemmend eingestuft.
Diskussion: Die Studie unterstreicht die Machbarkeit, Bedeutung sowie die Barrieren von elektronischen Überleitungen von Pflegebedürftigen. Pflegende aus dem Krankenhaus- und Langzeitpflegebereich können eine auf dem HL7 CDA Standard ePflegebericht basierende Anwendung in ihre Abläufe integrieren und erhalten dadurch bessere und vollständigere Informationen. Zur langfristigen Sicherstellung der Versorgungskontinuität sollte der HL7 CDA Standard ePflegebericht Teil der deutschen Telematik-Infrastruktur werden.
Der zunehmende Einsatz von Informations- und Kommunikationstechnologie im Gesundheitswesen verlangt auch von Angehörigen der Pflegeberufe Kompetenzen zur Nutzung der entsprechenden Systeme und Verfahren. Vor diesem Hintergrund haben sich die AG „Informationsverarbeitung in der Pflege“ der Deutschen Gesellschaft für Medizinische Informatik, Biometrie und Epidemiologie (GMDS), die Österreichische Gesellschaft für Pflegeinformatik (ÖGPI) und die Schweizerische Interessensgruppe Pflegeinformatik (IGPI) innerhalb des Schweizer Berufsverband der Pflegefachfrauen und Pflegefachmänner (SBK) entschlossen, gemeinsame Empfehlungen für benötigte Kernkompetenzfelder in Pflegeinformatik zu erarbeiten. Auf Basis einer iterativen multimethodischen Vorgehensweise unter Einbeziehung von einer großen Anzahl von Fachexperten aus Deutschland, Österreich und der Schweiz (D-A-CH) wurden 24 notwendige Felder von Kernkompetenzen definiert und deren Relevanz für fünf typische Berufsfelder in der Pflege bewertet. Damit liegt erstmalig eine wissenschaftlich fundierte Empfehlung für zu vermittelnde Kernkompetenzfelder in der Pflegeinformatik für verschiedene pflegerische Berufsfelder vor. Sie richtet sich an alle Personen mit Verantwortung für die Planung von Studium, Lehre, Aus- und Weiterbildung in der Pflege.
Greenwashing, defined by the Oxford Dictionary as “disinformation disseminated by an organization so as to present an environmentally responsible public image” can cause multifarious problems for companies. The phenomenon of greenwashing has, however, not attracted much attention in the event marketing literature to date. The purpose of this paper is twofold. It first describes and analyses the specific characteristics and features of greenwashing in event marketing. It then seeks to identify the current fundamental approaches of how to avoid greenwashing in event marketing and to assess their potential. A two-step literature analysis with complementary search approaches served as a methodical framework. First, journals related to event marketing were screened for the keywords “greenwashing” and “greenwash”. Next, the general literature was consulted for the same keywords. The results clearly demonstrate that the subject of greenwashing has been widely neglected in the event literature. There appears to be no overall concept or approach that allows event actors to avoid greenwashing, albeit various individual initiatives exist. However, it also became clear that initiatives against greenwashing in event marketing can be developed and implemented in the short and long term, for example by integrating different stakeholders. Additional political and juridical efforts based on specific guidelines are also necessary to prevent greenwashing in the future. The study is the first one to provide a systematic approach to the topic of greenwashing in the context of event marketing, including relevant approaches for its avoidance. It can thus help practitioners to better detect and avoid greenwashing in the event industry and to guide similar research in the future.
Hospice-Care-Modellkonzept - Entwicklung eines konzeptuellen Pflegemodells für stationäre Hospize

Die Hospizpflege sieht sich zunehmend der Herausforderung gegenüber, ihr Selbstverständnis sowie ihre Individualität in ein theoriegeleitetes Handeln zu überführen, das in einem geeigneten Pflegemodell seinen Ausdruck finden kann. Bemüht man jedoch die gängige Pflegefachliteratur, so lassen sich kaum Hinweise auf ein hospizspezifisches Pflegemodell finden. Mitunter wird der Versuch unternommen, bereits bestehende, etablierte Pflegetheorien auf das pflegerische Hospizgeschehen zu übertragen, was häufig nur in Ansätzen gelingt. Einmal mehr stellt sich daher die Frage, ob Hospize für sich den Bedarf für ein hospizspezifisches Pflegemodell sehen, und wie dieses ausgestaltet werden kann.
