Der N. medianus besitzt die Fähigkeit, bei Extremitätenbewegungen in seinem umliegenden Gewebe zu gleiten. Dieses wichtige physiologische Phänomen minimiert bei Engpasssyndromen Zug und Druck auf den Nerv. Das Karpaltunnelsyndrom (CTS) stellt in der Praxis die am häufigsten auftretende und stark behandlungsresistente periphere Neuropathie dar. Die verminderte Dynamik des N. medianus kann ein beitragender Faktor eines lange bestehenden CTS sein. Die vorliegende Querschnittstudie untersuchte die transversale Gleitfähigkeit des N. medianus mittels sonografischer Diagnostik an CTS-Patienten (Gruppe A). Die Nervenbewegung wurde hierfür durch passive Bewegungsmanöver an der HWS mittels einer Kontralateralflexion und einem Cervical lateral glide sowie durch aktive Bewegungen der Finger und der Schulter provoziert. Die Ergebnisse wurden im Anschluss mit den Resultaten einer früheren Untersuchung gleicher Art an gesunden Probanden (Gruppe B) verglichen. Die Bewegungsmanöver der oberen Extremität wurden zudem an gesunden Probanden durchgeführt (Gruppe B2). Das Ziel der Studie war es herauszufinden, ob die Manöver einen signifikanten Einfluss auf die Gleitfähigkeit sowie auf die Größe des Flächeninhalts des N. medianus ausüben und ob sich die Veränderungen in der CTS-Gruppe im Gegensatz zur Kontrollgruppe signifikant unterschiedlich darstellen. Sowohl distale Bewegungen der Fingergelenke als auch die zervikalen Bewegungsmanöver beeinflussten das transversale und longitudinale Gleiten des N. medianus. Am Unterarm zeigten sich deutliche Unterschiede in der Nervenbewegung zwischen den gesunden und den CTS-Probanden, was auf einen möglichen Einfluss des Cervical lateral glide auf die Neurodynamik des N. medianus bei CTS hindeutet.

Je reibungsloser das Zusammenspiel von Management und Controlling, desto effektiver können die Entscheidungsprobleme des Krankenhausmanagements gelöst werden. In der Praxis treten allerdings regelmäßg Reibungsverluste in der Zusammenarbeit auf. Die Arbeitsbeziehung zwischen Controller und Manager ist häufig konfliktgeladen und wenig partnerschaftlich. Ursächlich für die negative Dynamik der Beziehung sind unterschiedliche Führungsentwürfe und Rollenbilder zum Controlling. Der Beitrag widmet sich den Anforderungen und der Struktur eines Controllings im Krankenhaus, das den Besonderheiten der Expertenorganisation gerecht werden kann.
Beziehungsgestaltung in der ambulanten Pflege : Ergebnisse einer qualitativ-explorativen Studie

Die Beziehungsgestaltung zwischen Pflegenden und pflegebedürftigen Menschen ist ein wichtiger Aspekt der ambulanten Pflege, der bislang kaum erforscht ist. In einer qualitativen Studie wurde die pflegerische Beziehungsgestaltung im deutschen, ambulanten Pflegesetting untersucht. Die Studie orientierte sich an den Prinzipien der Grounded-Theory nach Strauss und Corbin. Acht Interviews mit beruflich Pflegenden und 24 teilnehmende Beobachtungen wurden durchgeführt. Dem Ergebnis zufolge investieren Pflegende zu Beginn in den Beziehungsaufbau, um einen Zugang zum Gegenüber zu erhalten und förderliche sowie hinderliche Einflussfaktoren auf die Beziehung auszubalancieren. Die Ausrichtung der Beziehung auf dieser Ebene ist funktional in enger Anlehnung an pflegerische Tätigkeiten. Ausgehend von einem erfolgreichen Beziehungsaufbau auf der funktionalen Ebene können Pflegende optional die Beziehung auf einer menschlich-privaten Ebene gestalten. Diese ist geprägt von Reziprozität, dem Zeigen von Geßihlen und verstärktem Sich privat involvieren. Die Ergebnisse zeigen, dass es einer Sensibilisierung der Pflegepraxis für Beziehungsentwicklung bedarf. Es gilt, bereits in der Ausbildung Fähigkeiten zur Reflexion und Abgrenzung zu fördern, damit Pflegende Beziehungen bewusst gestalten lernen.

Das Lehrbuch vermittelt die Grundzüge der Bilanzierung nach internationalen Rechnungslegungsvorschriften IAS/ IFRS und nach neuem deutschem HGB, gemäß BilMoG. Es entspricht dem Curriculum, nach dem an deutschen und ausländischen Hochschulen Bilanzierung bzw. Accounting gelehrt wird.
Zahlreiche Beispiele und eine durchgängige Fallstudie veranschaulichen den Lernstoff.

Ausgehend vom Begriff der Bildungslandschaft und einem veränderten Management zwischen Bildungsorganisationen wird in diesem Beitrag ein theoretisches Modell entwickelt. Dieses Modell beleuchtet die Bestimmung eines konzeptionellen Kerns im Bereich vernetzter Bildung, um Zielperspektiven bewertbar zu machen. Dabei werden Entkoppelungen beschrieben, die den konzeptionellen Kern von Bildungslandschaft als erziehungswissenschaftlicher Kategorie ausmachen. Basics of Accounting targets students in international business study programs. It covers the widely applied syllabus of Accounting at universities on bachelor’s and master’s level. In this book, the application of the methods comes first. The Basics teach how to do Accounting by a case study based approach. All cases were taken from former exam papers at international universities and calculated completely and illustrated understandably.
Bookkeeping and Financial Accounting covers the preparation of financial statements based on IFRS. Bookkeeping is taught as far as it is required for the understanding by managers – more formal aspects about how to keep financial records are cut short to the minimum.

Kanning stellt sich in seiner Kolumne die Frage, ob wir dieser Methode aufgrund ihrer Alltagsplausibilität wirklich ohne Vorbehalt begegnen können – und beantwortet sie gewohnt zugespitzt.
Wettbewerbsrisiken frühzeitig erkennen, um Maßnahmen in der strategischen Planung zu berücksichtigen, ist eine originäre Aufgabe des Managements. Die Branchenstrukturanalyse stellt ein methodisches Instrument zur Identifikation von unternehmerischen Risiken dar, die aus der Unternehmungsumwelt Einfluss nehmen. Dabei differenziert die Branchenstrukturanalyse nach Michael E. Porter fünf Herkunftsfelder der Risiken. Eine fehlende Konkretisierung macht eine spezifische Anwendung jedoch schwierig. Der folgende Beitrag greift diese Kritik auf und analysiert den Einfluss der fünf Kräfte am Beispiel der Stationären Altenhilfe, insbesondere des Landkreises Osnabrück.
Dekubitusprophylaxe. Die Rechtsprechung empfiehlt bei dekubitusgefährdeten Patienten ein Lagerungs- und Bewegungsprotokoll mit Einzelleistungsnachweis zu führen. Der Expertenstandard hingegen rät explizit nicht zu einer standardisierten Planung eines längerfristigen Lagerungsintervalls. Dieses Spannungsfeld sorgt für Unsicherheit speziell im Hinblick auf das neue Strukturmodell zur Entbürokratisierung der Pflegedokumentation.

