background: Musculoskeletal problems (MP) are widespread in performing artists and are due to the special demands of instrument playing, singing or dancing. Additionally, various other factors might contribute to performance-related MP. To provide a specific physiotherapeutic management for performing artists it is important to gain information about the performing art, the individual demands and contributing factors. The subjective examination (SE) is the basis of the clinical reasoning process and the hypothesis forming for further clinical examination and biomechanical analysis. In the present protocol, the SE consists of a questionnaire-based section and an interview-based section and is part of the evaluation process of the neuromusculoskeletal examination of a performing artist specific reference laboratory.
purpose: To develop a standardized SE protocol divided into a questionnaire-based section (Part 1) via web application and an interview-based SE (Part 2) to address MP of performing artists.
methods: The questionnaires for part 1 were selected based on the expertise of the research group and the psychometric properties of each possible questionnaire. A common physiotherapeutic recording of findings which addresses the relevant questions of the SE of MP was used. To adapt the anamnesis to performing-associated MP questions specific to instrument playing, singing and dancing were selected on the basis of a literature search and the expertise of the research group.

Background: Musculoskeletal problems (MP) are widespread in performing artists and are due to the special demands of instrument playing, singing or dancing. In order to specifically evaluate these problems, a reference laboratory is under development. The evaluation covers 4 steps: a subjective examination (SE) including (1) a questionnaire-based online survey and (2) an interview-based anamnesis. On the basis of the results of the SE, hypotheses are formed for (3) an individual musculoskeletal clinical examination and a (4) biomechanical analysis. Here, the focus is on the clinical examination.
Purpose: to develop a standardized protocol for a clinical examination addressing especially musculoskeletal problems in performing artists.
Methods: A common physiotherapeutic clinical examination should be supplemented with techniques, which are specific to performance-related musculoskeletal problems and/or their risk factors. The development was based on a literature search and the clinical expertise of the physiotherapeutic research group.
Summary of content/Results: The performing arts specific clinical examination includes the common analysis of posture as well as passive/active movement capacities and specific differentiating tests in relation to the signs and symptoms of the individual artist. The examination is supplemented by a functional demonstration focusing on the special demands of playing the respective instrument, while singing or dancing. Common overuse risk factors like hypermobility, special anthropometric data or an examination of the motor control of different body regions are addressed. Optional, the various parts of the examination process could be skipped, if not relevant for the individual artist.
Significance: This clinical examination protocol should help clinicians evaluating the musculoskeletal health of performing artists in a standardized and specific way.
AIM: In this prospective longitudinal study, the physical and psychological health status of music students is assessed at the beginning of their university music study and tracked over time. Analysis strategies and interim results from the first-year cohort, including 1-year incidences, monthly prevalences, and predictors of developing musculoskeletal health complaints (MHC), are presented.
METHODS: This prospective longitudinal study is calculated to enlist a total sample of 370 participants, including musicians and non-musicians, over 5 years. Baseline measurements include a self-designed questionnaire, core strength endurance, hypermobility, finger-floor-distance, motor control, mechanosensitivity, health-related quality of life (SF36), and stress and coping inventory (SCI). The occurrence of MHC is based on monthly online questionnaires.
RESULTS: The first-year subcohort enrolled 33 music students and 30 non-music control students. The mean monthly completion rate for the questionnaire was 55.7±8.7%. At baseline, music students showed significantly more stress symptoms, reduced physical function¬ing, and increased bodily pain compared to control students. The 1-year incidence of MHC was 59% for music students and 44% for controls. Risk factors for MHC included being a music student, previous pain, reduced physical functioning, stress symptoms, reduced emotional functioning, and mechanosensitivity. Being a music student, physical functioning, sleep duration, positive thinking, and general mechanosensitivity had a predictive ability of 0.77 (ROC curve) for MHC.
CONCLUSION: A total of 63 students enrolled in the first cohort is in line with the precalculated sample size. This prospective study design enables the measurement of MHC incidence and provides insight into mechanisms in the development of MHC among music students, including the interaction of physical, psychological, and psychosocial factors.
There is clinical evidence that cervical lateral glide (CLG) improves neurodynamics and alleviates pain in patients who suffer from neurogenic arm pain. Cervical lateral flexion (CLF) is also a treatment method and a means of testing neurodynamics. However, for both techniques nerve movement has not yet been investigated using ultrasound imaging (US). The purpose of this study was to quantify median nerve movement in the arm during CLG and CLF. For this study 27 healthy participants were recruited. Longitudinal movement of the median nerve was measured using US during CLG and CLF with the shoulder in 30° abduction in the middle and distal forearm (Fad). Data could be obtained from 11 participants (6 women and 5 men, average age 25.6 years, ±2.25) at the middle forearm (Fam) and from 9 participants (5 women and 4 men, average age 27.2 years, ±2.75) at the Fad. When applying CLF, the median nerve moved 2.3 mm (SEM ± 0.1 mm) at the Fam. At the same measuring point the median nerve moved 3.3 mm (SEM ± 0.3 mm, p = 0.005) by applying CLG. At the Fad the difference between CLF and CLF amounted to 0.6 mm (CLF: 1.9 mm (SEM ± 0.2 mm, CLG: 2.5 mm (SEM ± 0.2 mm, p ≤ 0.05). The movements during CLG are larger than during CLF. This difference is statistically significant. However, the statistical relevance cannot be extrapolated to a clinical relevance.
Objectives:
The purpose of this study was to determine the motor function of the abdominal muscles in singers with and without functional voice disorders and to examine them for possible differences. Additionally, the breathing behaviour and posture control was investigated.
Study Design
Observational study.