Auf der Basis der Ergebnisse einer Interviewbefragung, an der bundesweit 190 stationäre Hospize teilnahmen, wird mit der vorliegenden Arbeit nun der Versuch unternommen, einen Entwurf für ein Pflegemodell für stationäre Hospize zu entwickeln. Das ‚Hospice-Care-Modell‘ bietet als konzeptuelles Modell erste Ansätze zu einem theoretischen Bezugs- oder Handlungsrahmen für die palliative Pflege und Betreuung sterbender Menschen und deren Zugehörigen. Dieser Bezugs- oder Handlungsrahmen gibt keine normativen Strukturen vor, sondern eröffnet theoriegeleitete Räume, die es ermöglichen, dem Sterbenden in der konkreten Pflegehandlung situativ, individuell, flexibel und intuitiv zu begegnen. Der Modellentwurf soll Anstoß und Anleitung für praktisches Handeln geben und zugleich zu einem besseren palliativen Pflegeverständnis beitragen. Er versteht sich als ein erster Schritt, eine theoriebasierte, hospizliche Pflegepraxis zu ermöglichen.
Elektronisch unterstützte transsektorale Kommunikation im Gesundheitswesen ist eine der essentiellen Säulen von eHealth. Sie ist eine menschliche Handlung, die eine Verbesserung der Versorgung Einzelner und ganzer Bevölkerungsgruppen bewirken soll. Ethik bewertet menschliches Handeln in Bezug auf dessen Auswirkungen und die ihm zugrunde liegenden Werte und Normen. Dabei werden die Auswirkungen auf Individuen und Allgemeinheiten betrachtet. Im Gesundheits- und Sozialwesen gelten die Prinzipien der Autonomie, der Schadensverhütung, der Fürsorge und der Gerechtigkeit als Maßstäbe. Es gilt also die Fragen herauszuarbeiten, die an elektronische transsektorale Kommunikation aus ethischer Sicht gestellt werden müssen, um zu untersuchen, ob sie innerhalb der genannten Prinzipien ethischen Anforderungen genügt.
Aus den Ergebnissen einer systematischen Literaturrecherche wurden zunächst allgemein Aussagen zum Thema Information und Technologie im Zusammenhang mit Ethik extrahiert, und daraufhin geprüft, auf welche Fragen sie Antworten anbieten. Diese wurden innerhalb der genannten fünf Prinzipien als Fragen an elektronische transsektorale Kommunikation formuliert.
Aus den Aussagen der Literatur ließen sich sieben Fragen ableiten und den ethischen Prinzipien zuordnen, um mit ihnen elektronische transsektorale Kommunikation zu untersuchen. Auf diese Weise kann geprüft werden ob diese in der Lage sind, das Wohl Einzelner wie auch von Gemeinschaften im Gesundheitswesen zu fördern, wovon Betroffene, Professionelle und das Gesundheitssystem insgesamt profitieren könnten.
Die ambulante Wochenbettbetreuung. Eine qualitative Studie zum Professionellen Handeln von Hebammen

Die aufsuchende Versorgung in der Zeit nach der Geburt eines Kindes ist eine wichtige gesundheitliche Dienstleistung in einer Übergangsphase im Leben von jungen Familien. Im Mittel-punkt der vorliegenden Arbeit steht das professionelle Handeln der in der ambulanten Wochen-bettbetreuung tätigen Hebammen. Aus der Perspektive der Hebammen konnten Handlungsansätze und Entscheidungsmaxime vor dem Hintergrund veränderter Anforderungen in der Berufspraxis dargestellt werden. Ziel der vorliegenden Studie war es, einen theoretischen Beitrag für die professionelle Gestaltung der Wochenbettbetreuung zu geben und die Bedeutung dieses komplexen Handlungsfeldes herauszustellen.
