Im Rahmen dieser Arbeit sollen anhand der PEFA Bewertung, welche im Jahr 2018 stattfand
und die Qualität des öffentlichen Finanzmanagements der Stadt Osnabrück auf Grundlage von internationalen Standards darstellt, Handlungsempfehlungen zur Verbesserung der Transparenz erarbeitet werden. Die entwickelten Maßnahmen sollen den Grundsätzen der Open Government Partnership entsprechen und die Weiterentwicklung der Stadt Osnabrück,
insbesondere im Bereich der Transparenz, unterstützen. Sie sind somit dem übergeordneten
Begriff Open Data zuzuordnen. Um Handlungsempfehlungen zu erarbeiten, werden aus den insgesamt 31 Leistungsindikatoren mit den verschiedenen Dimensionen, die bewertet wurden, diejenigen ausgesucht, welche schlechter als A bewertet wurden. Die gesamten 31 Leistungsindikatoren wurden im Jahr 2018 von A bis D auf einer vierstufigen Ordinalskala bemessen. Anhand der Leistungsindikatoren, die Verbesserungsbedarf aufweisen, werden Maßnahmen entwickelt, die sowohl die Bewertung der Stadt Osnabrück in der PEFA-Analyse verbessern, als auch die Transparenz, das Bürgerengagement und die Modernisierung der Verwaltung im Mindesten unterstützen, wenn nicht sogar verbessern.
Im Zuge der Digitalisierung ist das Interesse an offenen Daten und Informationen enorm gestiegen. Für die öffentliche Verwaltung ist das eine große Chance, die genutzt werden sollte, um Transparenz, Beteiligung und Zusammenarbeit mit der Öffentlichkeit zu fördern und der Rechenschaftspflicht nachzukommen. Die Daten werden als Open Data uneinge-schränkt, kostenlos und zur freien Weiterverwendung zur Verfügung gestellt. Im Rahmen einer Dokumentenanalyse und persönlicher Interviews mit ausgewählten Mitarbeitern des Landkreises Osnabrück sollen Potenziale und Herausforderungen der Kreisverwaltung hin-sichtlich Open Data und der Einführung eines Open Data-Portals aufgezeigt werden. Die australische Hauptstadt Canberra wird als Best-Practice-Beispiel für die erfolgreiche Um-setzung von Open Data und eines Open Data-Portals angeführt. Aus den Untersuchungen ergeben sich vielfältige Handlungsmöglichkeiten und -empfehlungen, das kommunale Ver-waltungshandeln offener und transparenter zu gestalten. So sollen die Bürgerbeteiligung und die Zusammenarbeit mit der Öffentlichkeit verbessert und gleichzeitig dem politischen Auftrag im Sinne der Rechenschaftspflicht nachgekommen werden.
Mit der Akademisierung von praktischen Gesundheitsberufen stehen die Hochschulen und Universitäten gleichermaßen vor der Herausforderung, Pflege- und Therapiekräfte für ein noch nicht definiertes Berufsfeld zu qualifizieren. Die Wissenschaft hat die Fragen nach den genauen Inhalten und der Abgrenzung akademisierter Gesundheitsberufe zu anderen, seit langem etablierten Tätigkeitsbereichen in der Patientenversorgung zu beantworten und weiterführend zukünftigen Entwicklungen aufzuzeigen. Hier erwächst der Bedarf nach einer weitergehenden Arbeitsforschung in den akademisierten, praktischen Gesundheitsberufen. Der in diesem Beitrag vorgestellte Ansatz kombiniert die Weiterqualifizierung mit der Arbeitsforschung im Berufsfeld. Ziel ist es, Weiterbildungsmodule zu konzipieren, bei denen das praktische Arbeitsfeld mit den jeweiligen Inhalten der Module systematisch in Bezug gesetzt wird. Grundlage hierzu sind didaktische Ansätze der Theorie-Praxis-Relation aus dualen Studiengängen. Zentrales Instrument ist eine reflexionsorientierte Fallstudie, die von den Teilnehmern der Weiterbildung für jedes Modul erstellt wird, in der die Teilnehmer theoriebasierte Inhalte des Moduls aufgreifen und dazu nutzen, ihr jeweiliges praktisches Arbeitsfeld mit der fachlichen Perspektive des Moduls zu reflektieren. Die Teilnehmer werden so zu Praxisforschern und die Weiterbildung gleichzeitig zu einer wissenschaftlichen Berufsfeldreflexion, aus der die Praxis Impulse zur Weiterentwicklung erhält.
Emotionale Bewertungen stellen ein zentrales Element der Nutzungserfahrung dar. Aus diesem Grund untersucht die vorliegende Bachelorarbeit, ob eine hohe Nutzungshäufigkeit von Mobiltelefon oder Sprachsteuerung einen Einfluss auf die emotionalen Aspekte des Benutzererlebens hat. Zu diesem Zweck wurde eine Online-Befragung (N= 836) durchgeführt und mittels Korrelationen, hierarchischen Regressionsanalysen, multivariaten und univariaten Varianzanalysen sowie kanonischen Diskriminanzfunktionen ausgewertet. Die Ergebnisse offenbarten, dass sich Nutzungshäufigkeiten unterschiedlich auf positive und negative Emotionen auswirken. Personen, die ihr Mobiltelefon häufig verwendeten, erfuhren mehr positive Emotionen gegenüber dem Gerät. Negative Emotionen gegenüber dem Mobiltelefon konnten hingegen nicht durch eine häufige Nutzung reduziert werden. Dies konnte unabhängig von dem Ausmaß, in welchem das Mobiltelefon zur zwischenmenschlichen Kommunikation verwendet wurde, beobachtet werden und wurde nicht durch die Nutzungshäufigkeit der Sprachsteuerung moderiert. Dennoch zeigte sich, dass die Nutzungshäufigkeit der Sprachsteuerung sowohl die positiven als auch eingeschränkt die negativen Emotionen gegenüber dem Mobiltelefon beeinflusst, was vermutlich auf eine größere Gesamtnutzung des Mobiltelefons zurückzuführen ist. Insgesamt bestätigt die Studie die Übertragung von Herzbergs Zwei-Faktoren-Theorie auf das Nutzungserleben nur teilweise. Positive Emotionen scheinen einen stärkeren Bezug zum Benutzungserlebnis aufzuweisen, während negative Emotionen wahrscheinlich durch schlechte Gebrauchstauglichkeit hervorgerufen werden. Als Folge dessen wirken sich Nutzungshäufigkeiten vorwiegend auf positive Emotionen aus, doch die gemeinsame Betrachtung von positiven und negativen Emotionen kann einen Unterschied in Bezug auf die Ausprägung negativer Emotionen gegenüber dem Mobiltelefon machen, wie sich bei der Nutzungshäufigkeit der Sprachsteuerung herausstellte.
background: Musculoskeletal problems (MP) are widespread in performing artists and are due to the special demands of instrument playing, singing or dancing. Additionally, various other factors might contribute to performance-related MP. To provide a specific physiotherapeutic management for performing artists it is important to gain information about the performing art, the individual demands and contributing factors. The subjective examination (SE) is the basis of the clinical reasoning process and the hypothesis forming for further clinical examination and biomechanical analysis. In the present protocol, the SE consists of a questionnaire-based section and an interview-based section and is part of the evaluation process of the neuromusculoskeletal examination of a performing artist specific reference laboratory.
purpose: To develop a standardized SE protocol divided into a questionnaire-based section (Part 1) via web application and an interview-based SE (Part 2) to address MP of performing artists.
methods: The questionnaires for part 1 were selected based on the expertise of the research group and the psychometric properties of each possible questionnaire. A common physiotherapeutic recording of findings which addresses the relevant questions of the SE of MP was used. To adapt the anamnesis to performing-associated MP questions specific to instrument playing, singing and dancing were selected on the basis of a literature search and the expertise of the research group.
summary of content/results: Part 1 consists of three topics. (1) information about the performing art, professional level and sociodemographic data, (2) information about pain and pain processing, and (3) the anatomical location of the main MP including a body region-specific questionnaire. Part 2 is based on the five aspects of clinical practice described by Maitland and the Musculoskeletal Clinical Translation Framework by Mitchell et al. The performing arts specific part is particularly focused on performing art specific considerations, physical and psychosocial contributing factors and art-specific activities.

Background: Musculoskeletal problems (MP) are widespread in performing artists and are due to the special demands of instrument playing, singing or dancing. The evaluation covers 4 steps: a subjective examination (SE) including (1) a questionnaire-based online survey and (2) an interview-based anamnesis. On the basis of the results of the SE, hypotheses are formed for (3) an individual musculoskeletal clinical examination and a (4) biomechanical analysis. Here, the focus is on the clinical examination.
Purpose: to develop a standardized protocol for a clinical examination addressing especially musculoskeletal problems in performing artists.
Methods: A common physiotherapeutic clinical examination should be supplemented with techniques, which are specific to performance-related musculoskeletal problems and/or their risk factors. The development was based on a literature search and the clinical expertise of the physiotherapeutic research group.
Summary of content/Results: The performing arts specific clinical examination includes the common analysis of posture as well as passive/active movement capacities and specific differentiating tests in relation to the signs and symptoms of the individual artist. The examination is supplemented by a functional demonstration focusing on the special demands of playing the respective instrument, while singing or dancing. Common overuse risk factors like hypermobility, special anthropometric data or an examination of the motor control of different body regions are addressed. Optional, the various parts of the examination process could be skipped, if not relevant for the individual artist.
