Das Buch stellt ein Grundlagenwerk dar, in dem wissenschaftlich basierte und in der Praxis bewährte Methoden und Instrumente zur kontinuierlichen und systematischen Qualitätsentwicklung in der Pflege vorgestellt und diskutiert werden. Die Autoren stützen sich in ihren Beiträgen auf langjährige Forschungs- und Entwicklungsarbeit, die im Rahmen des deutschen Netzwerks für Qualitätssicherung in der Pflege (DNQP) im Dialog mit Kooperationspartnern auf nationaler und internationaler Ebene geleistet wurde. Der Schwerpunkt liegt dabei auf den Expertenstandards in der Pflege.
Das Buch ist ein Grundlagenwerk, in dem innovative Konzepte sowie wissenschaftlich basierte und in der Praxis bewährte Methoden und Instrumente zur kontinuierlichen und systematischen Qualitätsentwicklung in der Pflege vorgestellt und diskutiert werden. Der inhaltliche Schwerpunkt liegt dabei auf den Expertenstandards des DNQP - als einem wichtigen Motor für die Förderung der Pflegequalität in Krankenhäusern, ambulanten und stationären Pflegeeinrichtungen im gesamten deutschsprachigen Raum. Ein wesentlicher Teil der Beiträge stützt sich auf die langjährige Forschungs- und Entwicklungsarbeit des DNQP zu dieser Thematik. An der 2. Auflage haben 14 namhafte Fachexperten aus Pflegewissenschaft und -praxis, Gesundheitsökonomie, Sozialrecht und Verbraucherschutz mitgewirkt.
Qualität in der häuslichen Pflege : Ansätze zu einer lebensweltorientierten Weiterentwicklung

Die Entwicklung und Sicherstellung pflegerischer Qualität wird nicht erst seit der Einführung der Pflegeversicherung intensiv diskutiert. Im Zuge dieser Auseinandersetzung zeigt sich, dass insbesondere in der ambulanten Versorgung unterschiedliche Ansichten darüber bestehen, wie Pflegequalität zu definieren, zu entwickeln und zu sichern ist. Dieser Beitrag stellt daher zentrale Merkmale sowie Rahmenbedingungen ambulanter Pflege dar, zeigt auf, welche unterschiedlichen Formen der Pflege und Unterstützung in der häuslichen Versorgung zum Tragen kommen und wie die Diskussion um die Qualität der ambulanten Pflege durch die unterschiedlichen Perspektiven der beteiligten Akteure bestimmt wird. Daran anschließend werden Ansatzpunkte für eine weitere Diskussion um die Qualität in der ambulanten Pflege aufgezeigt.
Background: For more than 30 years, there has been close cooperation between Japanese and German scientists with regard to information systems in health care. Collaboration has been formalized by an agreement between the respective scientific associations. Following this agreement, two joint workshops took place to explore the similarities and differences of electronic health record systems (EHRS) against the background of the two national healthcare systems that share many commonalities.
Objectives: To establish a framework and requirements for the quality of EHRS that may also serve as a basis for comparing different EHRS.
Methods: Donabedian's three dimensions of quality of medical care were adapted to the outcome, process, and structural quality of EHRS and their management. These quality dimensions were proposed before the first workshop of EHRS experts and enriched during the discussions.
Results: The Quality Requirements Framework of EHRS (QRF-EHRS) was defined and complemented by requirements for high quality EHRS. The framework integrates three quality dimensions (outcome, process, and structural quality), three layers of information systems (processes and data, applications, and physical tools) and three dimensions of information management (strategic, tactical, and operational information management).
Conclusions: Describing and comparing the quality of EHRS is in fact a multidimensional problem as given by the QRF-EHRS framework. This framework will be utilized to compare Japanese and German EHRS, notably those that were presented at the second workshop.
