Personal health records (PHR) are instruments to compile, store and present health and wellness related data digitally with proven effects on self-management of diseases. The aim of this study was to investigate whether there were differences in the intention to use (ITU) and perceived usefulness (PU) of two technologies allowing users to access the PHR, i.e. a kiosk system and a smart phone based app (access as usual). The study also aimed at modelling ITU and PU with multiple linear regressions. A total of 46 subject participated in the study who were randomly assigned to one of the two experimental groups (nkiosk = 22; napp = 24). The task for both groups was to digitise their “Medikationsplan” (medical record) and upload it to the PHR. There was no significant difference in ITU and PU between the two technologies. ITU could only be significantly explained by PU (R2 = .55, p < 0.001), while PU was determined by perceived ease of use and psychological factors (R2 = .64, p < 0.001). Severity of disease did not play any significant role. The German “Terminservice- und Versorgungsgesetz” underpins the importance and timeliness of this study. The assumption that both – the publicly accessible kiosk and the app – are equally acceptable for people of different gender, age and technology background demonstrates the opportunity to master a potential digital divide among the population and allows users to get access to their PHR in multiple ways.
Introduction: Establishing continuity of care in handovers at changes of shift is a challenging endeavor that is jeopardized by time pressure and errors typically occurring during synchronous communication. Only if the outgoing and incoming persons manage to collaboratively build a common ground for the next steps of care is it possible to ensure a proper continuation. Electronic systems, in particular electronic patient record systems, are powerful providers of information but their actual use might threaten achieving a common understanding of the patient if they force clinicians to work asynchronously. In order to gain a deeper understanding of communication failures and how to overcome them, we performed a systematic review of the literature, aiming to answer the following four research questions: (1a) What are typical errors and (1b) their consequences in handovers? (2) How can they be overcome by conventional strategies and instruments? (3) electronic systems? (4) Are there any instruments to support collaborative grounding?
Methods: We searched the databases MEDLINE, CINAHL, and COCHRANE for articles on handovers in general and in combination with the terms electronic record systems and grounding that covered the time period of January 2000 to May 2012.
Results: The search led to 519 articles of which 60 were then finally included into the review. We found a sharp increase in the number of relevant studies starting with 2008. As could be documented by 20 studies that addressed communication errors, omission of detailed patient information including anticipatory guidance during handovers was the greatest problem. This deficiency could be partly overcome by structuring and systematizing the information, e.g. according to Situation, Background, Assessment and Recommendation schema (SBAR), and by employing electronic tools integrated in electronic records systems as 23 studies on conventional and 22 articles on electronic systems showed. Despite the increase in quantity and quality of the information achieved, it also became clear that there was still the unsolved problem of anticipatory guidance and presenting “the full story” of the patient. Only a small number of studies actually addressed how to establish common ground with the help of electronic tools.
Discussion: The increase in studies manifests the rise of great interest in the handover scenario. Electronic patient record systems proved to be excellent information feeders to handover tools, but their role in collaborative grounding is unclear. Concepts of how to move to joint information processing and IT-enabled social interaction have to be implemented and tested.
Objectives: to identify the errors in daily intensive nursing care and analyze them according to the theory of human error. Method: quantitative, descriptive and exploratory study, undertaken at the Intensive Care Center of a hospital in the Brazilian Sentinel Hospital Network. The participants were 36 professionals from the nursing team. The data were collected through semistructured interviews, observation and lexical analysis in the software ALCESTE®. Results: human error in nursing care can be related to the approach of the system, through active faults and latent conditions. The active faults are represented by the errors in medication administration and not raising the bedside rails. The latent conditions can be related to the communication difficulties in the multiprofessional team, lack of standards and institutional routines and absence of material resources. Conclusion: the errors identified interfere in nursing care and the clients’ recovery and can cause damage. Nevertheless, they are treated as common events inherent in daily practice. The need to acknowledge these events is emphasized, stimulating the safety culture at the institution.
The excitement sparked by the emergence of AI open platforms has encountered significant scrutiny from educators and educational planners, who have raised valid concerns about issues such as plagiarism, testing protocols, and the authenticity of content submitted by students. While these concerns are timely and crucial, it's essential not to overlook other pressing issues that often go unnoticed in the lived educational experience of learners, particularly within the field of social sciences. This paper aims to advocate for a humanistic approach with a focus on education in the generative AI Era.
Masterarbeit im Rahmen des MBA-Studiengangs Hochschul- und Wissenschaftsmanagement an der Hochschule Osnabrück.
Im Kontext des steigenden Digitalisierungsbedarfs und der Herausforderung, Mitarbeitenden die flexible Arbeit von zu Hause aus zu ermöglichen, hat im Zuge der Corona-Pandemie die Diskussion über neue hybride Arbeitsmodelle (im Sinne einer Verzahnung von Präsenzarbeit am Unternehmens-/ Organisationsstandort und orts- und zeitflexibler Arbeit außerhalb) als neue Normalität erheblich zugenommen.
Der Wandel der Arbeitswelt beschäftigt auch die deutschen Hochschulen, wo sich die Arbeitsweise in Reaktion auf die Veränderungen der letzten Jahre transformiert hat und die Corona-Pandemie als Verstärker für neue Arbeitsformen erkannt wird. Vor dem Hintergrund der dynamischen Weiterentwicklung ist Gegenstand der Arbeit, mit einer Bestandsaufnahme zu klären, inwieweit orts- und zeitflexibles Arbeiten insbesondere im Rahmen hybrider Arbeit an deutschen Hochschulen ermöglicht wird bzw. werden kann.
Im Frühsommer 2023 wurde eine anonyme Umfrage unter Personalverantwortlichen an deutschen Hochschulen bundesweit online durchgeführt. Der Analyse wurde New Work im Sinne eines ganzheitlichen Gestaltungsrahmens, der die Dimensionen People (Entwicklung und Führung von Mitarbeitenden), Places (Schaffung flexibler Arbeitsumgebungen) und Tools (digitale Plattformen und Werkzeuge zur Umsetzung neuer Arbeitsweisen) berücksichtigt, modellhaft zugrunde gelegt.
Die Ergebnisse zeigen, dass flexible Arbeitsbedingungen sowohl bei wissenschaftlichem Personal als auch in der Verwaltung, zugenommen haben. Es zeigt sich deutlich, dass hybrides Arbeiten an deutschen Hochschulen zum vorherrschenden Modell werden wird, wobei verschiedene Konzepte existieren.
Die Einführung hybrider Modelle bringt vielfältige Herausforderungen mit sich. Die hiermit angesprochene Themenpalette reicht dabei von Digitalisierung und Raumumgestaltung des Präsenzarbeitsortes bis hin zu Arbeitskultur und soziale Beziehungen. Die Hochschulen haben sich auf den Weg gemacht, um für sich eine passende Balance zwischen strukturierenden Rahmenbedingungen und Flexibilität zu erreichen.
Die umfassende Umsetzung hybrider Arbeit macht einen ganzheitlichen New Work-Ansatz empfehlenswert, um sicherzustellen, dass alle Beteiligten in dieser Transformation berücksichtigt werden. Insgesamt wird hybrides Arbeiten zunehmend zur Normalität an deutschen Hochschulen, was zu einer flexibleren und vielfältigeren Arbeitsumgebung beiträgt.
Pflegebedürftige, die sich für die Geldleistungsoption in der Pflegeversicherung entschieden haben, sind verpflichtet, jährlich zwei bzw. vier „Beratungsbesuche“ durch einen zugelassenen ambulanten Pflegedienst abzurufen und nach Ausführung der Pflegekasse anzuzeigen. Obwohl mehr als 2 Millionen dieser Beratungsbesuche jährlich in Deutschland stattfinden, ist nur wenig über ihre Durchführung und die Erfahrungen pflegebedürftiger Menschen und ihrer Angehörigen dazu bekannt. Ziel dieses Projektes war es daher, Einsichten in diese häusliche Pflegeberatung zu erhalten und einen inhaltlichen Ansatz zu ihrer Durchführung zu entwickeln. In zwei Regionen Nordrhein-Westfalens wurden mithilfe eines Fragebogens und ergänzender Interviews Daten zu der generellen Durchführung der Beratungsbesuche, der Perspektive pflegebedürftiger Menschen und ihrer Angehörigen sowie der Sichtweise der ambulanten Pflegedienste erhoben. Basierend auf den Befunden wurde ein familienorientierter Beratungsansatz entwickelt und in 80 häuslichen Pflegearrangements erprobt. Die Ergebnisse zeigen, dass trotz beachtlicher Entwicklungen in einigen Pflegediensten zur Gestaltung der Beratung die Angemessenheit und Qualität aus Sicht der Nutzer/innen vorwiegend vom Zufall abhängt. Der familienorientierte Beratungsansatz eröffnet die Möglichkeit für ein vertiefendes Verständnis der häuslichen Pflegearrangements, auf dessen Grundlage gezielte und den Problemen angemessene Empfehlungen gegeben werden können.