Significance: This clinical examination protocol should help clinicians evaluating the musculoskeletal health of performing artists in a standardized and specific way.
Problem-based learning (PBL) has become established as a successful didactic approach far beyond the field of medicine. Although there is no single concept of PBL, there is agreement on its objectives and implementation. Of central importance is the case that supports autonomous and reflective learning. Even before COVID-19, digital methods were used in traditional PBL. These served to support, for example, the provision of learning materials. As a result of university closures during the COVID-19 pandemic, technical solutions were made available at an unprecedented speed, which made it possible to implement the different requirements of traditional PBL in a digital PBL (DPBL). The present study results based on two scoping reviews demonstrated that PBL can be implemented digitally and that different digital methods, both asynchronous and synchronous, are available for the different steps. They show that DPBL not only leads to comparable student performance, but can also develop further competences, e.g. digital communication. With the findings, a concept for the implementation of DPBL as well as recommendations for the further development of DPBL are available.
Hypothesis/Aims of study
Many results related to the effectiveness of surgical and non-surgical proce- dures for treating urinary incontinence (UI) are reported in the literature. Following the principles of evidence based-practice, besides the interpreta- tion of study results based on statistical significance, authors should consid- er evaluating the clinical relevance of treatment effects in this field.
The minimal important difference (MID) of clinical outcomes could be used to assess the clinical relevance of interventions. MID is defined as “the small- est difference in score in the domain of interest that patients perceive as important, either beneficial or harmful, and which would lead the clinician to consider a change in the patient’s management’’[1]. One common way to obtain MID for outcomes of interest is by using anchor-based methods. These methods apply one anchor that analyzes the change in the patient's health status according to the patient’s perception.
However, MIDs should be provided according to appropriate calculations and methods and based on the definition of a MID. However, there are a lot of misconceptions and misunderstandings related to the MID. These mis- understandings have led to incorrect reports of these values. Moreover, it is still not known which criteria the authors considered during the analysis of the MID in the Women’s Health area. Therefore, with this preliminary re- port, we aimed to identify and report all anchor-based methods to estimate MIDs for outcomes measures related to UI available in the literature; and analyze which concepts and levels of improvement in the health status of the patient have been considered by the authors to calculate the MID.
Study design, materials and methods
This systematic review was conducted according to PRISMA guide- lines. The study protocol was registered in the PROSPERO database (CRD42022299686). A systematic search was performed using Ovid Med- line, Embase, Web of Science, and Scopus from May to June 2021. Any study generating MIDs for UI that included women with more than 18 years, stress urinary incontinence (SUI), urgency urinary incontinence (UUI) and/ or miXed urinary incontinence (MUI) was included. The primary outcome was the MID for outcomes related to UI. No limits were applied on the data- bases for the date, language or publication range.
Studies were classified into three categories according to the level of im- provement in health status assessed by the anchor and considered by the authors during the MID calculation: 1) slight improvement: if authors in- cluded participants that evaluated their health status as “a little better” in their analysis; 2) moderate improvement: if authors considered women that reported a “better” or a “much better” status of the condition; or 3) strong imporvement: if all patients that improved (“very much better” or if authors grouped all the patients that improved in one single category) were consid- ered in one group against other group that did not report any improvement. After classifying the papers, we counted and reported how many studies were considering only the minimal level of improvement to reported the MID, according to previous definition and recommendation.
The initial electronic search resulted in a total of 1,662. After removing du- plicates (n=719), 943 were screened, and at the end of the selection stages, nine papers that reported anchor-analysis were included in this preliminary report. Seven studies included women with SUI (total sample size= 2,436), while one study included only women with UUI (n=307), and the other one evaluated women with SUI and MUI (n=288). SiX studies analyzed data and provided the MID after a non-surgical treatment of UI, while three analyzed the results after surgery to correct UI. Eleven different questionnaires to measure the patient-related outcomes related to UI with their MIDs were identified. All the tools were related to measuring the impact, distress, or quality of life of women with UI.
Different anchors were used to analyze MID, including scales that evaluated the improvement and satisfaction of the patient, and the visual analogue scale, measures of urinary leakage and questionnaires that measure the se- verity and impact of UI. The MID of siX tools was determined according to the smallest difference detected by the patients, using the Patient Global Im- pression of Improvement questionnaire and the self-reported satisfaction to assess the change of the condition. Most of the MIDs (n=28, 80%) were mis- calculated considering a moderate or a strong improvement of the patients, and not a minimal improvement as suggested by the literature (Table 1).
Interpretation of results
Although previous systematic reviews have reported the psychometric prop- erties of different questionnaires to measure UI outcomes, this is the first study to analyze methods of obtaining MIDs for UI outcomes from the pa- tients perspective (anchor based methods). All the tools with their respec- tives MIDs were related to the impact, distress, and/or quality of life of women with UI. The use of these outcomes measures is in line with the as- sociated impairments of social, psychological, financial, and sexual aspects of a women’s life produced by UI.
Most of the authors in this field did not consider the smallest difference identified by the participants to calculate the MID, which does not follow the original definition of MID proposed by Jaeschke et al.,1 This could gen- erate underestimation or over-estimation of MID, which may directly im- pact the interpretation of the findings from the clinical trials[2] and biased interpretation of the results of the clinical significance from the interven- tions used to manage female UI. Therefore, the interpretation of the clinical significance related to UI outcomes should be done with caution.
Concluding message
Few studies that aimed to calculate the MID using anchor-based methods for outcomes related to female UI were found in the literature. Eleven different questionnaires to measure the outcomes related to UI with their MIDs were identified. However, most studies had not considered the smallest change of improvement (as perceived by the patients) in their analysis, which does not follow the definition of the MID. This could impact decision making. Future research should provide clear guidelines on how to calculate, report, and interpret MIDs in this field.
In der öffentlichen Wahrnehmung gilt die Universität Duisburg-Essen als Best-practice-Beispiel für das Deutschlandstipendien-Programm, das seit dem Jahr 2011 bundesweit leistungsstarke Studierende fördert und auf Basis des nahezu baugleichen NRW-Stipendiums entwickelt wurde. Zur Finanzierung der Stipendienprogramme dienen Reverse Matching Funds-Konstruktionen, innerhalb derer die durch private Förderer zur Verfügung gestellten Spenden durch Staatsmittel bis zu einer festgelegten Obergrenze verdoppelt werden. Die Hochschulen werden dadurch motiviert, stärker in das in Deutschland gegenüber dem internationalen Vergleich bisher stark unterentwickelte Hochschulfundraising zu investieren, um die benötigten privaten Förderer zu akquirieren und an die Hochschule zu binden. Als Finanzierungs- und Steuerungsinstrument ist das Matching Funds-Stipendienprogramm seiner Architektur nach ein Hybridwesen aus Elementen des Hochschulfundraisings und von Public Private Partnerships, zu denen beiden es bereits zahlreiche wissenschaftliche Untersuchungen gibt - anders als zum Phänomen der voraussetzungsreichen Matching Funds.
Die vorliegende Studie erörtert zunächst die Finanzierung staatlicher Hochschulen in Deutschland, um die drei Finanzierungsinstrumente Fundraising, Public Private Partnership und Matching Funds darin einordnen zu können. Die drei Instrumente werden umfangreich beschrieben und analysiert, um aufgrund des bestehenden Forschungsdesiderats zu Matching Funds einen selbstentwickelten, geschlossenen Kriterien-Katalog aufzustellen, der als Bewertungsraster der Untersuchung dienen soll. Der Darstellung der Matching Funds-Architektur des Stipendienprogramms und seiner konkreten Umsetzung an der Universität Duisburg-Essen als Fallbeispiel folgt eine kritische Analyse und Bewertung sowohl der Matching Funds-Konstruktion als solcher, als auch des vermeintlichen Best-practice-Beispiels.
Führungskräfte in Nonprofit-Organisationen befinden sich in sich stetig wandelnden Handlungsfeldern. Damit sie in dieser Komplexität konstruktiv agieren können, sind Kompetenzen erforderlich: Ganz zentral ist die Fähigkeit, sich initiativ und eigenverantwortlich mit den Herausforderungen unserer Zeit auseinanderzusetzen und im besten Fall geeignete Lösungen dafür zu finden sowie diese zielführend kommunizieren zu können. Aus diesem Grund wird genau diese Fähigkeit bei Studierenden aus den Masterstudiengängen Management in Nonprofit-Organisationen und Soziale Arbeit der Hochschule Osnabrück gefördert.
Im Rahmen des Moduls Handlungsfelder II entwickelten rund 30 Studierende im Wintersemester 2021/2022 in einer Denkwerkstatt Forschungsergebnisse und Lösungsideen zum Thema „Osnabrück 2030 – Nachhaltigkeit“. Die Studierenden wählten in einem partizipativen Prozess ihre eigenen Schwerpunktthemen wie zum Beispiel Mobilität oder Bildung aus und arbeiteten dann ein Semester lang an den Inhalten. Bei der Erarbeitung wurden sie durch Marlene Eimterbäumer begleitet, die Modelle, Methoden und Coaching zur Unterstützung bereit stellte. Die Modelle und Methoden finden sich teilweise in den Beiträgen der Studierenden wieder (unter anderem das Socio Ecological Model und der Business Model Canvas).
