Körperhaltung und Muskelspannung beeinflussen den Klang der Stimme. Aber gibt es auch einen Zusammenhang zwischen der motorischen Kontrolle der Nacken-, Gesichts- und Kieferregion und der Stimme? Die Pilotstudie mit 12 Sängerinnen ging dieser Frage nach und zeigt: Es ist sinnvoll, die motorische Kontrolle zu testen, wenn Patient*innen mit Stimmproblemen zur Physiotherapie kommen.
Methods: Systematic review of randomized controlled trials (RCT). Searches were conducted in five electronic databases. Studies were selected if they included patients with NP over 18 years old treated with aerobic exercise (AE) (e.g., cycling, running, hiking, and walking). The main outcome of interest was pain intensity. Qualitative and quantitative data were extracted. The risk of bias (RoB) was determined using the Cochrane RoB Tool-2 and the overall certainty of the evidence with the GRADE recommendations.
Results: Out of 21,585 initial records screened, a total of six individual studies published in ten manuscripts were included. There was a great heterogeneity between protocols, comparisons, and studies’ results (different magnitudes and directions). When looking at the effect of aerobic exercise versus control groups or other interventions on pain intensity measured with the VAS, not statistically (nor clinical) significant differences between aerobic exercise and control groups (MD [95%CI] 5.16 mm [-6.38, 16.70]) were identified. The combined effect of AE plus other interventions seems to be effective. Strength exercise obtained better effects than aerobic exercises (MD [95%CI]: -11.34 mm [-21.6, -1.09]).
Conclusions: Aerobic exercise presented positive results to reduce pain intensity, and improving disability, and physical and emotional functioning. However, the evidence is restricted, low quality, and heterogeneous.
Methods: The searches were conducted on five electronic databases. RCTs or CTs with patients over 18 years old of both sexes with OFP diagnoses were targeted. The intervention of interest was AE (i.e., walking, cycling, and running), compared to any other conservative and non-conservative therapy. The primary outcome was pain intensity. Risk of bias (RoB) was done with the Cochrane RoB tool (RoB 2). The overall certainty of the evidence was evaluated with GRADE.
Results: Out of 21,585 initial records found in the initial database search, only one study (reported on three manuscripts) was included. The diagnosis of interest was headache plus temporomandibular disorders (TMD). Three treatment groups (strengthening (Str) exercise + manual therapy (MT) (G1); AE + MT + Str exercises (G2); AE (G3)) were compared. The main outcome was pain; the secondary outcomes included disability, strength, anxiety, and quality of life. The combined treatment (AE+MT+Str exercises) had the strongest effect to decrease pain and headache intensity in patients with OFP (SMD: 9.99 [95%CI: 7.19, 12.80].
Conclusions: a multimodal treatment strategy achieved the greatest positive effects on pain and other outcomes in the short/medium term. AE seems to be an important component of this strategy. However, the scientific evidence supporting AE’s isolated effect is limited, indicating a research gap in this scientific field.

Schmerz hat einen entscheidenden Einfluss auf die humane Lebensqualität. Allein eine Wissensvergrößerung über neurobiologische Vorgänge kann das subjektive Schmerzempfinden sowie psychometrische Variablen positiv beeinflussen. Es gibt verschiedene Formen der präoperativen Patientenedukation, welche u. a. zum Ziel haben, den postoperativen Schmerz zu erklären. Laut der aktuellen Literatur liegt einer präoperativen biomedizinischen Edukation eine geringe Evidenz zugrunde. Sie kann das präoperative Angst- und Stresslevel der Patienten steigern, was sich negativ auf das postoperative Outcome auswirkt. Im Gegensatz zur biomedizinischen Edukation betrachtet das neurobiologische Verständnis den postoperativen Schmerz unter den Gesichtspunkten der Plastizität des Nervensystems und bezieht Sensibilisierungsprozesse im zentralen und peripheren Nervensystem mit ein.
Ziel
Systematische Untersuchung von Kurz- und Langzeiteffekten einer neurobiologischen (Schmerz‑)Edukation bei Patienten vor einer Wirbelsäulenoperation
Material und Methoden
Bei der Literaturrecherche wurde nach dem PI(C)O(Population Intervention Comparison Outcome)‑Schema in den medizinischen Datenbanken gesucht. 83 Artikel kamen in die engere Auswahl. Entsprechend den Ein- und Ausschlusskriterien konnten letztendlich neun Artikel eingeschlossen werden.
Ergebnisse
Durch eine präoperative neurobiologische (Schmerz‑)Edukation können postoperative Katastrophisierungstendenzen sowie die postoperative Kinesiophobie positiv beeinflusst werden. Keinen Einfluss hat eine präoperative neurobiologische Edukation auf postoperativen Schmerz und Funktion. Inkonsistenz besteht bislang in der Herangehensweise der PNE (Pain Neuroscience Education).
Schlussfolgerung
Eine präoperative Reduktion von Angst und schmerzaufrechterhaltenden Faktoren v. a. auf psychologischer und sozialer Ebene hat einen positiven Effekt auf die postoperative subjektive Schmerzbewertung, was sich in einer Reduktion von Angst, Katastrophisierungstendenzen und einer geringeren Inanspruchnahme von postoperativen Leistungen im Gesundheitswesen widerspiegelt.
Introduction
Observations show that foam rolling improves joint movements. Likewise, it can be stated that a vibration stimulation of the tissue leads to improved joint mobility.
Method
This study investigates whether the combination of foam rolling and vibrations (31 Hz) can influence the sliding of the thoracolumbar fascia more effectively than normal foam rolling. 45 subjects participated in the study and were divided into a foam roll with additional vibration group (FRV), a foam roll group (FR) and a control group (CG). The intervention groups rolled out the gluteal muscles, the lateral trunk and the upper and lower back. Mobility measures were taken pre and post the respective intervention. Subsequent cross correlation software analysis quantified the sliding of the fascia and calculated its shear strain mobility (SSM).
Results
The sliding of the thoracolumbar fascia improved significantly within the FRV by 2.83 mm (SD ± 1.08/p < .001), in the FR by 0.96 mm (SD ± 0.43/p < .001) and in the CG decreased the sliding by 0.1401 mm (SD ± 0.28/p = .076). The fascia/fascia SSM increased in the FRV by 22.61% (SD ± 15.64/p < .001), in the FR by 11.41% (SD ± 20.38/p = .056) and in the CG decreased the SSM by 0.9473% (SD ± 11.35/p < .751). The lumbar movement increased in both intervention groups, but showed no significant result.
Conclusion
The use of a foam roll with additional vibration and standard intervention have increased thoracolumbar fascia sliding and lumbar movements. The improved shear strain mobility can be attributed to the multi-activity of mechanoreceptors, such as Pacini- and Ruffini-Bodies.
The management of patients experiencing chronic orofacial pain is a great challenge, due to the complexity of chronic pain itself, combined with an increased peripheral sensitization in the craniofacial itself. Therefore, patients with orofacial pain may present a clear distortion of the somatorepresentation after some time. In this review, the authors develop a neurophysiological explanation of orofacial distortion, as well as propose assessment and treatment options, based on scarcely available scientific evidence and their own clinical experience. The assessments of facial somatosensory, cognitive-affective and motor dysfunctions are crucial to establish the most accurate treatment; the assessment tools are described in the article. Two-point discrimination, laterality recognition and emotion recognition are altered in patients with orofacial pain. Other sensorimotor assessment tools, such as motor acuity and auditory acuity, are also explained. Finally, the authors review their treatment proposals, based on the integration of brain training techniques and biobehavioral interventions. Somatosensory reintegration (tactile acuity training), facial emotion recognition, movement representation techniques, orofacial motor training and therapeutic patient education are explained in detail, and this may challenge new directions in rehabilitation and research.
BACKGROUND: The Craniofacial Pain and Disability Inventory (CF-PDI) is a cross-culturally adapted instrument designed from a biopsychosocial perspective to measure pain, disability, and function in orofacial head and neck pain with shown psychometric properties; however, the German cross-cultural adaption is lacking.
OBJECTIVES: To carry out a transcultural translation of CF-PDI into German and assess its psychometric properties in patients with painful temporomandibular disorders (TMD) with respect to construct and clinical validity, internal consistency and reproducibility.
STUDY DESIGN: Multicenter, prospective, cross-sectional design.
SETTING: Patients (n = 398) were recruited from dental and physical therapy clinics in middle and south Germany.
METHODS: Structural validity was assessed using exploratory factor analysis (EFA) and confirmatory factor analysis (CFA). We investigated know-group validity by means of the scale’s potential to discriminate between affected and unaffected subjects. Multiple linear regression analysis was used to estimate convergent validity. We tested test-retest reliability by the intraclass correlation coefficient and the Internal consistency by Cronbach’s alpha, or each dimension separately, and the total score. Multiple linear regression analysis was used to estimate convergent validity.
RESULTS: Two hundred forty-six heterogeneous chronic craniofacial pain patients and 152 patients without complaints were recruited from the middle and south of Germany. The German version CF-PDI-G presents 21 items, 4 factors, and adequate psychometric properties. The test-retest reliability and internal consistency of the CF-PDI-G were both excellent for the entire instrument and also for all sub-scales (intraclass correlation coefficient [ICC] > 0.90) except for the comorbidities and interference with work which was acceptable (ICC = 0.69). Standard error of the measurement (SEM) and minimal detectable change values are sufficiently low. Assessment of clinical validity shows good potential of discrimination and classification into categories “no,” “mild,” “moderate,” and “severe.” The multiple linear regression model showed a strong association between neck disability index, Visual Analog Scale, and anamnestic questionnaire (supporting the scale’s convergent validity).
LIMITATIONS: Our sample has a higher prevalence of women and the sample was not recruited consecutively, which may lead to a biased estimation of psychometric properties.
CONCLUSIONS: The CF-PDI-G represents valid and reliable instrument to assess pain and disability in patients with orofacial pain and headache suitable for research and clinical practice.