Datengrundlage dieser qualitativen Studie sind offene Leitfadeninterviews mit 28 Hebammen, die über mindestens zwei Jahre Berufserfahrung verfügten und in unterschiedlichen Betreuungssettings sowie zu unterschiedlichen strukturellen Rahmenbedingungen arbeiteten. Die Datenerhebung erfolgte in zwei Erhebungsrunden von April 2013 bis Januar 2015. Die Daten wurden inhaltsanalytisch und rekonstruktiv hermeneutisch analysiert.
Es zeigt sich ein Zusammenspiel von handlungsleitenden Sichtweisen, Handlungsgrundlagen und professionellen Handlungsdeterminanten. Neben konkreten Betreuungskonzepten konnten mit den vorliegenden Ergebnissen verschiedene Handlungsdeterminanten, wie die gesundheitliche Versorgung von Mutter und Kind, die Beziehungsgestaltung zwischen Frau und Hebamme, der Einbezug familialer Unterstützungsleistungen und verschiedene strukturelle Handlungsdeterminanten analysiert werden. Grundelemente der Hebammenarbeit sind die Begleitung und Beobachtung physiologischer mütterlicher Umstellungs- sowie kindlicher Entwicklungsprozesse, aus denen sich sowohl diagnostische, pflegerisch-therapeutische, praktisch-anleitende und beratende Handlungsformen als auch Handlungsformen mit psychosozialer Ausrichtung ableiten. Die Ergebnisse verweisen auf ein heterogenes Praxisfeld, welches einer breiten Expertise der Hebammen bedarf. Die professionelle Gestaltung Hebammenkundlicher Handlungsprozesse sollte im Sinne einer Praxis erfolgen, die Wissen, Können und Reflexion bewusst verbindet und die Autonomie der Lebenspraxis der Frauen und Familien respektiert. Ein genau definierter Versorgungsauftrag sowie evidenzbasierte Leitlinien und Expertenstandards könnten die Hebammen unterstützen, ihre Ressourcen entsprechend einer bestmöglichen postnatalen Versorgung von Frauen und ihren Familien zu nutzen.
Für etwa 13% der Familien mit Säuglingen in Deutschland besteht Unterstützungsbedarf im Rahmen Früher Hilfen. Frühe Hilfen streben an, die Entwicklungsmöglichkeiten von Eltern und ihren Kindern frühzeitig und nachhaltig zu verbessern. Ein zentrales Ziel der Frühen Hilfen, die Kooperation der Berufsgruppen des Sozial- und Gesundheitswesens, ist eine Herausforderung. Zu den Kooperationspartnerinnen/-partnern der Netzwerke gehören freiberufliche Hebammen, deren Perspektive in diesem Kontext jedoch kaum erforscht ist.
Ziel der Studie ist es, die Sichtweisen und Erfahrungen freiberuflicher Hebammen im Rahmen der interprofessionellen Kooperation im Kontext Früher Hilfen zu untersuchen. Der Fokus liegt dabei insbesondere auf der Kooperation der Hebammen mit den Akteurinnen/Akteuren des Sozialwesens.
27 problemzentrierte Interviews mit freiberuflichen Hebammen in 13 deutschen Bundesländern bilden die Basis der qualitativen Untersuchung. Im Vorfeld wurde eine Gruppendiskussion durchgeführt, um die Erhebungsinstrumente für die Haupterhebungsphase daraus abzuleiten. Durch diese Vorgehensweise wurden theoretische Vorüberlegungen gestützt sowie die Authentizität des Forschungsvorhabens gesichert. Der Feldzugang erfolgte über lokale E-Mail-Verteiler, Anzeigen in Hebammenzeitschriften, Besuche von Kongressen sowie durch Unterstützung von Multiplikatorinnen/Multiplikatoren. Die Interviews wurden mittels Leitfaden geführt, digital aufgezeichnet und transkribiert. Die Auswertung erfolgte über die qualitative Inhaltsanalyse nach Kuckartz. Das Vorhaben wurde durch die Ethik-Kommission der Universität Witten/Herdecke bewilligt.
