The distribution of a species along a thermal gradient is commonly approximated by a unimodal response curve, with a characteristic single optimum near the tempera‐ture where a species is most likely to be found, and a decreasing probability of occur‐rence away from the optimum. We aimed at identifying thermal response curves (TRCs) of European freshwater species and evaluating the potential impact of climate warming across species, taxonomic groups, and latitude. We first applied generalized additive models using catchment‐scale global data on distribution ranges of 577 freshwater species native to Europe and four different temperature variables (the current annual mean air/water temperature and the maximum air/water temperature of the warmest month) to describe species TRCs. We then classified TRCs into one of eight curve types and identified spatial patterns in thermal responses. Finally, we in‐tegrated empirical TRCs and the projected geographic distribution of climate warm‐ing to evaluate the effect of rising temperatures on species’ distributions. For the different temperature variables, 390–463 of 577 species (67.6%–80.2%) were char‐acterized by a unimodal TRC. The number of species with a unimodal TRC decreased from central toward northern and southern Europe. Warming tolerance (WT = maxi‐mum temperature of occurrence—preferred temperature) was higher at higher lati‐tudes. Preferred temperature of many species is already exceeded. Rising temperatures will affect most Mediterranean species. We demonstrated that fresh‐water species’ occurrence probabilities are most frequently unimodal. The impact of the global climate warming on species distributions is species and latitude depend‐ent. Among the studied taxonomic groups, rising temperatures will be most detri‐mental to fish. Our findings support the efforts of catchment‐based freshwater management and conservation in the face of global warming.
Recognition of Emotional Facial Expressions and Alexithymia in Patients with Chronic Facial Pain

Objectives
Alexithymia, conceived as difficulties to identify emotions, is said to be related with several pain syndromes. This study examined the recognition of facially expressed emotions and its relation to alexithymia in subjects with chronic facial pain.
Methods
A total of 62 subjects were recruited, with n=20 patients with chronic facial pain and n=42 healthy controls. All subjects were tested for the recognition of facially expressed emotions (Facially Expressed Emotion Labelling Test (FEEL test). The Toronto Alexithymia Scale (TAS-26) was used for the diagnosis of alexithymia.
Results
Patients with chronic facial pain performed worse than controls at the FEEL task (p<.001) and showed higher total TAS scores (p<.001). This indicates the presence of alexithymia and facial emotion recognition deficits in the facial pain group.
Discussion
It was concluded from the results that both the recognition of facially expressed emotions, and the ability to identify and describe one’s own feelings (TAS), are restricted in chronic orofacial pain patients. This relationship is particularly important in the treatment of chronic facial pain, indicating that it should become part of the treatment in addition to the therapeutic key issues, to influence the quality of life of the affected patients positively.
Background
Osteoarthritis of the knee is the most common cause for disability and limited mobility in the elderly, with considerable individual suffering and high direct and indirect disease-related costs. Nonsurgical interventions such as exercise, enhanced physical activity, and self-management have shown beneficial effects for pain reduction, physical function, and quality of life (QoL), but access to these treatments may be limited. Therefore, home therapy is strongly recommended. However, adherence to these programs is low. Patients report lack of motivation, feedback, and personal interaction as the main barriers to home therapy adherence. To overcome these barriers, electronic health (eHealth) is seen as a promising opportunity. Although beneficial effects have been shown in the literature for other chronic diseases such as chronic pain, cardiovascular disease, and diabetes, a systematic literature review on the efficacy of eHealth interventions for patients with osteoarthritis of knee is missing so far.
Objective
The aim of this study was to compare the efficacy of eHealth-supported home exercise interventions with no or other interventions regarding pain, physical function, and health-related QoL in patients with osteoarthritis of the knee.
Methods
MEDLINE, CENTRAL, CINAHL, and PEDro were systematically searched using the keywords osteoarthritis knee, eHealth, and exercise. An inverse variance random-effects meta-analysis was carried out pooling standardized mean differences (SMDs) of individual studies. The Cochrane tool was used to assess risk of bias in individual studies, and the quality of evidence across studies was evaluated following the Grading of Recommendations, Assessment, Development, and Evaluation approach.