Bruxismus ist keine Krankheit, sondern ein Sammelbegriff für Parafunktionen wie Zähnepressen oder Knirschen. Um Begleitsymptome des Bruxismus zu beeinflussen, ist eine Untersuchung des orofazialen und kraniozervikalen Systems sinnvoll. Die gefundenen auffälligen Zeichen leiten die Therapeuten in der muskuloskelettalen Therapie, die aus Hands-on und -off-Techniken besteht.

Arne Vielitz und Dr. Claus Beyerlein, beide Herausgeber der MSK, baten daher Vertreter/-innen der 5 Mitglieder der Dachorganisation OMT-Deutschland um ein Statement zu folgenden Fragen: Was beinhaltet die muskuloskelettale Physiotherapie für Sie? Wie sieht eine gelungene/zeitgemäße praktische Umsetzung/Anwendung aus? Wie sehen Sie die muskuloskelettale Physiotherapie in der Zukunft? Wo geht die Reise hin?
Die Statements der 5 auf diese Fragen sind in alphabetischer Reihenfolge geordnet.
Die Lumbale Spinalkanalstenose (LSS) bezeichnet eine symptomatische Verengung des Spinalkanals aufgrund einer kongenitalen Erkrankung (primäre LSS) oder eines degenerativen Prozesses (sekundäre LSS). Laut Schätzungen unterziehen sich 40 % aller von LSS betroffenen Patienten innerhalb der ersten 10 Jahre einem chirurgischen Eingriff. Ziel dieses Reviews ist, die Effektivität einer Rehabilitation, einschließlich individualisierter Physiotherapie, mit der herkömmlichen Versorgung nach einer Operation der LSS zu vergleichen.
Die Datenbanken CENTRAL, MEDLINE, DIMDI, PEDro und PubMed wurden systematisch nach randomisierten kontrollierten Studien durchsucht, die bis November 2018 durchgeführt wurden. Vier Studien wurden in den Review eingeschlossen. Die Gesamtqualität der Evidenz erwies sich dabei als moderat. Die Interventionen erfolgten unmittelbar während des Krankenhausaufenthalts oder innerhalb von 6–12 Wochen postoperativ und beinhalteten statt individualisierter Physiotherapie lediglich unspezifische Gruppentherapie. Die Kontrollgruppen erhielten herkömmliche Versorgung oder Empfehlungen zum postoperativen Verhalten. Die Analyse ergab keine Unterschiede zwischen den Gruppen in Bezug auf funktionellen Status und Rückenschmerzen kurz- und langfristig. Lediglich bezüglich Beinschmerzen zeigte sich eine signifikante Differenz zugunsten der Interventionsgruppe (SMD –0,22, 95 % KI –0,43 bis –0,01).
Nicht individuelle Physiotherapie zeigt demnach im Vergleich zur herkömmlichen Versorgung keine kurzfristigen Effekte hinsichtlich Funktion und Schmerz, jedoch einen kleinen klinisch relevanten und signifikanten langfristigen Effekt im Hinblick auf Beinschmerzen. Die geringe Anzahl an eingeschlossenen Studien und die moderate Qualität der Evidenz unterstreichen die dringende Notwendigkeit qualitativ hochwertiger Studien, die die Wirkung einer individualisierten, patientenzentrierten und evidenzbasierten Physiotherapie untersuchen.
Objectives:
The purpose of this study was to determine the motor function of the abdominal muscles in singers with and without functional voice disorders and to examine them for possible differences. Additionally, the breathing behaviour and posture control was investigated.
Study Design
Observational study.
Methods:
Female subjects (n = 20) with differing levels of professional competence were used to provide the data for analysis. By using the Singing Voice Handicap Index (SVHI) the grade of dysphonia could be measured, and the subjects were organized in groups. The change of muscle thickness of the M. transversus abdominis (TVA) and the M. obliquus internus abdominis (OIA) during different singing tasks was measured by using ultrasound. The subjects were then asked to perform the Abdominal Hollowing Test (AHT) with the STABILIZER. Finally, the subjects were all filmed while singing. The videos recordings of the singing sessions were analysed by an independent clinical expert regarding breathing and secondary motor activities (SMA). For the statistical analysis, the Mann-Whitney-U Test and the Chi-Square-Test was mainly used.
Results:
The results showed a significantly thinner TVA in the group with dysphonia in comparison to the group without dysphonia. Ultrasound measurements showed significantly higher changes of muscle thickness of the TVA during singing tasks in the group with dysphonia. Regarding the AHT there was a significant difference between the two groups. The group with dysphonia was not able to increase the pressure by 15mmHg. Furthermore, the healthy subjects demonstrated abdominal breathing, while the group with dysphonia present with thoracic breathing. Additionally, it was noted that the subjects with dysphonia showed a higher level of associated movements especially at and/or on the lumbar spine, cervical spine and the left arm and shoulder.
Conclusion:
Differences in TVA-recruitment, breathing behaviour and secondary motor activities while singing were found. This study sparks new ideas for neuromusculoskeletal assessments and therapy.