Employee cooperation is fundamental for accomplishing successful organizational change processes. Therefore, it is important to understand how employees' cooperation can be supported in the context of organizational change. Based on the group engagement model, we hypothesized how procedural justice affects organizational identification which in turn should have an affect on employees' cooperation (commitment to change, values-congruence fit, and change-supporting behavior) in the context of organizational change. To test the fit of the proposed model, structural equation models were calculated using both cross-sectional (N = 315) and longitudinal (N = 110) data of academic staff at a German university. Results indicated adequate data fit to our proposed model and revealed that organizational identification mediated the positive effects of procedural justice on affective commitment to change and values-congruence fit. The assumed mediating effect of organizational identification on the positive relationship between procedural justice and change-supporting behavior could only be supported using cross-sectional data.
Einsatzbereich
Ab 16 Jahren. Einsatzbereiche sind Mitarbeiterbefragungen, Evaluation, Organisationsentwicklung/Change Management, Personalentwicklung, Führung (z. B. Einschätzung der Zufriedenheit mit Führungsverhalten durch Mitarbeitende oder Einschätzung der Zufriedenheit mit Mitarbeitenden durch Führungskräfte), Leistungsbeurteilung, interne Personalauswahl, Forschung. Die Durchführung ist sowohl im Einzel- als auch Gruppensetting möglich. Das IAZ kann branchenübergreifend eingesetzt werden, eignet sich jedoch nicht für die Untersuchung selbstständiger Personen.
Inhalt
Das IAZ ist ein Selbstbeurteilungsinstrument zur differenzierten Erfassung der Arbeitszufriedenheit. Es stehen eine Langversion mit 94 Items sowie eine ökonomische Kurzversion mit 8 Items zur Verfügung. Die Langversion erfasst 14 Facetten (Primärskalen) der Arbeitszufriedenheit, die sich 8 Bereichen zuordnen lassen: 1. Arbeitsaufgaben (Zufriedenheit mit Arbeitsaufgaben), 2. Rahmenbedingungen (Zufriedenheit mit Arbeitszeit, Arbeitsplatz, Entlohnung), 3. Entwicklungsmöglichkeiten (Zufriedenheit mit Entwicklungsmöglichkeiten), 4. Führung (Zufriedenheit mit Mitarbeiterorientierung, Aufgabenorientierung der Führungskraft), 5. Kolleginnen und Kollegen (Zufriedenheit mit Kollegialität, Leistung), 6. Arbeitgeber (Zufriedenheit mit Arbeitgeber), 7. Kundinnen und Kunden (Zufriedenheit mit Zusammenarbeit, Feedback), 8. Mitarbeiterinnen und Mitarbeiter (Zufriedenheit mit Leistung/Zusammenarbeit, Feedback). Bei der Auswertung wird zusätzlich eine Sekundärskala Allgemeine Arbeitszufriedenheit (AAZU) als Globalmaß gebildet. Falls für die befragte Person ein einzelner Bereich nicht einschätzbar ist (z. B. weil kein Kundenkontakt besteht), kann der entsprechende Abschnitt des Fragebogens übersprungen werden. Die Kurzversion erfasst ausschließlich die allgemeine Arbeitszufriedenheit (AZU). Auch in der Kurzversion können bis zu zwei Fragen, die aufgrund der Arbeitstätigkeit nicht passend sind, übersprungen werden. Auswertung und Interpretation erfolgen sowohl rohwertbasiert als auch normwertbasiert: Auf der Basis der Rohwerte (Mittelwerte der Punktwerte pro Skala) ist eine Einschätzung der absoluten Zufriedenheit einer Person möglich, d. h. die Feststellung, ob eine untersuchte Person unzufrieden, neutral eingestellt oder zufrieden ist. Auf Basis der Normwerte ist eine Einschätzung der relativen Zufriedenheit möglich, d. h. die Feststellung, ob eine untersuchte Person im Vergleich zur Normstichprobe unterdurchschnittlich, durchschnittlich oder überdurchschnittlich zufrieden ist. Es stehen Auswertungsmaterialien sowohl für die Auswertung einzelner Fragebögen als auch für Gruppenauswertungen über mehrere Fragebögen hinweg (Einschätzungen von Beschäftigten aus einem oder mehreren Teams) zur Verfügung.
Zuverlässigkeit
Die Innere Konsistenz (Cronbachs Alpha) der Primärskalen des IAZ variiert in der Gesamtnormierungsstichprobe zwischen .78 und .97 und beträgt für die Globalskala AAZU .97. Die Innere Konsistenz (Cronbachs Alpha) der Skala AZU des IAZ-K beträgt .83.
Testgültigkeit
Es liegen 13 Studien vor, in denen der Zusammenhang zu Lebenszufriedenheit, Arbeitszufriedenheit, Commitment, Leistungsmotivation, berufliche Leistung, Stresserleben, Psychosomatische Symptome, Krankheitstage, Humor, Sozialkompetenz der Vorgesetzen, Organisationsklima, Persönlichkeitsmerkmalen (Big 5), Gehalt und Arbeitszeit berechnet wird. Darüber hinaus wurde die Anfälligkeit für die sozial erwünschte Selbstdarstellung sowie die soziale Validität des Verfahrens überprüft.
Normen
Das IAZ und das IAZ-K wurde an jeweils insgesamt 4199 Personen im Altersbereich 18 bis 67 Jahre normiert. Neben den Normen (Prozentränge, Stanine-Werte, Standardwerte) der Gesamtnormierungsstichprobe liegen Normen für Berufstätige mit vs. ohne Führungsverantwortung vor. Dabei wird zusätzlich jeweils zwischen weiblichen und männlichen Personen unterschieden. Bei der Untersuchung von 16- oder 17-Jährigen wird ausschließlich eine rohwertbasierte Auswertung und Interpretation vorgenommen.
Bearbeitungsdauer
Die Bearbeitungszeit der Langversion beträgt ca. 15 Minuten. Die Bearbeitung der Kurzversion IAZ-K nimmt weniger als 5 Minuten Zeit in Anspruch. Es besteht keine Zeitbegrenzung.

Inhalt: Mit Hilfe des Inventars zur Erfassung von Arbeitsmotiven wird erfasst, welche Motive eine Person durch ihre berufliche Tätigkeit befriedigen möchte. Dabei werden 16 grundlegende Arbeitsmotive (Primärmotive) unterschieden: Selbstbezug, Autonomie, Entwicklung, Abwechslung, Selbstwert, Führung, Materielles, Macht, Ansehen, Leistung, Komfort, Prosozialität, Anschluss, Aktivität, Sicherheit und Work-Life-Balance. Die Primärmotive lassen sich zu 4 übergeordneten Arbeitsmotiven (Sekundärmotive) gruppieren: Individualität, Karriere, Soziales und Privatleben. Es liegen zwei Selbstbeurteilungsformen vor, eine Langversion mit 97 Items (IEA) und eine Kurzversion mit 32 Items (IEA-K). Mit Hilfe der Langversion können sowohl die Primär- als auch die Sekundärmotive untersucht werden. Die Kurzform IEA-K erfasst ausschließlich die vier Sekundärmotive.
Zusätzlich existieren zwei Fragebögen zur Arbeitsplatzanalyse (IEA-A und IEA-A-K), welche sich auf die identischen Arbeitsmotive beziehen wie die beiden Selbstbeurteilungsbögen. Mit den beiden Fragebögen können konkrete Arbeitsplätze dahingehend untersucht werden, inwieweit sie den Arbeitsplatzinhabern die Möglichkeit bieten, bestimmte Arbeitsmotive zu befriedigen. Um eine möglichst messgenaue und aussagekräftige Arbeitsplatzanalyse zu erhalten, sollten immer mehrere Arbeitsplatzexperten (ca. 2-5 Personen) befragt werden. Je nach Fragestellung lassen sich die Selbstbeurteilungsbögen und die Fragebögen zur Arbeitsplatzanalyse gemeinsam oder unabhängig voneinander einsetzen. Bei einer kombinierten Anwendung sollte darauf geachtet werden, dass entweder beide Langversionen (IEA und IEA-A) oder beide Kurzversionen (IEA-K und IEA-A-K) gemeinsam eingesetzt werden.