Results
The literature search yielded a total of 648 results. After screening of titles, abstracts, and full-texts, seven randomized controlled trials were included. Pooling the data of individual studies demonstrated beneficial short-term (pain SMD=−0.31, 95% CI −0.58 to −0.04, low quality; QoL SMD=0.24, 95% CI 0.05-0.43, moderate quality) and long-term effects (pain −0.30, 95% CI −0.07 to −0.53, moderate quality; physical function 0.41, 95% CI 0.17-0.64, high quality; and QoL SMD=0.27, 95% CI 0.06-0.47, high quality).
Conclusions
eHealth-supported exercise interventions resulted in less pain, improved physical function, and health-related QoL compared with no or other interventions; however, these improvements were small (SMD<0.5) and may not make a meaningful difference for individual patients. Low adherence is seen as one limiting factor of eHealth interventions. Future research should focus on participatory development of eHealth technology integrating evidence-based principles of exercise science and ways of increasing patient motivation and adherence.
Hintergrund
Mehr als die Hälfte aller schwangeren Frauen wird bereits zu einem frühen Zeitpunkt der Geburt in den Kreißsaal aufgenommen. Von Seiten der Frauen bestehen Unsicherheiten insbesondere darüber, ob die Geburt begonnen hat. Ist der Geburtsbeginn diagnostiziert und die Frau in den Kreißsaal aufgenommen, wird ein deutliches Voranschreiten der Geburt von Geburtshelferinnen und -helfern sowie den werdenden Eltern erwartet. Wissenschaftliche Evidenzen weisen auf einen Zusammenhang von früher Kreißsaalaufnahme und häufigeren Diagnosen von Geburtsdystokien und vermehrten Interventionen hin. In diesem Zusammen-hang wird darüber diskutiert, dass Frauen während der beginnenden Geburt möglicherweise nicht die optimale Betreuung erhalten. Die Forschenden fordern, Managemententscheidungen unter Berücksichtigung der Physiologie der Latenzphase und aktiven Geburtsphase aufeinander abzustimmen. Diese international übliche Unterteilung der ersten Geburtsphase erfährt in Deutschland eine zunehmende Aufmerksamkeit. Sie birgt die Chance, Frauen in der Latenzphase besser über den zu erwartenden Geburtsfortschritt aufzuklären, sie gezielter zu unterstützen und die Rate der diagnostizierten Geburtsdystokien zu reduzieren.
Zielsetzung
Der Einsatz eines selbstentwickelten Dokumentenformulars – Dokumentation Latenzphase – zur Diagnose des Geburtsbeginns und der Latenzphase und zur Unterstützung der klinischen Entscheidungsfindung von Hebammen1 in einem Hebammenkreißsaal, sollte das professionelle, abwartende Verhalten von Hebammen während der frühen Phase der Geburt fördern. Es wurde erwartet, dass die Berücksichtigung der Latenzphase und des damit verbundenen langsamen Geburtsfortschritts im Betreuungsprozess dazu führt, dass weniger Frauen während der Geburt vom Hebammenkreißsaal in den üblichen Kreißsaal weitergeleitet werden müssen. Zudem wurde eine nominelle Verlängerung der Dauer der dokumentierten Eröffnungsperiode erwartet.