Am Ende des Semesters stand eine Präsentation vor den Kommiliton*innen, der Lehrenden und auch vor externen Gästen, die aus unterschiedlichen Expertisegebieten im Themenbereich Nachhaltigkeit kamen und dementsprechend Feedback aus der Praxis geben konnten. Die Entwicklung des Moduls erfolgte durch Gesa Birnkraut und Marlene Eimterbäumer und wurde von der Hochschule im Rahmen der Innovativen Lehre an der Fakultät Wirtschafts- und Sozialwissenschaften gefördert. Für die Studierenden stellte das Modul durchaus eine Herausforderung dar, denn in der Denkwerkstatt musste unter hoher Komplexität stark prozessbezogen gearbeitet werden im Gegensatz zu der sonstigen hohen Ergebnisorientierung. Die durchweg sehr guten Ergebnisse zeigen, dass der Einsatz und die kontinuierliche Kommunikation und Zusammenarbeit gelohnt haben.
Aufgeteilt ist das vorliegende Buch in die fünf Themen: Mobilität, nachhaltige Beschaffung, nachhaltige Bildung, Stadtplanung und Wirtschaft. Zu diesen Themen finden Sie jeweils erst einen Beitrag der Forscher*innen und darauffolgend einen Beitrag zur Darstellung einer Lösungsidee.
Mit der Öffnung der Hochschulen rücken beruflich vorgebildete Personen aus dem Berufsfeld der Gesundheitsversorgung auch ohne formale Hochschulzugangsberechtigung in den Kreis der Zielgruppen der Hochschulbildung. Diese Personen wählen wissenschaftliche Zertifikatsangebote im Übergang vom beruflichen zum hochschulischen Lernen, um auf dem aktuellsten Stand des Wissens weitergebildet zu werden. Im Rahmen der zeitlich kleineren Bildungseinheiten der Zertifikatsangebote streben sie mit Blick auf zukünftige Herausforderungen im Gesundheitswesen danach, einen wissenschaftlich fundierten Kompetenzaufbau und -ausbau in einem klar definierten Themenfeld zu erhalten, ohne dafür zwangsläufig ein vollständiges Studium absolvieren zu müssen.
Die Zielgruppe dieser nicht-traditionell Studierenden ist durch eine starke Heterogenität u.a. hinsichtlich Alter, Berufsbiografie und Familienverpflichtungen geprägt. Die besondere Angebots- und Organisationsstruktur der wissenschaftlichen Zertifikatsangebote ist auf diese Zielgruppe ausgerichtet. Begleitend zu der didaktischen Ausgestaltung des Curriculums sowie der methodischen Vernetzung von Theorie und Praxis ist die Wertschätzung und Validierung bereits beruflich erworbener Kompetenzen ein Anliegen der Zielgruppe. Insofern erweisen sich gezielte Beratungs- und Unterstützungsmaßnahmen, eine zentrierte Förderung der Selbstreflexionsfähigkeit sowie eine individuelle Lern(fortschritts)begleitung zur aktiven Teilnahme an der wissenschaftlichen Weiterbildung als hilfreich. In diesem Beitrag werden Erfahrungen aus pilotierten Zertifikatsangeboten der wissenschaftlichen Weiterbildung und empirische Erkenntnisse aus dem BMBF-geförderten Projekt KeGL vorgelegt.
Objectives
To develop a time-efficient motor control (MC) test battery while maximising diagnostic accuracy of both a two-level and three-level classification system for patients with non-specific low back pain (LBP).
Design
Case–control study.
Setting
Four private physiotherapy practices in northern Germany.
Participants
Consecutive males and females presenting to a physiotherapy clinic with non-specific LBP (n=65) were compared with 66 healthy-matched controls.
Primary outcome measures
Accuracy (sensitivity, specificity, Youden index, positive/negative likelihood ratio, area under the curve (AUC)) of a clinically driven consensus-based test battery including the ideal number of test items as well as threshold values and most accurate items.
Results
For both the two and three-level categorisation system, the ideal number of test items was 10. With increasing number of failed tests, the probability of having LBP increases. The overall discrimination potential for the two-level categorisation system of the test is good (AUC=0.85) with an optimal cut-off of three failed tests. The overall discrimination potential of the three-level categorisation system is fair (volume under the surface=0.52). The optimal cut-off for the 10-item test battery for categorisation into none, mild/moderate and severe MC impairment is three and six failed tests, respectively.
Conclusion
A 10-item test battery is recommended for both the two-level (impairment or not) and three-level (none, mild, moderate/severe) categorisation of patients with non-specific LBP.
Der Beitrag analysiert den Umgang mit der Covid-19-Pandemie innerhalb der Krankenhäuser und leitet davon Handlungsempfehlungen für das Krankenhausmanagement ab. Dabei geht es sowohl um die Organisation und Ausgestaltung der Patienten- und Personalsteuerung (vorgelagerte Notaufnahmen, Aufnahmestationen, Triage, Covid-19-Testung etc.) als auch um die sich durch die Kontaktbeschränkung ergebene Herausforderung im Umgang mit Besuchern und Lieferanten. Des Weiteren wird auch die betriebswirtschaftliche Steuerung in einer – in allen Belangen – ungewohnten Situation beleuchtet.
Aims and Objectives:
Preventive home visits are a low-threshold counselling and support approach. They have been reported to achieve heterogeneous effects. However, preventive home visits have the potential to reduce the risk of becoming dependent on long-term care. The aim of this study is to investigate the effect of preventive home visits as a nursing intervention on health-related quality of life of older people in a longitudinal survey and to develop recommendations for which target groups preventive home visits have the highest benefit. The sample consisted of 75 people, aged between 65 and 85, who were able to understand and speak German, had not yet been eligible for benefits from the long-term care insurance and lived in the municipality under study.
Methodological Design and Justification:
A quantitative longitudinal study in order to investigate the effects of preventive home visits.
Ethical Issues and Approval:
There were no ethical concerns. Accordingly, ethical approval was granted.
Research Methods, Results and Conclusions:
The health-related quality of life was recorded four times between 01/2017 and 08/2020 with the Short-Form- Health- Survey- 12 and analysed using descriptive statistics. Results reveal that the physical health status cannot be easily influenced over a short period of time. The main effect, however, is that preventive home visits have a significant positive effect on the mental health status. The main topics during the home visits were mobility, nutrition and social participation. Increased knowledge and motivation for preventive behaviour extended the autonomy of older people. Accordingly, preventive home visits can support a self-determined life in a familiar environment. The results of the present study show that preventive home visits as a nursing intervention in rural areas are successful. In Germany, preventive home visits have not yet been implemented on a regular basis. In order to do so, a general definition of the concept is needed. Preventive home visits should be officially included in the regular health care services in Germany.
Einleitung: Whiteboards können als ein Instrument des Lean Managements zur Steuerung der Verweildauer auf Stationen eingesetzt werden, um aktuelle Patienteninformationen zu bündeln und in regelmäßigen strukturierten sowie interdisziplinären Besprechungen die Patientenversorgung zu steuern, die interdisziplinäre Zusammenarbeit zu optimieren und das Entlassungsmanagement zu verbessern. Das Ziel dieser Studie bestand darin, zu untersuchen, inwiefern die Einführung von Whiteboards in zwei Kliniken mit einer Veränderung der Verweildauer einherging.
Methode: Um die Forschungsfrage zu beantworten, wurden retrospektive Zeitreihen aus den DRG-Routinedaten vor und nach Installation der Whiteboards aus den beiden Kliniken in einem Interrupted Time Series Design genutzt. In der einen Klinik (Chirurgie) lagen 3.734 Fälle für den Zeitraum von Januar 2018 bis Dezember 2019 und in der anderen Klinik (Innere Medizin) 54.049 Fälle für den Zeitraum Juli 2013 bis Dezember 2019 vor.
Ergebnisse: In dem gemittelten Vergleich der Verweildauer (relative Verweildauerabweichung pro DRG von dem jeweiligen Verweildauermittel) konnte in der ersten Klinik kein signifikanter Unterschied zwischen den Werten vor und nach Einführung des Boards festgestellt werden. Am zweiten Klinikum zeigte sich sogar im Vorher-Nachher-Vergleich eine signifikante Verschlechterung der Verweildauer. Eine deskriptive Zeitreihenanalyse vor und nach Einführung zeigte in beiden Kliniken, dass kurz nach der Einführung der Boards sich die Verweildauer verschlechterte, anschließend jedoch verbesserte, d.h. dass die Patienten durchschnittlich früher entlassen wurden. Dieser Unterschied ging jedoch im Zeitverlauf wieder zurück.
Diskussion: Zusammenfassend lässt sich festhalten, dass keine Verbesserung in der Verweildauer im Zuge der Nutzung der Whiteboards durch einen reinen Vorher-Nachher-Vergleich nachweisbar war. In der anschließenden Zeitreihenbetrachtung zeigten sich starke Schwankungen, die zunächst mit einer kurzzeitigen Verschlechterung der Verweildauer nach der Implementierung einhergingen und dann zu einer Verbesserung führten. Im Zeitverlauf verblasste der Unterschied jedoch, sodass die Patienten wieder später entlassen wurden. Methodisch zeigt sich, dass im Gegensatz zu der reinen Vorher-Nachher-Analyse erst eine Zeitreihenbetrachtung einen Einblick in das Geschehen und seine Variabilität lieferte. Für die Praxis ergeben sich folgende Implikationen: Whiteboards können als ein hilfreiches Instrument von Lean Management zur Verweildauersteuerung angesehen werden, wie die zwischenzeitlichen Verbesserungen nahelegen. Dies erfordert jedoch eine kontinuierliche, unter Einbezug der Mitarbeiter durchgeführte Pflege der Informationen und einen erkennbaren Mehrwert. Perspektivisch empfiehlt sich zudem eine Digitalisierung der Boards, um den Nachteilen wie der manuellen Pflege entgegenzuwirken.