Zuverlässigkeit: Die Split-Half-Reliabilität der Primärskalen des IEA variiert in der Gesamtstichprobe (N = 4.662) zwischen r = .73 und r = .92, die der Sekundärskalen zwischen r = .81 und .92. Für das IEA-K liegen die Werte zwischen r = .83 und r = .89. Die Split-Half-Reliabilität der Primärskalen des IEA-A variiert zwischen r = .76 und r = .94, die der Sekundärskalen zwischen r = .89 und r = .97 (N = 300). Die Werte für die Kurzversion IEA-A-K liegen zwischen r = .84 und r = .94.
Testgültigkeit: Es liegen 13 Studien vor, in denen der Zusammenhang der Arbeitsmotive zu den Big Five sowie zu verschiedenen Motiven (Leistungsmotivation, Führungsmotivation, Anschlussmotivation, Gestaltungsmotivation), zu Kriterien bei der Auswahl potenzieller Arbeitgeber, zu allgemeinen Interessen, Berufsinteressen sowie zu Unterschieden zwischen Personengruppen (Studienfach, Berufstätigkeit mit vs. ohne Führungsverantwortung) untersucht wurden. Zudem wurde die Anfälligkeit des IEA und des IEA-K für die sozial erwünschte Selbstdarstellung sowie die soziale Validität des Verfahrens überprüft.
Normen: Das IEA und das IEA-K wurden an insgesamt 4.662 Personen normiert. Geschlechtsspezifische Normwerte (Prozentränge, Stanine-Werte, Standardwerte) liegen für die Gesamtstichprobe, für Berufstätige ohne Führungsverantwortung (N = 1.223), für Führungskräfte (N = 607) sowie für Nicht-Berufstätige (N = 2.832) vor. Für die Arbeitsplatzanalyse (IEA-A und IEA-A-K) entfällt eine Normierung.
Bearbeitungsdauer: Die Durchführung des IEA und des IEA-A dauert jeweils etwa 15 Minuten. Die Durchführung der Kurzversionen IEA-K und IEA-A-K nimmt jeweils etwa 5 Minuten in Anspruch. Es besteht keine Zeitbegrenzung.

Background: This paper describes an international nursing and health research immersion program. Minority students from the USA work with an international faculty mentor in teams conducting collaborative research. The Minority Health International Research Training (MHIRT) program students become catalysts in the conduct of cross-cultural research.
Aim: To narrow the healthcare gap for disadvantaged families in the USA and partner countries.
Methods: Faculty from the USA, Germany, Italy, Colombia, England, Austria and Thailand formed an international research and education team to explore and compare family health issues, disparities in chronic illness care, social inequities and healthcare solutions. USA students in the MHIRT program complete two introductory courses followed by a 3-month research practicum in a partner country guided by faculty mentors abroad. The overall program development, student study abroad preparation, research project activities, cultural learning, and student and faculty team outcomes are explored.
Results: Cross-fertilization of research, cultural awareness and ideas about improving family health occur through education, international exchange and research immersion. Faculty research and international team collaboration provide opportunities for learning about research, health disparities, cultural influences and healthcare systems. The students are catalysts in the research effort, the dissemination of research findings and other educational endeavours. Five steps of the collaborative activities lead to programmatic success.
Conclusions: MHIRT scholars bring creativity, enthusiasm, and gain a genuine desire to conduct health research about families with chronic illness. Their cultural learning stimulates career plans that include international research and attention to vulnerable populations.
Aims: Intercultural competence has become a key-competence, since the world has become more and more volatile, uncertain, complex and ambiguous. Therefore, insights in the development of intercultural competence and its´ links to individual traits are crucial for companies and researchers to face the requirements in a VUCA world. This study examines the relationship between the time, students spent abroad, personality traits and circumstances during this time with the student’s intercultural competence and integration performance in the target culture. The study had a correlative cross-sectional design. Design and sample: A total of 202 academic subjects were surveyed. The average age was 22 years. There was one measuring time, to which 58 % of the participants stated that they have had a stay abroad. Measurements: Metacognitive, cognitive, motivational and behavioural intercultural competence were measured with the Cultural Intelligence Scale. The personality traits involvement, discipline, social competence, cooperation, dominance and stability were captured with the "Bochum inventory for job-related personality description-6F". Work-related attitudes as patterns of behaviour and experience were measured using the "Work-related Behaviour and Experiencing Pattern 44" (German: Arbeitsbezogene Verhaltens- und Erlebensmuster, AVEM). This scale captures the way participants relate to work in general based on the measurement of personality traits and their fit to specific clusters, which describe, whether individuals have healthy or risky patterns. In addition, the demographic factors and characteristics of stays abroad as well as the integration into the target culture based on the Sociocultural Adaption Scale were examined. The data was tested for relationships and differences by tests for mean differences, variance and regression analyses. Results: There was a positive correlation between duration and cognitive, motivational and behavioural intercultural competence. The motivational competence is higher in subjects who have no risk pattern in the AVEM. The different types of competence influence each other at diverse times. Moreover, the suggested structural equation model could be confirmed. This showed the effect of the AVEM pattern on intercultural competence, moderated by the stay abroad and the social competence. Thus, the study contributes to the understanding of both the measurement of intercultural competence and the development process of intercultural competence in a globalized world.
Aims: This study examines the relationship between the time, students spent abroad, personality traits and circumstances during this time with the student’s intercultural competence and integration performance in the target culture. Design and sample: The study had a correlative cross-sectional design. 202 academic subjects were surveyed. The average age was 22 years. There was one measuring time, to which 58 % of the participants stated that they have had a stay abroad. Measurements: Metacognitive, cognitive, motivational and behavioural intercultural competence were measured with the Cultural Intelligence Scale. The personality traits involvement, discipline, social competence, cooperation, dominance and stability were captured with the "Bochum inventory for job-related personality description-6F". Work-related attitudes as patterns of behaviour and experience were measured using the "Work-related Behaviour and Experiencing Pattern 44" (Geman: Arbeitsbezogene Verhaltens- und Erlebensmuster; AVEM). In addition, the demographic factors and characteristics of stays abroad as well as the integration into the target culture based on the Sociocultural Adaption Scale were examined. The data was tested for relationships and differences by tests for mean differences, variance and regression analyses. Results: There was a positive correlation between duration and cognitive, motivational and behavioural intercultural competence. The motivational competence is higher in subjects who have no risk pattern in the AVEM. The different types of competence influence each other at diverse times. Moreover, the suggested structural equation model could be confirmed. This showed the effect of the AVEM pattern on intercultural competence, moderated by the stay abroad and the social competence.
Health IT adoption research is rooted in Rogers' Diffusion of Innovation theory, which is based on longitudinal analyses. However, many studies in this field use cross-sectional designs. The aim of this study therefore was to design and implement a system to (i) consolidate survey data sets originating from different years (ii) integrate additional secondary data and (iii) query and statistically analyse these longitudinal data. Our system design comprises a 5-tier-architecture that embraces tiers for data capture, data representation, logics, presentation and integration. In order to historicize data properly and to separate data storage from data analytics a data vault schema was implemented. This approach allows the flexible integration of heterogeneous data sets and the selection of comparable items. Data analysis is prepared by compiling data in data marts and performed by R and related tools. IT Report Healthcare data from 2011, 2013 and 2017 could be loaded, analysed and combined with secondary longitudinal data.
Multiple-group confirmatory factor analysis (MG-CFA) is among the most productive extensions of.structural equation modeling. Many researchers conducting cross-cultural or longitudinal studies are interested in testing for measurement and structural invariance. The aim of the present paper is to provide a tutorial in MG-CFA using the freely available R-packages lavaan, semTools, and semPlot. The combination of these packages enable a highly efficient analysis of the measurement models both for normally distributed as well as ordinal data. Data from two freely available datasets – the first with continuous the second with ordered indicators - will be used to provide a walk-through the individual steps.
Background: Crowding in emergency departments (ED) has a negative impact on quality of care and can be averted by allocating additional resources based on predictive crowding models. However, there is a lack in effective external overall predictors, particularly those representing public activity.
Objectives: This study, therefore, examines public activity measured by regional road traffic flow as an external predictor of ED crowding in an urban hospital.
Methods: Seasonal autoregressive cross-validated models (SARIMA) were compared with respect to their forecasting error on ED crowding data.
Results: It could be shown that inclusion of inflowing road traffic into a SARIMA model effectively improved prediction errors.