Methodik
Die Wirkung der Dokumentation Latenzphase wurde in einer pragmatischen quasi-experimentellen Studie getestet. Hebammen des Hebammenkreißsaals der Interventionsklinik wurden zu Inhalt und Anwendung der Dokumentation Latenzphase geschult. In der Kontrollklinik wurden keine Änderungen an der Dokumentation der Betreuung vorgenommen, die Hebammen erhielten keine Schulung zur Latenz-phase und aktiven Geburtsphase. Für den Basisdatensatz wurden in der Interventionsklinik n = 137 Geburten vor Einführung der Intervention retrospektiv aus den Patientenakten erhoben. Im Interventionszeitraum wurden n = 147 Geburten dokumentiert. Zum Vergleich der Ergebnisse wurden insgesamt n = 183 Datensätze aus einer Kontrollklinik mit Hebammenkreißsaal herangezogen. Der Effekt wurde gemessen an der Häufigkeit, der vom Hebammenkreißsaal in den üblichen Kreiß-saal weitergeleiteten Geburten und der Dauer der dokumentierten Eröffnungsperiode. Weitere Outcomes betrafen die Anwendung von Oxytocin und Amniotomie und einer Schmerzbehandlung sowie von speziellen Betreuungsmaßnahmen von Hebammen. Explorative Analysen bezogen sich auf die Anwendung von medizinischen Maßnahmen in Abhängigkeit von der Geburtsphase (Latenzphase/aktive Geburtsphase) bei stationärer Aufnahme in der Interventionsklinik.
Unterschiede wurden mithilfe von Chi2-Tests und Ereignisfunktionen ermittelt. Mögliche Einflüsse auf die Weiterleitung und Eröffnungsdauer wurden mit logistischen und Cox-Regressionen analysiert.
Ergebnisse
In beiden Kliniken werden keine signifikanten Veränderungen der Weiterleitung in den üblichen Kreißsaal vom Basis- zum Interventionszeitraum beobachtet. Entgegen dem Trend zu mehr Weiterleitungen von Erstgebärenden in der Kontrollklinik, blieb die Rate in der Interventionsklinik stabil. Die dokumentierte Dauer der Eröffnungsperiode stieg in der Interventionsklinik signifikant von durchschnittlich 6,00 auf 7,43 Stunden (p = 0,006). In der Kontrollklinik war keine Veränderungen nachzuweisen (5,19 vs. 4,89 Std.).
Schlussfolgerungen
Im Vergleich zur ansteigenden Weiterleitungsquote von Erstgebärenden in der Kontrollklinik und auf Basis der generell niedrigeren Weiterleitungsquote in der Interventionsklinik, ist die stabile Rate für Erstgebärende als Hinweis auf eine differenziertere Weiterleitungspraxis und ein stärker ausgeprägtes abwartendes Verhalten in der Interventionsklinik zu deuten. Die Verlängerung der Eröffnungsperiode zeigt auf, dass die Latenzphase über die Dokumentation Latenzphase erfolgreich in die Geburtszeit integriert wurde und Berücksichtigung fand.
Das Informationsmanagement steht im Zentrum erfolgreicher eHealth-Innovationsprozesse von Krankenhäusern. Im Kontext komplexer, zum Teil tradierter Krankenhausstrukturen kann die Gestaltungsfähigkeit des Informationsmanagements durch eine ausgeprägte Intrapreneurship-Kultur erhöht werden, wovon vermutlich auch der Digitalisierungsgrad der Einrichtungen profitiert. Vor diesem Hintergrund verfolgte die vorliegende Studie zwei Forschungsfragen: (1.) Welche Effekte hat Intrapreneurship auf den Digitalisierungsgrad der Krankenhäuser und (2.) inwiefern werden diese Effekte durch das Informationsmanagement beeinflusst? Zur Beantwortung der Forschungsfragen wurde ein konzeptionelles Untersuchungsmodell entwickelt, welches mit Daten von 224 IT-Leitern evaluiert wurde. Die Ergebnisse bestätigen, dass Intrapreneurship die Umsetzung von eHealth-Anwendungen positiv beeinflussen kann. Die identifizierten Effekte waren jedoch vorwiegend indirekter Art, vermittelt durch den Professionalisierungsgrad des Informationsmanagements. So kann Intrapreneurship auf IT-Leiter-Ebene und auf Ebene der Gesamtorganisation zu einer Professionalisierung des strategischen Informationsmanagements führen. Auf Ebene der IT-Abteilung profitiert vor allem das operative Informationsmanagement von einer ausgeprägten Intrapreneurship-Kultur.