Objectives
The aim of this Delphi survey was to establish an international consensus on the most useful outcome measures for research on the effectiveness of non-pharmacological interventions for migraine. This is important, since guidelines for pharmacological trials recommend measuring the frequency of headaches with 50% reduction considered a clinically meaningful effect. It is unclear whether the same recommendations apply to complementary (or adjunct) non-pharmacological approaches, whether the same cut-off levels need to be considered for effectiveness when used as an adjunct or stand-alone intervention, and what is meaningful to patients.
Setting
University-initiated international survey.
Participants
The expert panel was chosen based on publications on non-pharmacological interventions in migraine populations and from personal contacts. 35 eligible researchers were contacted, 12 agreed to participate and 10 completed all 3 rounds of the survey. To further explore how migraine patients viewed potential outcome measures, four migraine patients were interviewed and presented with the same measurement tools as the researchers.
Procedures
The initial Delphi round was based on a systematic search of the literature for outcome measures used in non-pharmacological interventions for headache. Suggested outcome measures were rated by each expert, blinded towards the other members of the panel, for its usefulness on a 5-point Likert scale ranging from definitely not useful to extremely useful. Results were combined using median values and IQRs. Tools rated overall as definitely or probably not useful were excluded from subsequent rounds. Experts further suggested additional outcome measures that were presented to the panel in subsequent rounds. Additionally, experts were asked to rank the most useful tools and provide information on feasible cut-off levels for effectiveness for the three highest ranked tools.
Results
Results suggest the use of the Migraine Disability Assessment (MIDAS), Headache Impact Test (HIT-6) and headache frequency as primary outcome measures. Patient experts suggested the inclusion of a measure of quality of life and evaluation of associated symptoms and fear of attacks.
Conclusions
Recommendations are for the use of the MIDAS, the HIT-6 and headache frequency, in combination with an outcome measure for quality of life. Associated symptoms and fear of attacks should also be considered as secondary outcomes, if relevant for the individual target population. The cut-off level for effectiveness should be lower for non-pharmacological interventions, especially when used as an adjunct to medication.
The current narrative literature review aims to discuss clinical reasoning based on nociceptive pain mechanisms for determining the most appropriate assessment and therapeutic strategy and to identify/map the most updated scientific evidence in relation to physical therapy interventions for patients with temporomandibular disorders (TMDs). We will also propose an algorithm for clinical examination and treatment decisions and a pain model integrating current knowledge of pain neuroscience. The clinical examination of patients with TMDs should be based on nociceptive mechanisms and include the potential identification of the dominant, central, or peripheral sensitization driver. Additionally, the musculoskeletal drivers of these sensitization processes should be assessed with the aim of reproducing symptoms. Therapeutic strategies applied for managing TMDs can be grouped into tissue-based impairment treatments (bottom-up interventions) and strategies targeting the central nervous system (top-down interventions). Bottom-up strategies include joint-, soft tissue-, and nerve-targeting interventions, as well as needling therapies, whereas top-down strategies include exercises, grade motor imagery, and also pain neuroscience education. Evidence shows that the effectiveness of these interventions depends on the clinical reasoning applied, since not all strategies are equally effective for the different TMD subgroups. In fact, the presence or absence of a central sensitization driver could lead to different treatment outcomes. It seems that multimodal approaches are more effective and should be applied in patients with TMDs. The current paper also proposes a clinical decision algorithm integrating clinical diagnosis with nociceptive mechanisms for the application of the most appropriate treatment approach.
Objective
To identify assessment tools used to evaluate patients with temporomandibular disorders (TMD) considered to be clinically most useful by a panel of international experts in TMD physical therapy (PT).
Methods
A Delphi survey method administered to a panel of international experts in TMD PT was conducted over three rounds from October 2017 to June 2018. The initial contact was made by email. Participation was voluntary. An e-survey, according to the Checklist for Reporting Results of Internet E-Surveys (CHERRIES), was posted using SurveyMonkey for each round. Percentages of responses were analysed for each question from each round of the Delphi survey administrations.
Results
Twenty-three experts (completion rate: 23/25) completed all three rounds of the survey for three clinical test categories: 1) questionnaires, 2) pain screening tools and 3) physical examination tests. The following was the consensus-based decision regarding the identification of the clinically most useful assessments. (1) Four of 9 questionnaires were identified: Jaw Functional Limitation (JFL-8), Mandibular Function Impairment Questionnaire (MFIQ), Tampa Scale for Kinesiophobia for Temporomandibular disorders (TSK/TMD) and the neck disability index (NDI). (2) Three of 8 identified pain screening tests: visual analog scale (VAS), numeric pain rating scale (NRS) and pain during mandibular movements. (3) Eight of 18 identified physical examination tests: physiological temporomandibular joint (TMJ) movements, trigger point (TrP) palpation of the masticatory muscles, TrP palpation away from the masticatory system, accessory movements, articular palpation, noise detection during movement, manual screening of the cervical spine and the Neck Flexor Muscle Endurance Test.
Conclusion
After three rounds in this Delphi survey, the results of the most used assessment tools by TMD PT experts were established. They proved to be founded on test construct, test psychometric properties (reliability/validity) and expert preference for test clusters. A concordance with the screening tools of the diagnostic criteria of TMD consortium was noted. Findings may be used to guide policymaking purposes and future diagnostic research.
Als Jane mit ihrer kleinen Schwester tobt, schießt ihr plötzlich ein starker Schmerz in Nacken und Kopf. So weit nicht ungewöhnlich und eine Indikation für Physiotherapie. Doch als die junge Studentin erzählt, dass der Kopfschmerz pulsierend ist, wird ihr Therapeut hellhörig und stellt die entscheidenden Fragen.
Eine Nackenflexion beeinflusst nachweislich die Mechanosensibilität von neurodynamischen Testungen der unteren Extremität. In der Literatur wird diskutiert, ob sich dieses Phänomen durch eine erhöhte (intraneurale) Spannung des Nervengewebes und/oder einen protektiven Reflexmechanismus des motorischen Systems erklären lässt. Bislang konnte kein protektiver Mechanismus in Form einer modifizierten muskulären Rekrutierung als mechanosensibler Parameter zwischen der passiven Nackenflexion und der unteren Extremität gemessen werden.
Alle in diese Querschnittsstudie eingeschlossenen 30 asymptomatischen, männlichen Probanden führten den neurodynamischen Straight-Leg-Raise-Test und Slump-Test durch. Um einen möglichen Einfluss der passiven Nackenflexion auf die muskuläre Rekrutierung der unteren Extremität festzustellen, wurden während dieser sensibilisierenden Bewegung die muskuläre Rekrutierung und das Bewegungsausmaß mittels Elektromyografie und Inertialsensorik (kinematische Messtechnik) gemessen.
Die Studienergebnisse bestätigten einen segmentübergreifenden Einfluss der passiven Nackenflexion auf die muskuläre Rekrutierung der unteren Extremität. Deren Muster deuten möglicherweise auf einen protektiven muskulären Mechanismus in endgradigen neurodynamischen Positionen hin.
In dieser Online-Befragung wird erforscht, ob eine OMT-Weiterbildung (orthopädische manuelle/manipulative Physiotherapie) die Employability (Beschäftigungsfähigkeit) verbessert und woran man eine Verbesserung erkennen könnte. Veränderungen durch die OMT-Weiterbildung, deren Vor- und Nachteile und weitere Themen waren Bestandteile der Befragung. Die statistische Auswertung erfolgte deskriptiv. 190 Fragebögen wurden ausgewertet, die Rücklaufquote lag bei 44,6 %.
Die Befragung zeigte, dass die OMT-Weiterbildung teilweise Vorteile mit sich bringt. Die Vorteile sind laut den Befragten eine verbesserte Behandlung und Beratung von Patienten, mehr Spaß und Motivation bei der Arbeit und eine erhöhte Patientenzufriedenheit. Keine Vorteile sind hingegen in Bezug auf eine bessere professionelle Position, Mehrverdienst oder eine verbesserte Zusammenarbeit mit den Krankenkassen zu erkennen. Zudem ist die Zufriedenheit mit dem Dachverband OMT-Deutschland eher gering. Dennoch würden über 90 % die OMT-Weiterbildung weiterempfehlen.
Es sollten weitere Studien in Bezug auf die Employability von Physiotherapeuten mit bestimmten Fort-/Weiterbildungen durchgeführt werden. Nach Aussagen der OMT-zertifizierten Physiotherapeuten nimmt die Employability nach der OMT-Weiterbildung zu. Die Beziehung zu dem Dachverband OMT-Deutschland scheint eher schwierig oder nicht vorhanden zu sein.
Objectives
To develop a time-efficient motor control (MC) test battery while maximising diagnostic accuracy of both a two-level and three-level classification system for patients with non-specific low back pain (LBP).
Design
Case–control study.
Setting
Four private physiotherapy practices in northern Germany.
Participants
Consecutive males and females presenting to a physiotherapy clinic with non-specific LBP (n=65) were compared with 66 healthy-matched controls.
Primary outcome measures
Accuracy (sensitivity, specificity, Youden index, positive/negative likelihood ratio, area under the curve (AUC)) of a clinically driven consensus-based test battery including the ideal number of test items as well as threshold values and most accurate items.
Results
For both the two and three-level categorisation system, the ideal number of test items was 10. With increasing number of failed tests, the probability of having LBP increases. The overall discrimination potential for the two-level categorisation system of the test is good (AUC=0.85) with an optimal cut-off of three failed tests. The overall discrimination potential of the three-level categorisation system is fair (volume under the surface=0.52). The optimal cut-off for the 10-item test battery for categorisation into none, mild/moderate and severe MC impairment is three and six failed tests, respectively.
Conclusion
A 10-item test battery is recommended for both the two-level (impairment or not) and three-level (none, mild, moderate/severe) categorisation of patients with non-specific LBP.
Aim
Manual cranial bone tissue techniques (CBTT) are used by physiotherapists as a tool for assessment and treatment of complaints in the craniocervical, face, and head regions. The goal of this study was to determine whether CBTT were able to discriminate between subjects with cervical and/or temporomandibular disorders (TMD) and a healthy group. In addition, the inter-rater reliability when applying CBTT was also investigated.
Methods
A cross-sectional study was conducted and included 60 participants. Six standardized passive techniques were applied and judged for resistance, compliance, and sensory answer. In order to evaluate the inter-rater reliability of these techniques a cohort of participants was measured twice (by two evaluators) prospectively. A logistic regression model and Receiver Operating Curve (ROC) analyses were used to determining the discriminative validity of these techniques.
Results
Logistic regression identified a significant difference for five techniques for resistance and/or compliance and/or the sensory answer between the groups. Based on the Area Under the Curve (AUC) analysis, the discriminative ability of the temporal rotation to distinguish between the groups was fair to good (for resistance AUC = 0.7775 and for compliance AUC = 0.8065). The highest agreement between the two assessors was for the resistance with occipital compression (73%) technique.
Conclusion
This study highlights that some of the CBTT could be potentially useful in distinguishing subjects with cervical and/or TMD from healthy subjects. Inter-rater reliability was moderate. CBTT could be potentially integrated in the examination of participants with complaints in the craniofacial region.

Geholfen hat ihm eine Kombination aus Hands-on-Techniken und zeitgemäßen Behandlungsmethoden auf neurowissenschaftlicher Basis.
Introduction
Tests to evaluate the integrity of the alar ligaments are important clinical tools for manual therapists, but there is limited research regarding their validity.
Method
A single blinded examiner assessed alar ligament integrity using the lateral shear test (LST), rotation stress test (RST) and side-bending stress test (SBST) on a sample of convenience comprising 7 subjects with MRI confirmed alar ligament lesions and 11 healthy people. Alar ligament lesions were identified using both supine and high-field strength upright MRI.
Results
The RST had a sensitivity of 80% and a specificity of 69.2%. The SBST and the LST both showed a sensitivity of 80% and a specificity of 76.9%. In cases where all three tests were positive, the specificity increased to 84.6%.
Discussion
Tests of manual examination of alar ligament integrity have some diagnostic utility; however, these findings require further corroboration in a larger sample.
Purpose
Differences between standard dysarthria treatment and the same treatment with the integration of neurodynamic techniques tailored to the severity of dysarthria in patients with Parkinson's disease were examined.
Method
In total, 10 subjects with idiopathic Parkinson's disease and rigid–hypokinetic dysarthria were enrolled in this quasi-randomized, controlled, single-blind, pre–post study. In each of 12 therapy sessions the control group (n = 5) received standard dysarthria treatment (usual care), while the intervention group (n = 5) received the same treatment with the addition of integrated neurodynamic treatment (special care).
Results
There was no significant difference between the two groups for either the pre-test (p = 0.739) or the post-test (p = 0.156) results. However, significant differences between the pre-test and post-test results within each group (intervention group p = 0.001; control group p = 0.003) were found.
Conclusions
The significant differences in the pre–post comparison within the groups may indicate a high probability of a positive effect of standard dysarthria treatment on the severity of dysarthria. In between-group comparisons, the study results indicated no evidence of a significant difference between standard dysarthria treatment with or without neurodynamics. Due to the small sample size, the effectiveness of the integration of neurodynamics into speech therapy cannot be definitively concluded for now. In order to be able to have generalized applicability, future studies with larger numbers of participants are required.
Background
Osteoarthritis of the knee is the most common cause for disability and limited mobility in the elderly, with considerable individual suffering and high direct and indirect disease-related costs. Nonsurgical interventions such as exercise, enhanced physical activity, and self-management have shown beneficial effects for pain reduction, physical function, and quality of life (QoL), but access to these treatments may be limited. Therefore, home therapy is strongly recommended. However, adherence to these programs is low. Patients report lack of motivation, feedback, and personal interaction as the main barriers to home therapy adherence. To overcome these barriers, electronic health (eHealth) is seen as a promising opportunity. Although beneficial effects have been shown in the literature for other chronic diseases such as chronic pain, cardiovascular disease, and diabetes, a systematic literature review on the efficacy of eHealth interventions for patients with osteoarthritis of knee is missing so far.
Objective
The aim of this study was to compare the efficacy of eHealth-supported home exercise interventions with no or other interventions regarding pain, physical function, and health-related QoL in patients with osteoarthritis of the knee.
Methods
MEDLINE, CENTRAL, CINAHL, and PEDro were systematically searched using the keywords osteoarthritis knee, eHealth, and exercise. An inverse variance random-effects meta-analysis was carried out pooling standardized mean differences (SMDs) of individual studies. The Cochrane tool was used to assess risk of bias in individual studies, and the quality of evidence across studies was evaluated following the Grading of Recommendations, Assessment, Development, and Evaluation approach.
Results
The literature search yielded a total of 648 results. After screening of titles, abstracts, and full-texts, seven randomized controlled trials were included. Pooling the data of individual studies demonstrated beneficial short-term (pain SMD=−0.31, 95% CI −0.58 to −0.04, low quality; QoL SMD=0.24, 95% CI 0.05-0.43, moderate quality) and long-term effects (pain −0.30, 95% CI −0.07 to −0.53, moderate quality; physical function 0.41, 95% CI 0.17-0.64, high quality; and QoL SMD=0.27, 95% CI 0.06-0.47, high quality).
Conclusions
eHealth-supported exercise interventions resulted in less pain, improved physical function, and health-related QoL compared with no or other interventions; however, these improvements were small (SMD<0.5) and may not make a meaningful difference for individual patients. Low adherence is seen as one limiting factor of eHealth interventions. Future research should focus on participatory development of eHealth technology integrating evidence-based principles of exercise science and ways of increasing patient motivation and adherence.
Bei dieser mit dem dbl-Nachwuchspreis 2018 ausgezeichneten Pilotstudie erhielten sechs ProbandInnen in der Akutphase ihres erstmaligen Schlaganfalls eine hochfrequente Therapie in Anlehnung an die Propriozeptive Neuromuskuläre Fazilitation (PNF) kombiniert mit motorischer Imagination einer autobiografischen Erinnerung oder nur in Anlehnung an PNF. Zentrale Fragestellungen vor und nach der Intervention waren die Veränderungen des Schweregrades der Fazialisparese und der krankheitsbezogenen Lebensqualität der Betroffenen.