Conclusion: The results provide evidence that circadian patterns of medical emergencies are connected to human activity levels in the region and could be captured by public monitoring of traffic flow. In order to corroborate this model, data from further years and additional regions need to be considered. It would also be interesting to study public activity by additional variables.

Die deutsche Ratifizierung der UN-Konvention über die Rechte von Menschen mit Behinderungen (UN-BRK) aus dem Jahre 2009 schließt ein für alle Menschen bestehendes Recht auf ein frühkindliches Bildungsangebot, den Zugang zum allgemeinbildenden Schulsystem sowie schließlich das Recht auf Teilhabemöglichkeiten am berufsbildenden Bereich und am Arbeitsmarkt ein. Auch wenn in einigen Bereichen bereits gesetzlich verbindliche Regelungen getroffen werden, steht Deutschland bei der Umsetzung dieses Rechts auf Inklusive Bildung im internationalen Vergleich erst noch am Beginn. Dieser Band versammelt ausgewählte internationale Perspektiven auf unterschiedliche politische, strategische und praktische Implementierungsbemühungen Inklusiver Bildung und macht verschiedene Vorstellungen des Inklusionsbegriffs sichtbar. Die Auseinandersetzung mit diesen unterschiedlichen Zugängen soll eine ermutigende Hilfestellung für die deutsche Umsetzungspraxis geben und Verständnis für eine international differenzierte Betrachtung des Inklusionsbegriffs herstellen.
Objectives
To investigate differences in pressure pain thresholds (PPTs) and longitudinal mechanosensitivity of the greater occipital nerve (GON) between patients with side-dominant head and neck pain (SDHNP) and healthy controls. Evaluation of neural sensitivity is not a standard procedure in the physical examination of headache patients but may influence treatment decisions.
Methods
Two blinded investigators evaluated PPTs on two different locations bilaterally over the GON as well as the occipitalis longsitting-slump (OLSS) in subjects with SDHNP (n = 38)) and healthy controls (n = 38).
Results
Pressure pain sensitivity of the GON was lower at the occiput in patients compared to controls (p = 0.001). Differences in pressure sensitivity of the GON at the nucheal line, or between the dominant headache side and the non-dominant side were not found (p > 0.05). The OLSS showed significant higher pain intensity in SDHNP (p < 0.001). In comparison to the non-dominant side, the dominant side was significantly more sensitive (p = 0.004).
Discussion
Palpation of the GON at the occiput and the OLSS may be potentially relevant tests in SDHNP. One explanation for an increased bilateral sensitivity may be sensitization mechanisms. Future research should investigate the efficacy of neurodynamic techniques directed at the GON.
This paper introduces ideas to reduce talent scarcity by binding female talent in India. As a theoretical lens Neo-Institutionalism in the Indian context is combined with the family-relatedness of work decision model. The qualitative research design and first results as well as propositions for companies are included.
Building on Hofstede's finding that individualism and social hierarchy are incompatible at the societal level, the authors examined the relationship between individualism-collectivism and orientations toward authority at the individual level. In Study 1, authoritarianism was related to three measures of collectivism but unrelated to three measures of individualism in a U.S. sample (N = 382). Study 2 used Triandis's horizontal-vertical individualism-collectivism framework in samples from Bulgaria, Japan, New Zealand, Germany, Poland, Canada, and the United States (total N = 1,018). Both at the individual level and the societal level of analysis, authoritarianism was correlated with vertical individualism and vertical collectivism but unrelated to horizontal collectivism. Horizontal individualism was unrelated to authoritarianism except in post-Communist societies whose recent history presumably made salient the incompatibility between state authority and self-determination.
Mit der Ausgestaltung von Ganztagsschulen korrespondieren organisatorische und pädagogische Merkmale, die einen Rahmen schaffen können, um eine neue individualisierende sowie unterrichtliche Bildung und außerunterrichtliche Erziehung zusammenführende Praxis zu etablieren. Insbesondere erweiterte Kompetenzanforderungen sowie neue Formen der intra- und interprofessionellen Kooperation zeigen sich hierbei als Merkmal professioneller Förderung. Im Beitrag interessiert uns daher die Frage, inwieweit die fachliche Maxime der individuellen Förderung im Schulalltag anschlussfähig an das pädagogische Handeln der in den Ganztag eingebunden Akteure ist. Hier zeigen die berichteten Ergebnisse unserer Studie eine kaum operationalisierbare und differenzierbare Matrix von solchen Handlungsprozessen, die auf einer systematischen methodischen Grundlage basieren. Es dominiert ein eher situatives, den alltäglichen Ausprägungen von Förderbedarfen gerecht werdendes Handeln, das jedoch kaum in Bezug zu einem Meta-Wissen, einem Verweisungs- und Analysewissen gesetzt wird. Gegenwärtig zeigen sich auf Seiten der Ganztagsakteure unterschiedliche Interventionssituationen und Rollenerwartungen im Kontext der Förderung, die es nicht anzugleichen, aber dennoch professionstheoretisch und -praktisch in ihrer Komplementarität zu verorten gilt.
Die ethischen Fragen und Spannungsfelder individueller Förderung in inklusiven Schulen werden in der Ausbildung von Lehrkräften bisher nicht ausreichend behandelt. Gleichwohl wird erwartet, dass Lehrkräfte neben fachlichen und methodischen Kompetenzen auch ethische Kompetenzen in die Förderdiagnostik und das anschließende Fördervorgehen einbringen: Sie sollen ethische Implikationen ihrer Entscheidungen reflektieren, über eine wertebasierte Urteilsfähigkeit verfügen sowie Verantwortung für ihr pädagogisches Handeln übernehmen. Um diesen Anforderungen gerecht werden zu können, wird in diesem Beitrag für die Einführung einer ethischen Ausbildung angehender Lehrkräfte plädiert. Vorgestellt wird eine Modellkonzeption für zwei konsekutive Module im bildungswissenschaftlichen Anteil von Lehramtsstudiengängen, in denen Studierende eine Ethical Literacy in Bezug auf ihr förderpädagogisches Handeln in inklusiven Schulen erwerben.
Im Zuge der Digitalisierung ist das Interesse an offenen Daten und Informationen enorm gestiegen. Für die öffentliche Verwaltung ist das eine große Chance, die genutzt werden sollte, um Transparenz, Beteiligung und Zusammenarbeit mit der Öffentlichkeit zu fördern und der Rechenschaftspflicht nachzukommen. Die Daten werden als Open Data uneinge-schränkt, kostenlos und zur freien Weiterverwendung zur Verfügung gestellt. Im Rahmen einer Dokumentenanalyse und persönlicher Interviews mit ausgewählten Mitarbeitern des Landkreises Osnabrück sollen Potenziale und Herausforderungen der Kreisverwaltung hin-sichtlich Open Data und der Einführung eines Open Data-Portals aufgezeigt werden. Die australische Hauptstadt Canberra wird als Best-Practice-Beispiel für die erfolgreiche Um-setzung von Open Data und eines Open Data-Portals angeführt. Aus den Untersuchungen ergeben sich vielfältige Handlungsmöglichkeiten und -empfehlungen, das kommunale Ver-waltungshandeln offener und transparenter zu gestalten. So sollen die Bürgerbeteiligung und die Zusammenarbeit mit der Öffentlichkeit verbessert und gleichzeitig dem politischen Auftrag im Sinne der Rechenschaftspflicht nachgekommen werden.

Die Forderung nach Inklusion, deren rechtliche Ausgestaltung und ihre kommunale Umsetzung müssen als einer der bedeutsamsten Paradigmenwechsel der letzten Jahre in der internationalen und nationalen Politik verstanden werden, die auch in der Kinder- und Jugendhilfe Fragen nach Ausgrenzungsmechanismen, Teilhabemöglichkeiten, strukturellen Ressourcen und persönlichen Einstellungen aufwirft.
Das Ziel der vorliegenden Studie liegt darin, den Stand der konzeptionellen Verankerung eines inklusiven Betreuungs- und Bildungsangebotes in Kindertageseinrichtungen der Stadt und des Landkreises Osnabrück empirisch zu erfassen. Hierzu wurde eine qualitative Inhaltsanalyse der pädagogischen Konzeptionen von 112 Einrichtungen vorgenommen. Die Ergebnisse zeigen, dass nur in 9,8 % der Konzeptionen der Begriff „Inklusion“ verwendet wird. Förder- und Unterstützungsbedarfe von Kindern mit Beeinträchtigungen werden in 48,2 % der Konzeptionen angesprochen. In weniger als der Hälfte der Konzeptionen (43,8 %) wird die interprofessionelle Zusammenarbeit im Team von Kindertageseinrichtungen thematisiert, während fast alle Einrichtungen externe Kooperationspartner benennen. Maßnahmen zur Personal- und Organisationsentwicklung im Kontext von Inklusion werden kaum benannt. Insgesamt ist im Zuge der Umsetzung von Inklusion eine Weiterentwicklung der pädagogischen Konzeptionen von Kindertageseinrichtungen erforderlich.