Die Verbreitung von Informationstechnologien (IT) im Gesundheitswesen sowie deren Einflussgrößen sind Betrachtungsobjekt der Adoptions- und Diffusionsforschung. Neues Wissen aus diesen Studien wird dabei häufig als summative Umfrageergebnisse disseminiert. Mit dem in diesem Beitrag vorgestellten Web-Portal werden die individuellen Umfrageergebnisse im Vergleich zu einer Referenzgruppe präsentiert. Das erfolgt in flexibler Form unter Verwendung von reliablen und validen Kennzahlen der IT-Prozessunterstützung, die in einer hierarchischen Struktur angeordnet sind. Es werden die Entwicklung des Web-Portals als Benchmarking Instrument, seine Anwendung und eine initiale Evaluation vorgestellt. Es zeigte sich, dass das Web-Portal anhand aktueller Benchmarking-Ergebnisse von 197 Krankenhäusern einsetzbar ist, seine Anwendung als nützlich und die Indikatoren als verständlich eingeschätzt werden.
Background: While health informatics recommendations on competencies and education serve as highly desirable corridors for designing curricula and courses, they cannot show how the content should be situated in a specific and local context. Therefore, global and local perspectives need to be reconciled in a common framework.
Objectives: The primary aim of this study is therefore to empirically define and validate a framework of globally accepted core competency areas in health informatics and to enrich this framework with exemplar information derived from local educational settings.
Methods: To this end, (i) a survey was deployed and yielded insights from 43 nursing experts from 21 countries worldwide to measure the relevance of the core competency areas, (ii) a workshop at the International Nursing Informatics Conference (NI2016) held in June 2016 to provide information about the validation and clustering of these areas and (iii) exemplar case studies were compiled to match these findings with the practice. The survey was designed based on a comprehensive compilation of competencies from the international literature in medical and health informatics.
Results: The resulting recommendation framework consists of 24 core competency areas in health informatics defined for five major nursing roles. These areas were clustered in the domains “data, information, knowledge”, “information exchange and information sharing”, “ethical and legal issues”, “systems life cycle management”, “management” and “biostatistics and medical technology”, all of which showed high reliability values. The core competency areas were ranked by relevance and validated by a different group of experts. Exemplar case studies from Brazil, Germany, New Zealand, Taiwan/China, United Kingdom (Scotland) and the United States of America expanded on the competencies described in the core competency areas.
Conclusions: This international recommendation framework for competencies in health informatics directed at nurses provides a grid of knowledge for teachers and learner alike that is instantiated with knowledge about informatics competencies, professional roles, priorities and practical, local experience. It also provides a methodology for developing frameworks for other professions/disciplines. Finally, this framework lays the foundation of cross-country learning in health informatics education for nurses and other health professionals.
Current frameworks postulate the success of health IT innovations to be determined by the professionalism of the information management (PIM). Still, empirical knowledge about PIM is scarce up until today. This study seeks to answer three research questions: (1.) How can PIM be measured in a reliable and valid way, (2.) how pronounced is PIM in German hospitals and (3.) do hospital characteristics have an impact on the degree of PIM? Based on the results of an expert workshop and frameworks for information management (IM) items for a PIM inventory were developed and the inventory sent to 1349 chief information officers of German hospitals. A principle component analysis based on the responses of 196 hospitals confirmed the three components that had been proposed by the frameworks: the strategic, the tactical and the operational level. The full inventory implied satisfying reliability and allowed a PIM composite-score to be calculated. The PIM scores for strategic and tactical IM were found to be far lower than for operational IM which hints at strong deficits in these areas. A stepwise regression model indicated that the degree of PIM significantly increased with the size of the hospital, which had been expected and hints the validity of the PIM inventory. This tool offers potentials for hospitals to classify and improve their IM.