Methods:
Female subjects (n = 20) with differing levels of professional competence were used to provide the data for analysis. By using the Singing Voice Handicap Index (SVHI) the grade of dysphonia could be measured, and the subjects were organized in groups. The change of muscle thickness of the M. transversus abdominis (TVA) and the M. obliquus internus abdominis (OIA) during different singing tasks was measured by using ultrasound. The subjects were then asked to perform the Abdominal Hollowing Test (AHT) with the STABILIZER. Finally, the subjects were all filmed while singing. The videos recordings of the singing sessions were analysed by an independent clinical expert regarding breathing and secondary motor activities (SMA). For the statistical analysis, the Mann-Whitney-U Test and the Chi-Square-Test was mainly used.
Results:
The results showed a significantly thinner TVA in the group with dysphonia in comparison to the group without dysphonia. Ultrasound measurements showed significantly higher changes of muscle thickness of the TVA during singing tasks in the group with dysphonia. Regarding the AHT there was a significant difference between the two groups. The group with dysphonia was not able to increase the pressure by 15mmHg. Furthermore, the healthy subjects demonstrated abdominal breathing, while the group with dysphonia present with thoracic breathing. Additionally, it was noted that the subjects with dysphonia showed a higher level of associated movements especially at and/or on the lumbar spine, cervical spine and the left arm and shoulder.
Conclusion:
Differences in TVA-recruitment, breathing behaviour and secondary motor activities while singing were found. This study sparks new ideas for neuromusculoskeletal assessments and therapy.

Musicians often suffer from disorders of the musculoskeletal system that are related to their instrument playing. Among the most frequent symptoms are complaints in the shoulder-neck area. Radial shock wave therapy is increasingly used in trigger point treatment, but only few high-level studies have examined of shock wave therapy used together with physical therapy in the treatment of musicians. METHODS: This randomized blinded study in musicians (n = 26) with nonspecific shoulder-neck problems was done to examine the effect of shock wave therapy in addition to current physical therapy on the symptoms and quality of life of the musicians as well as their habits of playing musical instruments (intervention group shock wave vs reference group placebo). The effects were documented by a pain VAS and other instruments. A questionnaire designed specifically for musicians (with initial and final questions) recorded intensity and manifestation of pain and handicaps in daily life, especially when practicing and playing. The Shoulder Pain and Disability Index (SPADI) and the Neck Pain Disability Index Questionnaire (NPDIQ) were also used. RESULTS: Both groups reported subjective improvement in pain, but significance was found only for the intervention group for the SPADI and NPDIQ. CONCLUSIONS: Trigger point treatment with radial shock wave used in combination with physical therapy makes the subjects feel temporarily relieved of neck and shoulder pains. The effects of radial shock wave without physical therapy will need to be examined in further studies.
Einleitung
Ziel dieser Studie war es, zu untersuchen, ob es einen Zusammenhang gibt zwischen einer mechanisch gesteuerten Traktion am Handgelenk und den nächtlichen Kribbelparästhesien sowie der Funktionsfähigkeit bei Patienten mit Karpaltunnelsyndrom.
Methode
In der vorliegenden Interventionsstudie wurden 21 Patienten mit Karpaltunnelsyndrom präoperativ sechsmal mittels des Traktionsgerätes Phystrac GR 10 behandelt. Die Messung der Effekte wurde vor sowie nach der ersten, dritten und sechsten Intervention durchgeführt. Als Assessment-Instrumente wurden die Sonografie, der Hand Grip Dynamometer, die Visuelle Analogskala und der Boston Carpal Tunnel Syndrome Questionnaire genutzt. Letzterer wurde nur zu Beginn und am Ende der Studie ausgefüllt.
Ergebnisse
Statistisch signifikante Ergebnisse konnten für eine Verbesserung der Funktionsfähigkeit (p = 0,023) und eine Reduktion der nächtlichen Kribbelparästhesien (p = 0,001) nach sechs Interventionen erzielt werden. Zudem konnte eine Korrelation zwischen diesen Parametern nachgewiesen werden (r = 0,497 zu Beginn und r = 0,591 am Ende der Studie). Die Parameter Querschnittsfläche sowie transversales und longitudinales Gleiten des N. medianus, Greifkraft, Schmerzen beim Greifen und Symptome veränderten sich nicht signifikant.
Schlussfolgerungen
Die vorliegende Studie konnte aufzeigen, dass eine mechanisch gesteuerte Traktion eine wirkungsvolle konservative Intervention bei Karpaltunnelsyndrom darstellt und als mögliche Alternative zu operativen Eingriffen am Karpaltunnel gesehen werden sollte. Es ist davon auszugehen, dass eine mechanisch gesteuerte Traktion Einfluss auf die intraneuralen physiologischen Eigenschaften und weniger auf die Mechanik des N. medianus nimmt.
Hintergrund
Das Bundesministerium für Bildung und Forschung (BMBF) initiierte 2016 die Open-Access-Initiative. Open Access bedeutet, Wissen online kostenlos freizugänglich zu machen. Dieser Anspruch setzt eine Digitalisierung voraus, die hochschulübergreifend professionelles Wissen öffentlich macht. Dazu passt die Etablierung einer entsprechenden Online-Datenbank.
Ziel
Mithilfe einer Online-Befragung die Erwartungen potentieller Nutzer an eine Volltextdatenbank zu untersuchen. Mittels eines Fragebogens sollen Nutzerverhalten, Bedürfnisse, Notwendigkeiten erforscht sowie Ideen, Anregungen, persönliche Einschätzungen und mögliche Vor- und Nachteile erfasst werden.
Methode
Durchführung einer Online-Befragung mit 20 Fragen, welche in vier Abschnitte eingeteilt wurden. Geschlossene Fragen wurden deskriptiv ausgewertet, offene Fragen wurden mit der Kernsatzmethode nach Leithäuser und Volmerg (1988) ausgewertet.