Long Range Wide Area Network (LoRaWAN) operates in the ISM band with 868 MHz, where the Time on Air (ToA) is regulated in the EU to 1 %. LoRaWAN nodes use the Adaptive Data Rate (ADR) algorithm to adapt their data rates during operation. The standard ADR algorithm works well with stationary nodes, however is very slow in the adaptation for mobile nodes. This paper introduces a new ADR algorithm for LoRaWAN that is supported by higher level meta-data for sensor streams, namely Quality of Information (QoI). With the help of QoI it is possible to provide additional information to the new ADR algorithm, reducing the convergence time and thus improving the Packet Delivery Ratio (PDR) of data from mobile sensor nodes. The new algorithm requires only modifications on network server side and keeps backwards compatibility with LoRaWAN nodes. Results show a significant better PDR compared to the standard ADR in scenarios with a limited number of mobile nodes.
Introduction Postoperative delirium (POD) is seen in approximately 15% of elderly patients and is related to poorer outcomes. In 2017, the Federal Joint Committee (Gemeinsamer Bundesausschuss) introduced a ‘quality contract’ (QC) as a new instrument to improve healthcare in Germany. One of the four areas for improvement of in-patient care is the ‘Prevention of POD in the care of elderly patients’ (QC-POD), as a means to reduce the risk of developing POD and its complications.
The Institute for Quality Assurance and Transparency in Health Care identified gaps in the in-patient care of elderly patients related to the prevention, screening and treatment of POD, as required by consensus-based and evidence-based delirium guidelines. This paper introduces the QC-POD protocol, which aims to implement these guidelines into the clinical routine. There is an urgent need for well-structured, standardised and interdisciplinary pathways that enable the reliable screening and treatment of POD. Along with effective preventive measures, these concepts have a considerable potential to improve the care of elderly patients.
Methods and analysis The QC-POD study is a non-randomised, pre–post, monocentric, prospective trial with an interventional concept following a baseline control period. The QC-POD trial was initiated on 1 April 2020 between Charité-Universitätsmedizin Berlin and the German health insurance company BARMER and will end on 30 June 2023. Inclusion criteria: patients 70 years of age or older that are scheduled for a surgical procedure requiring anaesthesia and insurance with the QC partner (BARMER). Exclusion criteria included patients with a language barrier, moribund patients and those unwilling or unable to provide informed consent. The QC-POD protocol provides perioperative intervention at least two times per day, with delirium screening and non-pharmacological preventive measures.
Ethics and dissemination This protocol was approved by the ethics committee of the Charité-Universitätsmedizin, Berlin, Germany (EA1/054/20). The results will be published in a peer-reviewed scientific journal and presented at national and international conferences.
Im Mittelpunkt des vorliegenden Beitrags stehen thematische Schwerpunkte, methodische Zugänge und Entwicklungspotentiale
qualitativer Wirkungsforschung, die sich im Kontext einer experimentell-naturwissenschaftlich dominierten Interventionsforschung
entwickelt. Eine Analyse ausgewählter aktueller Studien zur Wirkung physio-, ergo- und sprachtherapeutischer sowie
pflegerischer Interventionen der Schlaganfallrehabilitation verdeutlicht die Dominanz des Studienformats einer auflistenden
qualitativen Inhaltszusammenfassung. Qualitative Interventionsstudien dieses Formats werden weitgehend ohne Bezugnahme
auf etablierte qualitative Forschungstraditionen realisiert. Sie sind durch eine fallübergreifende Auflistung von Interviewaussagen
zu ausgewählten Themenbereichen gekennzeichnet. Basierend auf in den Pflegewissenschaften entstandenen Typologien der
Ergebnisse qualitativer Studien, werden Potentiale zur Weiterentwicklung der Komplexität und interpretativen Erklärungskraft
qualitativer Interventionsforschung vorgestellt. Ihre methodologische Fundierung und Qualitätssicherung werden als zentrale
Entwicklungsaufgaben diskutiert.

Um eine an Teilhabe und Selbstbestimmung orientierte Sprachtherapie planen, gestalten
und bewerten zu können, sind vielfältige und detaillierte Informationen über die
Teilhabesituation, die Teilhabeziele und das individuelle Erleben eines Menschen mit
Aphasie grundlegend. Eine therapierelevante und veränderungsorientierte Diagnostik
braucht methodische Zugänge, die der Individualität, Kontextbezogenheit und Subjektivität
menschlicher Teilhabe gerecht werden können. Nach Auffassung der Autorinnen
bietet die qualitative Sozialforschung der Sprachtherapie hierfür ein viel versprechendes
methodisches Repertoire. Der vorliegende Beitrag skizziert grundlegende Kennzeichen
dieses Forschungsstils, der Sprachtherapeutinnen und -therapeuten oftmals
wenig vertraut ist. Ausgewählte Ansätze einer qualitativen teilhabeorientierten Aphasie-
Diagnostik werden beispielhaft vorgestellt.