Going Mobile : An Empirical Model for Explaining Successful Information Logistics in Ward Rounds

Background: Medical ward rounds are critical focal points of inpatient care that call for uniquely flexible solutions to provide clinical information at the bedside. While this fact is undoubted, adoption rates of mobile IT solutions remain rather low.
Objectives: Our goal was to investigate if and how mobile IT solutions influence successful information provision at the bedside, i.e. clinical information logistics, as well as to shed light at socio-organizational factors that facilitate adoption rates from a user-centered perspective.
Methods: Survey data were collected from 373 medical and nursing directors of German, Austrian and Swiss hospitals and analyzed using variance-based Structural Equation Modelling (SEM).
Results: The adoption of mobile IT solutions explains large portions of clinical information logistics and is in itself associated with an organizational culture of innovation and end user participation.
Conclusion: Results should encourage decision makers to understand mobility as a core constituent of information logistics and thus to promote close end-user participation as well as to work towards building a culture of innovation.
The workflow-oriented dissemination of electronic patient data is a central goal of IT deployment in hospitals. Against this background, the present study examines two research questions: (1.) Are there differences in the availability of electronic patient data (AEPD) between different clinical workflows and data types and (2.) which structural and organizational factors determine AEPD? Based on a Germany wide hospital survey, AEPD was assessed along six clinical workflows. While AEPD was lowest for ward rounds, discharge showed the highest AEPD with pre- and post-surgery processes ranging in between. With regard to the data types analyzed, patient demographics and observation findings obtained the highest AEPD scores. Electrophysiological results, checklists and warnings were less common electronically and received lower AEPD scores. Multiple linear regression analysis resulted in a significant model that explained 34.4% of the variance of AEPD. Large hospitals and those with a professional information management, a high health IT related innovation culture and a nursing informatics officer possess higher AEPD scores and thus have better clinical information logistics mechanisms at their command.
Health IT systems are employed to support continuity of care via information continuity, while management continuity is often neglected. This study aims at investigating issues of management continuity when developing a collaborative decision support system for chronic wounds. Thirty-three experts from a variety of professions and disciplines discussed problems and possible solutions in four workshops. The following topics emerged from the discussion: existing networks involving payers, responsibilities as well as good discharge management. These topics clearly address management continuity and are also relevant for the scenario of inter-professional wound care across different settings.
The establishment of successful clinical information logistics (CIL) within the care processes is one of the main objectives of strategic health IT management in hospitals. While technical realisations in terms of useful, usable and interoperable IT solutions are essential precursors of CIL, there is limited empirical research on what socio-organisational factors underlie an innovation-friendly culture and how they can affect successful information provision. We applied factor analysis on survey data from 403 clinical directors from Germany, Austria and Switzerland and used the dimensions identified to explain the level of CIL with ordered logistic regression analysis. The intensity of collaboration and exchange with the IT department as well as the degree of executive IT leadership showed to be strongly associated with better CIL while personal views and attitudes of clinical directors were not. Analysing country differences revealed the degree of the exchange with the IT department to be significantly lower in German hospitals. This points at a potential strategic lever for German hospital executives to focus on.
This prospective longitudinal study aims at better understanding eHealth success factors in different European nations, esp. the role of eHealth-legislation in Switzerland and Germany. Qualitative interviews with 39 matched experts from a large variety of institutions in both nations were conducted. The individual statements in the interviews and the overall satisfaction rating indicate a clear trend for a more optimistic attitude towards the law in Switzerland than in Germany. This result is not surprising given the history of a telematics infrastructure in Germany. Cross-country learning topics for German politicians are the inclusion of the inpatient sector and the focus on one major application. In a next step, interview results from Austria will be included and with that the scope of study findings enriched.