Die gefundenen auffälligen Zeichen leiten die Therapeuten in der muskuloskelettalen Therapie, die aus Hands-on und -off-Techniken besteht.
Handovers need a common ground on the clinical cases between the members of the successive shifts to establish continuity of care. Conventional electronic patient record systems (EHR) proved to be only insufficiently suitable for supporting the grounding process. Against this background we proposed a basic concept for a handover EHR that extends general EHRs in particular openEHR based systems. The resulting handover information model was implemented in a database and evaluated based on 120 clinical cases. The information items of these cases could be mapped successfully to the model, however, the new class “anticipatory guidance” needed to be introduced. The evaluation also demonstrated the importance of highly aggregated information on the clinical case, opinions and meta-information such as the relevance of an item during handovers. Based on these findings, in particular the handover database, handover EHR applications are currently developed to support the grounding process.
This paper provides a discourse based upon the key development of nursing in response to the emerging 4Ds of health technology re-design. Building informatics capability among health professionals is a workforce issue necessitated through the increasing prevalence of information technology and digitization of healthcare affecting the entire health workforce, specifically front-line nurses. The key concepts will be explored of Digitization, Distribution, Disruption and Diversity, a framework recognising the tsunami of technology such as Big Data analytics, comprehensive decision support systems for nursing, nanobots, robotics, and pharmacogenomics and the impact these have upon the nursing workforce.
Die Akzente im Stadtmarketing haben sich in den letzten Jahren in vielen Städten verschoben: Stand früher die Ausrichtung auf externe Stakeholder wie Touristen, Unternehmen oder potenzielle Arbeitskräfte im Zentrum, so richtet sich die Aufmerksamkeit heute in zunehmendem Maße auf interne Stakeholder, insbesondere auf die Bürgerinnen und Bürger. Diese werden nicht nur auf ihre Rolle als Kunden reduziert, sondern sie werden als Mitgestalter in Beteiligungsprozesse integriert. Der Beitrag beschreibt die Entwicklung einer Dachmarke in einem beteiligungsorientierten Stadtmarketing-Prozess unter besonderer Berücksichtigung der Methode Design Thinking.
Career Decisions of Indian Female Talent: Implications for Gender-sensitive Talent Management

Purpose: Talent scarcity in emerging economies such as India poses challenges for companies,and limited labour market participation among well-educated women has been observed. The reasons that professionals decide not to pursue a further corporate career remain unclear. By investigating career decision making, this article aims to highlight (1) the contextual factors that impact those decisions, (2) individuals’ agency to handle them, and (3) the implications for talent management (TM).
Design/methodology/approach: Following a qualitative research design, computer-aided analysis was conducted on interviews with 24 internationally experienced Indian business professionals. A novel application of neo-institutionalism in the Indian context was combined with the family-relatedness of work decisions (FRWD) model.
Findings: Career decisions indicate that rebellion against Indian societal and family expectations is essential to following a career path, especially for women. TM as part of the current institutional framework serves as a legitimising façade veiling traditional practices that hinder females’ careers.
Research limitations: Interviewees adopted a retrospective perspective when describing their career decisions; therefore, different views might have existed at the moment of decision making.
Practical implications: Design and implementation of gender-sensitive TM adjusted to fit the specific Indian context can contribute to retaining female talent in companies and the labour market.
Originality/value: The importance of gender-sensitive TM can be concluded from an empirical study of the context-based career decision making of experienced business professionals from India. The synthesis of neo-institutionalism, the FRWD model and the research results provides assistance in mapping talent experiences and implications for overcoming the challenges of talent scarcity in India.
Background
Lay family caregivers of patients receiving palliative care often confront stressful situations in the care of their loved ones. This is particularly true for families in the home-based palliative care settings, where the family caregivers are responsible for a substantial amount of the patient’s care. Yet, to our knowledge, no study to date has examined the family caregivers’ exposure to critical events and distress with home-based palliative care has been reported from Germany. Therefore, we attempt to assess family caregiver exposure to the dying patient’s critical health events and relate that to the caregiver’s own psychological distress to examine associations with general health within a home-based palliative care situation in Germany.
Methods
A cross-sectional study was conducted among 106 family caregivers with home-based palliative care in the Federal State of North Rhine Westphalia, Germany. We administered the Stressful Caregiving Adult Reactions to Experiences of Dying (SCARED) Scale. Descriptive statistics and linear regression models relating general health (SF-36) were used to analyze the data.
Results
The frequency of the caregiver’s exposure, or witness of, critical health events of the patient ranged from 95.2% “pain/discomfort” to 20.8% “family caregiver thought patient was dead”. The highest distress scores assessing fear and helpfulness were associated with “family caregiver felt patient had enough’” and “family caregiver thought patient was dead”. Linear regression analyses revealed significant inverse associations between SCARED critical health event exposure frequency (beta = .408, p = .025) and total score (beta = .377, p = .007) with general health in family caregivers.
Conclusions
Family caregivers with home-based palliative care in Germany frequently experience exposure to a large number of critical health events in caring for their family members who are terminally ill. These exposures are associated with the family caregiver’s degree of fear and helplessness and are associated with their worse general health. Thus the SCARED Scale, which is brief and easy to administer, appears able to identify these potentially upsetting critical health events among family caregivers of palliative care patients receiving care at home. Because it identified commonly encountered critical events in these patients and related them to adverse general health of family caregivers, the SCARED may add to clinically useful screens to identify family caregivers who may be struggling.
In Germany, a lot of young children at risk of language difficulties still go undetected or are not assessed before preschool-age. For children where parents may suspect a disorder, this practice causes a lot of emotional distress alongside lost time for intervention. Thus, what contribution can parents and nursery staff make for the earlier detection of language difficulties? 34 children from four German kindergartens were tested with a standardized preschool screening for language problems by an SLT. Parents and nursery staff completed a questionnaire (FEE 3-4) that was designed to collect potential risk-factors and included the rating of children’s abilities across the main language domains. Outcomes from the FEE 3-4 were compared between parents and nursery staff as well as triangulated with results from the standardized screening. Agreement between parents and nursery staff re. individual children’s potential language difficulties was moderate (Kappa = 0.44, p = .050). Overall, nursery staff rated children’s language abilities more strictly and precisely than parents. Especially their rating of ‘word order’ (p = .022) and ‘verb endings’ contributed significantly to the identification of potential language difficulties similar to the standardized screening. The screening identified two children at risk without caregiver's concern, but not two others who were at risk of language disorder and for whom caregivers expressed concern. Caregiver’s awareness of early language difficulties appears to be rather intuitive. Young children at risk are most reliably detected if standardized instruments are used in combination with caregiver questionnaires. Ideally, this process includes data from parents and nursery staff to be interpreted by an experienced SLT, as the use of a standardized screening alone may lead to missed or mistaken identification where essential information about the child’s environment (e.g. risk factors) is not provided. If parents are concerned about children’s language, full assessment is clearly justified.