Recognition of Emotional Facial Expressions and Alexithymia in Patients with Chronic Facial Pain

Objectives
Alexithymia, conceived as difficulties to identify emotions, is said to be related with several pain syndromes. This study examined the recognition of facially expressed emotions and its relation to alexithymia in subjects with chronic facial pain.
Methods
A total of 62 subjects were recruited, with n=20 patients with chronic facial pain and n=42 healthy controls. All subjects were tested for the recognition of facially expressed emotions (Facially Expressed Emotion Labelling Test (FEEL test). The Toronto Alexithymia Scale (TAS-26) was used for the diagnosis of alexithymia.
Results
Patients with chronic facial pain performed worse than controls at the FEEL task (p<.001) and showed higher total TAS scores (p<.001). This indicates the presence of alexithymia and facial emotion recognition deficits in the facial pain group.
Discussion
It was concluded from the results that both the recognition of facially expressed emotions, and the ability to identify and describe one’s own feelings (TAS), are restricted in chronic orofacial pain patients. This relationship is particularly important in the treatment of chronic facial pain, indicating that it should become part of the treatment in addition to the therapeutic key issues, to influence the quality of life of the affected patients positively.
Für die Schmerzentstehung von Kokzygodynie können sowohl lokale als auch entfernte Strukturen verantwortlich sein. Deshalb gilt es, die Quellen der Symptome für jeden Einzelfall zu identifizieren und durch Clinical Reasoning zu untermauern. Da keine Korrelation zwischen den Symptomen und dem Grad der Steißbeinkrümmung besteht, lässt sich ein Behandlungserfolg nicht mit der Korrektur der Steißbeinstellung erklären. Der Therapieverlauf muss durch andere Parameter kontrolliert werden, wie z. B. Schmerzreduktion und Verbesserung der funktionalen Beeinträchtigungen. Da die Kokzygodynie eng mit psychosozialen Prozessen in Verbindung zu stehen scheint, ist bei Kindern und chronischen Schmerzgeschehen besondere Aufmerksamkeit geboten.
Eine gängige Behandlungsoption bei Kokzygodynie stellt die muskuloskeletale Therapie dar, über deren Wirksamkeit allerdings wenig bekannt ist. Aus diesem Grund ging dieser systematische Literaturreview der Forschungsfrage nach, ob sich Kokzygodynie mit muskuloskeletaler Therapie effektiv behandeln lässt. Obwohl die Validität der ausgewählten Studien eher gering ausfiel, waren ihre Ergebnisse durchweg positiv. Insgesamt erwies sich muskuloskeletale Therapie bei Kokzygodynie als effektive Behandlungsform.

Ein Fallbeispiel offenbart das komplexe Zusammenspiel von Kopfschmerzen und Alltagsbelastungen. Es macht deutlich, wie viele Ansätze es bei der Behandlung von Kopfschmerzerkrankungen gibt, und wie oft die Betroffenen von Pontius zu Pilatus geschickt werden, um adäquate Hilfe zu erhalten. Hierbei werden physiotherapeutische Interventionen außer Acht gelassen, oder sie dienen nur als ultimo ratio – zu Unrecht, wie aktuelle Studien belegen.
Objectives
To investigate differences in pressure pain thresholds (PPTs) and longitudinal mechanosensitivity of the greater occipital nerve (GON) between patients with side-dominant head and neck pain (SDHNP) and healthy controls. Evaluation of neural sensitivity is not a standard procedure in the physical examination of headache patients but may influence treatment decisions.
Methods
Two blinded investigators evaluated PPTs on two different locations bilaterally over the GON as well as the occipitalis longsitting-slump (OLSS) in subjects with SDHNP (n = 38)) and healthy controls (n = 38).
Results
Pressure pain sensitivity of the GON was lower at the occiput in patients compared to controls (p = 0.001). Differences in pressure sensitivity of the GON at the nucheal line, or between the dominant headache side and the non-dominant side were not found (p > 0.05). The OLSS showed significant higher pain intensity in SDHNP (p < 0.001). In comparison to the non-dominant side, the dominant side was significantly more sensitive (p = 0.004).
Discussion
Palpation of the GON at the occiput and the OLSS may be potentially relevant tests in SDHNP. One explanation for an increased bilateral sensitivity may be sensitization mechanisms. Future research should investigate the efficacy of neurodynamic techniques directed at the GON.
Objective
Gross mandibular position and masticatory muscle activity have been shown to influence cervical muscles electromyographic activity. The purpose of this study was to investigate the influence of three different mandible positions including conscious occlusion, tongue tip against the anterior hard palate (Palate tongue position) and natural resting position (Rest), on sagittal plane cervical spine range of motion (ROM) as well as the flexion-rotation test (FRT) in asymptomatic subjects.
Materials and methods
An experienced single blinded examiner evaluated ROM using an Iphone in 22 subjects (7 females; mean age of 29.91years, SD 5.44).
Results
Intra-rater reliability for range recorded was good for the FRT with ICC (intraclass correlation) 0.95 (95% CI: 0.88–0.98) and good for sagittal plane cervical ROM with ICC 0.90 (95% CI: 0.77–0.96). A repeated measures ANOVA determined that mean ROM recorded during the FRT differed significantly between assessment points (F(1.99, 41.83) = 19.88, P < 0.001). Bonferroni Post hoc tests revealed that both conscious Occlusion and Palate tongue position elicited a significant large reduction in ROM recorded during the FRT from baseline (p < 0.01). Despite this, one activation strategy did not influence ROM more than the other. An additional repeated measures ANOVA determined that mean sagittal cervical ROM did not significantly vary between assessment points (F(2, 42) = 8.18, P = 0.08).
Conclusion
This current study provided further evidence for the influence of the temporomandibular region on upper cervical ROM. Results suggest that clinicians should focus on the natural mandible rest position when evaluating upper cervical mobility.
Objective:
To compare the short- and long-term effects of a structural-oriented (convential) with an activity-oriented physiotherapeutic treatment in patients with frozen shoulder.
Design:
Double-blinded, randomized, experimental study.
Setting:
Outpatient clinic.
Subjects:
We included patients diagnosed with a limited range of motion and pain in the shoulder region, who had received a prescription for physiotherapy treatment, without additional symptoms of dizziness, a case history of headaches, pain and/or limited range of motion in the cervical spine and/or temporomandibular joint.
Interventions:
The study group received treatment during the performance of activities. The comparison group was treated with manual therapy and proprioceptive neuromuscular facilitation (conventional therapy). Both groups received 10 days of therapy, 30 minutes each day.
Main measures:
Range of motion, muscle function tests, McGill pain questionnaire and modified Upper Extremity Motor Activity Log were measured at baseline, after two weeks of intervention and after a three-month follow-up period without therapy.
Results:
A total of 66 patients were randomized into two groups: The activity-oriented group (n = 33, mean = 44 years, SD = 16 years) including 20 male (61%) and the structural-oriented group (n = 33, mean = 47 years, SD = 17 years) including 21 male (64%). The activity-oriented group revealed significantly greater improvements in the performance of daily life activities and functional and structural tests compared with the group treated with conventional therapy after 10 days of therapy and at the three-month follow-up (p < 0.05).
Conclusions:
Therapy based on performing activities seems to be more effective for pain reduction and the ability to perform daily life activities than conventional treatment methods.