Ergebnisse
Insgesamt 84,87 % der 337 Befragten zeigten Interesse an einer Volltextdatenbank für Abschlussarbeiten aus den Therapiewissenschaften. Wünsche der Befragten konnten in den Anforderungskatalog für die zu entwickelnde Volltextdatenbank aufgenommen werden. 67,95 % der Befragten würden sich ein Profil in einer Volltextdatenbank anlegen. Die Vorstellung, eine Abschlussarbeit in einem Videoporträt anzulegen, wurde hingegen von 36,50 % als nicht nützlich bewertet.
Schlussfolgerung
Anhand der Online-Befragung konnten die Rechercheaktivität und die Wünsche an eine Volltextdatenbank abgefragt werden. Die Nutzerakzeptanz kann erst zu einem späteren Zeitpunkt hinreichend bewertet werden, mithilfe einer Evaluationsstudie.
Due to new research results in the past few years, interest in the fascia of the human body has increased. Dysfunctions of the fascia are indicated by various symptoms, amongst others, musculoskeletal pain. As a result stronger focus has been put on researching therapeutic approaches in this area. The main aim of this study was to investigate the effect of Foam Roll exercises on the mobility of the thoracolumbar fascia (TLF). Study has been conducted in a randomized and controlled trial which sampled 38 healthy athletic active men and women.
The subjects were randomly assigned to a Foam Roll Group (FMG), a Placebo Group (PG) and a Control Group (CG). Depending on the assigned group the volunteers were either instructed to do exercises with the Foam Roll, received a pseudo treatment with the Foam Roll or received no treatment.
A total of three measurements were carried out. The most important field of research was the mobility of the TLF, which was determined using a sonographic assessment. In addition the lumbar flexion and the mechanosensivity of relevant muscles were determined.
After the intervention, the FMG showed an average increase of 1.7915 mm for the mobility of the TLF (p < 0.001/d = 0.756). In contrast, only an average improvement of 0.1681 mm (p = 0.397) was shown in the PG, while the CG showed a slight improvement of 0.0139 mm (p = 0.861). However, no significant changes were observed with regard to the lumbar flexion and mechanosensivity of the treated muscles.
Thus, evidence is that the use of Foam Roll exercises significantly improves the mobility of the thoracolumbar fascia in a healthy young population.

Präventive Maßnahmen werden im Rahmen der Gesundheitsförderung immer bedeutsamer. Die regelmäßige Bewegung führt u. a. zu einer Stärkung des Herz-Kreislauf-Systems sowie zur Reduktion von muskuloskelettalen Beschwerden. Angestellte von Hochschulen sowie Studierende leiden häufig unter muskuloskelettalen Beschwerden.
Ziel
Ziel der Kundenbefragung ist es, die Zufriedenheit mit dem hochschulinternen Fitnessstudio INMOVE abzufragen, das rein physiotherapeutisch konzipiert ist. Mithilfe des Fragebogens sollen v. a. die Bereiche der Zufriedenheit, der physiotherapeutischen Betreuung sowie die subjektive Lebensqualität beurteilt werden.
Methode
Es wurde eine Kundenbefragung mit 26 Fragen, welche in vier Abschnitte eingeteilt sind, durchgeführt. Die geschlossenen Fragen wurden deskriptiv ausgewertet und die acht offenen Fragen wurden mit der Kernsatzmethode nach Leithäuser und Volmerg (1988) ausgewertet.
Ergebnisse
An der Befragung nahmen insgesamt 70 Trainierende (weiblich: 22/31,2 %, männlich: 48/68,8 %) teil. Anhand der Ergebnisse zeigt sich, dass die Teilnehmenden die Frage „Inwieweit sind die Kunden des hier genannten INMOVE mit dem Fitnessstudio zufrieden?“ mit „ziemlich zutreffend“ und „zutreffend“ beantworteten. Die zweite Forschungsfrage „Ist die physiotherapeutische Betreuung adäquat für das Fitnessstudio?“ kann mit „ziemlich zutreffend“ beantwortet werden. Die dritte Forschungsfrage bezog sich auf die Verbesserung der subjektiven Lebensqualität. Die Antwortenverteilung spiegelt, dass die Teilnehmenden diesen Themenbereich mit überwiegend „trifft genau zu“ oder „trifft ziemlich zu“ beantworteten.
Schlussfolgerung
Anhand der Kundenbefragung konnte die Zufriedenheit sowie die Auswirkungen auf die subjektive Lebensqualität beurteilt werden. Es lässt sich aus den Ergebnissen ableiten, dass ein hochschulinternes Fitnessstudio dabei helfen kann, Alltagsstress abzubauen und somit präventiv bei muskuloskelettalen Beschwerden nützlich ist.
Hintergrund Die physiotherapeutische Dokumentation spielt im Therapieprozess eine wichtige Rolle, erfolgt jedoch auf unterschiedlichste Weise. Es existieren derzeit eine Vielzahl von Softwarelösungen für die physiotherapeutische Dokumentation, welche sich jedoch in ihren Funktionalitäten stark unterscheiden. Ziel Mithilfe einer Befragung soll ein Konsens von Expertenaus der Physiotherapie im Themengebiet der softwarebasierten Dokumentation ermittelt werden. Anhand der Ergebnisse wird ein Anforderungskatalog für die Entwicklung einer neuartigen und benutzerorientierten Dokumentationssoftware erstellt. Methode Online-Delphi-Befragung mit neun Experten über drei Befragungswellen.