Der Band entwirft ein Grundkonzept der Implementierung inklusiver Bildung auf unterschiedlichen Ebenen und geht dabei auf Fragen des Rechtes, der Organisation und Profession sowie auf kommunale Gestaltungsmöglichkeiten einer lebensweltlichen Vielfalt ein. Dabei stehen die Bereiche Schule und schulbezogene Kinder- und Jugendhilfe, Tagesbetreuung sowie Arbeit im Mittelpunkt der Analysen.
Schule und Kinder- und Jugendhilfe sind in der Kommune gleichermaßen Gegenstand von Fachplanungen. Gerade inklusive Bildung verlangt ein Zusammenwirken beider Felder nicht nur in der (sozial-)pädagogischen Praxis, sondern auch in der kommunalen Steuerung sowie Planung entsprechender Angebotsstrukturen. Wie gelingen derartig abgestimmte Prozesse? Welches Verständnis von Inklusion leitet dabei das Planungsgeschehen und welche Maßstäbe lenken Steuerungsentscheidungen?
Diesen Fragen geht das Buch nach, indem empirische Befunde aus Untersuchungen in drei niedersächsischen Kommunen dargestellt und Konsequenzen für Theorie, Forschung und Praxis in diesem Themenkomplex abgeleitet werden.
Die Umsetzung eines inklusiven Bildungsangebotes in frühpädagogischen
Einrichtungen ist eines der zentralen Themen in der aktuellen Debatte um die Konzeptentwicklung
in Kindertageseinrichtungen. Die Forschungsnotiz stellt eine Studie vor, die die Umsetzung
von inklusiver Bildung in Kindertageseinrichtungen in Niedersachsen in den Blick nimmt. Sie
beschreibt auf den Ebenen Person, Konzept und Struktur Umsetzungsprobleme aus Sicht der
beteiligten Personen. Auf dieser Grundlage werden Lösungsansätze für die Realisierung eines
frühkindlichen inklusiven Bildungsangebotes entwickelt. Eine besondere Rolle spielen in dieser
Studie Fragen zur Organisationsgestaltung und Personalentwicklung. Ziel ist es, durch Inklusion
bedingte Veränderungen von Arbeitsanforderungen und -bedingungen zu analysieren und daraus
konzeptionelle Empfehlungen für die Organisations- und Personalentwicklung zu entwickeln.
Gute Kooperationsstrukturen gelten als eine der wichtigsten Gelingensbedingungen bei der Umsetzung schulischer Inklusion. Eine enge und abgestimmte Zusammenarbeit im Schulteam ist eine Voraussetzung, um Kinder optimal fördern zu können und ihren individuellen Bedarfen gerecht zu werden. Dieser Band führt ein in das Praxismanual "Interprofessionelle Kooperation an inklusiven Grundschulen", kurz InproKiG: Die Modulinhalte des Manuals werden durch vielfältige Materialien vermittelt, zudem regen zahlreiche Methoden zum Erfahrungsaustausch und zur Vernetzung an. Ziel ist es, die berufsübergreifende Zusammenarbeit an inklusiven Grundschulen zu verbessern und somit praxisrelevante Impulse für die Schulentwicklung zu setzen.
Die Innerbetriebliche Leistungsverrechnung ist ein wesentlicher Bestandteil der Kosten-, Leistungs-, Erlös- und Ergebnisrechnung, denn über die Verrechnungsgrößen können unterschiedliche Preise generiert werden. Innovation braucht Freiraum

Innovationen sind positiv besetzt. Deshalb reicht es nicht aus, dass etwas „neu“ ist, es muss „innovativ“ sein. Viele verbinden das mit etwas Sensationellem, das große Aufmerksamkeit auf sich lenkt und das revolutionäre Veränderungen mit sich bringt – am besten schlagartig. Gerne werden auch alle technischen Neuerungen als Innovationen bezeichnet. Aber stimmt das denn auch?
Soziale Arbeit, etwa in Form von Kinder- und Jugendhilfe, ist ein fester Bestandteil der sozialen Infrastruktur in Kommunen. Sie wirkt aktiv mit an der Gestaltung von Lebens- und Entwicklungsräumen junger Menschen und Familien, bietet Unterstützung in schwierigen Erziehungssituationen sowie Schutz und Hilfe in prekären Lebenslagen. Schaut man auf die aktuellen Entwicklungsthemen und -ziele von Kommunen, so fällt auf, dass sie vermehrt jugendhilfespezifische und -relevante Gestaltungsziele in ihre Programme und Leitbilder der Stadtentwicklung aufnehmen (vgl. Maykus/Windheuser 2010). Die Themen Familienförderung und -bildung, Bildung junger Menschen, Kinderfreundlichkeit oder Kommune als Bestandteil einer lernenden Region sind längst bedeutsame Maximen der Kommunalentwicklung geworden.
Multinational health IT benchmarks foster cross-country learning and have been employed at various levels, e.g. OECD and Nordic countries. A bi-national benchmark study conducted in 2007 revealed a significantly higher adoption of health IT in Austria compared to Germany, two countries with comparable healthcare systems. We now investigated whether these differences still persisted. We further studied whether these differences were associated with hospital intrinsic factors, i.e. the innovative power of the organisation and hospital demographics. We thus performed a survey to measure the “perceived IT availability” and the “innovative power of the hospital” of 464 German and 70 Austrian hospitals. The survey was based on a questionnaire with 52 items and was given to the directors of nursing in 2013/2014. Our findings confirmed a significantly greater IT availability in Austria than in Germany. This was visible in the aggregated IT adoption composite score “IT function” as well as in the IT adoption for the individual functions “nursing documentation” (OR = 5.98), “intensive care unit (ICU) documentation” (OR = 2.49), “medication administration documentation” (OR = 2.48), “electronic archive” (OR = 2.27) and “medication” (OR = 2.16). “Innovative power” was the strongest factor to explain the variance of the composite score “IT function”. It was effective in hospitals of both countries but significantly more effective in Austria than in Germany. “Hospital size” and “hospital system affiliation” were also significantly associated with the composite score “IT function”, but they did not differ between the countries. These findings can be partly associated with the national characteristics. Indicators point to a more favourable financial situation in Austrian hospitals; we thus argue that Austrian hospitals may possess a larger degree of financial freedom to be innovative and to act accordingly. This study is the first to empirically demonstrate the effect of “innovative power” in hospitals on health IT adoption in a bi-national health IT benchmark. We recommend directly including the financial situation into future regression models. On a political level, measures to stimulate the “innovative power” of hospitals should be considered to increase the digitalisation of healthcare.
The paper presents and analyses the structure and consequences of limits to growth for the global economy. Apart from the famous report for the Club of Rome, a wide range of related literature, which all caution against the idea of unrestricted growth, is also covered. In presenting side tracks in economics, major differences towards standard economic reasoning are highlighted; these are particularly centred on the concepts of technology, sustainability, and resources. One of the central conclusions is that technological progress alone cannot be the knight in shining armour to save the day amid increasing stress on the sides of sources and sinks of the global system. Further, oil production, consumption, and reserve patterns are discussed, emphasising that a continuous underestimation of the challenges ahead would be distressing. Finally, the development of non-advanced countries is hindered, as the diminishing availability of oil reserves could prove as a significant constraint.
Introduction Development and implementation of appropriate health policy is essential to address the rising global burden of non-communicable diseases (NCDs). The aim of this study was to evaluate existing health policies for integrated prevention/management of NCDs among Member States of the Organisation for Economic Co-operation and Development (OECD). We sought to describe policies’ aims and strategies to achieve those aims, and evaluate extent of integration of musculoskeletal conditions as a leading cause of global morbidity.
Methods Policies submitted by OECD Member States in response to a World Health Organization (WHO) NCD Capacity Survey were extracted from the WHO document clearing-house and analysed following a standard protocol. Policies were eligible for inclusion when they described an integrated approach to prevention/management of NCDs. Internal validity was evaluated using a standard instrument (sum score: 0–14; higher scores indicate better quality). Quantitative data were expressed as frequencies, while text data were content-analysed and meta-synthesised using standardised methods.