Changeprozesse gehören heute zum Tagesgeschehen, mit dem sichOrganisationen auseinander setzen müssen. Die bisherige Forschungzum Change Management bezieht sich zum überwiegenden Teil aufprivatwirtschaftliche Organisationen und es ist nicht abschließendklar, inwiefern die beforschten Zusammenhänge auf den Bereich derNonprofit Organisationen (NPO) übertragbar sind. NPO unter-scheiden sich grundlegend von privatwirtschaftlichen Unternehmenund auf ihre Besonderheiten nimmt der vorliegende Beitrag Bezug.Hierbei wird eine ganzheitliche Sichtweise gewählt, die sowohl ge-sellschaftliche, organisationale als auch individuelle Dimensionenmiteinbezieht. Am Beispiel von NPO im Kindertagesstättenbereich,als typisches Beispiel für Drittleistungs-NPO, welche von einem ra-dikalen Wandel in der deutschen Bildungslandschaft betroffen sind,und deren Beschäftigten testen wir die aus der Literatur ent-wickelten Zusammenhänge anhand eines Mehrebenenmodells. Die Ergebnisse verdeutli-chen die positive Wirksamkeit von strategischer Personalentwicklung auf die Arbeitszu-friedenheit der Mitarbeiterinnen und Mitarbeiter.
In diesem Beitrag sollen aus einer psychologischen Perspektive Implikationen für das Change Monitoring formuliert werden. Hierzu werden im ersten Teil zunächst individuelle Reaktionen auf Veränderungen anhand unterschiedlicher einstellungs- und verhaltensbezogener Konzeptualisierungen modelliert. Hierbei werden Konstrukte wie Commitment to change, Zynismus gegenüber Wandel oder Widerstand bei Veränderungen thematisiert. Der zweite Teil zeigt auf, wie die individuellen Reaktionen auf Veränderungen durch verschiedene Faktoren wie personale Merkmale, Change Kontext, Change Prozess oder Change Charakteristika beeinflusst werden. Im dritten Teil steht die Frage im Vordergrund, welche Ansatzpunkte es gibt, um eine Verhaltensänderung im Sinne der Ziele eines Change Projekts zu unterstützen. Hierzu wird zunächst aufgezeigt, wann Bereitschaft für eine Veränderung entsteht. Außerdem wird die zentrale Rolle der Selbstwirksamkeitserwartung für die Änderung und Aufrechterhaltung von Verhalten dargestellt. Schließlich wird ein Überblick zu verschiedenen konkreten Interventionen gegeben, die Verhaltensänderungen unterstützen können. Im abschließenden Teil des Beitrags werden aufbauend auf den Vorüberlegungen konkrete Implikationen für ein psychologisch-fokussiertes Change Monitoring herausgearbeitet.
Characterisation of pain in people with hereditary neuropathy with liability to pressure palsy

Hereditary neuropathy with liability to pressure palsy (HNPP) has historically been considered a pain-free condition, though some people with HNPP also complain of pain. This study characterised persistent pain in people with HNPP. Participants provided cross-sectional demographic data, information on the presence of neurological and persistent pain symptoms, and the degree to which these interfered with daily life. The painDETECT and Central Sensitization Inventory questionnaires were used to indicate potential neuropathic, central sensitisation and musculoskeletal (nociceptive) pain mechanisms. Additionally, participants were asked if they thought that pain was related to/part of HNPP. 32/43 (74%) subjects with HNPP had persistent pain and experience this pain in the last week. Of those with pain, 24 (75%) were likely to have neuropathic pain and 27 (84%) were likely to have central sensitisation. All 32 participants felt that their pain could be related to/part of their HNPP. Significant negative impact of the pain was common. Pain characterisation identified neuropathic pain and/or central sensitisation as common, potential underlying processes. Pain may plausibly be directly related to the underlying pathophysiology of HNPP. Further consideration of including pain as a primary symptom of HNPP is warranted.
Characteristics of German Hospitals Adopting Health IT Systems : Results from an Empirical Study

Hospital characteristics that facilitate IT adoption have been described by the literature extensively, however with controversial results. The aim of this study therefore is to draw a set of the most important variables from previous studies and include them in a combined analysis for testing their contribution as single factors and their interactions. Total number of IT systems installed and number of clinical IT systems in the hospital were used as criterion variables. Data from a national survey of German hospitals served as basis. Based on a stepwise multiple regression analysis four variables were identified to significantly explain the degree of IT adoption (60% explained variance): 1) hospital size, 2) IT department, 3) reference customer and 4) ownership (private vs. public). Our results replicate previous findings with regard to hospital size and ownership. In addition our study emphasizes the importance of a reliable internal structure for IT projects (existence of an IT department) and the culture of testing and installing most recent IT products (being a reference customer). None of the interactions between factors was significant.
Charakteristika innovativer Krankenhäuser in Deutschland : Ergebnisse einer empirischen Untersuchung