Ergebnisse Hinsichtlich der Anforderungen an die Anamnese konnte ein Konsens erzielt werden. Bei der Gliederung des Befundes kam es zu keiner Übereinstimmung der Experten. Das Ergebnis lässt sich durch unterschiedliche manualtherapeutische Konzepte erklären, die für die Befunderhebung verwendet wurden. Schlussfolgerung Eine softwarebasierte Dokumentation sollte standardisierter als bisher erfolgen, um den ClinicalReasoning-Prozess zu unterstützten. Gleichzeitig ist dabei eine gewisse Flexibilität geboten. Die gesammelten Anforderungen können für die Entwicklung einer neuartigen und benutzerorientierten mobilen Anwendung zur Effizienzsteigerung in der physiotherapeutischen Dokumentation verwendet werden.
Wozu betriebliche Gesundheitsförderung? : Eine Bedarfsanalyse an der Fachhochschule Osnabrück

Hintergrund
Betriebliche Gesundheitsförderung ist ein Instrument, um auf die gesamtgesellschaftlichen und strukturellen Veränderungen, die an Hochschulen gestellt werden, zu reagieren. Um betriebliche Gesundheitsförderung an der Fachhochschule Osnabrück zu etablieren, wurde zunächst der Bedarf analysiert.
Methode
Der Bedarf an betrieblicher Gesundheitsförderung wurde mittels Web-basierter, schriftlicher Mitarbeiter- und Studierendenbefragung erhoben und hinsichtlich der Statusgruppen differenziert ausgewertet.
Ergebnisse
Es bestehen signifikante Unterschiede sowohl im Gesundheitsverhalten als auch im Gesundheitszustand der Beschäftigten und Studierenden. Nackenschmerzen sind bei Mitarbeitern häufiger, wohingegen Studierende stärkere psychische Belastungen angaben. Auch die Erwartungen an BGF unterscheiden sich hinsichtlich der Statusgruppen.
Schlussfolgerung
Die Angehörigen der Fachhochschule Osnabrück weisen insgesamt einen relativ guten Gesundheitszustand auf, dennoch zeigte die Bedarfsanalyse die Notwendigkeit und auch das Potential für BGF-Maßnahmen auf.
Objective
Brass players are exposed to high musculoskeletal strains during their instrumental play. Various assessments can be used to measure these strains, whereby a targeted therapy can also be supported. The aim of this study was to review literature concerning assessments used in quantitatively based studies about the analysis of musculoskeletal loads of brass players.
Data sources
The Cochrane Library, PubMed, CINAHL, PEDro as well as the journal “Medical Problems of Performing Artists” were searched for relevant studies.
Study selection
Two reviewers independently applied the inclusion and exclusion criteria to select potential studies. A third reviewer was involved in the case of discrepancies.
Data extraction
Two reviewers independently extracted the data.
Data synthesis
A total of 73 studies conducted between 2004 and 2019 were included. Within a total of 30 studies, 18 assessments could be found that collect 2-dimensional or 3-dimensional kinematic data using video- or image-based analysis of posture, sonographic, optoelectronic and various electromagnetic systems. In 7 studies kinetic data were measured by force-transducers, pressure platforms, stabilizer and dynamometer. Fifteen studies used clinical examinations and additional assessments to screen individual body regions and 9 studies derived electromyography measurements from a total of 25 muscles. Thirty-one partially validated questionnaires were used to record musculoskeletal pain of brass players.
Conclusions
A variety of assessments can be used to optimize analysis and treatment procedures in research and clinical work. Future studies should both examine quality criteria of the various assessment methods and validate clinical examinations and questionnaires.
Wie kombiniert man die in der osteopathischen Ausbildung erlernten Untersuchungs- und Behandlungstechniken zu einem Behandlungskonzept, das optimal auf den Patienten zugeschnitten ist? Antworten liefert das Buch mit Anleitungen zur systematischen Diagnose, zur Planung individueller Therapiekonzepte und zum problemorientierten Testen und Üben. Therapeuten in der Ausbildung erarbeiten sich damit Strategien für ein ganzheitliches Vorgehen, für ausgebildete Osteopathen eignet sich der Band als Nachschlagewerk (mit Index der Leitsymptome).
Seit der Bologna-Deklaration von 1999 entstanden in Deutschland 2001 erste Bachelor-Studiengänge für Physiotherapeuten. Bislang bestehen jedoch kaum Informationen, welche beruflichen Veränderungen der Bachelo-rabschluss für Physiothera-peuten gestattet. Deshalb wurde eine Absolventenbefragung des Bachelor-Studiengangs Physiotherapie an der FH Osnabrück durchgeführt, um Einblicke über die beruflichen Veränderungen der ehemaligen Studenten aufgrund des Studiums zu gewinnen. Die Ergebnisse stellen wir mit diesem Beitrag dar.
Die Akademisierung der deutschen Physiotherapie ist mittlerweile 10 Jahre alt. Anhand einer Absolventenbefragung der ersten Jahrgänge der Hochschule Osnabrück (vormals Fachhochschule Osnabrück) sollen Erwartungen an das Studium ebenso wie die erworbenen Kompetenzen und die veränderte Berufsauffassung exemplarisch dargestellt werden.
Background
Osteoarthritis of the knee is the most common cause for disability and limited mobility in the elderly, with considerable individual suffering and high direct and indirect disease-related costs. Nonsurgical interventions such as exercise, enhanced physical activity, and self-management have shown beneficial effects for pain reduction, physical function, and quality of life (QoL), but access to these treatments may be limited. Therefore, home therapy is strongly recommended. However, adherence to these programs is low. Patients report lack of motivation, feedback, and personal interaction as the main barriers to home therapy adherence. To overcome these barriers, electronic health (eHealth) is seen as a promising opportunity. Although beneficial effects have been shown in the literature for other chronic diseases such as chronic pain, cardiovascular disease, and diabetes, a systematic literature review on the efficacy of eHealth interventions for patients with osteoarthritis of knee is missing so far.