Introduction Development and implementation of appropriate health policy is essential to address the rising global burden of non-communicable diseases (NCDs). The aim of this study was to evaluate existing health policies for integrated prevention/management of NCDs among Member States of the Organisation for Economic Co-operation and Development (OECD). We sought to describe policies’ aims and strategies to achieve those aims, and evaluate extent of integration of musculoskeletal conditions as a leading cause of global morbidity.
Methods Policies submitted by OECD Member States in response to a World Health Organization (WHO) NCD Capacity Survey were extracted from the WHO document clearing-house and analysed following a standard protocol. Policies were eligible for inclusion when they described an integrated approach to prevention/management of NCDs. Internal validity was evaluated using a standard instrument (sum score: 0–14; higher scores indicate better quality). Quantitative data were expressed as frequencies, while text data were content-analysed and meta-synthesised using standardised methods.
Results After removal of duplicates and screening, 44 policies from 30 OECD Member States were included. Three key themes emerged to describe the general aims of included policies: system strengthening approaches; improved service delivery; and better population health. Whereas the policies of most countries covered cancer (83.3%), cardiovascular disease (76.6%), diabetes/endocrine disorders (76.6%), respiratory conditions (63.3%) and mental health conditions (63.3%), only half the countries included musculoskeletal health and pain (50.0%) as explicit foci. General strategies were outlined in 42 (95.5%) policies—all were relevant to musculoskeletal health in 12 policies, some relevant in 27 policies and none relevant in three policies. Three key themes described the strategies: general principles for people-centred NCD prevention/management; enhanced service delivery; and system strengthening approaches. Internal validity sum scores ranged from 0 to 13; mean: 7.6 (95% CI 6.5 to 8.7).
Conclusion Relative to other NCDs, musculoskeletal health did not feature as prominently, although many general prevention/management strategies were relevant to musculoskeletal health improvement.

The last two decades have been characterized by a fundamental change in the approaches, tools and instruments in the quality management at Higher Education Institutions. Comparison of two Higher Education Institutions in the Slovak Republic and Germany highlights similarities and trends in quality assurance. Both case studies show how multifaceted the quality management is, and the needs to be approached if a meaningful progress is to be made. Complexity has to be explicitly recognized and built into the approach chosen. Higher Education Institutions have to develop internal quality assurance processes. Quality culture is key for addressing the challenges.

Career Decisions of Indian Female Talent: Implications for Gender-sensitive Talent Management

Purpose: Talent scarcity in emerging economies such as India poses challenges for companies,and limited labour market participation among well-educated women has been observed. The reasons that professionals decide not to pursue a further corporate career remain unclear. By investigating career decision making, this article aims to highlight (1) the contextual factors that impact those decisions, (2) individuals’ agency to handle them, and (3) the implications for talent management (TM).
Design/methodology/approach: Following a qualitative research design, computer-aided analysis was conducted on interviews with 24 internationally experienced Indian business professionals. A novel application of neo-institutionalism in the Indian context was combined with the family-relatedness of work decisions (FRWD) model.
Findings: Career decisions indicate that rebellion against Indian societal and family expectations is essential to following a career path, especially for women. TM as part of the current institutional framework serves as a legitimising façade veiling traditional practices that hinder females’ careers.
Research limitations: Interviewees adopted a retrospective perspective when describing their career decisions; therefore, different views might have existed at the moment of decision making.
Practical implications: Design and implementation of gender-sensitive TM adjusted to fit the specific Indian context can contribute to retaining female talent in companies and the labour market.
Originality/value: The importance of gender-sensitive TM can be concluded from an empirical study of the context-based career decision making of experienced business professionals from India. The synthesis of neo-institutionalism, the FRWD model and the research results provides assistance in mapping talent experiences and implications for overcoming the challenges of talent scarcity in India.
Talent scarcity in many parts of the world leads to the necessity to enlarge talent pools in order to provide enough future holders of key positions. Taking the scholarly discussion at the overlap of talent management and current careers literature as a starting point our qualitative empirical research provides insights in talent’s career decisions in an eastern emerging market, India, and a western developed country, Germany. 49 interviews with internationally experienced knowledge-workers were held to find out how to they come to career decisions throughout their career. Special focus was the balancing act of professional and private life sphere. An inductive-deductive approach was used to develop categories in MaxQda. Results show the impact of institutional frame, cultural context, and gender differences. Consequently, a stronger focus on talent’s different life phases with context specific deviations when configuring Talent Management in Multinational Enterprises can be advised.
HRM processes are increasingly AI-driven, and HRM supports the general digital transformation of companies’ viable competitiveness. This paper points out possible positive and negative effects on HRM, workplaces, and workersorganizations along the HR processes and its potential for competitive advantage in regard to managerial decisions on AI implementation regarding augmentation and automation of work.
A systematic literature review that includes 62 international journals across different disciplines and contains top-tier academic and German practitioner journals was conducted. The literature analysis applies the resource-based view (RBV) as a lens through which to explore AI-driven HRM as a potential source of organizational capabilities.
The analysis shows four ambiguities for AI-driven HRM that might support sustainable company development or might prevent AI application: job design, transparency, performance and data ambiguity. A limited scholarly discussion with very few empirical studies can be stated. To date, research has mainly focused on HRM in general, recruiting, and HR analytics in particular.
The four ambiguities’ context-specific potential for capability building in firms is indicated, and research avenues are developed.
This paper critically explores AI-driven HRM and structures context-specific potential for capability building along four ambiguities that must be addressed by HRM to strategically contribute to an organization’s competitive advantage.
We investigated whether design experts or laypersons evaluate webpages differently. Twenty participants, 10 experts and 10 laypersons, judged the aesthetic value of a webpage in an EEG-experiment. Screenshots of 150 webpages, judged as aesthetic or as unaesthetic by another 136 participants, served as stimulus material. Behaviorally, experts and laypersons evaluated unaesthetic webpages similarly, but they differed in their evaluation of aesthetic ones: experts evaluated aesthetic webpages as unaesthetic more often than laypersons did. The ERP-data show main effects of level of expertise and of aesthetic value only. There was no interaction of expertise and aesthetics. In a time-window of 110–130 ms after stimulus onset, aesthetic webpages elicited a more negative EEG-amplitude than unaesthetic webpages. In the same time window, experts had more negative EEG-amplitudes than laypersons. This patterning of results continued until a time window of 600–800 ms in which group and aesthetic differences diminished. An interaction of perceiver characteristics and object properties that several interactionist theories postulate was absent in the EEG-data. Experts seem to process the stimuli in a more thorough manner than laypersons. The early activation differences between aesthetic and unaesthetic webpages is in contrast with some theories of aesthetic processing and has not been reported before.
Despite normal neurological bedside and electrodiagnostic, some patients with non-specific neck arm pain (NSNAP) have heightened nerve mechanosensitivity upon neurodynamic testing [1, 2]. It remains however unclear whether this is associated with a minor nerve injury. The aim of this study was to evaluate potential differences in somatosensory function among patients with unilateral NSNAP with and without positive neurodynamic tests and healthy controls.
Quantitative sensory testing was performed in 40 patients with unilateral NSNAP; 23 with positive upper limb neurodynamic tests (ULNTPOS) and 17 with negative neurodynamic tests (ULNTNEG). The protocol comprised thermal and mechanical detection and pain thresholds as well as mechanical pain sensitivity, wind-up ratio and dynamic mechanical allodynia. All parameters were measured in the maximal pain area on the affected side as well as over the corresponding area on the unaffected side. Symptom severity, functional deficits, psychological parameters, quality of life and sleep disturbance were also recorded.
Fifty-seven percent of patients with NSNAP had positive neurodynamic tests despite normal bedside neurological integrity tests and nerve conduction parameters. Clinical profiles did not differ between patient groups. Somatosensory profiling revealed a more pronounced loss of function phenotype in ULNTPOS patients compared to healthy controls. Hyperalgesia (cold, heat and pressure pain) was present bilaterally in both NSNAP group. The ULNTNEG subgroup represented an intermediate phenotype between ULNTPOS patients and healthy controls in both thermal and pressure pain thresholds as well as mechanical detection thresholds.
In conclusion, heightened nerve mechanosensitivity was present in over half of patients with NSNAP. Our data suggest that NSNAP presents as a spectrum with some patients showing signs suggestive of a minor nerve dysfunction.

Studiengänge zur Versorgungsforschung in Deutschland : aktueller Stand und zukünftige Perspektiven

Ziel der Studie: Ziel dieser Untersuchung war es, die derzeit akkreditierten Versorgungsforschungsstudiengänge in Deutschland im Hinblick auf ihre Strukturdaten und Inhalte zu analysieren.
Methodik: Im Rahmen eines deskriptiven qualitativen Designs wurden die aktuellen Studienverlaufspläne, die Modulhandbücher und die Steckbriefe akkreditierter Masterstudiengänge der Versorgungsforschung inhaltsanalytisch ausgewertet.
Ergebnisse: Die 6 analysierten Studiengänge zeigen sich hinsichtlich ihrer strukturellen Merkmale und lehr-lerninhaltlichen Ausrichtung in den wesentlichen Punkten als homogen. Ziel aller Studiengänge ist es, fach- und disziplinübergreifende personale und soziale Kompetenzen zu fördern, um Versorgungsbedarfe im Gesundheitswesen zu identifizieren, Innovationen der Versorgung zu entwickeln, zu initiieren, zu evaluieren und kritisch zu reflektieren.