Germany is Europe’s most populated country and it is actual locomotive for growth. The Germans have a positive image in China. However, this appreciation is not mutual. Various polls indicate that Germans are reserved concerning China’s rise; some even dislike outward foreign direct investment (OFDI) from Chinese Companies flowing to Germany. Nevertheless, Chinese investments in Germany will accelerate in the near future; already it increased from 600 million € 2 years ago to more than 800 million €. Experts predict a further massive increase. Asian OFDIs in Europe will be an integral part of the Asian Century, transforming the host countries, just as western OFDIs changed Asia during the last decades. This article analyses the German “China readiness” in the field of public economic policy at a local level.
Model-derived relationships between chlorophyll a (Chl-a) and nutrients and temperature have fundamental implications for understanding complex interactions among water quality measures used for lake classification, yet accuracy comparisons of different approaches are scarce. Here, we (1) compared Chl-a model performances across linear and nonlinear statistical approaches; (2) evaluated single and combined effects of nutrients, depth, and temperature as lake surface water temperature (LSWT) or altitude on Chl-a; and (3) investigated the reliability of the best water quality model across 13 lakes from perialpine and central Balkan mountain regions. Chl-a was modelled using in situ water quality data from 157 European lakes; elevation data and LSWT in situ data were complemented by remote sensing measurements. Nonlinear approaches performed better, implying complex relationships between Chl-a and the explanatory variables. Boosted regression trees, as the best performing approach, accommodated interactions among predictor variables. Chl-a–nutrient relationships were characterized by sigmoidal curves, with total phosphorus having the largest explanatory power for our study region. In comparison with LSWT, utilization of altitude, the often-used temperature surrogate, led to different influence directions but similar predictive performances. These results support utilizing altitude in models for Chl-a predictions. Compared to Chl-a observations, Chl-a predictions of the best performing approach for mountain lakes (oligotrophic–eutrophic) led to minor differences in trophic state categorizations. Our findings suggest that both models with LSWT and altitude are appropriate for water quality predictions of lakes in mountain regions and emphasize the importance of incorporating interactions among variables when facing lake management challenges.

Patientenschulungen sind in der ambulanten wie stationären pädiatrischen Versorgung von Kindern und Jugendlichen mit chronischen Erkrankungen fest verankert. In diesem Beitrag werden nach Angaben zur Definition und Verbreitung chronischer Erkrankungen des Kindes- und Jugendalters die wichtigsten Ziele und Vorgehensweisen von Schulungsprogrammen für Patienten und ihre Familien dargelegt. Ausführlich vorgestellt werden Schulungen bei chronischem Kopfschmerz, Neurodermitis, Typ-1 Diabetes und juveniler idiopathischer Arthritis. Abschließend wird auf zukünftige Bedarfe bei der Evaluation und Weiterentwicklung dieser Programme eingegangen.
Angesichts der wachsenden Zahl chronisch erkrankter Menschen auch in der reproduktiven Lebensphase gewinnt das Thema der Versorgung betroffener Frauen in Schwangerschaft, Geburt und Wochenbett an Aktualität. Frauen mit chronischen Erkrankungen müssen in der Zeit rund ums Mutterwerden sowohl die grundsätzlichen Herausforderungen dieser Lebensphase als auch die Besonderheiten ihrer Erkrankung bewältigen. Ihr Erleben und Bewältigungshandeln in diesem Spannungsfeld ist Gegenstand der vorliegenden Dissertation. Die Ergebnisse sollen auch der Entwicklung angepasster Versorgungs-konzepte rund um die Geburt dienen.
Die Untersuchung wurde entsprechend der Grounded Theory Methodologie durchgeführt. Datengrundlage sind 27 Interviews mit Frauen, die ein Kind geboren haben und mindestens zwei Jahre vor der Schwangerschaft chronisch erkrankten. Die Auswertung der Daten und Identifizierung der Kategorien folgte den Prinzipien des Theoretischen Samplings, der komparativen Analyse und des begleitenden Schreibens von Memos.
In den Ergebnissen kann gezeigt werden, dass betroffene Frauen in der Schwangerschaft die Sicherstellung größtmöglicher Gesundheit in gleichzeitiger Sorge um zwei bedürftige Körper, des eigenen und den des Ungeborenen, zum Ziel haben. Allgemeine Bedingungen der Erkrankung wirken auf dieses Geschehen ein. Der Einsatz verschiedener Strategien wie die Beobachtung der körperlichen Veränderungen und des kindlichen Wachstums, des Generierens von Wissen und die Aushandlungsprozesse mit den Professionen des Gesundheitssystems führen zu einer positiven oder negativen Bewertung und Bewältigung der Situation. Diese münden entsprechend in einem Gefühl neuer Potenz und Selbstermächtigung oder in einer Irritation gegenüber dem Krankheitsmanagement und potenziellen Handlungsoptionen.
Für die Frauen mit chronischen Erkrankungen hat das Gesundheitssystem in der Phase des Mutterwerdens eine relevante Bedeutung. Dabei wird deutlich, dass die Anforderungen an die Krankheits-, Biografie- und Alltagsarbeit bei ihnen verstärkt erlebt werden. Auch unterscheidet sich ihr Erleben, trotz einer für fast alle Frauen geltenden risikoorientierten Schwangerenbetreuung, in der Ausprägung, Dringlichkeit und Komplexität von dem der Frauen ohne Krankheitsdiagnose oder mit akuten Schwangerschaftskomplikationen. Dies hat auch Auswirkungen auf die Beziehung zum Kind und die Wahrnehmung gesellschaft-licher Teilhabe und Normalität.
Land cover change is a dynamic phenomenon driven by synergetic biophysical and socioeconomic effects. It involves massive transitions from natural to less natural habitats and thereby threatens ecosystems and the services they provide. To retain intact ecosystems and reduce land cover change to a minimum of natural transition processes, a dense network of protected areas has been established across Europe. However, even protected areas and in particular the zones around protected areas have been shown to undergo land cover changes. The aim of our study was to compare land cover changes in protected areas, non-protected areas, and 1 km buffer zones around protected areas and analyse their relationship to climatic and socioeconomic factors across Europe between 2000 and 2012 based on earth observation data. We investigated land cover flows describing major change processes: urbanisation, afforestation, deforestation, intensification of agriculture, extensification of agriculture, and formation of water bodies. Based on boosted regression trees, we modelled correlations between land cover flows and climatic and socioeconomic factors. The results show that land cover changes were most frequent in 1 km buffer zones around protected areas (3.0% of all buffer areas affected). Overall, land cover changes within protected areas were less frequent than outside, although they still amounted to 18,800 km2 (1.5% of all protected areas) from 2000 to 2012. In some parts of Europe, urbanisation and intensification of agriculture still accounted for up to 25% of land cover changes within protected areas. Modelling revealed meaningful relationships between land cover changes and a combination of influencing factors. Demographic factors (accessibility to cities and population density) were most important for coarse-scale patterns of land cover changes, whereas fine-scale patterns were most related to longitude (representing the general east/west economic gradient) and latitude (representing the north/south climatic gradient).