Objective
The aim of this study was to compare the efficacy of eHealth-supported home exercise interventions with no or other interventions regarding pain, physical function, and health-related QoL in patients with osteoarthritis of the knee.
Methods
MEDLINE, CENTRAL, CINAHL, and PEDro were systematically searched using the keywords osteoarthritis knee, eHealth, and exercise. An inverse variance random-effects meta-analysis was carried out pooling standardized mean differences (SMDs) of individual studies. The Cochrane tool was used to assess risk of bias in individual studies, and the quality of evidence across studies was evaluated following the Grading of Recommendations, Assessment, Development, and Evaluation approach.
Results
The literature search yielded a total of 648 results. After screening of titles, abstracts, and full-texts, seven randomized controlled trials were included. Pooling the data of individual studies demonstrated beneficial short-term (pain SMD=−0.31, 95% CI −0.58 to −0.04, low quality; QoL SMD=0.24, 95% CI 0.05-0.43, moderate quality) and long-term effects (pain −0.30, 95% CI −0.07 to −0.53, moderate quality; physical function 0.41, 95% CI 0.17-0.64, high quality; and QoL SMD=0.27, 95% CI 0.06-0.47, high quality).
Conclusions
eHealth-supported exercise interventions resulted in less pain, improved physical function, and health-related QoL compared with no or other interventions; however, these improvements were small (SMD<0.5) and may not make a meaningful difference for individual patients. Low adherence is seen as one limiting factor of eHealth interventions. Future research should focus on participatory development of eHealth technology integrating evidence-based principles of exercise science and ways of increasing patient motivation and adherence.
Kreuzbandrisse (Rupturen des vorderen Kreuzbands, VKB) ziehen oft eine langwierige Rehabilitation und Wiedereingliederung nach sich. Die physiotherapeutische Nachbehandlung beinhaltet aktive und passive Maßnahmen. Bislang existieren keine Studien zu neurodynamischen Aspekten der Rehabilitation von Kreuzbandrissen. Das Ziel der vorliegenden Studie bestand darin, die Veränderung der Bewegung des N. tibialis des betroffenen und nicht betroffenen Beins bei Probanden nach einer VKB-Ruptur zu untersuchen.
Bestandteil der verblindeten Untersuchung der 19 Probanden waren der Knee Outcome Survey für alltägliche Aktivitäten (KOS) und verschiedene Funktionsuntersuchungen (Slump-Test mit passiver Knieextension, Long Sitting Slump Test mit passiver Dorsalextension des oberen Sprunggelenks, Straight Leg Raise Test, belastete Dorsalextension im Stand). Die longitudinale Nervenexkursion des N. tibialis wurde mit bildgebendem Ultraschall in sitzender Slump-Position untersucht. Die Auswertung der Ultraschallvideos erfolgte mit einer Frame-by-frame Cross Correlation Software.
Die Ergebnisse zeigten eine signifikant geringere Nervenexkursion des N. tibialis am betroffenen Knie. Allerdings fand sich kein klinisch signifikanter Zusammenhang zu den Funktionsuntersuchungen.
Otto Frisch, ein 74-jähriger Rentner, hat starke Rücken- und Beinschmerzen. Der Arzt diagnostiziert eine Ischialgie, spritzt ihn und verschreibt Ibuprofen. Aber nichts hilft, die Schmerzen werden unerträglich. Als Otto Frisch zu Physiotherapeutin Fiona Morrison kommt, schrillen bei ihr schnell alle Alarmglocken.
Nach einer Bandscheiben-OP entwickelt Ina Zeiß diffuse Schmerzen im linken Bein und dumpfe Kopfschmerzen. Auch der seit fünf Jahren bestehende Tinnitus hat sich verstärkt. Die Symptome sind sehr wechselhaft und nicht durch Physiotherapie zu beeinflussen. Physiotherapeutin Fiona Morrison merkt schnell, dass dies keine typischen postoperativen Beschwerden sind, und schickt die Patientin zurück zum Arzt.

First, the component planning of the innovative reference laboratory structure is carried out, on which the clinical and technical development of the reference laboratory is based. Parallel to this, various analysis tools are being developed for biomechanical data as well as for physiotherapeutic diagnostics and merged at the end of this work package for individual results for different groups of performing artists. Overlapping with the previous work packages, the reference laboratory will be tested for application and manageability. Clinical and technical data are brought together and checked by means of individual case trials on various groups of artists. In terms of an agile software development process, the results flow back into the previous work packages in order to optimize them and then test them again.
Results:
At the end of the project a functional and proven reference laboratory for the comprehensive analysis of performing artists will be developed.

Background: Muscle fatigue has been reported as a risk factor for the
development of performance-related musculoskeletal disorders (PRMD) in
musicians. However, little research exists to support this claim. The aim of
this study was to investigate whether changes occur in muscle activity
patterns during high string performance over a prolonged playing period, and
whether this is influenced by PRMD.
Methods: High string musicians were divided into a PRMD and a non-PRMD
group. They played a chromatic scale pre and post and a self-chosen “hard”
(Borg scale 16-17) piece of music for one hour. Electromyography data
recorded from arm, shoulder and trunk muscles was analyzed: the amplitude
to measure muscle activity characteristics and the lower frequency to
measure muscle fatigue. Differences between and within groups and the
frequency spectrum were analyzed using linear mixed models.
Results: Fifteen musicians participated (7 PRMD: 22.8 years, 2 male/5 female
and 8 non-PRMD: 34.3 years, 2 male/6 female). Changes in muscle activation
patterns were observed between and within both groups, however changes
varied significantly depending on group affiliation. Significant low frequency
spectrum changes between groups were observed in overall muscles of the
right arm (p=0.04) and left forearm flexors (p=0.05) following one hour of
playing.