Results After removal of duplicates and screening, 44 policies from 30 OECD Member States were included. Three key themes emerged to describe the general aims of included policies: system strengthening approaches; improved service delivery; and better population health. Whereas the policies of most countries covered cancer (83.3%), cardiovascular disease (76.6%), diabetes/endocrine disorders (76.6%), respiratory conditions (63.3%) and mental health conditions (63.3%), only half the countries included musculoskeletal health and pain (50.0%) as explicit foci. General strategies were outlined in 42 (95.5%) policies—all were relevant to musculoskeletal health in 12 policies, some relevant in 27 policies and none relevant in three policies. Three key themes described the strategies: general principles for people-centred NCD prevention/management; enhanced service delivery; and system strengthening approaches. Internal validity sum scores ranged from 0 to 13; mean: 7.6 (95% CI 6.5 to 8.7).
Conclusion Relative to other NCDs, musculoskeletal health did not feature as prominently, although many general prevention/management strategies were relevant to musculoskeletal health improvement.

Purpose
Differences between standard dysarthria treatment and the same treatment with the integration of neurodynamic techniques tailored to the severity of dysarthria in patients with Parkinson's disease were examined.
Method
In total, 10 subjects with idiopathic Parkinson's disease and rigid–hypokinetic dysarthria were enrolled in this quasi-randomized, controlled, single-blind, pre–post study. In each of 12 therapy sessions the control group (n = 5) received standard dysarthria treatment (usual care), while the intervention group (n = 5) received the same treatment with the addition of integrated neurodynamic treatment (special care).
Results
There was no significant difference between the two groups for either the pre-test (p = 0.739) or the post-test (p = 0.156) results. However, significant differences between the pre-test and post-test results within each group (intervention group p = 0.001; control group p = 0.003) were found.
Conclusions
The significant differences in the pre–post comparison within the groups may indicate a high probability of a positive effect of standard dysarthria treatment on the severity of dysarthria. In between-group comparisons, the study results indicated no evidence of a significant difference between standard dysarthria treatment with or without neurodynamics. Due to the small sample size, the effectiveness of the integration of neurodynamics into speech therapy cannot be definitively concluded for now. In order to be able to have generalized applicability, future studies with larger numbers of participants are required.

Identification of differences in clinical presentation and underlying pain mechanisms may assist the classification of patients with neck–arm pain which is important for the provision of targeted best evidence based management. The aim of this study was to: (i) assess the inter-examiner agreement in using specific systems to classify patients with cervical radiculopathy and patients with non-specific neck–arm pain associated with heightened nerve mechanosensitivity (NSNAP); (ii) assess the agreement between two clinical examiners and two clinical experts in classifying these patients, and (iii) assess the diagnostic accuracy of the two clinical examiners. Forty patients with unilateral neck–arm pain were examined by two clinicians and classified into (i) cervical radiculopathy, (ii) NSNAP, (iii) other. The classifications were compared to those made independently by two experts, based on a review of patients' clinical assessment notes. The experts' opinion was used as the reference criterion to assess the diagnostic accuracy of the clinical examiners in classifying each patient group. There was an 80% agreement between clinical examiners, and between experts and 70%–80% between clinical examiners and experts in classifying patients with cervical radiculopathy (kappa between 0.41 and 0.61). Agreement was 72.5%–80% in classifying patients with NSNAP (kappa between 0.43 and 0.52). Clinical examiners' diagnostic accuracy was high (radiculopathy: sensitivity 79%–84%; specificity 76%–81%; NSNAP: sensitivity 78%–100%; specificity 71%–81%). Compared to expert opinion, clinicians were able to identify patients with cervical radiculopathy and patients with NSNAP in 80% of cases, our data supporting the reliability of these classification systems.
Possessing skills in social and intercultural interaction is vitally important for employees who work in globalized environments, especially as people's working lives tend to involve an increasingly large amount of service-related activities. As a consequence, universities offer cultural studies courses and strive to enable their students to study abroad for a period of time. However, there is still no widely shared agreement on how intercultural experiences and cultural preparation courses predict the perception, thinking and acting of individuals. Therefore, the study at hand uses a cross-sectional design with N = 430 participants in order to investigate whether students of cultural studies gain more intercultural competencies during the time spent studying abroad, compared to studies of other subjects. The results reveal that students of cultural subjects show significantly higher levels of cultural empathy and openness in the post hoc measurement, even though there was no interaction effect with the amount of time spent studying abroad. Length of stay abroad had a significant indirect effect on social competence via all the dimensions of the Multicultural Personality Questionnaire. Moreover, results indicate that flexibility to adapt one's behaviour to cultural norms may predict problems when returning to one's home country.

Objective
To identify assessment tools used to evaluate patients with temporomandibular disorders (TMD) considered to be clinically most useful by a panel of international experts in TMD physical therapy (PT).
Methods
A Delphi survey method administered to a panel of international experts in TMD PT was conducted over three rounds from October 2017 to June 2018. The initial contact was made by email. Participation was voluntary. An e-survey, according to the Checklist for Reporting Results of Internet E-Surveys (CHERRIES), was posted using SurveyMonkey for each round. Percentages of responses were analysed for each question from each round of the Delphi survey administrations.
Results
Twenty-three experts (completion rate: 23/25) completed all three rounds of the survey for three clinical test categories: 1) questionnaires, 2) pain screening tools and 3) physical examination tests. The following was the consensus-based decision regarding the identification of the clinically most useful assessments. (1) Four of 9 questionnaires were identified: Jaw Functional Limitation (JFL-8), Mandibular Function Impairment Questionnaire (MFIQ), Tampa Scale for Kinesiophobia for Temporomandibular disorders (TSK/TMD) and the neck disability index (NDI). (2) Three of 8 identified pain screening tests: visual analog scale (VAS), numeric pain rating scale (NRS) and pain during mandibular movements. (3) Eight of 18 identified physical examination tests: physiological temporomandibular joint (TMJ) movements, trigger point (TrP) palpation of the masticatory muscles, TrP palpation away from the masticatory system, accessory movements, articular palpation, noise detection during movement, manual screening of the cervical spine and the Neck Flexor Muscle Endurance Test.
Conclusion
After three rounds in this Delphi survey, the results of the most used assessment tools by TMD PT experts were established. They proved to be founded on test construct, test psychometric properties (reliability/validity) and expert preference for test clusters. A concordance with the screening tools of the diagnostic criteria of TMD consortium was noted. Findings may be used to guide policymaking purposes and future diagnostic research.
Background
A wide range of physical tests have been published for use in the assessment of musculoskeletal dysfunction in patients with headache. Which tests are used depends on a physiotherapist's clinical and scientific background as there is little guidance on the most clinically useful tests.
Objectives
To identify which physical examination tests international experts in physiotherapy consider the most clinically useful for the assessment of patients with headache.
Design/methods
Delphi survey with pre-specified procedures based on a systematic search of the literature for physical examination tests proposed for the assessment of musculoskeletal dysfunction in patients with headache.
Results
Seventeen experts completed all three rounds of the survey. Fifteen tests were included in round one with eleven additional tests suggested by the experts. Finally eleven physical examination tests were considered clinically useful: manual joint palpation, the cranio-cervical flexion test, the cervical flexion-rotation test, active range of cervical movement, head forward position, trigger point palpation, muscle tests of the shoulder girdle, passive physiological intervertebral movements, reproduction and resolution of headache symptoms, screening of the thoracic spine, and combined movement tests.
Conclusions
Eleven tests are suggested as a minimum standard for the physical examination of musculoskeletal dysfunctions in patients with headache.
Objective: To pilot benchmark measures of health information and communication technology (ICT) availability and use to facilitate cross-country learning.
Materials and Methods: A prior Organization for Economic Cooperation and Development–led effort involving 30 countries selected and defined functionality-based measures for availability and use of electronic health records, health information exchange, personal health records, and telehealth. In this pilot, an Organization for Economic Cooperation and Development Working Group compiled results for 38 countries for a subset of measures with broad coverage using new and/or adapted country-specific or multinational surveys and other sources from 2012 to 2015. We also synthesized country learnings to inform future benchmarking.
Results: While electronic records are widely used to store and manage patient information at the point of care—all but 2 pilot countries reported use by at least half of primary care physicians; many had rates above 75%—patient information exchange across organizations/settings is less common. Large variations in the availability and use of telehealth and personal health records also exist.
Discussion: Pilot participation demonstrated interest in cross-national benchmarking. Using the most comparable measures available to date, it showed substantial diversity in health ICT availability and use in all domains. The project also identified methodological considerations (e.g., structural and health systems issues that can affect measurement) important for future comparisons.