Ziel dieser Studie ist es, die Auswirkungen einer Kinesiophobie auf die Emotions- sowie Lateralitätserkennung zu untersuchen.
Material und Methoden
67 Probanden mit chronischen muskuloskeletalen Schmerzen wurden untersucht. Hierbei erzielten 24 Probanden einen Wert von >37 auf der Tampa-Skala für Kinesiophobie und wurden in die Querschnittstudie eingeschlossen. Die Fähigkeit zur Erkennung mimisch codierter Basisemotionen wurde mittels des Facially-Expressed-Emotion-Labeling(FEEL)-Tests und die Lateralitätserkennung anhand eines speziellen Face-Mirroring-Assessment-and-Treatment-Programms ermittelt. Die Toronto-Alexithymie-Skala (TAS) 26 diente zur Erfassung einer Alexithymie.
Ergebnisse
Der FEEL-Score der Probanden mit Kinesiophobie war signifikant niedriger (p = 0,019). Die Basisemotionen Angst (p = 0,026), Ärger (p = 0,027) und Überraschung (p = 0,014) verdeutlichten einen signifikanten Unterschied zwischen den Ergebnissen der Querschnittstudie und gesunden Probanden. Der Alexithymiefragebogen TAS-26 zeigte lediglich in der TAS 1 (Schwierigkeiten bei der Identifikation von Gefühlen; p = 0,008) einen signifikanten Unterschied zwischen Probanden mit Kinesiophobie.
Schlussfolgerung
Die Ergebnisse zeigen, dass kinesiophobische Patienten Veränderungen in der Emotions- und Lateralitätserkennung sowie alexithyme Merkmale aufweisen.
Due to new research results in the past few years, interest in the fascia of the human body has increased. Dysfunctions of the fascia are indicated by various symptoms, amongst others, musculoskeletal pain. As a result stronger focus has been put on researching therapeutic approaches in this area. The main aim of this study was to investigate the effect of Foam Roll exercises on the mobility of the thoracolumbar fascia (TLF). Study has been conducted in a randomized and controlled trial which sampled 38 healthy athletic active men and women.
The subjects were randomly assigned to a Foam Roll Group (FMG), a Placebo Group (PG) and a Control Group (CG). Depending on the assigned group the volunteers were either instructed to do exercises with the Foam Roll, received a pseudo treatment with the Foam Roll or received no treatment.
A total of three measurements were carried out. The most important field of research was the mobility of the TLF, which was determined using a sonographic assessment. In addition the lumbar flexion and the mechanosensivity of relevant muscles were determined.
After the intervention, the FMG showed an average increase of 1.7915 mm for the mobility of the TLF (p < 0.001/d = 0.756). In contrast, only an average improvement of 0.1681 mm (p = 0.397) was shown in the PG, while the CG showed a slight improvement of 0.0139 mm (p = 0.861). However, no significant changes were observed with regard to the lumbar flexion and mechanosensivity of the treated muscles.
Thus, evidence is that the use of Foam Roll exercises significantly improves the mobility of the thoracolumbar fascia in a healthy young population.
The primary objective of this study was to determine the structural and known-group validity as well as the inter-rater reliability of a test battery to evaluate the motor control of the craniofacial region. Seventy volunteers without TMD and 25 subjects with TMD (Axes I) per the DC/TMD were asked to execute a test battery consisting of eight tests. The tests were video-taped in the same sequence in a standardised manner. Two experienced physical therapists participated in this study as blinded assessors. We used exploratory factor analysis to identify the underlying component structure of the eight tests. Internal consistency (Cronbach's α), inter-rater reliability (intra-class correlation coefficient) and construct validity (ie, hypothesis testing-known-group validity) (receiver operating curves) were also explored for the test battery. The structural validity showed the presence of one factor underlying the construct of the test battery. The internal consistency was excellent (0.90) as well as the inter-rater reliability. All values of reliability were close to 0.9 or above indicating very high inter-rater reliability. The area under the curve (AUC) was 0.93 for rater 1 and 0.94 for rater two, respectively, indicating excellent discrimination between subjects with TMD and healthy controls. The results of the present study support the psychometric properties of test battery to measure motor control of the craniofacial region when evaluated through videotaping. This test battery could be used to differentiate between healthy subjects and subjects with musculoskeletal impairments in the cervical and oro-facial regions. In addition, this test battery could be used to assess the effectiveness of management strategies in the craniofacial region.

Geoffrey Maitlands Behandlungsweise prägt die Physiotherapie bis heute und hat nichts an Aktualität verloren. 1992 gründete er mit von ihm ausgebildeten Instruktoren die Internationale Maitland Teacher Association (IMTA). Diese feiert am 27. Januar 2018 in Stuttgart ihr 25-jähriges Bestehen im Rahmen des physiokongresses. Es erwarten Sie zehn Vorträge von hochkarätigen Referenten.
Schmerzen im Bereich des Steißbeins werden häufig unter dem Begriff Kokzygodynie zusammengefasst. Ursache der Beschwerden können hierbei lokal oder entfernt liegende Strukturen sein. Außerdem gibt es eine Reihe von inneren und psychischen Erkrankungen, die eine Kokzygodynie vortäuschen können und differenzialdiagnostisch abgeklärt werden müssen.
Dieser Artikel gibt eine Übersicht über Anatomie, Ätiopathologie, Diagnostik, Differenzialdiagnostik und Therapiemöglichkeiten bei Kokzygodynie. Der Schwerpunkt liegt dabei auf muskuloskeletaler Therapie. Es werden Techniken am Steißbein aus 4 manualtherapeutischen Konzepten vorgestellt.
Kreuzbandrisse (Rupturen des vorderen Kreuzbands, VKB) ziehen oft eine langwierige Rehabilitation und Wiedereingliederung nach sich. Die physiotherapeutische Nachbehandlung beinhaltet aktive und passive Maßnahmen. Bislang existieren keine Studien zu neurodynamischen Aspekten der Rehabilitation von Kreuzbandrissen. Das Ziel der vorliegenden Studie bestand darin, die Veränderung der Bewegung des N. tibialis des betroffenen und nicht betroffenen Beins bei Probanden nach einer VKB-Ruptur zu untersuchen.
Bestandteil der verblindeten Untersuchung der 19 Probanden waren der Knee Outcome Survey für alltägliche Aktivitäten (KOS) und verschiedene Funktionsuntersuchungen (Slump-Test mit passiver Knieextension, Long Sitting Slump Test mit passiver Dorsalextension des oberen Sprunggelenks, Straight Leg Raise Test, belastete Dorsalextension im Stand). Die longitudinale Nervenexkursion des N. tibialis wurde mit bildgebendem Ultraschall in sitzender Slump-Position untersucht. Die Auswertung der Ultraschallvideos erfolgte mit einer Frame-by-frame Cross Correlation Software.
Die Ergebnisse zeigten eine signifikant geringere Nervenexkursion des N. tibialis am betroffenen Knie. Allerdings fand sich kein klinisch signifikanter Zusammenhang zu den Funktionsuntersuchungen.
Die Kieferregion und die HWS stehen neuroanatomisch und biomechanisch in enger Verbindung zueinander. Darüber hinaus hat die kraniomandibuläre Region durch ihren funktionellen Aufbau, ihre anatomische Positionierung und ihre neurologische Wertigkeit eine Bedeutung für den gesamten menschlichen Organismus.
Das Ziel der Studie war es zu beobachten, ob eine durch eine veränderte dentale Okklusion hervorgerufene Veränderung im Mandibulabereich einen direkten Einfluss auf die aktive und passive physiologische HWS-Beweglichkeit ausübt.
Neben der Gruppenauswertung wurden die 61 Probanden zusätzlich von einem Zahnarzt in die verschiedenen Angle-Klassifikationen (Klasse 1, 2a, 2b und 3) eingeteilt und auf Unterschiede untersucht. Bei allen Probanden wurde die physiologische Beweglichkeit der HWS mit dem CROM Device gemessen und der Flexions-Rotations-Test (FRT) für die passive physiologische Bewegungsüberprüfung mit einem digitalen Goniometer durchgeführt. Die Messungen erfolgten jeweils 2-mal innerhalb eines kurzen Zeitabstands, und zwar einmal ohne und einmal mit Veränderung der Okklusion durch einen Papierstreifen.
Beim Vergleich der beiden Messungen der gesamten Stichprobe zeigte sich eine signifikant vergrößerte Beweglichkeit in jeder gemessenen Bewegungsrichtung. Zwischen den Angle-Klassifikationen waren keine signifikanten Unterschiede zu beobachten.
Viele „Overhead Athletes“ leiden am Glenohumeral Internal Rotation Deficit Syndrome (GIRD; glenohumerales Innenrotationsdefizit). Die persistierenden Schulterbeschwerden werden durch Überbelastung im oberen Quadranten verursacht. Bislang ist der Einfluss der orofazialen Region und der dentalen Okklusion auf die (motorische) Funktion bei den betroffenen Patienten nicht systematisch erforscht.
Diese Fallserie untersuchte bei (semi)professionellen Overhead Athletes die Einschränkung ihrer Innen- und Außenrotation sowie der statischen Kraft in 4 Bewegunsrichtungen. Aufgrund der Resultate wurde aus der 1. Gruppe eine 2. Gruppe gebildet, die 3 manualtherapeutische Behandlungen und anschließend eine Dentalschiene erhielt.
Die multidisziplinäre Behandlung durch den Zahnarzt und einen (spezialisierten) Manualtherapeuten reduzierte signifikant die Beschwerden und Dysfunktionen. Eine orofaziale klinische Diagnose nach den Richtlinien der Diagnostic Criteria/Temporomandibular Dysfunctions (DC/TDM) und der okklusale kinästhetische Sensibilisierungstest (OKST) scheinen prognostische Indikatoren für die Behandlung des GIRD-Syndroms zu sein.
Die funktionelle Dysphonie (FD) präsentiert sich klinisch mit ventralen Halsschmerzen, Heiserkeit und Stimmstörungen. Organisch liegt kein pathologischer Befund vor, aber möglicherweise spielen laryngeale Strukturen, die Muskulatur und neurale Ansteuerung eine ursächliche Rolle. Die FD wird zwar primär von Logopäden und Sprachtherapeuten behandelt, aber spezialisierte Manualtherapeuten können wesentlich dazu beitragen, dass die Therapie erfolgreich verläuft.