Conclusions: Muscle activity and frequency spectrum shifts differ in high
string musicians with and without PRMD, suggesting possible differential
muscle fatigue effects between the groups.
BACKGROUND: Muscle fatigue seems to be a risk factor in the development of performance-related musculoskeletal disorders (PRMDs) in musicians, but it is unclear how muscle activity characteristics change between musicians with and without PRMDs over a prolonged playing period.
PURPOSE: To investigate muscle activity patterns in muscles of the arms, shoulder, and back of high string musicians during prolonged performance.
METHODS: Fifteen professional or university high string musicians were divided into PRMD and non-PRMD groups. All musicians played a chromatic scale, then an individual “heavy” piece for 1 hr, and finally the chromatic scale again. Surface electromyography (sEMG) data were recorded from 16 muscles of the arm, shoulder, and trunk on both sides of the body. Two parameters were analyzed: the percentage load in relation to the respective maximum force during the chromatic scale, and the low-frequency spectrum to determine the fatigue behavior of muscles during the 1-hr play.
RESULTS: Changes in muscle activation patterns were observed at the beginning and end of the trial duration; however, these varied depending on whether musicians had PRMDs or no PRMDs. In addition, low-frequency spectrum changes were observed after 1 hr of playing in the PRMD musicians, consistent with signs of muscular fatigue.
CONCLUSION: Differences in muscle activity appear between high string musicians with and without PRMDs as well as altered frequency spectrum shifts, suggesting possible differential muscle fatigue effects between the groups. The applied sEMG analysis proved a suitable tool for detailed analysis of muscle activation characteristics over prolonged playing periods for musicians with and without PRMDs.
Rationale:
Dance as an intense kind of performance is associated with high loads on the musculoskeletal system. In particular, the lower limb is exposed to these high loads, which is reported by a high prevalence. Most dancers are affected by injuries during their careers, most in the lower limb. Typical risk factors for dancers include the compensatory turnout, hypermobility, and core stability. The correlation between these factors and lower limb injuries is not fully understood.
Purpose:
The aim of this study was to evaluate the correlation between lower limb injuries and the risk factors compensated turnout, hypermobility and core stability. Based on these results, hypotheses can be generated for further studies.
Methods:
This explorative pilot study was conducted at Osnabrück University of Applied Sciences. All eligible participants were scheduled for a single research appointment if they fulfilled the inclusion criteria of dance experience (ballet, modern, contemporary or jazz) at least one year with dance lessons of at least 5 hours per week. Parameters for the correlation analysis were (1) core stability, measured by the motor control tests battery proposed by Luomajoki, (2) hypermobility, measured by the Beighton Score (3) compensated turnout, measured by the difference between functional turnout and external rotation of both hips, (4) dance hours per week, (5) dance experience in years, (6) professional dance experience in years and (7) dance style (ballet, modern, contemporary, jazz). These parameters were correlated with the numbers of lower limb injuries. Significance level was set at 90% because of the explorative character of the study with the purpose to generate hypotheses.
Results:
Sixteen female dancers with different levels of dance experience (mean 11.3 years) and dance lessons per week (mean 17.6 hours) fulfilled the inclusion criteria and participated in this study. Significant correlations with the numbers of lower limb injuries were found at core stability (rs=0.489, p=0.034, medium effect), dance hours per week (rs=0.459, p=0.048, medium effect) and professional dance experience in years (rs=0.396, p=0.093, medium effect). Two dance styles (modern dance: rs=0.388, p=0.101 and contemporary dance: rs=0.385, p=0.104) were close to the significance level.
Conclusions:
These results show correlations between core stability, dance hours per week, professional dance experience in years and lower limb injuries in dancers.
Due to the limitations of a small and heterogeneous sample size as well as the medium effects, these results should be interpreted with caution, but may provide a basis for further research to this topic in the field of dance research and can be helpful in generating research hypotheses.
Rationale
Playing the trombone can lead to playing-related musculoskeletal disorders (PRMD). Previous research suggests that professional trombonists predominantly struggle with PRMD on the left body side. An increasing volume leads to an increasing contact pressure on the mouthpiece of the trombone, but it is still unclear how the muscle activity relates to this and whether it differs in musicians with PRMD from those without PRMD.
Purpose
The purpose of this study was to investigate the relationship between the activity of different muscles of the left body side, the contact pressure on the mouthpiece and the volume level in healthy trombonists.
Methods
Six male healthy tenor trombonists were included in this study and run through a standardized protocol which consisted of playing a b-flat major scale with three different volume levels (pianissimo, mezzoforte, fortissimo). Analyzed parameters were (1) the activity of several muscles of the left body side (measured with surface electromyography), (2) the contact pressure on the mouthpiece (measured with a force sensor) and (3) the different volume levels.
Results
Analysis of variance reveals significant differences of the muscle activity for the three volume levels. Depending on the volume level and the selected muscle, results show very weak to moderate correlations between contact pressure on the mouthpiece and muscle activity (Spearman´s rho between .11 and .58). The strongest correlation across all muscles occurs during fortissimo playing.
Conclusions
These results show a relationship in healthy trombonists between volume level, contact pressure on the mouthpiece and muscle activity when playing a b-flat major scale. Future research should include trombonists with PRMD to enable comparison between PRMD and non-PRMD musicians.