Stephan Maykus zeigt in seinem Artikel auf, was Integrierte Planung alles beinhalten kann, wenn sie in der Kommune konsequent umgesetzt wird. So könnte eine kommunale Struktur entstehen, die es ermöglicht, dass Bildung vor Ort geklärt ist, institutionelle Konzept- und Organisationsentwicklung mit Vernetzung verbindet und die dem datenbasierten kommunalen Bildungsmanagement ein Ziel gibt: eine pädagogische Entwicklung der Kommune, um Jugend zu ermöglichen.
The impact of pulsed light (PL) treatment on naturally occurring microorganisms, mycotoxins, and on physicochemical properties in red pepper powder was investigated. Powder samples were exposed to different PL treatments up to 61 pulses, with fluence ranging from 1.0 to 9.1 J/cm2. The highest fluence applied (9.1 J/cm2, 61 pulses, 20 s) resulted in 2.7, 3.1, and 4.1 log CFU/g reduction of yeasts, molds, and total plate counts (TPC), where initial microbial loads were 4.6, 5.5, and 6.5 log CFU/g, respectively. At the same fluence intensity, a maximum reduction of 67.2, 50.9, and 36.9% of aflatoxin B1 (AFB1), total aflatoxins (AF), and ochratoxin A (OTA) were detected, respectively. Proportional increase in temperature of the samples was observed from the absorbed PL energy, reaching maximum of 59.8°C. The inactivation of investigated microorganisms and mycotoxins followed first-order kinetics (R2 > 0.95). The fluence intensity at 6.9 and 9.1 J/cm2 did not cause degradation, but rather a significant (p < .05) and apparent increase of total phenols. Total color difference (ΔE*) revealed only “slight differences,” compared to the untreated sample. In conclusion, higher reduction of microbial load and mycotoxins in red pepper powder could be achieved, when higher treatment intensity was applied. This suggests the PL as a potential technology for decontamination of red pepper powder and other spice powders.
Among all nonthermal food processing technologies, high intensity pulsed electric fields (PEF) is one of the most appealing due to its short treatment times and reduced heating effects. The kiwifruit processing industry is focused on product yield maximization and keeping energy costs and waste effluents to a minimum while maintaining high product quality. In our study, pulsed electric field (PEF) pretreatment enhanced kiwifruit processing to facilitate peelability and specific peeling process and enhanced valorization of kiwifruit waste. PEF optimization was applied to obtain the best treatment parameters. A 32 factorial design of response surface methodology was applied to find the effect of time elapsed after PEF treatment and the PEF-specific energy input on specific peeling force and kiwifruit firmness as response criteria. Under the optimized condition, the specific peeling force decreased by 100, and peelability increased by 2 times. The phenolic content and antioxidant capacity of PEF-treated kiwifruit bagasse were 5.1% and 260% richer than the control sample. Overall, the optimized PEF pretreatments incorporated into kiwifruit processing led to decreased energy demand and increased productivity.
In this study the effect of PEF pre-treatment on the microstructure of freeze-dried strawberry dices was investigated. The PEF treatment has been performed at an electric field intensity of 1.07 kV/cm and a specific energy input of 1 kJ/kg. The samples were freeze-dried at a temperature of 45 °C and a pressure of 1 mbar. The microstructure of dried material was evaluated by different physical and optical methods, such as SEM, μ-CT and thermogravimetry. Moreover, mechanical and acoustic properties as well as the colour of processed material have been analyzed. PEF pre-treated strawberry dices showed a more uniform shape, a better retention of volume and a visual better quality compared to untreated ones. Moreover, PEF pre-treatment led to a more homogeneous distribution and a greater thickness of pores. In accordance, analysis of textural properties evidenced that PEF treated freeze-dried strawberry dices were crispier than untreated ones. Measurement of L*a*b*-values showed that PEF treated material was characterized by a more preserved colour after freeze-drying than untreated ones.