Schlussfolgerungen: Trotz ähnlicher Lehr-Lerninhalte bestehen spezifische Ausrichtungen, für die sich die Studierenden vor dem Hintergrund ihrer individuellen Präferenzen entscheiden können. Diese Ergebnisse könnten unter Berücksichtigung aktueller Diskurse und weiterer Erkenntnisse als Grundlage für Empfehlungen zur Entwicklung eines Kerncurriculums dienen.
Background: Pancreatic cancer is one of the malignant diseases with the highest cancer-specific mortality. At the time of diagnosis, life expectancy is often already very limited, as it is usually discovered late and in an advanced stage. Coping with cancer is a complex process. Coping strategies of patients with pancreatic cancer probably differ from those of other malignancies. Yet to date, there exists no pancreatic cancer-specific coping model.
Objective: The objective of this scoping review is to explore and characterize the academic literature related to coping processes in patients with pancreatic cancer.
Methods/Design: The JBI's three-step search strategy, combined with the Arksey and O'Malley framework, will be used to identify articles via PubMed/MEDLINE, CINAHL, Cochrane Library, Google Scholar, CAMbase, CareLit, CC Med, Scopus, and PsycARTICLES (Arksey & O'Malley, 2005; Peters et al., 2017). It follows the PRISMA guidelines for scoping reviews (Tricco et al., 2018). Primary and secondary studies and reviews which report on coping with pancreatic cancer (adenocarcinoma) in adults in English or German language will be included in this scoping review, regardless of publication date or study design.

Im Rahmen eines Unterauftrags durch die Universität Bremen, die vom Verein Geschäftsstelle Qualitätsausschuss Pflege e.V. mit der Entwicklung eines wissenschaftlich fundierten Verfahrens zur einheitlichen Bemessung des Personalbedarfs in Pflegeeinrichtungen nach qualitativen und quantitativen Maßstäben nach § 113c SGB XI beauftragt wurde, hat die Hochschule Osnabrück Personalfragen in der ambulanten Pflege untersucht.
Im Rahmen des Forschungsprojekts „Gesunde Personalbemessung: Arbeitsschutz und Gesundheitsförderung in Kontexten der systematischen Personalbemessung für die Pflege“ (GePAG) haben die Hochschule und die Universität Osnabrück, gefördert von der Berufsgenossenschaft für Gesundheitsdienst und Wohlfahrtspflege (BGW) die Machbarkeit einer systematischen Integration von Arbeitsschutz und Gesundheitsförderung in Instrumentarien der Personalbemessung für die Pflege untersucht. Es wurden zwei systematische Literaturstudien, Expert*inneninterviews und eine Fokusgruppe durchgeführt. Zentrale Ergebnisse der Studie sind unter anderem: dass eine systematische Integration von Arbeitsschutz und Gesundheitsförderung in Instrumentarien der Personalbemessung in der Pflege maßgeblich für die Sicherstellung von gesunder Pflegearbeit ist; bisherige Arbeiten zu wissenschaftlich fundierten Instrumenten der Personalbemessung um komplexe, nicht vollständig operationalisierbare Aspekte („Beyond Numbers“), die bspw. ethische Anforderungen betreffen, erweitert werden müssen sowie dass Initiativen zur Beförderung von Arbeitsschutz und Gesundheitsförderung in der Pflege die Umsetzung bereits bekannter Ansätze aus anderen Disziplinen und ihre Auswirkungen auf eine gesundheitsorientierte Personalbemessung in organisationalen Zusammenhängen untersuchen sollten.
Der vorliegende Bericht fasst die Ergebnisse der durch den Sozialverband VdK in Auftrag gegebenen Pflegestudie „Wunsch und Wirklichkeit in der häuslichen Pflege“ zusammen. Viele Menschen haben dazu beigetragen, dass die Studie durchgeführt werden konnte. An erster Stelle und vor allem sei den Menschen herzlich gedankt, die sich an der Befragung beteiligt haben. Mehr als 50.000 Teilnehmerinnen und Teilnehmer haben jegliche Erwartung zur Teilnahme an der Studie deutlich übertroffen.
Objectives: This paper addresses recent steps for reforming the eligibility criteria of the German long-term care insurance that have been initiated to overcome shortcomings in the current system.
Methods: Based on findings of a survey of international long-term care systems, assessment tools and the relevant literature on care needs a new tool for determining eligibility in the German long-term care insurance was developed.
Results: The new tool for determining long-term care eligibility broadens the understanding of what ‚dependency on nursing care' implies for the person affected. The assessment results in a degree of dependency from personal help provided by formal or informal caregivers. This degree of dependency can be used for determining eligibility for and the amount of long-term care benefits.
Discussion: The broader understanding of "dependency on nursing care' and the new tool are important steps to adapt the German long-term care insurance to the challenges of the demographic and societal changes in the future
Zukünftige Nonprofit-Manager*innen sind mit komplexen Herausforderungen in einem sich stetig wandelnden Umfeld konfrontiert. Themen wie Klimaschutz, soziale Ungleichheit oder auch Demokratieförderung haben einen starken Einfluss auf die Arbeitsfelder der Nonprofit-Organisationen. Eine zentrale Aufgabe der Führungskräfte ist daher in dieser Komplexität konstruktiv agieren zu können. Dafür sind Kompetenzen erforderlich: Ganz zentral ist die Fähigkeit, sich initiativ und eigenverantwortlich mit den Herausforderungen unserer Zeit auseinanderzusetzen und im besten Fall geeignete Lösungen dafür zu finden sowie diese zielführend kommunizieren zu können. Aus diesem Grund wird genau diese Fähigkeit bei Studierenden aus den Masterstudiengängen Management in Nonprofit-Organisationen und Soziale Arbeit der Hochschule Osnabrück gefördert. Im Rahmen des Moduls Handlungsfelder II entwickelten rund 30 Studierende im Wintersemester 2022/2023 in einer Denkwerkstatt Forschungsergebnisse und Lösungsideen zum Thema „Osnabrück 2030 – Demokratie leben“. Die Studierenden wählten in einem partizipativen Prozess ihre eigenen Schwerpunktthemen wie zum Beispiel Mobilität oder Bildung aus und arbeiteten dann ein Semester lang an den Inhalten. Bei der Erarbeitung wurden sie durch Marlene Eimterbäumer begleitet, die Modelle, Methoden und Coaching zur Unterstützung bereit stellte. Die Modelle und Methoden finden sich teilweise in den Beiträgen der Studierenden wieder (unter anderem das Social Ecological Model und der Business Model Canvas). Am Ende des Semesters stand eine Präsentation vor den Kommiliton*innen, der Lehrenden und auch vor externen Gästen, die aus unterschiedlichen Expertisegebieten im Themenbereich „Demokratie“ kamen und dementsprechend Feedback aus der Praxis geben konnten. Das Modul selbst wurde von der Hochschule im Rahmen der Innovativen Lehre an der Fakultät Wirtschafts- und Sozialwissenschaften gefördert. Für die Studierenden stellte das Modul durchaus eine Herausforderung dar, denn in der Denkwerkstatt musste unter hoher Komplexität stark prozessbezogen gearbeitet werden im Gegensatz zu der sonstigen hohen Ergebnisorientierung. Die durchweg sehr guten Ergebnisse zeigen, dass der Einsatz und die kontinuierliche Kommunikation und Zusammenarbeit sich gelohnt haben. Aufgeteilt ist das vorliegende Buch in die fünf Themen: Wahlen, Partizipation, Partizipative Raumentwicklung, Politische Bildung und Medienkompetenz. Zu diesen Themen finden Sie jeweils erst einen Beitrag der Forscher*innen und darauffolgend einen Beitrag zur Darstellung einer Lösungsidee.

Aus diesem Grunde wird genau diese Fähigkeit bei Studierenden aus den Masterstudiengängen Management in Nonprofit-Organisationen und Soziale Arbeit der Hochschule Osnabrück gefördert.
Im Rahmen des Moduls Handlungsfelder II entwickelten rund 30 Studierende im Wintersemester 2020/2021 in einer Denkwerkstatt ihre eigenen Lösungen in Bezug auf Forschung, Produkte / Dienstleistungen und Kommunikation. Die Studierenden wählten in einem partizipativen Prozess ihre eigenen Schwerpunktthemen aus und arbeiteten dann ein Semester lang an den Inhalten.
Begleitet wurden sie durch ein Teamteaching von Prof. Dr. Gesa Birnkraut und Marlene Eimterbäumer, die Modelle, Methoden und Coaching zur Unterstützung anboten. Die Modelle und Methoden finden sich in den Beiträgen der Studierenden wieder (unter anderem das socio-ecological model, der Business Model Canvas, der story telling canvas, das design thinking).
Am Ende des Semesters stand eine Präsentation vor den Kommiliton*innen und den Lehrenden, aber auch vor externen Gästen, die aus unterschiedlichen Expertisegebieten kamen und dementsprechend Feedback gaben. Das Modul selbst wurde von der Hochschule im Rahmen der Innovativen Lehre an der Fakultät Wirtschafts- und Sozialwissenschaften gefördert. Für die Studierenden stellte das Modul durchaus eine große Herausforderung dar, denn in der Denkwerkstatt musste unter hoher Komplexität stark prozessbezogen gearbeitet werden im Gegensatz zu der sonstigen hohen Ergebnisorientierung.