BACKGROUND/AIMS: Research in music medicine has reported incidence rates of musculoskeletal disorders of approx. 70% in instrumental musicians. String players have the highest risk, with rates of performance-related musculoskeletal disorders (PRMDs) of 65% to 88%. Playing the violin or viola requires complex neuromusculoskeletal skills, and the high frequency of repetitive movements, dynamic and static muscle load, awkward postures, poor technique, and practice time are factors causing musculoskeletal strain. In ergonomic terms, these disorders can be categorized based on extrinsic and intrinsic loads. Identification of intrinsic loads, such as muscle utilization and joint motion, is necessary to understand factors influencing musculoskeletal disorders associated with violin playing. The aim of this study was to review the literature on musculoskeletal demands in violin and viola playing.
METHODS: A literature search was conducted in the PubMed, COCHRANE, and CINAHL electronic databases from 1999 to 2015 using the search terms violin, viola, high strings, movement, posture, and synonyms. A manual search of Medical Problems of Performing Artists was also conducted. Additional references were identified by searching the citations and reference lists of all identified relevant studies. RESULTS: The results suggest that an asymmetric playing posture, the associated muscle activity, and joint mobility may contribute to musculoskeletal problems in violin and viola players. Evidence suggests an increased load of intrinsic factors in violin/viola performance.
CONCLUSION: The identification of intrinsic loads in violin and viola playing may facilitate the development of prevention strategies and interventions.
Hintergrund
Der Professionalisierungsprozess der deutschen Physiotherapie ist naturgemäß mit der Frage nach vermehrter beruflicher Autonomie verknüpft. Mit der berufspolitischen Diskussion um die Einführung des Patient Self-referral in Deutschland besteht auch die Forderung, die Kompetenzen deutscher Physiotherapeuten zu erweitern und an internationale Standards anzupassen.
Ziel
Entwicklung eines Analyseverfahrens zur Ermittlung des Bildungsbedarfs deutscher Physiotherapeuten in Vorbereitung auf die Anforderungen im Patient Self-referral.
Methode
Unter Verwendung verschiedener repräsentativer internationaler und nationaler Richtlinien zur physiotherapeutischen Ausbildung wurde ein spezifisches Verfahren zur Analyse des Bildungsbedarfs entwickelt und exemplarisch angewendet.
Ergebnisse
Das entwickelte Analyseverfahren deckte Bildungsdefizite in den Bereichen der Fach- und personalen Kompetenz auf. Der Hauptanteil identifizierter Bildungsdefizite besteht hinsichtlich der neuen Anforderungen wie Diagnosestellung oder Einleitung angemessener Überweisungsprozesse.
Schlussfolgerungen
Die Studie unterstreicht den Bedarf nach einheitlichen Ausbildungsstandards in der deutschen Physiotherapie und der Formulierung eines verbindlichen Kompetenzprofils. Das Analyseverfahren ist eine Möglichkeit zur Darstellung des Ist-Zustands deutscher Physiotherapeuten und liefert die Grundlage zur Vermittlung fehlender Kompetenzen, die sich an internationalen Standards orientieren.
rationale: Musculoskeletal problems are widespread in performing artists and are due to the special demands of instrument playing, singing or dancing. Additionally, various other factors might contribute to performance-related musculoskeletal problems. In order to provide a specific physiotherapeutic management for performing artists, it is important to gain information about the performing art, the individual (biomechanical) demands and contributing factors. The subjective examination is the basis of the clinical reasoning process and the hypothesis forming, which is important for goal setting in further clinical examination and biomechanical analysis. In the present protocol, the subjective examination consists of a questionnaire based section and an interview based section and is part of the evaluation process of the neuromusculoskeletal examination of a performing artist specific reference laboratory.
purpose: The aim of this study was to develop a standardized protocol for an interview based subjective examination of performing artists with musculoskeletal problems. The results of this section of the subjective examination will be combined with the results of the questionnaire based subjective examination , in order to gather as much relevant information as possible to specifically address the individual’s musculoskeletal health status.
methods: A common physiotherapeutic recording of findings which addresses the relevant questions of the subjective examination of musculoskeletal problems was used. In order to adapt the anamnesis to performing-related musculoskeletal problems, questions specific to instrument playing, singing and dancing were selected on the basis of a literature search and the expertise of the research group.
results: The protocol is based on the five aspects of clinical practice described by Maitland and the Musculoskeletal Clinical Translation Framework by Mitchell et al. (2017) .The performing art specific questions especially address (1) performing art specific considerations including style, genre, education and professional level, practice habits, repertoire, and external factors, e.g. concert conditions, (2) performing art specific physical and psychosocial contributing factors, and (3) performing art specific activities which trigger the musculoskeletal problems and help to form hypotheses for the following clinical examination. Individual tracks lead through the subjective examination so that irrelevant questions are skipped depending on the kind of musculoskeletal problem and the performing art practiced by the individual client.
conclusions: A pretest of this standardized anamnesis protocol is ongoing in a special physiotherapy clinic for performing artists since spring 2022. In an iterative approach, the protocol will be continuously improved based on patient feedback and clinical considerations.
OBJECTIVE: The aim of the study was to identify and systematically categorize musculoskeletal assessments used in quantitatively based studies about posture and movement in musicians playing high string instruments (violin, viola) in order to inform further research and help to identify gaps in knowledge.
METHODS: A systematic literature search was conducted in the databases Cochrane, CINAHL, and PubMed as well as the journal Medical Problems of Performing Artists. Additional studies were searched within the reference lists of relevant articles. Sixty-four studies conducted since 1999 until May 2017 were included according to predefined inclusion criteria.
RESULTS: Various biomechanical measurement instruments were used in 24 of the selected studies in order to collect two- and three-dimensional kinematic data: optoelectronic systems with active and passive markers, electromagnetic systems, ultrasonic motion analysis systems, and image-based analysis of posture. In 11 studies, kinetics were measured by optoelectronic systems, force sensors, and a posturographic device. Further studies used electromyography to evaluate muscle activity (19 studies) as well as instruments to measure range of motion (2 studies). Additionally, a broad range of clinical examination techniques (35 studies) and self-reports (36 studies) were used to assess the musculoskeletal system of high string players.