Freiberufliche Hebammen kooperieren häufiger mit Berufsgruppen des Gesundheitswesens als mit jenen des Sozialwesens. Bei der Betreuung von Frauen, Kindern und Familien mit einem besonderen Unterstützungsbedarf spielen informelle Kontakte innerhalb des Gesundheitswesens für freiberufliche Hebammen eine zentrale Rolle.
Kontinuierliche Kontakte, gegenseitiges Interesse und eine Kommunikation, die durch Respekt und klare Absprachen gekennzeichnet ist, tragen zu einer gelingenden Kooperation mit den Akteurinnen/Akteuren in den Frühen Hilfen bei. Fehlendes Wissen der Hebammen über die Strukturen und Angebote Früher Hilfen und die unterschiedliche berufliche Sozialisation der Berufsgruppen des Gesundheits- und Sozialwesens stellen zentrale Herausforderungen für die Zusammenarbeit dar.
Am Übergang von der Hebammen- zur Familienhebammenversorgung sowie zu Angeboten der Kinder- und Jugendhilfe ist die Vertrauensbeziehung zwischen Klientinnen und freiberuflichen Hebammen ein wichtiger Faktor, um Familien mit einem besonderen Unterstützungsbedarf zur Annahme dieser Angebote zu motivieren.
Damit freiberufliche Hebammen ihr gesundheitsförderndes Potential in den Frühen Hilfen entfalten können, bedarf es einer systematischen Einbeziehung dieser Akteurinnen in die Netzwerke der Frühen Hilfen. Zur Stärkung dieses Prozesses sind auch gesundheitspolitische Entscheidungen notwendig. Das Thema Kooperation sollte in der Aus- und Weiterbildung und im Studium von Hebammen berücksichtigt werden.
Background
Forced migration significantly endangers health. Women face numerous health risks, including sexual violence, lack of contraception, sexually transmitted disease, and adverse perinatal outcomes. Therefore, sexual and reproductive healthcare is a significant aspect of women asylum seekers’ health.
Even when healthcare costs of asylum seekers are covered by the government, there may be strong barriers to healthcare access and specific needs may be addressed inadequately. The study’s objectives were a) to assess the accommodation and healthcare services provided to women asylum seekers in standard and specialised health care, b) to assess the organisation of healthcare provision and how it addresses the sexual and reproductive healthcare needs of women asylum seekers.
Methods
The study utilised a multi-method approach, comprising a less-dominant quantitative component and dominant qualitative component. The quantitative component assessed accommodation conditions for women in eight asylum centres using a survey. The qualitative component assessed healthcare provision on-site, using semi-structured interviews with health and social care professionals (n = 9). Asylum centres were selected to cover a wide range of characteristics. Interview analysis was guided by thematic analysis.
Results
The accommodation in the asylum centres provided gender-separate rooms and sanitary infrastructure. Two models of healthcare were identified, which differed in the services they provided and in their organisation: 1) a standard healthcare model characterised by a lack of coordination between healthcare providers, unavailability of essential services such as interpreters, and fragmented healthcare, and 2) a specialised healthcare model specifically tailored to the needs of asylum-seekers. Its organisation is characterised by a network of closely collaborating health professionals. It provided essential services not present in the standard model. We recommend the specialised healthcare model as a guideline for best practise.
Conclusions
The standard, non-specialised healthcare model used in some regions in Switzerland does not fully meet the healthcare needs of women asylum seekers. Specialised healthcare services used in other regions, which include translation services as well as gender and culturally sensitive care, are better suited to address these needs. More widespread use of this model would contribute significantly toward protecting the sexual and reproductive integrity and health of women asylum seekers.