Die durchweg sehr guten Ergebnisse zeigen, dass der Einsatz und das Aushalten der Unsicherheit sich gelohnt haben. Aufgeteilt ist das vorliegende Buch in die zwei Schwerpunktthemen Ressourcenknappheit / Wirtschaft und Wasserknappheit. Communication deficits belong to the most frequent errors in patient handovers calling upon specialized training approaches to be implemented. This study aims to harness problem-based learning (PBL) methods in handover education and evaluated the learning process. A digitally enabled PBL course was developed and implemented at Klinikum Osnabrück from which eight nurses participated in the course. They agreed on the stimulating effect of the setting regarding self-directed learning and on the potential to translate the new knowledge and skills into the daily clinical practice. In conclusion, the findings are promising that a digitally enabled PBL course is a suitable learning format for handover education.
Introduction: Handovers are a central process for ensuring information continuity in patient care and, therefore, possess a major influence on patient safety as errors due to poor handovers can lead to life-threatening events. Education to improve handovers and ensure safe patient care can be supported by using critical incident reporting systems (CIRS). The aim of the study is to perform a content analysis of a national CIRS-database with regard to identifying adverse events in handovers situations and to derive competencies for the development of continuing education from these findings.
Methods: A meta model served as a research framework to merge the empirical findings with the London protocol of analysing critical events and the Canadian framework of safety competencies. Relevant cases to be investigated were searched in a freely accessible German CIRS database.
Results: A total of 253 case descriptions were found and analysed. Team factors emerged as the most frequently reported influencing factors following the analysis of the London protocol. Communication errors and missing information as well as a lack of appropriate standards and processes appeared to be the main reasons for critical events to occur. Most of the events happened in units involving surgery and intensive care. A mapping of patient safety competences with the reasons for critical events was conducted in order to determine the practical, concrete and handover related competencies.
Conclusion: Data from a CIRS database and theoretical frameworks can be combined to extract meaningful information about patient safety risks in handover situations. The results are useful for developing curricula to improve handovers based on patient safety competencies.
The article describes an analysis of the use of e-learning to improve the learning transfer to practice in continuing education. Therefore an e-learning offer has been developed as a part between two attendance periods of a training course in the field of Ambient Assisted Living (AAL). All participants of the course were free to use the e-learning offer. After the end of the e-learning part we compared the e-learning users to the other participants. Using an online questionnaire we explored if there are differences in the activities in the field AAL after the training course. The results show that e-learning is beneficial especially for communication processes. Due to the fact that the possibility to talk about the learning content is an essential factor for the learning transfer, e-learning can improve the learning success.
An Iterative Methodology for Developing National Recommendations for Nursing Informatics Curricula

The increasing importance of IT in nursing requires educational measures to support its meaningful application. However, many countries do not yet have national recommendations for nursing informatics competencies. We thus developed an iterative triple methodology to yield validated and country specific recommendations for informatics core competencies in nursing. We identified relevant competencies from national sources (step 1), matched and enriched these with input from the international literature (step 2) and fed the resulting 24 core competencies into a survey (120 invited experts from which 87 responded) and two focus group sessions with a total of 48 experts (steps 3a/3b). The subsequent focus group sessions confirmed and expanded the findings. As a result, we were able to define role specific informatics core competencies for three countries.
Current challenges are a result of complex and multi-layered factors. They require systemic solutions: problems must be solved holistically by identifying interconnections between the different problems which are results of the same root cause. Social entrepreneurs are agents of change, their mission is to generate societal impact. I investigated how a social entrepreneurship think tank can be conceptualized to support systems change. Think tanks can conduct in-depth research to inform about the systems and share knowledge. Entrepreneurs can make strategic interventions based on the research findings. To create a framework of how social entrepreneurship think tanks can promote systems change I conducted 14 case studies. The framework is created based on the method of Theory of Change, it displays how different activities can contribute to succeeding in systems change. I outline eleven activities which include strategies in capacity building, research and collaboration with the other actors in the field. In the end, the framework is applied to the evolving case of a think tank founded in 2018.
Background:
Large health organizations often struggle to build complex health information technology (HIT) solutions and are faced with ever-growing pressure to continuously innovate their information systems. Limited research has been conducted that explores the relationship between organizations’ innovative capabilities and HIT quality in the sense of achieving high-quality support for patient care processes.
Objective:
The aim of this study is to explain how core constructs of organizational innovation capabilities are linked to HIT quality based on a conceptual sociotechnical model on innovation and quality of HIT, called the IQHIT model, to help determine how better information provision in health organizations can be achieved.
Methods:
We designed a survey to assess various domains of HIT quality, innovation capabilities of health organizations, and context variables and administered it to hospital chief information officers across Austria, Germany, and Switzerland. Data from 232 hospitals were used to empirically fit the model using partial least squares structural equation modeling to reveal associations and mediating and moderating effects.
Results:
The resulting empirical IQHIT model reveals several associations between the analyzed constructs, which can be summarized in 2 main insights. First, it illustrates the linkage between the constructs measuring HIT quality by showing that the professionalism of information management explains the degree of HIT workflow support (R²=0.56), which in turn explains the perceived HIT quality (R²=0.53). Second, the model shows that HIT quality was positively influenced by innovation capabilities related to the top management team, the information technology department, and the organization at large. The assessment of the model’s statistical quality criteria indicated valid model specifications, including sufficient convergent and discriminant validity for measuring the latent constructs that underlie the measures of HIT quality and innovation capabilities.
Conclusions:
The proposed sociotechnical IQHIT model points to the key role of professional information management for HIT workflow support in patient care and perceived HIT quality from the viewpoint of hospital chief information officers. Furthermore, it highlights that organizational innovation capabilities, particularly with respect to the top management team, facilitate HIT quality and suggests that health organizations establish this link by applying professional information management practices. The model may serve to stimulate further scientific work in the field of HIT adoption and diffusion and to provide practical guidance to managers, policy makers, and educators on how to achieve better patient care using HIT.
Going Mobile : An Empirical Model for Explaining Successful Information Logistics in Ward Rounds

Background: Medical ward rounds are critical focal points of inpatient care that call for uniquely flexible solutions to provide clinical information at the bedside. While this fact is undoubted, adoption rates of mobile IT solutions remain rather low.
Objectives: Our goal was to investigate if and how mobile IT solutions influence successful information provision at the bedside, i.e. clinical information logistics, as well as to shed light at socio-organizational factors that facilitate adoption rates from a user-centered perspective.
Methods: Survey data were collected from 373 medical and nursing directors of German, Austrian and Swiss hospitals and analyzed using variance-based Structural Equation Modelling (SEM).
Results: The adoption of mobile IT solutions explains large portions of clinical information logistics and is in itself associated with an organizational culture of innovation and end user participation.
Conclusion: Results should encourage decision makers to understand mobility as a core constituent of information logistics and thus to promote close end-user participation as well as to work towards building a culture of innovation.
The establishment of successful clinical information logistics (CIL) within the care processes is one of the main objectives of strategic health IT management in hospitals. While technical realisations in terms of useful, usable and interoperable IT solutions are essential precursors of CIL, there is limited empirical research on what socio-organisational factors underlie an innovation-friendly culture and how they can affect successful information provision. We applied factor analysis on survey data from 403 clinical directors from Germany, Austria and Switzerland and used the dimensions identified to explain the level of CIL with ordered logistic regression analysis. The intensity of collaboration and exchange with the IT department as well as the degree of executive IT leadership showed to be strongly associated with better CIL while personal views and attitudes of clinical directors were not. Analysing country differences revealed the degree of the exchange with the IT department to be significantly lower in German hospitals. This points at a potential strategic lever for German hospital executives to focus on.
Hospital CIOs play a central role in the adoption of innovative health IT. Until now, it remained unclear which particular conditions constitute their capability to innovate in terms of intrapersonal as well as organisational factors. An inventory of 20 items was developed to capture these conditions and examined by analysing data obtained from 164 German hospital CIOs. Principal component analysis resulted in three internally consistent components that constitute large portions of the CIOs innovation capability: organisational innovation culture, entrepreneurship personality and openness towards users. Results were used to build composite indicators that allow further evaluations.
Die Mobilität Studierender und Beschäftigter an Hochschulen trägt maßgeblich zu ihrem Einfluss auf Umwelt und Klima bei. Um mögliche negative Effekte zu minimieren, zielt die vorliegende Arbeit auf die Evaluation des aktuellen Mobilitätsverhaltens und die Initiierung von Aktivitäten zur nachhaltigen Mobilität an der Hochschule Mittweida (HSMW) und der Hochschule für Technik und Wirtschaft Dresden (HTW Dresden). Dafür wurde der aktuelle Status der Wege zur Hochschule und der Dienstreisen im Herbst 2019 mittels Online-Befragungen erfasst. Das Mobilitätsverhalten und zugrundeliegende Motive wurden analysiert. Basierend auf der Ist-Analyse wurden Ziele und Handlungsansätze zur Förderung nachhaltiger Mobilität für beide Hochschulen entwickelt.