Objective: The more people there are who use clinical information systems (CIS) beyond their traditional intramural confines, the more promising the benefits are, and the more daunting the risks will be. This review thus explores the areas of ethical debates prompted by CIS conceptualized as smart systems reaching out to patients and citizens. Furthermore, it investigates the ethical competencies and education needed to use these systems appropriately.
Methods: A literature review covering ethics topics in combination with clinical and health information systems, clinical decision support, health information exchange, and various mobile devices and media was performed searching the MEDLINE database for articles from 2016 to 2019 with a focus on 2018 and 2019. A second search combined these keywords with education.
Results: By far, most of the discourses were dominated by privacy, confidentiality, and informed consent issues. Intertwined with confidentiality and clear boundaries, the provider-patient relationship has gained much attention. The opacity of algorithms and the lack of explicability of the results pose a further challenge. The necessity of sociotechnical ethics education was underpinned in many studies including advocating education for providers and patients alike. However, only a few publications expanded on ethical competencies. In the publications found, empirical research designs were employed to capture the stakeholders’ attitudes, but not to evaluate specific implementations.
Conclusion: Despite the broad discourses, ethical values have not yet found their firm place in empirically rigorous health technology evaluation studies. Similarly, sociotechnical ethics competencies obviously need detailed specifications. These two gaps set the stage for further research at the junction of clinical information systems and ethics.
The current narrative literature review aims to discuss clinical reasoning based on nociceptive pain mechanisms for determining the most appropriate assessment and therapeutic strategy and to identify/map the most updated scientific evidence in relation to physical therapy interventions for patients with temporomandibular disorders (TMDs). We will also propose an algorithm for clinical examination and treatment decisions and a pain model integrating current knowledge of pain neuroscience. The clinical examination of patients with TMDs should be based on nociceptive mechanisms and include the potential identification of the dominant, central, or peripheral sensitization driver. Additionally, the musculoskeletal drivers of these sensitization processes should be assessed with the aim of reproducing symptoms. Therapeutic strategies applied for managing TMDs can be grouped into tissue-based impairment treatments (bottom-up interventions) and strategies targeting the central nervous system (top-down interventions). Bottom-up strategies include joint-, soft tissue-, and nerve-targeting interventions, as well as needling therapies, whereas top-down strategies include exercises, grade motor imagery, and also pain neuroscience education. Evidence shows that the effectiveness of these interventions depends on the clinical reasoning applied, since not all strategies are equally effective for the different TMD subgroups. In fact, the presence or absence of a central sensitization driver could lead to different treatment outcomes. It seems that multimodal approaches are more effective and should be applied in patients with TMDs. The current paper also proposes a clinical decision algorithm integrating clinical diagnosis with nociceptive mechanisms for the application of the most appropriate treatment approach.
Clinically Significant Differences in Acute Pain Measured on Self-report Pain Scales in Children

Objectives
The objective was to determine the minimum and ideal clinically significant differences (MCSD, ICSD) in pain intensity in children for the Faces Pain Scale–Revised (FPS-R) and the Color Analog Scale (CAS) and to identify any differences in these estimates based on patient characteristics.
Methods
This was a prospective study of children aged 4 to 17 years with acute pain presenting to two urban pediatric emergency departments. Participants self-reported their pain intensity using the FPS−R and CAS and qualitatively described their changes in pain. Changes in pain score reported using the FPS-R and CAS that were associated with “a little less” and “much less” pain (MCSD and ICSD, respectively) were identified using a receiver operating characteristic–based method and expressed as raw change score and percent reductions. Estimates of MCSD and ICSD were determined for each category of initial pain intensity (mild, moderate, and severe) and patient characteristics (age, sex, and ethnicity). Post hoc exploratory analyses evaluated categories of race, primary language, and etiology of pain.
Results
A total of 314 children with acute pain were enrolled; mean (±SD) age was 9.8 (±3.8) years. The FPS-R raw change score and percent reduction MCSD estimates were 2/10 and 25%, with ICSD estimates of 3/10 and 60%. For the CAS, raw change score and percent reduction MCSD estimates were 1/10 and 15%, with ICSD estimates of 2.75/10 and 52%. For both scales, raw change score and percent reduction estimates of the MCSD remained unchanged in children with either moderate or severe pain. For both scales, estimates of ICSD were not stable across categories of initial pain intensity. There was no difference in MCSD or ICSD based on age, sex, ethnicity, race, primary language, or etiology of pain.
Conclusions
The MCSD estimates can be expressed as raw change score and percent reductions for the FPS-R and CAS. These estimates appear stable for children with moderate to severe pain, irrespective of age, sex, and ethnicity. Estimates of ICSD were not stable across different categories of initial pain intensity, therefore limiting their potential generalizability.