Frequent users of emergency departments (ED) pose a significant challenge to hospital emergency services. Despite a wealth of studies in this field, it is hardly understood, what medical conditions lead to frequent attendance. We examine (1) what ambulatory care sensitive conditions (ACSC) are linked to frequent use, (2) how frequent users can be clustered into subgroups with respect to their diagnoses, acuity and admittance, and (3) whether frequent use is related to higher acuity or admission rate. We identified several ACSC that highly increase the risk for heavy ED use, extracted four major diagnose subgroups and found no significant effect neither for acuity nor admission rate. Our study indicates that especially patients in need of (nursing) care form subgroups of frequent users, which implies that quality of care services might be crucial for tackling frequent use. Hospitals are advised to regularly analyze their ED data in the EHR to better align resources.
As health IT supports processes along the entire patient trajectory and involves different types of professional groups, eHealth is inter-professional by nature. The aim of this study, therefore, is to investigate which competencies are at the intersection of the individual groups of health professionals. 718 international experts provided relevance ratings of eHealth competencies for different professional roles in an online survey. Communication and leadership proved to be important competencies across all professions, not only for executives. None or very little differences between professions were found between physicians and nurses, between IT experts at different levels and between IT experts and executives. However, there were a number of competencies rated differently when contrasting direct patient care specialists with executives. These findings should encourage organisations issuing educational recommendations to specify areas of shared competencies more extensively.
Elektronisch unterstützte transsektorale Kommunikation im Gesundheitswesen ist eine der essentiellen Säulen von eHealth. Sie ist eine menschliche Handlung, die eine Verbesserung der Versorgung Einzelner und ganzer Bevölkerungsgruppen bewirken soll. Ethik bewertet menschliches Handeln in Bezug auf dessen Auswirkungen und die ihm zugrunde liegenden Werte und Normen. Dabei werden die Auswirkungen auf Individuen und Allgemeinheiten betrachtet. Im Gesundheits- und Sozialwesen gelten die Prinzipien der Autonomie, der Schadensverhütung, der Fürsorge und der Gerechtigkeit als Maßstäbe. Es gilt also die Fragen herauszuarbeiten, die an elektronische transsektorale Kommunikation aus ethischer Sicht gestellt werden müssen, um zu untersuchen, ob sie innerhalb der genannten Prinzipien ethischen Anforderungen genügt.
Aus den Ergebnissen einer systematischen Literaturrecherche wurden zunächst allgemein Aussagen zum Thema Information und Technologie im Zusammenhang mit Ethik extrahiert, und daraufhin geprüft, auf welche Fragen sie Antworten anbieten. Diese wurden innerhalb der genannten fünf Prinzipien als Fragen an elektronische transsektorale Kommunikation formuliert.
Aus den Aussagen der Literatur ließen sich sieben Fragen ableiten und den ethischen Prinzipien zuordnen, um mit ihnen elektronische transsektorale Kommunikation zu untersuchen. Auf diese Weise kann geprüft werden ob diese in der Lage sind, das Wohl Einzelner wie auch von Gemeinschaften im Gesundheitswesen zu fördern, wovon Betroffene, Professionelle und das Gesundheitssystem insgesamt profitieren könnten.
Die ambulante Wochenbettbetreuung. Eine qualitative Studie zum Professionellen Handeln von Hebammen

Die aufsuchende Versorgung in der Zeit nach der Geburt eines Kindes ist eine wichtige gesundheitliche Dienstleistung in einer Übergangsphase im Leben von jungen Familien. Im Mittel-punkt der vorliegenden Arbeit steht das professionelle Handeln der in der ambulanten Wochen-bettbetreuung tätigen Hebammen. Aus der Perspektive der Hebammen konnten Handlungsansätze und Entscheidungsmaxime vor dem Hintergrund veränderter Anforderungen in der Berufspraxis dargestellt werden. Ziel der vorliegenden Studie war es, einen theoretischen Beitrag für die professionelle Gestaltung der Wochenbettbetreuung zu geben und die Bedeutung dieses komplexen Handlungsfeldes herauszustellen.