Background
A wide range of physical tests have been published for use in the assessment of musculoskeletal dysfunction in patients with headache. Which tests are used depends on a physiotherapist's clinical and scientific background as there is little guidance on the most clinically useful tests.
Objectives
To identify which physical examination tests international experts in physiotherapy consider the most clinically useful for the assessment of patients with headache.
Design/methods
Delphi survey with pre-specified procedures based on a systematic search of the literature for physical examination tests proposed for the assessment of musculoskeletal dysfunction in patients with headache.
Results
Seventeen experts completed all three rounds of the survey. Fifteen tests were included in round one with eleven additional tests suggested by the experts. Finally eleven physical examination tests were considered clinically useful: manual joint palpation, the cranio-cervical flexion test, the cervical flexion-rotation test, active range of cervical movement, head forward position, trigger point palpation, muscle tests of the shoulder girdle, passive physiological intervertebral movements, reproduction and resolution of headache symptoms, screening of the thoracic spine, and combined movement tests.
Conclusions
Eleven tests are suggested as a minimum standard for the physical examination of musculoskeletal dysfunctions in patients with headache.

Obwohl chronische Schulterschmerzen sehr weit verbreitet sind und myofasziale Triggerpunkte (mTrP) als häufig gelten, bleibt ihr Einfluss auf das Schmerzgeschehen unklar. Nur in wenigen kontrolliert angelegten Studien wurde der Effekt einer manuellen Triggerpunktintervention untersucht.
Zielsetzung
Diese randomisierte, kontrollierte Studie (RCT) vergleicht die kurzfristigen Effekte einer manuellen Druckdehnmanipulation (n = 6) bei unilateralen Schulterschmerzen und vorliegendem myofaszialem Syndrom (MFS) im Vergleich mit einer manuellen Scheintherapie (Sham-Therapie; n = 6).
Material und Methoden
Die Messungen erfolgten vor der ersten und nach der zweiten Intervention. Neben der Druckschmerzschwelle [„pressure pain thresholds“ [PPT]) der mTrP wurden symmetrisch lokalisierte Vergleichspunkte auf der symptomlosen Seite sowie neutrale Hyperalgesiepunkte erhoben, um eine potenzielle unilaterale oder generalisierte Hyperalgesie zu erkennen. Daneben wurden der Schmerzwert auf der visuellen Analogskala in Ruhe und bei Bewegung sowie der Neck Disability Index (NDI) und der Disability-of-Arm-Shoulder-Hand (DASH)-Fragebogen erhoben.
Ergebnisse
Beide Behandlungsmodalitäten führten zu einer signifikanten Verbesserung, allerdings war die manuelle Triggerpunktintervention der Sham-Therapie, gemessen an verschiedenen Parametern, signifikant überlegen.
Schlussfolgerung
Die signifikante PPT-Verbesserung in der Interventionsgruppe auch an unbehandelten Stellen lässt auf eine zentrale Modulation der Reizschwelle durch eine Druckdehnmanipulation schließen. Die signifikanten Effekte der Sham-Therapie sind möglicherweise darauf zurückzuführen, dass es sich um eine Hands-on-Technik handelt und dass es durch die Diagnostik und PPT-Messungen mehrmals zu einer Gewebevorspannung in der Triggerpunktregion kam.

Objectives
Cervical movement impairment has been identified as a core component of cervicogenic headache evaluation. However, normal range of motion values in children has been investigated rarely and no study has reported such values for the flexion–rotation test (FRT). The purpose of this study was to identify normal values and side-to-side variation for cervical spine range of motion (ROM) and the FRT, in asymptomatic children aged 6–12 years. Another important purpose was to identify the presence of pain during the FRT.
Methods
Thirty-four asymptomatic children without history of neck pain or headache (26 females and 8 males, mean age 125.38 months [SD 13.14]) were evaluated. Cervical spine cardinal plane ROM and the FRT were evaluated by a single examiner using a cervical ROM device.
Results
Values for cardinal plane ROM measures are presented. No significant gender difference was found for any ROM measure. Mean difference in ROM for rotation, side flexion, and the FRT were less than one degree. However, intra-individual variation was greater, with lower bound scores of 9.32° for rotation, 5.30° for side flexion, and 10.89° for the FRT. Multiple linear regression analysis indicates that movement in the cardinal planes only explains 19% of the variance in the FRT. Pain scores reported following the FRT were less than 2/10.
Discussion
Children have consistently greater cervical spine ROM than adults. In children, side-to-side variation in rotation and side flexion ROM and range recorded during the FRT indicates that the clinician should be cautious when using range in one direction to determine impairment in another. Range recorded during the FRT is independent of cardinal movement variables, which further adds to the importance of the FRT, as a test that mainly evaluates range of movement of the upper cervical spine.
Einleitung
Lymphödeme können als direkte Erkrankung oder als Symptome bei vielen Krankheiten auftreten. Die gängigste Methode zur Behandlung von Lymphödemen ist die Komplexe Physikalische Entstauungstherapie (KPE), die sich aus Manueller Lymphdrainage (MLD), Kompression, Bewegungsübungen und Hautpflege zusammensetzt. In Deutschland wird meist nur MLD verschrieben. Das Ziel dieses systematischen Reviews ist es, die Effektivität der MLD zu überprüfen.
Methodik
Es wurde eine systematische Literaturrecherche durchgeführt und zusätzlich Kontakt zur Deutschen Gesellschaft für Lymphologie (DGL) aufgenommen. Die Studien wurden anhand von Kriterien der Cochrane Collaboration auf ihr Evidenzlevel hin überprüft und die methodische Qualität mithilfe der PEDro-Skala beurteilt.
Ergebnisse
37 in diese Übersichtsarbeit eingeschlossene Studien umfassen die Behandlungsmethoden MLD alleine, MLD in Kombination mit Kompression und MLD mit zusätzlicher Physiotherapie sowie mechanische Druckkompression und KPE. Die Evidenzlevel waren unterschiedlich und die methodische Qualität der Studien mäßig. Alle Studien zum Thema Lymphödembehandlung bei Brustkrebspatienten zeigten gute Erfolge, unabhängig von der Art der Intervention.
Schlussfolgerungen
Während zur Behandlung von Lymphödemen infolge von Brustkrebs einige Studien vorhanden sind, gibt es zu allen anderen Krankheitsbildern, die im Heilmittelkatalog angegeben sind, kaum Studien. Hier wird dringend weitere Forschung benötigt.
Einleitung
Ziel dieser Studie war es, zu untersuchen, ob es einen Zusammenhang gibt zwischen einer mechanisch gesteuerten Traktion am Handgelenk und den nächtlichen Kribbelparästhesien sowie der Funktionsfähigkeit bei Patienten mit Karpaltunnelsyndrom.
Methode
In der vorliegenden Interventionsstudie wurden 21 Patienten mit Karpaltunnelsyndrom präoperativ sechsmal mittels des Traktionsgerätes Phystrac GR 10 behandelt. Die Messung der Effekte wurde vor sowie nach der ersten, dritten und sechsten Intervention durchgeführt. Als Assessment-Instrumente wurden die Sonografie, der Hand Grip Dynamometer, die Visuelle Analogskala und der Boston Carpal Tunnel Syndrome Questionnaire genutzt. Letzterer wurde nur zu Beginn und am Ende der Studie ausgefüllt.
Ergebnisse
Statistisch signifikante Ergebnisse konnten für eine Verbesserung der Funktionsfähigkeit (p = 0,023) und eine Reduktion der nächtlichen Kribbelparästhesien (p = 0,001) nach sechs Interventionen erzielt werden. Zudem konnte eine Korrelation zwischen diesen Parametern nachgewiesen werden (r = 0,497 zu Beginn und r = 0,591 am Ende der Studie). Die Parameter Querschnittsfläche sowie transversales und longitudinales Gleiten des N. medianus, Greifkraft, Schmerzen beim Greifen und Symptome veränderten sich nicht signifikant.
Schlussfolgerungen
Die vorliegende Studie konnte aufzeigen, dass eine mechanisch gesteuerte Traktion eine wirkungsvolle konservative Intervention bei Karpaltunnelsyndrom darstellt und als mögliche Alternative zu operativen Eingriffen am Karpaltunnel gesehen werden sollte. Es ist davon auszugehen, dass eine mechanisch gesteuerte Traktion Einfluss auf die intraneuralen physiologischen Eigenschaften und weniger auf die Mechanik des N. medianus nimmt.
Paul Brand, 51 Jahre, hat seit sechs Wochen Schmerzen im rechten Nacken-, Hals- und Schläfenbereich. Zudem plagen ihn im rechten Auge pulsierende Schmerzen. Passive manuelle Bewegungen von Gelenk- und Neuralstrukturen werden häufig mit einem ersten (R 1 = erster fühlbarer Widerstand innerhalb einer passiven Bewegung) und zweiten Widerstand (R2 = fühlbarer Widerstand am Bewegungsende) gemessen.
Die vorliegende Arbeit gibt einen Überblick über die Zuverlässigkeit der Messung von R1 und R2. In den Datenbanken PubMed (Medline), The Cochrane Database, CINAHL und Web of Science wurde in einer systematischen Literaturrecherche nach Diagnostikstudien zu R1 und R2 gesucht und die eingeschlossenen Studien mit dem Quality Appraisal of Reliability Studies Tool (QAREL) bewertet.
Die Ergebnisse zeigten widersprüchliche Reliabilität bei neurodynamischen Tests. Passive Außenrotationen bei Schulterpathologien erzielten eine gute, passive akzessorische intervertebrale Bewegungen (PAIVM) eher eine geringere Reliabilität.
R2 scheint eine höhere Reliabilität als R1 aufzuweisen, und die Intrarater-Reliabilität bei neurodynamischen Tests besser zu sein als die Interrater-Reliabilität. Für die geringe Reliabilität werden verschiedene Ursachen diskutiert. Trotz der methodischen Mängel in einigen Studien scheinen Tests mit größerem Hebel eine höhere Reliabilität zu erreichen als PAIVM.
A patient presenting with local pain and limitation of movement in the temporomandibular region following surgery of the left temporomandibular joint (TMJ) is described. Manual techniques like distraction of the TMJ combined with motor control exercises to restore TMJ function were not sufficient to relieve the patient's symptoms and her orofacial functions. However, during manual assessment and treatment of cranial nervous tissue, in this case the auriculotemporal nerve and its interface, pain was relieved and orofacial functions improved.