Das Konzept der Co-Creation hat sich zu einem vieldiskutierten Thema in der Veranstaltungsbranche entwickelt. Dieses Kapitel analysiert das Konzept von Co-Creation, auch vor dem Hintergrund ihrer Entstehung und Entwicklung im Rahmen der service-orientierten Sichtweise des Marketings. Ziel des Co-Creation-Ansatzes ist die Schaffung eines Mehrwertes für alle Beteiligten. Im Sinne eines gelungenen Wissenstransfers ist es für Veranstaltungs organisatoren dabei auch wichtig zu beachten, wie die Teilnehmer lernen und wie sie Informationen aufnehmen und behalten. Dazu werden verschiedene Lerntypen und Lernstile aus der Lernpsychologie vorgestellt. Die Ausführungen belegen die hohe Bedeutung von partizipativen Elementen, die somit neben den konventionellen Formaten (z. B. Frontalvorträgen) Bestandteil einer erfolgreichen Veranstaltung sein sollten. Das Kapitel stellt die prominentesten partizipativen Formate und deren Vor- und Nachteile vor und geht auch auf mögliche Herausforderungen bei der Umsetzung ein.
Co-financing arrangements in which investors from outside the motion picture industry become co-owners of the completed films are a common phenomenon in Hollywood. Kay H. Hofmann analyzes the conflicts of interest and the organizational problems that may arise between the experienced major studios and investors with comparably low industry expertise. Guided by principal agent theory, the empirical analysis provides evidence for adverse selection and moral hazard. Based on his findings, the author develops solutions that are not only relevant for investors but also for film producers who rely on the long-term availability of external funds.
Seit den 90er Jahren des letzten Jahrhunderts zählt Coaching zu den etablierten Methoden der Personalentwicklung. Ursprünglich handelte es sich dabei um eine Beratungssituation, in der eine Führungskraft in mehreren Einzelsitzungen mit einem Coach Probleme des Arbeitsalltags bespricht, Lösungsansätze entwickelt und deren Umsetzung in die Wege leitet. Im Fahrwasser dieses wirtschaftlich erfolgreichen Ansatzes werden inzwischen unterschiedlichste Methoden unter dem Label „Coaching“ vermarktet, die nicht einmal mehr Plausibilität für sich in Anspruch nehmen können. Drei dieser Ansätze – Horse Sense, Organisationsaufstellung und Spirituelles Coaching – sollen kurz vorgestellt und kritisch hinterfragt werden. Am Ende des Vortrags steht eine Analyse der Vermarktungsstrategien sowie der Gründe dafür, dass derartige Methoden überhaupt Abnehmer finden.
Background: Clinical handovers at changes of shifts are typical scenarios of time restricted and information intensive communication, which are highly cognitively demanding. The currently available applications supporting handovers typically present complex information in a textual checklist-like manner. This presentation style has been criticised for not meeting the specific user requirements.
Objectives: We, therefore, aimed at developing a concept for visualising the overview of a clinical case that serves as an alternative way to checklist-like presentations in clinical handovers. We also aimed at implementing this concept in a handoverEHR in order to support the pre-handover phase, the actual handover, and the post-handover phase as well as at evaluating its usability and attractiveness.
Results: We developed and implemented a concept that draws on Tolman's pioneering work on cognitive maps that we designed in accordance with Gestalt principles. These maps provide a pictorial overview of a clinical case. The application to build, manipulate, and store the cognitive maps was integrated into an openEHR based handover record that extends conventional records with handover specific information. Usability (n = 28) and attractiveness (n = 26) testing with experienced clinicians resulted in good ratings for suitability for the task as well as for attractiveness and pragmatism.
Conclusion: We propose cognitive maps to represent and visualise the clinical case in situations where there is limited time to present complex information.
Patient handovers are cognitively demanding, crucial for information continuity and patient safety, but error prone. This study investigated the effect of an electronic handover tool, i.e. the handoverEHR, on the memory and care planning performance of nurse students (n=32) in a randomised, controlled cross-over design with the factors handover task and handover role. On a descriptive level, handover recipients could improve their memory performance with electronic support, handover givers their performance of writing care plans. Statistically meaningful differences occurred, however, only when the participants were givers. Without handover experience and with low fluency to word problems, givers performed badly in the most demanding of the handover tasks. Final recommendations, however, can only be made after replicating this study in a clinical setting with mixed groups.
Background: Multilingual children with suspected SLCN are often overlooked or their needs not accurately differentiated regarding the necessity of language support or therapy. The purpose of the study was to conceptualize, carry out and evaluate a local language support (LS) project within linguistically and culturally diverse (LCD) families and its effects on all collaborating participants.
Methods: Eight SLT students and one lecturer took part in the LS-project, alongside equivalent numbers of family liaison personnel. Students visited more than 10 young children aged between 2-6 years, and for each child 10 weekly home visits were carried out. Language enhancement was documented, several case studies with children and interviews with five liaison personnel conducted.
Results: All SLT students perceived changes in the behaviour and communication of participating children. Children in the case studies developed from pre-verbal to verbal means of communication and family liaison personnel reported positive changes alongside parental wishes to continue the support.
Conclusion: Local language support projects with LCD families can lead to positive differences regarding their children's communication development and better inclusion in mainstream society. SLT students benefit from working with LCD families and their collaborative support together with family liaison personnel, and vice versa.

Comparison of quantitative sensory testing profles between people living in Germany and Australia

While Nursing Informatics competencies seem essential for the daily work of nurses, they are not formally integrated into nursing education in Austria, Germany and Switzerland, nor are there any national educational recommendations. The aim of this paper is to show how such recommendations can be developed, what competency areas are most relevant in the three countries and how the recommendations can be implemented in practice. To this end, a triple iterative procedure was proposed and applied starting with national health informatics recommendations for other professionals, matching and enriching these findings with topics from the international literature and finally validating them in an expert survey with 87 experts and in focus group sessions. Out of the 24 compiled competency areas, the relevance ratings of the following four recommended areas achieved values above 90%: nursing documentation (including terminologies), principles of nursing informatics, data protection and security, and quality assurance and quality management. As there were no significant differences between the three countries, these findings laid the foundation of the DACH Recommendations of Nursing Informatics as joint German (D), Austrian (A), and Swiss (CH) recommendations in Nursing Informatics. The methodology proposed has been utilized internationally, which demonstrates the added value of this study also outside the confines of Austria, Germany, Switzerland.
Current challenges are a result of complex and multi-layered factors. They require systemic solutions: problems must be solved holistically by identifying interconnections between the different problems which are results of the same root cause. Social entrepreneurs are agents of change, their mission is to generate societal impact. I investigated how a social entrepreneurship think tank can be conceptualized to support systems change. Think tanks can conduct in-depth research to inform about the systems and share knowledge. Entrepreneurs can make strategic interventions based on the research findings. To create a framework of how social entrepreneurship think tanks can promote systems change I conducted 14 case studies. The framework is created based on the method of Theory of Change, it displays how different activities can contribute to succeeding in systems change. I outline eleven activities which include strategies in capacity building, research and collaboration with the other actors in the field. In the end, the framework is applied to the evolving case of a think tank founded in 2018.
Conceptualising event value co-destruction and developing a future agenda for events research

Purpose
Value co-destruction has received little attention in an event-related context. This appears surprising, given that the interactions among actors at an event may also reduce the value for other participants, stakeholders and that of the entire event or the event's service ecosystem. This paper first aims to conceptualise value co-destruction and to provide an overview of related research in an event context. Second, a future research agenda for value co-destruction processes in an event context is developed.
Design/methodology/approach
Journals of the “Scimago Journal and Country Rank” were systematically reviewed for the keywords “value co-destruction”, “value destruction” and “negative value co-creation”. A second literature review specifically aimed at the events context extended the search scope to non-Scimago journals, Google Scholar and Google Web using the same keywords. All identified articles were qualitatively analysed concerning (1) the conceptualisation of value co-destruction and (2) reasons for value co-destruction.
Findings
The review of previous research highlights a limited scope of analysis, a focus on value co-destruction as an outcome and on interactions at the meso-level. Based on these findings, a holistic definition of value co-destruction is proposed. The paper identifies two major directions for future studies on value co-destruction at events and suggests specific examples.
Originality/value
The paper contributes to a more holistic understanding of value co-creation and co-destruction in an event setting. For example, a clearer understanding of the interactions that reduce the overall value of an event may assist to better design valuable events in the future.
Background:
The evaluation of somatosensory dysfunction is important for diagnostics and may also have implications for prognosis and management. The current standard to evaluate somatosensory dysfunction is quantitative sensory testing (QST), which is expensive and time consuming. This study describes a low-cost and time-efficient clinical sensory test battery (CST), and evaluates its concurrent validity compared to QST.
Method: Three patient cohorts with carpal tunnel syndrome (CTS, n=86), non-specific neck and arm pain (NSNAP, n=40) and lumbar radicular pain/radiculopathy (LR n=26) were included. The CST consisted of 13 tests, each corresponding to a QST parameter and evaluating a broad spectrum of sensory functions using mechanical and thermal detection and pain thresholds and testing both loss and gain of function. Agreement rate, significance and strength of correlation between CST and QST were calculated.
Results: Several CST parameters (cold and warm detection, cold pain, mechanical detection, mechanical pain for loss of function, pressure pain) were significantly correlated with QST, with a majority demonstrating >60% agreement rates and weak to relatively strong correlations. However, agreement varied among cohorts. Gain of function parameters showed stronger correlation in the CTS and NSNAP cohort, whereas loss of function parameters performed better in the LR cohort. Other CST parameters (vibration detection, heat pain, mechanical pain for gain of function, windup ratio) did not significantly correlate with QST.
Conclusion: Some, but not all tests in the CST battery can detect somatosensory dysfunction as determined with QST. The CST battery may perform better when the somatosensory phenotype is more pronounced.