Conclusion: While health policies and priorities differ, many nations aim to increase access, quality, and/or efficiency of care through effective ICT use. By identifying variations and describing key contextual factors, benchmarking offers the potential to facilitate cross-national learning and accelerate the progress of individual countries.

Aber lassen sich "westlich" geprägte Vorgehensweisen im Unternehmenswandel als Best-Practice-Ansätze global nutzen oder sind kulturell bedingte Anpassungen notwendig? In einer quantitativen Studie wurden die Bewertungen verschiedener Veränderungsszenarien durch deutsche und südkoreanische Teilnehmer erhoben.
Background:
One of the main problems of Internet-delivered interventions for a range of disorders is the high dropout rate, yet little is known about the factors associated with this. We recently developed and tested a Web-based 6-session program to enhance motivation to change for women with anorexia nervosa, bulimia nervosa, or related subthreshold eating pathology.
Objective:
The aim of the present study was to identify predictors of dropout from this Web program.
Methods:
A total of 179 women took part in the study. We used survival analyses (Cox regression) to investigate the predictive effect of eating disorder pathology (assessed by the Eating Disorders Examination-Questionnaire; EDE-Q), depressive mood (Hopkins Symptom Checklist), motivation to change (University of Rhode Island Change Assessment Scale; URICA), and participants’ age at dropout. To identify predictors, we used the least absolute shrinkage and selection operator (LASSO) method.
Results:
The dropout rate was 50.8% (91/179) and was equally distributed across the 6 treatment sessions. The LASSO analysis revealed that higher scores on the Shape Concerns subscale of the EDE-Q, a higher frequency of binge eating episodes and vomiting, as well as higher depression scores significantly increased the probability of dropout. However, we did not find any effect of the URICA or age on dropout.
Conclusions:
Women with more severe eating disorder pathology and depressive mood had a higher likelihood of dropping out from a Web-based motivational enhancement program. Interventions such as ours need to address the specific needs of women with more severe eating disorder pathology and depressive mood and offer them additional support to prevent them from prematurely discontinuing treatment.
The central objective of early prevention in Germany is an improved cooperation between professional groups of the health services and child and youth welfare in interprofessional networks. This objective derives from the realisation that proper care for families with infants can only be achieved if the various groups act in close integration. The ‘Federal Initiative early prevention’ explicitly calls for freelance midwives to be integrated in this context. However, only a few scientific findings on midwives’ cooperation in networks of early prevention have been published to date. This integrative review aims to identify the central themes of interprofessional cooperation of midwives in out-of-hospital obstetrical care from national and international research literature. A systematic search of five research databases for publications between 2005 and 2015 was performed, complemented by a manual search. 25 studies were identified describing various contexts where midwives in out-of-hospital obstetrical care cooperate with other professional groups. Four key themes were analysed: contexts of cooperation, benefits of cooperation, facilitating and restrictive factors of cooperation, and competencies of cooperation. The studies show that there is only limited research coverage of the midwives’ perspective regarding interprofessional cooperation. The existing studies examine the cooperation of midwives primarily with health care professionals, and secondarily with professionals in the social services. In order to expand knowledge on the cooperation of freelance midwives in the networks of early prevention, future research should focus on the perspective of midwives regarding cooperation with other professional groups, both in the health care sector and in the field of social services.

Familienzentren, beschreibt Netzwerkarbeit als unverzichtbare Grundlage einer erfolgreichen Betreuungspraxis der Zukunft und konturiert das komplexe Anliegen „Interprofessionalität“ in sozialen Handlungsfeldern. Den Schwerpunkt machen Ausführungen zur Konzeption und die Vorstellung von Modulen der Qualifizierung von Interprofessionalität in der Praxis aus: „INPRO“ wird den LeserInnen als systematische Arbeitsgrundlage für die Konzept-, Netzwerk-, Team und Organisationsarbeit präsentiert. Zusätzlich bieten Beiträge internationaler Partner interessante Einblicke in die interprofessionelle Praxis anderer europäischer Länder und regen die Reflexion der hiesigen Praxis an. Im Ergebnis werden notwendige Bedingungen für das anspruchsvolle Vorhaben der Interprofessionalität systematisiert.

In der inklusiven Grundschule arbeiten zunehmend verschiedene Professionen, wie bspw. Lehrer*innen, Sonderpädagog*innen oder Schulsozialarbeiter*innen, interprofessionell zusammen. Dieser Wandel in schulischen Arbeitskontexten geht mit veränderten Professionalisierungsanforderungen für die Lehr- und Fachkräfte einher (vgl. Kleina & Tan, 2019, S. 63). Die Praxis interprofessioneller Kooperation in der inklusiven Grundschule ist dabei bisher nur in Teilfragen erforscht (vgl. Moldenhauer, 2019), kaum aber mit Blick auf die Erfahrungen und Erwartungen der pädagogischen Lehr- und Fachkräfte selbst. Hierzu werden Ergebnisse einer quantitativen Bedarfserhebung (N=271), welche im Rahmen des vom BMBF geförderten Forschungs- und Entwicklungsprojektes InproKiG bei Lehrkräften und pädagogischen Fachkräften von Schulteams in Niedersachsen und Hessen durchgeführt wurde, vorgestellt. Die Ergebnisse geben Aufschluss über die Entwicklung und Gestaltung von Qualifizierungsmaßnahmen in diesem Bereich. Neben der Qualifizierung der bereits tätigen Lehr- und Fachkräfte, kann das InproKiG-Manual auch in der Lehrer*innenausbildung eingesetzt werden.
Die alltägliche pädagogische Praxis von Grundschulen hat sich durch die Implementierung unterschiedlicher multiprofessioneller Kooperationsprozesse maßgeblich geändert (vgl. Beck & Maykus 2016, S. 148f.). So kooperieren laut Thieme (2019) seit dem Ausbau von ganztägig-inklusiven Schulsettings zunehmend auch Lehrkräfte und sozialpädagogische Fachkräfte unterschiedlicher Fachbereiche und Institutionen (vgl. Thieme, 2019, S. 247). Die Schulsozialarbeit an Grundschulen ist dabei ein sozialpädagogisches Arbeitsfeld, welches als Schnittstelle zwischen verschiedenen pädagogischen Berufsgruppen gilt (vgl. Thimm 2017, S. 108) und dem der Vernetzungs- und Kooperationsgedanken als Leitmotiv (sozial-)pädagogischer Praxis inhärent ist. Vor allem die Kooperation zwischen den Lehrkräften und der Schulsozialarbeit ist dabei an bestimmte Voraussetzungen geknüpft. Das BMBF-geförderte Forschungs- und Entwicklungsprojekt „Qualifizierungsmodule für die interprofessionelle Kooperation in inklusiven Grundschulen – Entwicklung und Evaluation des Manuals InproKiG“ (kurz: InproKiG) hat mit unterschiedlichen Erhebungsinstrumenten u.a. diese Voraussetzungen und Herausforderungen in der alltäglichen Kooperationspraxis näher analysiert. Der Beitrag stellt Ergebnisse einer quantitativen Erhebung zum Stand der Kooperation und zu Kooperationsbedarfen von Grundschulteams vor, welche in den Jahren 2018 und 2019 an Grundschulen in Hannover, Osnabrück und Gießen durchgeführt wurde. In Einzelanalysen werden vor diesem Hintergrund vertiefend die Perspektiven von Fachkräften der Schulsozialarbeit und ihre Erfahrungen interprofessioneller Kooperation untersucht.
The political geography of central government debt has hardly been investigated. We propose a method for calculating implicit interregional transfers stemming from central government debt.
We apply this method to Belgium over the 1970-2016 period. The share of poorer Francophone Belgium in debt-financed central government spending was persistently larger than its share in central government revenue used to pay the resulting interest bills. The opposite holds for richer Flanders. Also, a primary deficit in one particular year leads to an interest bill in each of the following years as long as debt caused by that primary deficit is not repaid. All the above caused debt-related transfers from Flanders to Francophone Belgium of over 7% of Flemish GDP during many years.
Interregional interest transfers may also be large in the many other democracies suffering from both high central government debt and considerable geographic income disparities.
The size of these transfers may in turn explain the size and persistence of central government deficits. This is also because poorer, less densely populated regions such as Francophone Belgium tend to be overrepresented within central governments. This strengthens their ability to cause deficits.
We recommend more fiscal decentralisation or at least smaller central government deficits.