Basisemotionen werden über die Mimik ausgedrückt und sind ein wichtiger Kommunikator zur Außenwelt. Patienten mit Parkinson verlieren diese Fähigkeit aufgrund des häufig auftretenden so genannten Maskengesichts.
Ziel
Das Ziel dieser Arbeit war es, Unterschiede hinsichtlich der Emotionserkennung und -wahrnehmung zwischen Patienten mit Parkinson und gesunden Menschen zu identifizieren.
Methode
34 medikamentös eingestellte Patienten mit Parkinson wurden anhand des Facially Expressed Emotion Labeling Tests (FEEL) auf ihre Emotionserkennung anderer Personen und anhand der Toronto-Alexithymie-Skala-26 (TAS) auf ihre Wahrnehmung der eigenen Emotionen untersucht. Die Ergebnisse wurden mit den Daten aus bereits vorliegenden Studien verglichen.
Ergebnisse
Patienten mit Parkinson hatten signifikante Probleme beim Erkennen von Emotionen in der Mimik im Vergleich zu Gesunden (p ≤ 0,001). Zusätzlich brauchten sie signifikant länger, die Ausdrücke den korrekten Emotionen zuzuordnen (p ≤ 0,001). Dabei korrelierte das Endergebnis des FEEL-Tests sehr stark mit der Reaktionszeit (p ≤ 0,001; r = –0,665). Auch die Wahrnehmung der eigenen Emotionen (TAS-26) war bei ihnen stark eingeschränkt (p ≤ 0,001). Der Zusammenhang zwischen den Ergebnissen des FEEL-Tests und der TAS-26 war gering (p = 0,020; r = –0,404).
Schlussfolgerung
Patienten mit Parkinson zeigten Beeinträchtigungen, Gesichtsausdrücke zu erkennen und sie den passenden Emotionen zuzuordnen. Merkmale einer Alexithymie konnten aber nicht festgestellt werden. Patienten mit einer guten Emotionserkennung scheinen geringere Schwierigkeiten zu haben, ihre eigenen Gefühle wahrzunehmen.
Hintergrund
Persistierende chronische Schmerzen führen zu kortikalen Veränderungen in Arealen, die an der Emotionserkennung beteiligt sind. Wand et al. belegen einen engen Zusammenhang zwischen der affektiven Schmerzkomponente und dem Ausmaß dieser Veränderungen. In der Folge kann sich das Emotionsprofil verändern und es können Schwierigkeiten in der emotionalen Kommunikation auftreten.
Material und Methoden
49 Patienten mit „chronic low back pain“ (CLBP) wurden mithilfe der Graded Chronic Pain Scale (GCPS) nach GCPS-Grad 1 + 2 und GCPS-Grad 3 + 4 unterteilt. Bei allen Patienten wurde die Fähigkeit zur Erkennung mimisch codierter Basisemotionen mit dem Facially-Expressed-Emotion-Labeling-Test (FEEL) untersucht. Zudem wurde mit der Toronto-Alexithymie-Skala-26 (TAS-26) überprüft, ob die Patienten alexithyme Merkmale zeigten. Eine differenzierte Auswertung fand für die Gruppe mit GCPS-Grad 3 + 4 (n = 35) statt.
Ergebnisse
Der FEEL-Test verdeutlichte, dass die Gruppe mit GCPS-Grad 3 + 4 die Basisemotion Überraschung signifikant häufiger (p = 0,001) als Gesunde (Vergleich mit Normwerten) erkannte und ein gesteigertes Niveau an momentan erlebtem Ärger zeigte. Nach der TAS-26 zeigten 28,5 % der CLBP-Patienten alexithyme Merkmale.
Schlussfolgerung
Die Ergebnisse offenbaren, dass Patienten mit chronifizierten lumbalen Rückenschmerzen Veränderungen in der Emotionserkennung sowie alexithyme Merkmale zeigen. Zur Bestätigung dieser Ergebnisse sowie zur Absicherung der erkannten Tendenzen sind weitere Studien mit einer größeren Stichprobe nötig.
Zielsetzung
Bis dato konnte keine Studie eine Veränderung der Druckempfindlichkeit der myofaszialen Tender- oder Triggerpunkte (MTrP) bei Patienten mit kraniomandibulärer Dysfunktion (CMD) im extratrigeminalen Versorgungsgebiet nachweisen. Ziel dieser Studie war herauszufinden, ob die Druckempfindlichkeit der MTrP bei CMD-Patienten sowohl im trigeminalen als auch im extratrigeminalen Versorgungsgebiet im Vergleich zu gesunden Kontrollen erhöht ist. Außerdem sollte untersucht werden, inwieweit MTrP als Marker geeignet sind, eine Hyperalgesie bei CMD-Patienten zu diagnostizieren.
Probanden und Methoden
Für die Studie wurden 34 CMD-Patienten und 30 Probanden ohne CMD rekrutiert. In beiden Gruppen wurde die mechanische Schmerzschwelle an trigeminalen und extratrigeminalen MTrP gemessen und verglichen. Zusätzlich wurde mittels ROC-Kurve untersucht, welcher Marker am besten geeignet ist, CMD-Patienten als hyperalgetisch zu klassifizieren.
Ergebnis
Die Druckempfindlichkeit aller MTrP der trigeminalen und extratrigeminalen Körperregionen war in der CMD-Gruppe signifikant erhöht. Selbst nach Korrektur für multiples Testen blieben die meisten Effekte signifikant. Von den untersuchten MTrP war der M. trapezius am besten geeignet, CMD-Patienten als hyperalgetisch zu klassifizieren. Bei einer falsch-positiven Rate <5 % wurden ca. 42 % der CMD-Patienten als hyperalgetisch klassifiziert.
Schlussfolgerung
Die signifikanten Unterschiede der Druckempfindlichkeit der MTrP in trigeminalen und extratrigeminalen Körperregionen lassen auf eine Hyperalgesie bei CMD-Patienten schließen. Diese kann möglicherweise mit dem Marker des M. trapezius diagnostiziert werden. Hierzu sind jedoch weitere Studien notwendig, die u. a. geschlechts- und altersspezifische Referenzwerte ermitteln.
Objective
The purpose of this case report is to describe the use of mobilization and eccentric exercise training for a patient with ankle pain and a history of chronic ankle sprains and discuss the course of diagnostic decision making when the patient did not respond to care.
Clinical Features
A 48-year-old police officer who had sustained multiple ankle sprains throughout his life presented with pain and restriction in his ability to walk, run, and work. The Global Rating of Change Scale score was − 6, the Numeric Pain Rating Scale score was 7/10, and the Lower Extremity Functional Scale score was − 33. Palpation of the peroneus longus and brevis muscles and inversion with overpressure reproduced the chief concern (Numeric Pain Rating Scale 7/10). The patient was initially diagnosed with chronic peroneal tendinopathy.
Intervention and Outcome
Treatment included lateral translation mobilization of the talocrural joint combined with eccentric exercise using an elastic band for the peroneal muscles. The patient reported improvement in pain and function during the course of intervention but not as rapidly as expected. Therefore, follow-up ultrasonographic imaging and radiography were performed. These studies revealed partial rupture of the peroneal brevis muscle and total rupture of the peroneal longus muscle.
Conclusion
A patient with long-term concerns of the foot complex with a diagnosis of peroneal tendinopathy showed slight improvement with eccentric exercises combined with manual therapy of the talocrural joint. After a course of treatment but minimal response, a diagnosis of tendon rupture was confirmed with diagnostic ultrasonography. Clinicians should be aware that when injuries do not improve with care, tendon rupture should be considered.