Datengrundlage dieser qualitativen Studie sind offene Leitfadeninterviews mit 28 Hebammen, die über mindestens zwei Jahre Berufserfahrung verfügten und in unterschiedlichen Betreuungssettings sowie zu unterschiedlichen strukturellen Rahmenbedingungen arbeiteten. Die Datenerhebung erfolgte in zwei Erhebungsrunden von April 2013 bis Januar 2015. Die Daten wurden inhaltsanalytisch und rekonstruktiv hermeneutisch analysiert.
Es zeigt sich ein Zusammenspiel von handlungsleitenden Sichtweisen, Handlungsgrundlagen und professionellen Handlungsdeterminanten. Neben konkreten Betreuungskonzepten konnten mit den vorliegenden Ergebnissen verschiedene Handlungsdeterminanten, wie die gesundheitliche Versorgung von Mutter und Kind, die Beziehungsgestaltung zwischen Frau und Hebamme, der Einbezug familialer Unterstützungsleistungen und verschiedene strukturelle Handlungsdeterminanten analysiert werden. Grundelemente der Hebammenarbeit sind die Begleitung und Beobachtung physiologischer mütterlicher Umstellungs- sowie kindlicher Entwicklungsprozesse, aus denen sich sowohl diagnostische, pflegerisch-therapeutische, praktisch-anleitende und beratende Handlungsformen als auch Handlungsformen mit psychosozialer Ausrichtung ableiten. Die Ergebnisse verweisen auf ein heterogenes Praxisfeld, welches einer breiten Expertise der Hebammen bedarf. Die professionelle Gestaltung Hebammenkundlicher Handlungsprozesse sollte im Sinne einer Praxis erfolgen, die Wissen, Können und Reflexion bewusst verbindet und die Autonomie der Lebenspraxis der Frauen und Familien respektiert. Ein genau definierter Versorgungsauftrag sowie evidenzbasierte Leitlinien und Expertenstandards könnten die Hebammen unterstützen, ihre Ressourcen entsprechend einer bestmöglichen postnatalen Versorgung von Frauen und ihren Familien zu nutzen.
Background
Forced migration significantly endangers health. Women face numerous health risks, including sexual violence, lack of contraception, sexually transmitted disease, and adverse perinatal outcomes. Therefore, sexual and reproductive healthcare is a significant aspect of women asylum seekers’ health.
Even when healthcare costs of asylum seekers are covered by the government, there may be strong barriers to healthcare access and specific needs may be addressed inadequately. The study’s objectives were a) to assess the accommodation and healthcare services provided to women asylum seekers in standard and specialised health care, b) to assess the organisation of healthcare provision and how it addresses the sexual and reproductive healthcare needs of women asylum seekers.
Methods
The study utilised a multi-method approach, comprising a less-dominant quantitative component and dominant qualitative component. The quantitative component assessed accommodation conditions for women in eight asylum centres using a survey. The qualitative component assessed healthcare provision on-site, using semi-structured interviews with health and social care professionals (n = 9). Asylum centres were selected to cover a wide range of characteristics. Interview analysis was guided by thematic analysis.
Results
The accommodation in the asylum centres provided gender-separate rooms and sanitary infrastructure. Two models of healthcare were identified, which differed in the services they provided and in their organisation: 1) a standard healthcare model characterised by a lack of coordination between healthcare providers, unavailability of essential services such as interpreters, and fragmented healthcare, and 2) a specialised healthcare model specifically tailored to the needs of asylum-seekers. Its organisation is characterised by a network of closely collaborating health professionals. It provided essential services not present in the standard model. We recommend the specialised healthcare model as a guideline for best practise.
Conclusions
The standard, non-specialised healthcare model used in some regions in Switzerland does not fully meet the healthcare needs of women asylum seekers. Specialised healthcare services used in other regions, which include translation services as well as gender and culturally sensitive care, are better suited to address these needs. More widespread use of this model would contribute significantly toward protecting the sexual and reproductive integrity and health of women asylum seekers.