In Germany, a lot of young children at risk of language difficulties still go undetected or are not assessed before preschool-age. For children where parents may suspect a disorder, this practice causes a lot of emotional distress alongside lost time for intervention. Thus, what contribution can parents and nursery staff make for the earlier detection of language difficulties? 34 children from four German kindergartens were tested with a standardized preschool screening for language problems by an SLT. Parents and nursery staff completed a questionnaire (FEE 3-4) that was designed to collect potential risk-factors and included the rating of children’s abilities across the main language domains. Outcomes from the FEE 3-4 were compared between parents and nursery staff as well as triangulated with results from the standardized screening. Agreement between parents and nursery staff re. individual children’s potential language difficulties was moderate (Kappa = 0.44, p = .050). Overall, nursery staff rated children’s language abilities more strictly and precisely than parents. Especially their rating of ‘word order’ (p = .022) and ‘verb endings’ contributed significantly to the identification of potential language difficulties similar to the standardized screening. The screening identified two children at risk without caregiver's concern, but not two others who were at risk of language disorder and for whom caregivers expressed concern. Caregiver’s awareness of early language difficulties appears to be rather intuitive. Young children at risk are most reliably detected if standardized instruments are used in combination with caregiver questionnaires. Ideally, this process includes data from parents and nursery staff to be interpreted by an experienced SLT, as the use of a standardized screening alone may lead to missed or mistaken identification where essential information about the child’s environment (e.g. risk factors) is not provided. If parents are concerned about children’s language, full assessment is clearly justified.
The increasing complexity of caseloads in SLT practice, e.g. due to higher comorbidity, lacking information or experience in the
treatment of complicated cases, calls for support from experienced as well as specialist practitioners from within the field - especially
for novice therapists. One way to tackle these challenges may be peer coaching and how it can be employed within the educational
and professional SLT setting.
Peer coaching was implemented across five semesters of a successive SLT study programme at a University of Applied Sciences in
Germany. The approach was embedded in a clinical reasoning seminar with 25 SLT students who each presented a challenging case
study from their current workload. All participants completed a short online survey to evaluate the feasibility of the team approach
within this setting as well as their personal benefit and development re. the discussed case studies.
Students felt encouraged by being able to share their experience and tackle actual challenges. They particularly valued receiving
answers from a broad range of other SLTs but also contributing to other students’ queries and providing practical solutions for
them. All participants felt that peer coaching was an appropriate approach for clinical reasoning to support their professional as well
as personal development. Other outcomes were a perceived increased ability to employ metacognitive reflection to be used with
their whole caseload but also a prospective need for further training. Some students suggested the employment of peer coaching
within their work setting.
In the educational as well as professional SLT setting, peer coaching can be successfully employed, triggering metacognitive
reflection re. practitioner’s thinking and acting, resulting in an increased awareness of needs and skills as part of the clinical
reasoning process.
Purpose
Attracting skilled students is an important aim of many cities in a knowledge-based society. This paper focuses on urban factors of attractiveness from a student's perspective and analyses their influence on locational choices of students. The criteria found were also used to evaluate how the City of Osnabrück, Germany, is rated in terms of these criteria and to reveal the greatest discrepancies.
Design / Methodology / Approach
The paper is based on a multi-level empirical research concept, including qualitative and quantitative approaches. A survey of 2,300 students was conducted in Osnabrück on the basis of focus group discussions with students and interviews with various experts such as a neighbourhood manager, an urban planner, a district mayor, a college president, a real estate manager.
Originality/value
To date, little research has been undertaken to empirically examine the specific requirements that German students look for in a place to live and study. According to the author’s present state of knowledge (January 2018), a comparable study has not been done.
The main contribution of this paper is the empirical analysis of what makes cities attractive to students. In contrast to the findings of Richard Florida about the Creative Class, the cleanliness of a city, beautiful city scenery, and attractive apartments are more important to students than cultural offers, interesting job opportunities, or a multicultural population.
Practical Implications
Insights from the empirical survey can both help to analyse important factors in students' decision-making process and provide possible measures that the city stakeholders can take.