This review aimed to synthesize the current evidence on the effectiveness of invasive treatments for complex regional pain syndrome in children and adolescents. Studies on children and adolescents with complex regional pain syndrome that evaluated the effects of invasive treatment were identified in PubMed (search March 2013). Thirty-six studies met the inclusion criteria. Articles reported on a total of 173 children and adolescents with complex regional pain syndrome. Generally, many studies lack methodological quality. The invasive treatments applied most often were singular sympathetic blocks, followed by epidural catheters and continuous sympathetic blocks. Rarely, spinal cord stimulation and pain-directed surgeries were reported. An individual patient frequently received more than one invasive procedure. Concerning outcome, for approximately all patients, an improvement in pain and functional disability was reported. However, these outcomes were seldom assessed with validated tools. In conclusion, the evidence level for invasive therapies in the treatment of complex regional pain syndrome in children and adolescents is weak.
Das ISK ist ein breit angelegtes, multidimensionales Selbstbeurteilungsinstrument, mit dem in umfassender Weise grundlegende soziale Kompetenzen abgebildet werden. Es werden »situationsübergreifende« (allgemeine) soziale Kompetenzen erfasst, welche in einer Vielzahl sozialer Situationen wirken und Bestandteil der Persönlichkeit eines Menschen sind. Das Verfahren liegt in einer Langform mit 108 Items (ISK) und einer Kurzform mit 33 Items (ISK-K) vor. Die Kurzform ISK-K erfasst ausschließlich die vier Sekundärskalen, welche übergeordnete Kompetenzen widerspiegeln.

Inhalt: Das IGIP erfasst die Glaubwürdigkeit von Selbstauskünften, die eine Person zu einem bestimmten Zeitpunkt gibt. Das Verfahren besteht aus 15 Items, wobei jedes Item eine Aussage über die eigene Person enthält. Die Probanden müssen auf einer dichotomen Skala („Ja“ vs. „Nein“) angeben, ob die fragliche Aussage auf sie zutrifft oder nicht. Die Aussagen wurden im Rahmen der Testentwicklung derart konstruiert und ausgewählt, dass ein bestimmtes Antwortverhalten als wahrscheinlich unglaubwürdige Aussage interpretiert werden darf.
Das IGIP kann immer dann eingesetzt werden, wenn die Gefahr besteht, dass im Rahmen einer Untersuchung die Probanden stark motiviert sind, die Unwahrheit zu sagen. Das primäre Anwendungsfeld des IGIP liegt in der Personalauswahl: Durch das IGIP lassen sich Bewerber identifizieren, die in der Auswahlsituation eine besonders verzerrte Selbstpräsentation liefern.
Zuverlässigkeit: Die Konsistenzkoeffizienten des IGIP liegen in den Normierungsstichproben zwischen .76 (Nichtbewerber) und .81 (Bewerber).
Testgültigkeit: Die Überprüfung der faktoriellen Validität bestätigt die einfaktorielle Struktur. Es bestehen hypothesenkonforme Zusammenhänge zu verschiedenen Skalen zur Messung sozial erwünschten Antwortverhaltens, Integrität, „Big Five“ und verschiedenen Skalen zur Messung sozialer Kompetenzen.
Normen: Es liegen Normwerte (Prozentränge) aus zwei getrennten Normierungsstichproben von Bewerbern (N = 1.954) und Nichtbewerbern (N = 979) vor.

Background: The majority of health IT adoption research focuses on the later stages of the IT adoption process: namely on the implementation phase. The first stage, however, which is defined as the knowledge-stage, remains widely unobserved. Following Rogers’ Diffusion of Innovation Theory (DOI) this paper presents a research framework to examine the possible lack of shared IT awareness-knowledge, i.e. an information gradient, of two crucial stakeholders, the Chief Information Officer (CIO) and the Director of Nursing (DoN). This study shall answer the following research questions: (1.) Does this gradient exist? (2.) Which direction does it have? (3.) Are certain health IT (HIT) attributes associated with a potential gradient? (4.) Which determinants of diffusion go along with this gradient?
Method: Results of two surveys that focused on the topic “IT support of clinical workflows” from the viewpoint of CIOs and DoNs with corresponding datasets from 75 hospitals were used in a secondary data analysis. The gradient was operationalised by measuring the disagreement of CIOs and DoNs on the availability and implementation status of 29 IT functions. HIT attributes tested were relevance and market penetration of the IT functions, determinants of diffusion were inter-professional leadership and IT service density.
Results: The analysis revealed a significant disagreement on the availability of 9 out of 29 HIT functions. In 23 HIT functions, the CIOs reported a higher implementation status than the DoNs, which pointed to a trend for a unidirectional gradient. The disagreement was significantly lower when the relevance of the IT function was high. Both determinants of diffusion correlated significantly negative with the degree of disagreement.
Conclusion: This is the first study to empirically examine shared awareness-knowledge of two IT-stakeholders that are crucial for triggering IT adoption on the frontline level in hospitals. It could be shown that a gradient and thus a lack of shared awareness-knowledge existed and was associated with certain factors. In conclusion, hospitals should implement improved cooperation between IT staff and clinicians and IT service density when establishing the prerequisites for successful IT adoption processes.
While the topic of artificial intelligence (AI) in multinational enterprises has been receiving attention for some time, small and medium enterprises (SMEs) have recently begun to recognize the potential of this new technology. However, the focus of previous research and AI applications has therefore mostly been on large enterprises. This poses a particular issue, as the vastly different starting conditions of various company sizes, such as data availability, play a central role in the context of AI. For this reason, our systematic literature review, based on the PRISMA protocol, consolidates the state of the art of AI with an explicit focus on SMEs and highlights the perceived challenges regarding implementation in this company size. This allowed us to identify various business activities that have been scarcely considered. Simultaneously, it led to the discovery of a total of 27 different challenges perceived by SMEs in the adoption of AI. This enables SMEs to apply the identified challenges to their own AI projects in advance, preventing the oversight of any potential obstacles or risks. The lack of knowledge, costs, and inadequate infrastructure are perceived as the most common barriers to implementation, addressing social, economic, and technological aspects in particular. This illustrates the need for a wide range of support for SMEs regarding an AI introduction, which covers various subject areas, like funding and advice, and differentiates between company sizes.
Pediatric headache is an increasingly reported phenomenon. Cervicogenic headache (CGH) is a subgroup of headache, but there is limited information about cervical spine physical examination signs in children with CGH. Therefore, a cross-sectional study was designed to investigate cervical spine physical examination signs including active range of motion (ROM), posture determined by the craniovertebral angle (CVA), and upper cervical ROM determined by the flexion–rotation test (FRT) in children aged between 6 and 12 years. An additional purpose was to determine the degree of pain provoked by the FRT. Thirty children (mean age 120.70 months [SD 15.14]) with features of CGH and 34 (mean age 125.38 months [13.14]) age-matched asymptomatic controls participated in the study. When compared to asymptomatic controls, symptomatic children had a significantly smaller CVA (p < 0.001), significantly less active ROM in all cardinal planes (p < 0.001), and significantly less ROM during the FRT (p < 0.001), especially towards the dominant headache side (p < 0.001). In addition, symptomatic subjects reported more pain during the FRT (p < 0.001) and there was a significant negative correlation (r = −0.758, p < 0.001) between the range recorded during the FRT towards the dominant headache side and FRT pain intensity score. This study found evidence of impaired function of the upper cervical spine in children with CGH and provides evidence of the clinical utility of the FRT when examining children with CGH.
Aim
Manual cranial bone tissue techniques (CBTT) are used by physiotherapists as a tool for assessment and treatment of complaints in the craniocervical, face, and head regions. The goal of this study was to determine whether CBTT were able to discriminate between subjects with cervical and/or temporomandibular disorders (TMD) and a healthy group. In addition, the inter-rater reliability when applying CBTT was also investigated.
Methods
A cross-sectional study was conducted and included 60 participants. Six standardized passive techniques were applied and judged for resistance, compliance, and sensory answer. In order to evaluate the inter-rater reliability of these techniques a cohort of participants was measured twice (by two evaluators) prospectively. A logistic regression model and Receiver Operating Curve (ROC) analyses were used to determining the discriminative validity of these techniques.
Results
Logistic regression identified a significant difference for five techniques for resistance and/or compliance and/or the sensory answer between the groups. Based on the Area Under the Curve (AUC) analysis, the discriminative ability of the temporal rotation to distinguish between the groups was fair to good (for resistance AUC = 0.7775 and for compliance AUC = 0.8065). The highest agreement between the two assessors was for the resistance with occipital compression (73%) technique.
Conclusion
This study highlights that some of the CBTT could be potentially useful in distinguishing subjects with cervical and/or TMD from healthy subjects. Inter-rater reliability was moderate. CBTT could be potentially integrated in the examination of participants with complaints in the craniofacial region.