Für beide untersuchten Hochschulen steht die Reduzierung der Kohlendioxid-Äquivalent-Emissionen im Fokus zum Thema nachhaltige Mobilität. Durch die verschiedenen Ausgangssituationen bezüglich der Mobilität auf den Wegen zur Hochschule wird jedoch der Schwerpunkt zur Umsetzung von Ansätzen zur Förderung nachhaltiger Mobilität unterschiedlich gesetzt. Für die HSMW werden verstärkt Handlungsansätze zur Förderung des öffentlichen Personen-Nahverkehrs und eines klimafreundlichen motorisierten Individualverkehrs entworfen (Beispiele sind Semester- und Jobtickets für Studierende und Beschäftigte). An der HTW Dresden haben Ansätze zum nichtmotorisierte Individualverkehr eine wichtige Priorität (z. B. die Beteiligung an kommunalen Initiativen zum Radverkehr in Dresden). Für die HSWM und die HTW Dresden werden außerdem Ansätze zur nachhaltigen Durchführung oder gar – sofern möglich – zur Vermeidung von Dienstreisen wie die Anerkennung der Reise- als Dienstzeit vorgestellt. Aufgrund der schwierigen Übertragbarkeit und der Chance viele Hochschulangehörige durch Befragungen partizipieren zu lassen, wird die individuelle Betrachtung der Ist-Situation auch zukünftig an anderen Hochschulen empfohlen.
The current narrative literature review aims to discuss clinical reasoning based on nociceptive pain mechanisms for determining the most appropriate assessment and therapeutic strategy and to identify/map the most updated scientific evidence in relation to physical therapy interventions for patients with temporomandibular disorders (TMDs). We will also propose an algorithm for clinical examination and treatment decisions and a pain model integrating current knowledge of pain neuroscience. The clinical examination of patients with TMDs should be based on nociceptive mechanisms and include the potential identification of the dominant, central, or peripheral sensitization driver. Additionally, the musculoskeletal drivers of these sensitization processes should be assessed with the aim of reproducing symptoms. Therapeutic strategies applied for managing TMDs can be grouped into tissue-based impairment treatments (bottom-up interventions) and strategies targeting the central nervous system (top-down interventions). Bottom-up strategies include joint-, soft tissue-, and nerve-targeting interventions, as well as needling therapies, whereas top-down strategies include exercises, grade motor imagery, and also pain neuroscience education. Evidence shows that the effectiveness of these interventions depends on the clinical reasoning applied, since not all strategies are equally effective for the different TMD subgroups. In fact, the presence or absence of a central sensitization driver could lead to different treatment outcomes. It seems that multimodal approaches are more effective and should be applied in patients with TMDs. The current paper also proposes a clinical decision algorithm integrating clinical diagnosis with nociceptive mechanisms for the application of the most appropriate treatment approach.
Continuity of care is a concept that is defined as the uninterrupted and coordinated care provided to a patient and that includes an informational dimension which describes the information exchange between the parties involved. In nursing, the nursing summary is the main instrument to ensure informational continuity of care. The aim of this paper is to present an HL7 Clinical Document Architecture based document standard for the eNursing Summary and to discuss the need for harmonizing these results at international level. The eNursing Summary proposed in this paper was developed on the basis of several internationally accepted concepts, primarily the nursing process, the ISO 18104 Reference Terminology Model for Nursing and various data sets. The standardisation process embraced several phases of involving nursing experts for validating its structure and content. It was finally evaluated by a network of 100 healthcare organizations. We argue that the eNursing Summary is a good starting point for standardising nursing discharge and transfer documents on a global level. However, further work is needed to bring together the different national and international strands in standardisation.
Handovers need a common ground on the clinical cases between the members of the successive shifts to establish continuity of care. Conventional electronic patient record systems (EHR) proved to be only insufficiently suitable for supporting the grounding process. Against this background we proposed a basic concept for a handover EHR that extends general EHRs in particular openEHR based systems. The resulting handover information model was implemented in a database and evaluated based on 120 clinical cases. The information items of these cases could be mapped successfully to the model, however, the new class “anticipatory guidance” needed to be introduced. The evaluation also demonstrated the importance of highly aggregated information on the clinical case, opinions and meta-information such as the relevance of an item during handovers. Based on these findings, in particular the handover database, handover EHR applications are currently developed to support the grounding process.
Die verfasste Bachelorarbeit verknüpft in Form der "Offenen Kinder- und Jugendarbeit" sowie dem Aspekt der "Migration", zwei relevante Bereiche, die in der Sozialen Arbeit wesentlich thematisiert werden. Beide Hauptthemenschwerpunkte dieser Arbeit basieren auf einem Facettenreichtum ihrer Spezifika. Auf Grundlage der Tatsache, dass das Jugendzentrum des "Stadtteiltreff Haste" in Osnabrück, einen Besucheranteil von ca. 90% von Kindern und Jugendlichen mit Migrationshintergrund aufweist, befasst sich diese Arbeit mit der Frage, wie so ein großer prozentualer Anteil an Jugendlichen mit Migrationsstatus als Besucherklientel eines Jugendzentrums zustande kommt. Mittels einer im Stadtteiltreff Haste durchgeführten, quantitativen Forschung (Befragung per Fragebogen) von insgesamt 55 befragten Kindern und Jugendlichen mit Migrationshintergrund, sollte herausgefunden werden, wovon aus der Perspektive der Besucher/-innen, eine erfolgreiche offene Kinder- und Jugendarbeit mit dem Migrationsklientel abhängig ist.
In der inklusiven Grundschule arbeiten zunehmend verschiedene Professionen, wie bspw. Lehrer*innen, Sonderpädagog*innen oder Schulsozialarbeiter*innen, interprofessionell zusammen. Dieser Wandel in schulischen Arbeitskontexten geht mit veränderten Professionalisierungsanforderungen für die Lehr- und Fachkräfte einher (vgl. Kleina & Tan, 2019, S. 63). Die Praxis interprofessioneller Kooperation in der inklusiven Grundschule ist dabei bisher nur in Teilfragen erforscht (vgl. Moldenhauer, 2019), kaum aber mit Blick auf die Erfahrungen und Erwartungen der pädagogischen Lehr- und Fachkräfte selbst. Hierzu werden Ergebnisse einer quantitativen Bedarfserhebung (N=271), welche im Rahmen des vom BMBF geförderten Forschungs- und Entwicklungsprojektes InproKiG bei Lehrkräften und pädagogischen Fachkräften von Schulteams in Niedersachsen und Hessen durchgeführt wurde, vorgestellt. Die Ergebnisse geben Aufschluss über die Entwicklung und Gestaltung von Qualifizierungsmaßnahmen in diesem Bereich. Neben der Qualifizierung der bereits tätigen Lehr- und Fachkräfte, kann das InproKiG-Manual auch in der Lehrer*innenausbildung eingesetzt werden.
Aims: Intercultural competence has become a key-competence, since the world has become more and more volatile, uncertain, complex and ambiguous. Therefore, insights in the development of intercultural competence and its´ links to individual traits are crucial for companies and researchers to face the requirements in a VUCA world. This study examines the relationship between the time, students spent abroad, personality traits and circumstances during this time with the student’s intercultural competence and integration performance in the target culture. The study had a correlative cross-sectional design. Design and sample: A total of 202 academic subjects were surveyed. The average age was 22 years. There was one measuring time, to which 58 % of the participants stated that they have had a stay abroad. Measurements: Metacognitive, cognitive, motivational and behavioural intercultural competence were measured with the Cultural Intelligence Scale. The personality traits involvement, discipline, social competence, cooperation, dominance and stability were captured with the "Bochum inventory for job-related personality description-6F". Work-related attitudes as patterns of behaviour and experience were measured using the "Work-related Behaviour and Experiencing Pattern 44" (German: Arbeitsbezogene Verhaltens- und Erlebensmuster, AVEM). This scale captures the way participants relate to work in general based on the measurement of personality traits and their fit to specific clusters, which describe, whether individuals have healthy or risky patterns. In addition, the demographic factors and characteristics of stays abroad as well as the integration into the target culture based on the Sociocultural Adaption Scale were examined. The data was tested for relationships and differences by tests for mean differences, variance and regression analyses. Results: There was a positive correlation between duration and cognitive, motivational and behavioural intercultural competence. The motivational competence is higher in subjects who have no risk pattern in the AVEM. The different types of competence influence each other at diverse times. Moreover, the suggested structural equation model could be confirmed. This showed the effect of the AVEM pattern on intercultural competence, moderated by the stay abroad and the social competence. Thus, the study contributes to the understanding of both the measurement of intercultural competence and the development process of intercultural competence in a globalized world.
Do multipliers have to be more sensitized for the issue diversity? Do they have to develop specific competences? Which do they already have? These questions were analysed by a qualitative investigation. Ca. 70 interviews with managers of the large DAX companies and employees were conducted. The results show a field of tension between self-perception and perception of others and the assessment of the relevance of diversity attitudes and measures, competences and their actual implementation. The results indicate the need of promotion of competences, especially regarding the intercultural competence. We position ourselves in a functionalist perspective, in line with the work on paradigms of Burell and Morgan (2017) and Deetz (1996) in social sciences and Cross-Cultural Management. We present these results from a functionalist perspective in order to ensure the greatest possible "objectivity".