Chronic facial pain has many of the clinical characteristics found in other persistent musculoskeletal conditions, such as low back and cervical pain syndromes. Unique to this condition, however, is that painful facial movements may result in rigidity or altered ability to demonstrate mimicry, defined as the natural tendency to adopt the behavioral expressions of other persons involved in the interaction. Loss of ability to communicate through emotional expression can lead to impaired processing of emotions and ultimately social isolation. Diminished quality and quantity of facial expression is associated with chronic face pain, tempromandibular dysfunction, facial asymmetries, and neurological disorders. This report provides a framework for assessment of impaired emotional processing and associated somatosensory alterations. Principles for management for chronic facial pain should include graded motor imagery, in addition to standard treatments of manual therapy, exercise, and patient education. A case study is provided which illustrates these principles.
Die Behandlung des Schädels ist derzeit ein sehr schnell wachsendes Gebiet im Bereich des Assessments und der neuromuskuloskeletalen Behandlung. Ziel dieses systematischen Literaturreviews war es, eine Übersicht zu Studien bezüglich der Wirkung von passiven Techniken auf das Kranium zu geben.
Bei den 37 gefundenen Studien zu kieferorthopädischer Schienen-, kraniosakraler oder Manueller Therapie als passive Maßnahmen waren kraniomandibuläre Dysfunktionen das am häufigsten vorkommende Krankheitsbild.
Hinsichtlich der Wirkung der unterschiedlichen Therapieansätze unter anderem auf Kopfschmerzen ebenso wie auf psychische Probleme ergab sich für alle Behandlungstechniken nur geringe Evidenz.
Insgesamt lässt die derzeitige Studienlage keine eindeutige Aussage zur Effektivität der untersuchten Maßnahmen zu.
Zervikogene Kopfschmerzen werden als durch Dysfunktionen in der hochzervikalen Wirbelsäule verursachte Kopfschmerzen beschrieben. Einige medizinische Disziplinen betrachten diese Kopfschmerzform aufgrund unzureichender pathobiologischer Erklärungsmodelle kritisch oder halten sie teilweise sogar für nicht existent, während sie die neuromuskeloskeletale Therapie als eigenständige Entität anerkennt.
Anhand einer systematischen Literaturrecherche reflektiert die vorliegende Arbeit sowohl die gängigen Diagnosekriterien als auch die Unterschiede und Überlappungen von zervikogenem Kopfschmerz zu Migräne ohne Aura bzw. Spannungskopfschmerz unter Einbeziehung des tatsächlich vorherrschenden pathobiologischen Mechanismus.
Mit der Überlegung peripherer und zentraler Sensibilisierungsprozesse zeigten sich deutliche Überschneidungen im Bereich der pathobiologischen Mechanismen von zervikogenem Kopfschmerz, Migräne ohne Aura und Spannungskopfschmerz. Daher sollten die Diagnosekriterien um diesen Hintergrund erweitert bzw. angepasst werden. Aus manualtherapeutischer Sicht ergibt sich die mögliche Behandlung dieser Kopfschmerzarten nach eingehender struktureller Untersuchung und Screening angrenzender Faktoren unter Beachtung der zugrundeliegenden Schmerzmechanismen.

Sprunggelenksverletzungen (SGV) sind die häufigsten Verletzungen des muskuloskeletalen Systems. Neben Schmerz, Schwellung und Funktionseinschränkung werden Zusammenhänge zwischen einem Sprunggelenkstrauma und Veränderungen am Becken bzw. Sakroiliakalgelenk (SIG) diskutiert. In der vorliegenden Studie wird geprüft, ob Wechselwirkungen von SGV und Veränderungen am Becken bzw. SIG bestehen.
Material und Methoden
In dieser Querschnittsstudie ohne Verblindung wurden 18 Probanden mit SGV und 22 gesunde Probanden am Becken und SIG untersucht. Der Zustand nach der SGV wurde anhand des FAAM-G-Fragebogens ermittelt. Die Evaluation der Beckenposition erfolgte mit Photometrie. Dabei wurden die Referenzpunkte SIAS und SIPS zueinander verglichen. Am SIG erfolgten Schmerzprovokationstests, um Veränderungen am SIG zu ermitteln. Die in beiden Gruppen erhobenen Daten wurden statistisch ausgewertet und verglichen.
Ergebnisse
Der funktionelle Zustand der Sprunggelenke unterschied sich zwischen der Kontrollgruppe und der Experimentalgruppe signifikant. Die Unterschiede bei den photometrischen Ergebnissen waren für die Beckensymmetrie nicht signifikant (SIAS p = 0,426; SIPS p = 0,779). Hinsichtlich der Schmerzhaftigkeit des SIG zeigte sich ebenfalls kein signifikanter Unterschied (p = 0,477).
Schlussfolgerung
Es konnten keine Positionsveränderungen des Beckens infolge eines Sprunggelenktraumas beobachtet werden. Auch zeigten sich keine Assoziationen zwischen SGV und Becken- bzw. SIG-Position.
There is clinical evidence that cervical lateral glide (CLG) improves neurodynamics and alleviates pain in patients who suffer from neurogenic arm pain. Cervical lateral flexion (CLF) is also a treatment method and a means of testing neurodynamics. However, for both techniques nerve movement has not yet been investigated using ultrasound imaging (US). The purpose of this study was to quantify median nerve movement in the arm during CLG and CLF. For this study 27 healthy participants were recruited. Longitudinal movement of the median nerve was measured using US during CLG and CLF with the shoulder in 30° abduction in the middle and distal forearm (Fad). Data could be obtained from 11 participants (6 women and 5 men, average age 25.6 years, ±2.25) at the middle forearm (Fam) and from 9 participants (5 women and 4 men, average age 27.2 years, ±2.75) at the Fad. When applying CLF, the median nerve moved 2.3 mm (SEM ± 0.1 mm) at the Fam. At the same measuring point the median nerve moved 3.3 mm (SEM ± 0.3 mm, p = 0.005) by applying CLG. At the Fad the difference between CLF and CLF amounted to 0.6 mm (CLF: 1.9 mm (SEM ± 0.2 mm, CLG: 2.5 mm (SEM ± 0.2 mm, p ≤ 0.05). The movements during CLG are larger than during CLF. This difference is statistically significant. However, the statistical relevance cannot be extrapolated to a clinical relevance.

Pediatric headache is an increasingly reported phenomenon. Cervicogenic headache (CGH) is a subgroup of headache, but there is limited information about cervical spine physical examination signs in children with CGH. Therefore, a cross-sectional study was designed to investigate cervical spine physical examination signs including active range of motion (ROM), posture determined by the craniovertebral angle (CVA), and upper cervical ROM determined by the flexion–rotation test (FRT) in children aged between 6 and 12 years. An additional purpose was to determine the degree of pain provoked by the FRT. Thirty children (mean age 120.70 months [SD 15.14]) with features of CGH and 34 (mean age 125.38 months [13.14]) age-matched asymptomatic controls participated in the study. When compared to asymptomatic controls, symptomatic children had a significantly smaller CVA (p < 0.001), significantly less active ROM in all cardinal planes (p < 0.001), and significantly less ROM during the FRT (p < 0.001), especially towards the dominant headache side (p < 0.001). In addition, symptomatic subjects reported more pain during the FRT (p < 0.001) and there was a significant negative correlation (r = −0.758, p < 0.001) between the range recorded during the FRT towards the dominant headache side and FRT pain intensity score. This study found evidence of impaired function of the upper cervical spine in children with CGH and provides evidence of the clinical utility of the FRT when examining children with CGH.
There is evidence that temporomandibular disorder (TMD) may be a contributing factor to cervicogenic headache (CGH), in part because of the influence of dysfunction of the temporomandibular joint on the cervical spine. The purpose of this randomized controlled trial was to determine whether orofacial treatment in addition to cervical manual therapy, was more effective than cervical manual therapy alone on measures of cervical movement impairment in patients with features of CGH and signs of TMD. In this study, 43 patients (27 women) with headache for more than 3-months and with some features of CGH and signs of TMD were randomly assigned to receive either cervical manual therapy (usual care) or orofacial manual therapy to address TMD in addition to usual care. Subjects were assessed at baseline, after 6 treatment sessions (3-months), and at 6-months follow-up. 38 subjects (25 female) completed all analysis at 6-months follow-up. The outcome criteria were: cervical range of movement (including the C1-2 flexion-rotation test) and manual examination of the upper 3 cervical vertebra. The group that received orofacial treatment in addition to usual care showed significant reduction in all aspects of cervical impairment after the treatment period. These improvements persisted to the 6-month follow-up, but were not observed in the usual care group at any point. These observations together with previous reports indicate that manual therapists should look for features of TMD when examining patients with headache, particularly if treatment fails when directed to the cervical spine.
Die Zahl der Migräneerkrankungen im Kindesalter nimmt stetig zu. Die betroffenen Kinder und ihre Angehörigen sind einem großen Leidensdruck ausgesetzt. Das klinische Bild der kindlichen Migräne ist bisher jedoch nicht umfassend erforscht, was zu Einschränkungen hinsichtlich der Diagnose- und Therapiemöglichkeiten führt.
Die vorliegende Querschnittstudie untersuchte den Einfluss neuromuskuloskeletaler Faktoren auf die kindliche Migräne aus dem Blickwinkel der manualtherapeutischen Untersuchung und Behandlung.
Beim Vergleich von Kindern mit Migräne und einer symptomfreien Kontrollgruppe (Schmerzintensität, kraniozervikaler Winkel, aktives physiologisches Bewegungsausmaß der HWS, Druckschmerzschwelle, Lageschwindel) fanden sich signifikante Unterschiede bezüglich der Schmerzintensität und dem kraniozervikalen Winkel.