The CST consisted of 13 tests, each corresponding to a QST parameter and evaluating a broad spectrum of sensory functions using thermal (coins, ice cube, hot test tube) and mechanical (cotton wool, von Frey hairs, tuning fork, toothpicks, thumb and eraser pressure) detection and pain thresholds testing both loss and gain of function. Agreement rate, statistical significance and strength of correlation (phi coefficient) between CST and QST parameters were calculated.
Results
Several CST parameters (cold, warm and mechanical detection thresholds as well as cold and pressure pain thresholds) were significantly correlated with QST, with a majority demonstrating >60% agreement rates and moderate to relatively strong correlations. However, agreement varied among cohorts. Gain of function parameters showed stronger agreement in the CTS and LR cohorts, whereas loss of function parameters had better agreement in the NSNAP cohort. Other CST parameters (16 mN von Frey tests, vibration detection, heat and mechanical pain thresholds, wind-up ratio) did not significantly correlate with QST.
Conclusion
Some of the tests in the CST could help detect somatosensory dysfunction as determined with QST. Parts of the CST could therefore be used as a low-cost screening tool in a clinical setting.
Significance
Quantitative sensory testing, albeit considered the gold standard to evaluate somatosensory dysfunction, requires expensive equipment, specialized examiner training and substantial time commitment which challenges its use in a clinical setting. Our study describes a CST as a low-cost and time-efficient alternative. Some of the CST tools (cold, warm, mechanical detection thresholds; pressure pain thresholds) significantly correlated with the respective QST parameters, suggesting that they may be useful in a clinical setting to detect sensory dysfunction.

Abstract: Thermal response curves that depict the probability of occurrence along a thermal gradient are used to derive various species’ thermal properties and abilities to cope with warming. However, different thermal responses can be expected for different portions of a species range. We focus on differences in thermal response curves (TRCs) and thermal niche requirements for four freshwater fishes (Coregonus sardinella, Pungitius pungitius, Rutilus rutilus, Salvelinus alpinus) native to Europe at (1) the global and (2) European continental scale. European ranges captured only a portion of the global thermal range with major differences in the minimum (Tmin), maximum (Tmax) and average temperature (Tav) of the respective distributions. Further investigations of the model-derived preferred temperature (Tpref), warming tolerance (WT = Tmax − Tpref), safety margin (SM = Tpref − Tav) and the future climatic impact showed substantially differing results. All considered thermal properties either were under- or overestimated at the European level. Our results highlight that, although continental analyses have an impressive spatial extent, they might deliver misleading estimates of species thermal niches and future climate change impacts, if they do not cover the full species ranges. Studies and management actions should therefore favor whole global range distribution data for analyzing species responses to environmental gradients.

Communication deficits belong to the most frequent errors in patient handovers calling upon specialized training approaches to be implemented. This study aims to harness problem-based learning (PBL) methods in handover education and evaluated the learning process. A digitally enabled PBL course was developed and implemented at Klinikum Osnabrück from which eight nurses participated in the course. They agreed on the stimulating effect of the setting regarding self-directed learning and on the potential to translate the new knowledge and skills into the daily clinical practice. In conclusion, the findings are promising that a digitally enabled PBL course is a suitable learning format for handover education.

Data warehouse systems have become a basic technological infrastructure in management decision making. Nevertheless, the overall utility of data warehouses remains unmeasured in most practical cases. As a consequence of this, IT-managers do not possess appropriate means to evaluate warehouse benefits in order to decide about investments in warehousing technology. This paper develops a controlling instrument for data warehouse systems based on the Balanced Scorecard (BSC) approach. On the basis of the technological aspects of data warehouse systems, the BSC perspectives are developed and populated with relevant objectives and measures for datawarehouse success. These perspectives are integrated into a consistent data warehouse scorecard. Finally, this instrument provides a holistic approach to drive the performance of data warehouse systems.

Controlling wird als eine betriebswirtschaftliche Teildisziplin dargestellt, die vor allem im Gesundheitsbereich Daten, Analysen und Auswertungen für Nicht-Ökonomen bereitstellt. Das Werk bereitet aktuelle Forschungsansätze für die Praxis auf, um darauf aufbauend anwendungsorientierte Controllinginstrumente vorzustellen, die mit Daten und Zahlen aus der Praxis hinterlegt sowie beispielhaft erläutert sind.
The well-documented social gradient in health in the developed world will not just disappear on its own. Tackling health inequalities by introducing a universal coverage healthcare system recently became an important notion in the U.S. Using cross-sectional data from Germany which has maintained its compulsory egalitarian healthcare system for more than 50 years now, we apply logistic and negative binomial regression to uncover utilisation behaviour patterns under universal coverage. We find that lower education and unemployment raise the risks for all diseases under consideration. Unemployment increases the chance of contacting a physician, while income and education do not apparently affect the healthcare utilisation behaviour. Those diseases concentrated among unemployed and less educated, however, are associated with intensified healthcare utilisation. We conclude that universal coverage may make access to health care easier for those facing the worst health; the unemployed and lower educated.
Highlights • We investigate how peer learning impacts elite footballers' performance. • We interview twelve elite footballers from German Bundesliga clubs. • Our findings highlight that peers are important knowledge sources in football. • Knowledge sharing positively impacts elite players' development and performance. • The findings call for management tactics to use this untapped potential. Abstract This research focuses on coworker learning and knowledge sharing amongst elite footballers. The authors provide an in-depth understanding on how elite footballers learn from their peers and which channels are used to share their knowledge. The authors also analyze how peer learning impacts an elite footballer's development and performance and to what extent elite football clubs actively support peer learning. Data were collected through semi-structured interviews with 12 elite footballers from first and second division German Bundesliga clubs. The findings demonstrate that peers are very important sources of knowledge for elite footballers. There are four main knowledge sharing channels: observing/imitating, peer exchange/peer communication, labor mobility and knowledge brokers. The findings highlight the positive impact of knowledge sharing on elite players' development and performance and call for future (knowledge) management tactics to specifically use this untapped potential.