CONCLUSION: The identified assessments could be used both in further research and in practical work in order to evaluate posture and movement in high string players. For future research, qualitatively based studies as well as psychometrically tested quantitatively based self-report assessments are required.
Der N. medianus besitzt die Fähigkeit, bei Extremitätenbewegungen in seinem umliegenden Gewebe zu gleiten. Dieses wichtige physiologische Phänomen minimiert bei Engpasssyndromen Zug und Druck auf den Nerv. Das Karpaltunnelsyndrom (CTS) stellt in der Praxis die am häufigsten auftretende und stark behandlungsresistente periphere Neuropathie dar. Die verminderte Dynamik des N. medianus kann ein beitragender Faktor eines lange bestehenden CTS sein. Die vorliegende Querschnittstudie untersuchte die transversale Gleitfähigkeit des N. medianus mittels sonografischer Diagnostik an CTS-Patienten (Gruppe A). Die Nervenbewegung wurde hierfür durch passive Bewegungsmanöver an der HWS mittels einer Kontralateralflexion und einem Cervical lateral glide sowie durch aktive Bewegungen der Finger und der Schulter provoziert. Die Ergebnisse wurden im Anschluss mit den Resultaten einer früheren Untersuchung gleicher Art an gesunden Probanden (Gruppe B) verglichen. Die Bewegungsmanöver der oberen Extremität wurden zudem an gesunden Probanden durchgeführt (Gruppe B2). Das Ziel der Studie war es herauszufinden, ob die Manöver einen signifikanten Einfluss auf die Gleitfähigkeit sowie auf die Größe des Flächeninhalts des N. medianus ausüben und ob sich die Veränderungen in der CTS-Gruppe im Gegensatz zur Kontrollgruppe signifikant unterschiedlich darstellen. Sowohl distale Bewegungen der Fingergelenke als auch die zervikalen Bewegungsmanöver beeinflussten das transversale und longitudinale Gleiten des N. medianus. Am Unterarm zeigten sich deutliche Unterschiede in der Nervenbewegung zwischen den gesunden und den CTS-Probanden, was auf einen möglichen Einfluss des Cervical lateral glide auf die Neurodynamik des N. medianus bei CTS hindeutet.
Hintergrund
Eine gute Gleichgewichtsfähigkeit ist die wesentliche Voraussetzung, um im Alter mobil und unabhängig zu bleiben. Bedingt durch die zunehmende Lebenserwartung gewinnt der Erhalt der körperlichen Funktionstüchtigkeit von Senioren immer weiter an Bedeutung. Das Ziel dieser randomisierten kontrollierten Studie war es, zu untersuchen, ob sich durch ein 3-wöchiges BalanceSlack-Training die Gleichgewichtsfähigkeit von Menschen im Alter ab 60 Jahren verbessert.
Methode
An der Studie nahmen 34 Probanden im Alter zwischen 60–72 Jahren teil. Die Teilnehmer wurden in eine Interventionsgruppe (n=17) und eine Kontrollgruppe (n=17) randomisiert. Die Interventionsgruppe trainierte über einen Zeitraum von 3 Wochen 2-mal wöchentlich für je 20 min auf einem „BalanceSlack“, die Kontrollgruppe bekam kein Training. Die Datenerhebung erfolgte in einer Eingangs- und einer Abschlussmessung. Es kamen zwei klinische (Functional-reach-Test und Gleichgewichtstest nach Wydra) und zwei biomechanische (posturographische Messungen auf einer Kraftmessplatte und das System BalensoSenso) Assessments zur Evaluation der dynamischen und quasistatischen Gleichgewichtsfähigkeit zum Einsatz.
Ergebnisse
Es wurden höchst signifikante Verbesserungen der dynamischen Gleichgewichtsfähigkeit der am BalanceSlack-Training teilnehmenden Senioren nachgewiesen. Hinsichtlich der quasistatischen Gleichgewichtsfähigkeit zeigte sich, dass die Probanden der Interventionsgruppe ihre Leistung im statischen Teil des Gleichgewichtstests nach Wydra höchst signifikant steigern konnten und die Länge des Schwankungsweges im Nahstand auf der Kraftmessplatte nach der Intervention signifikant verringert wurde. Alle weiteren Messungen unter quasistatischen Bedingungen zeigten keine signifikanten Verbesserungen der Interventionsgruppe, allerdings waren Tendenzen eines Trainingseffekts erkennbar. Die Kontrollgruppe verbesserte sich in keinem Test signifikant.
Schlussfolgerung
Die Ergebnisse legen den Schluss nahe, dass BalanceSlack-Training geeignet ist, die Gleichgewichtsfähigkeit von alten Menschen innerhalb kurzer Zeit deutlich zu verbessern.
Sabine Krener ist enttäuscht. Sie bereitet sich auf einen Halbmarathon vor, hat aber zunehmend Probleme beim Laufen. Dass ihre Achillessehne schmerzt, kennt sie schon seit Jahren – nun wird es aber schlimmer, und es treten zudem ständig Krämpfe im rechten Bein auf. Zu allem Überfluss hat sie in den letzten sechs Wochen acht Kilo zugenommen.
Als Jane mit ihrer kleinen Schwester tobt, schießt ihr plötzlich ein starker Schmerz in Nacken und Kopf. So weit nicht ungewöhnlich und eine Indikation für Physiotherapie. Doch als die junge Studentin erzählt, dass der Kopfschmerz pulsierend ist, wird ihr Therapeut hellhörig und stellt die entscheidenden Fragen.