Grundlagen in personalpsychologischen Gebieten können dabei unterstützen, effizienter Qualitätskriterien in der Personalauswahl zu treffen, zielführende Testverfahren zu entwickeln, soziale Kompetenzen besser einschätzen und fördern zu können sowie die Arbeitszufriedenheit zu messen. Dieses Buch ist eine Einführung in das weite Feld der Personalpsychologie und möchte in einfacher Sprache zeigen, wie man diese theoretischen Überlegungen in der Praxis anwenden kann. Dabei gilt es auch, unseriöse Methodiken zu kritisieren, die gerade in populären Medien große Beliebtheit erfahren, aber nicht zielführend sind.
In this paper we want to review and discuss research on the effects of occupational health management activities and services on different outcomes like employee health and well-being or absenteeism and other economic outcomes. In part I we outline goals and functions of occupational health management (OHM), then characterize typical occupational health promotion interventions and describe principles for implementing and organizing OHM. Part II focuses on different OHM activities and services such as creating healthy and safe workplace, reducing work-family conflicts, providing counselling via employee assistance programs and implementing health circles or stress management interventions. We will also discuss intervention design and sample studies as well as meta-analytic data relating to the effectiveness of these interventions. Finally, part III is about the economic impact of OHM. Findings from a management evaluation approach for OHM will be discussed. Then we will concentrate especially on data linking health promotion interventions to absenteeism and financial outcomes expressed as cost savings or cost-benefit ratios. The concluding part summarizes key findings of this paper.
The conservation of freshwater ecosystems has lagged behind that of marine and terrestrial ecosystems and often requires the integration of large-scale approaches and transboundary considerations. This study aims to set the foundations of a spatial conservation strategy by identifying the most important catchments for the conservation of freshwater biodiversity in Europe.
Using data on 1296 species of fish, mollusc, odonate and aquatic plant, and the key biodiversity area criteria (species Red List status, range restriction and uniqueness of species assemblages), we identified a network of Critical Catchments for the conservation of freshwater biodiversity. Applying spatial prioritisation, we show how the prioritised network differs from the ideal case of protecting all Critical Catchments and how it changes when protected areas are included, and we also identify gaps between the prioritised network and existing protected areas.
Critical Catchments (n = 8423) covered 45% of the area of Europe, with 766 qualifying (‘trigger’) species located primarily in southern Europe. The prioritised network, limited to 17% of the area of Europe, comprised 3492 catchments mostly in southern and eastern Europe and species targets were met for at least 96% of the trigger species.
We found the majority of Critical Catchments to be inadequately covered by protected areas. However, our prioritised network presents a possible solution to augment protected areas to meet policy targets while also achieving good species coverage.
Policy implications. While Critical Catchments cover almost half of Europe, priority catchments are mostly in southern and eastern Europe where the current level of protection is not sufficient. This study presents a foundation for a Europe-wide systematic conservation plan to ensure the persistence of freshwater biodiversity. Our study provides a powerful new tool for optimising investment on the conservation of freshwater biodiversity and for meeting targets set forth in international biodiversity policies, conventions and strategies.
HRM processes are increasingly AI-driven, and HRM supports the general digital transformation of companies’ viable competitiveness. This paper points out possible positive and negative effects on HRM, workplaces, and workersorganizations along the HR processes and its potential for competitive advantage in regard to managerial decisions on AI implementation regarding augmentation and automation of work.
A systematic literature review that includes 62 international journals across different disciplines and contains top-tier academic and German practitioner journals was conducted. The literature analysis applies the resource-based view (RBV) as a lens through which to explore AI-driven HRM as a potential source of organizational capabilities.
The analysis shows four ambiguities for AI-driven HRM that might support sustainable company development or might prevent AI application: job design, transparency, performance and data ambiguity. A limited scholarly discussion with very few empirical studies can be stated. To date, research has mainly focused on HRM in general, recruiting, and HR analytics in particular.
The four ambiguities’ context-specific potential for capability building in firms is indicated, and research avenues are developed.

BACKGROUND: The Craniofacial Pain and Disability Inventory (CF-PDI) is a cross-culturally adapted instrument designed from a biopsychosocial perspective to measure pain, disability, and function in orofacial head and neck pain with shown psychometric properties; however, the German cross-cultural adaption is lacking.
OBJECTIVES: To carry out a transcultural translation of CF-PDI into German and assess its psychometric properties in patients with painful temporomandibular disorders (TMD) with respect to construct and clinical validity, internal consistency and reproducibility.
STUDY DESIGN: Multicenter, prospective, cross-sectional design.
SETTING: Patients (n = 398) were recruited from dental and physical therapy clinics in middle and south Germany.
METHODS: Structural validity was assessed using exploratory factor analysis (EFA) and confirmatory factor analysis (CFA). We investigated know-group validity by means of the scale’s potential to discriminate between affected and unaffected subjects. Multiple linear regression analysis was used to estimate convergent validity. We tested test-retest reliability by the intraclass correlation coefficient and the Internal consistency by Cronbach’s alpha, or each dimension separately, and the total score. Multiple linear regression analysis was used to estimate convergent validity.
RESULTS: Two hundred forty-six heterogeneous chronic craniofacial pain patients and 152 patients without complaints were recruited from the middle and south of Germany. The German version CF-PDI-G presents 21 items, 4 factors, and adequate psychometric properties. The test-retest reliability and internal consistency of the CF-PDI-G were both excellent for the entire instrument and also for all sub-scales (intraclass correlation coefficient [ICC] > 0.90) except for the comorbidities and interference with work which was acceptable (ICC = 0.69). Standard error of the measurement (SEM) and minimal detectable change values are sufficiently low. Assessment of clinical validity shows good potential of discrimination and classification into categories “no,” “mild,” “moderate,” and “severe.” The multiple linear regression model showed a strong association between neck disability index, Visual Analog Scale, and anamnestic questionnaire (supporting the scale’s convergent validity).
LIMITATIONS: Our sample has a higher prevalence of women and the sample was not recruited consecutively, which may lead to a biased estimation of psychometric properties.
CONCLUSIONS: The CF-PDI-G represents valid and reliable instrument to assess pain and disability in patients with orofacial pain and headache suitable for research and clinical practice.