The central aim of the investigation at hand is to deal with the problem areas of Human Resource Management, which arise by demographic changes and migration. The paper focuses on mutual relationships. Managers and human resource managers are considered as multipliers. Older employees, migrants and women are important potential. Therefore, following research questions have been investigated: Which competences are necessary to promote to recognise the potential of migrants correctly and to promote them? Do the multipliers have to be more sensitized for the issue diversity? Do they have to develop specific competences to make the system more permeable and to make the entry and promotion of migrants possible? Which competences should be promoted to increase the sensitivity for diversity? The questions were examined by a qualitative investigation to develop hypotheses for a quantitative study. Overall, 30 interviews with managers, human resource managers and diversity representatives of the large DAX companies were conducted. Furthermore, 17 employees with immigration background and 15 employees without an immigration background were interviewed. The data was transcribed and analysed by the qualitative content analysis according to Mayring (2010). Comparative analyses were made with single items with Likert Scales. The investigation of managers and employees is a highly diversified issue. Therefore, the main focus of the project lays on the problem areas, conflicts and competences of human resources managers in demographic-sensitive personnel management. In comparison, employees with and without an immigration background were asked. The results show an interesting field of tension between self-perception and perception of others and the assessment of the relevance of diversity attitudes and measures, competences and their implementation. Furthermore, a contrary perception regarding strains and stresses of person with and without immigration background is determined, which is developed in the consequences of migrations stress and experiences of discrimination. The results indicate the need of promotion of competences, especially regarding intercultural competence. A critical analysis of the results will be presented.
Kurzfassung: Die vorliegende Untersuchung setzt sich mit den Problemfeldern der Personalauswahl und -entwicklung auseinander, welche die demografischen Veränderungen in Bezug auf Migration mit sich bringen. Explorativ soll die Ausprägung von sozialen und interkulturellen Kompetenzen und der Aspekt Diversity in Unternehmen untersucht werden. Hierzu wurden 69 Interviews durchgeführt und die intervallskalierten Selbsteinschätzungsskalen quantitativ verglichen. Die Ergebnisse zeigen, dass Mitarbeiter die Relevanz von Diversity-Einstellungen und Maßnahmen, Kompetenzen und Umsetzung dieser deutlich höher bewerten als die Führungskräfte. Die Ergebnisse weisen auf den Bedarf einer Kompetenzförderung hin, insbesondere in Bezug auf die interkulturelle Kompetenz.
Kurzfassung: Die vorliegende Untersuchung untersucht die Auswirkungen
der Dauer eines Auslandsaufenthaltes von Studierenden sowie von arbeitsbezogenen Persönlichkeitsfaktoren und Charakteristika auf die interkulturelle Kompetenz und die Integration der Teilnehmer. Die Untersuchung erfolgte in Form einer korrelativen Querschnittstudie. Insgesamt 202 Probanden mit akademischer Ausbildung wurden untersucht. Es wurden die interkulturelle Kompetenz, Persönlichkeitsmerkmale und arbeitsbezogene Einstellungen gemessen, sowie demographische Faktoren des Auslandsaufenthaltes und die Integration in die Zielkultur. Es gibt einen positiven Zusammenhang zwischen der Dauer von Auslandsaufenthalten und kognitiver, motivationaler und verhaltensbezogener interkultureller
Kompetenz.
The increasing diversity of cultural backgrounds offers potential for a more elaborated information processing, yet has been shown to be challenging for individuals, due to intergroup hostility, prejudices, and difficulties of intercultural communication. Current research thus focusses on the interaction of employees with different cultural heritages, as well as on their intergroup attitudes and competences. Thereby, leaders have been shown to shape the way diversity is considered in their teams in a top-down process. However, their perception of diversity as well as related challenges and chances are poorly investigated. The current paper thus aims to contribute to the understanding of leader’s perspective on and role in diversity-management by building on a comparative qualitative study with sixteen employees and twenty-two leaders. Research questions are how employees and leaders perceive cultural diversity from their specific point of view, which experiences are likely to contribute to their opinion on and perception of diversity and in how far do employees and leaders differ in these aspects. The results show that employees are aware of the topic of diversity in general but have poor competences in dealing with diversity in their daily working life. This seems to be associated with lack of experience with intercultural interaction and a lack of support from the respective organizations/leaders. Leaders are rarely aware of this lack of support. The results indicate that leaders’ experiences with cultural and age diversity, their identification with the company and previous diversity measures in the context of the companies’ policy towards diversity seem to be relevant for the formation of leaders’ attitudes. As a consequence, most participants focus on the question whether they do or do not want diversity within their teams and companies, while they actually fail to perceive the diversity that is already there.
Possessing skills in social and intercultural interaction is vitally important for employees who work in globalized environments, especially as people's working lives tend to involve an increasingly large amount of service-related activities. As a consequence, universities offer cultural studies courses and strive to enable their students to study abroad for a period of time. However, there is still no widely shared agreement on how intercultural experiences and cultural preparation courses predict the perception, thinking and acting of individuals. Therefore, the study at hand uses a cross-sectional design with N = 430 participants in order to investigate whether students of cultural studies gain more intercultural competencies during the time spent studying abroad, compared to studies of other subjects. The results reveal that students of cultural subjects show significantly higher levels of cultural empathy and openness in the post hoc measurement, even though there was no interaction effect with the amount of time spent studying abroad. Length of stay abroad had a significant indirect effect on social competence via all the dimensions of the Multicultural Personality Questionnaire. Moreover, results indicate that flexibility to adapt one's behaviour to cultural norms may predict problems when returning to one's home country.
Hintergrund und Ziele:
In der Physio- und Ergotherapie sowie in der Logopädie in Deutschland herrscht bereits seit mehreren Jahren ein Fachkräftemangel. Die Gründe für diesen Mangel sind vielschichtig, in vorhandenen Studien werden jedoch nur einzelne Ursachen genauer beleuchtet. Ziel dieser Arbeit ist es daher auf der einen Seite, die Einflussgrößen auf die Fachkräfteentwicklung zusammenzustellen. Da durch Diskussionen immer wieder die These aufgestellt wird, dass eine Vollakademisierung einen negativen Effekt auf die Fachkräfteentwicklung hat, sollen auf der anderen Seite die Veränderungen der Studierenden-, Absolventen- und Beschäftigtenzahlen in Österreich und der Schweiz vor und nach der Akademisierung untersucht und dargestellt werden.
Methode:
Zur Untersuchung der verschiedenen Gründe für den Fachkräftemangel in den Therapieberufen wurde eine systematische Literaturrecherche im Sinne eines Scoping Reviews durchgeführt. Die Daten zu Studierenden-, Absolventen- und Beschäftigtenzahlen in Österreich und der Schweiz wurden als Sekundärdatenanalyse erhoben und interpretiert.
Ergebnisse:
Die Gründe für den Fachkräftemangel in den Therapieberufen sind viel-schichtig. Als Kategorien wurden der demografische Wandel, die Bedingungen der Ausbildung sowie die Berufsflucht recherchiert. Die Ursachen zum Verlassen des Berufes findet man in finanziellen Aspekten, den Arbeitsbedingungen insgesamt und der fehlenden Anerkennung und Wertschätzung. Die erhobenen Daten aus Österreich und der Schweiz zeigen insgesamt einen positiven Trend in den letzten Jahren. Die Studierenden- und Absolventenzahlen sind in den genannten Ländern stetig gestiegen und führten daher auch zu einer Erhöhung der Beschäftigtenzahlen.
Schlussfolgerung:
Die verschiedenen Gründe für den Fachkräftemangel konzentrieren sich grundsätzlich darauf, dass die Attraktivität der Berufsfelder gesteigert werden muss, um Schulabsolventen für die Berufe zu gewinnen und Beschäftigte im Beruf zu halten. Als Lösungsmöglichkeit wird die Akademisierung gefordert, die zu einer Attraktivitätssteigerung führen könnte. Die Zahlen aus Österreich und der Schweiz zeigen, dass es durch die Akademisierung nicht zu einem Einbruch der Zahlen von Personen in Ausbildung in den untersuchten Berufsgruppen gekommen ist und weiterhin sehr viele junge Menschen an einer akademischen Ausbildung in den Heilmittelberufen interessiert sind.
Einleitung: Aktuelle Erkenntnisse der Faszienforschung zeigen, dass Faszien weitaus mehr Einfluss auf den menschlichen Bewegungsapparat haben, als bisher angenommen. Besonders die Fascia Thoracolumbalis (FTL) stellt eine große Faszie dar, deren patholo-gische Veränderungen zu Beschwerden im Bereich des unteren Rückens führen können. Auf der Grundlage dieser Erkenntnisse wurde die Behandlungsmethode mit der Foam Roll entwickelt, um Muskeln und Faszien gezielt zu behandeln und Beschwerden vorzubeugen. Beobachtungen zeigen, dass das Foam Rolling die Dehnfähigkeit der Muskulatur verbes-sert. Jedoch wurde bisher nicht untersucht, wie sich Übungen mit der Foam Roll auf die Beweglichkeit der Faszien auswirken. Ziel dieser Studie ist es daher, aufzuzeigen, ob das Foam Rolling Einfluss auf die Beweglichkeit der FTL hat. Zu diesem Zweck wurden sowohl die kurzfristigen, als auch die langfristigen Auswirkungen untersucht.
Methode: Es nahmen 38 Probanden an der Studie teil. Diese wurden in eine Foam Roll Gruppe, eine Placebo- und eine Kontrollgruppe eingeteilt. Das Bewegungsausmaß einer Rumpfflexion von 30° wurde mit Videos eines Ultraschallgerätes, als Grundlage der Unter-suchungen über die Beweglichkeit der FTL, aufgezeichnet. Anschließend erfolgte die Ana-lyse mit der Cross Correlation Software nach Dr. Andrew Dilley. Hierdurch konnte das Bewegungsausmaß der Faszie ermittelt werden. Neben der Faszienbeweglichkeit wurden Messungen bezüglich der lumbalen, sowie thoracolumbalen Flexion und der Mechanosen-sivität einzelner Muskeln durchgeführt. Zur Ermittlung der Werte gab es eine Messung vorab und zehn Minuten nach der Intervention. Die Messwerte wurden sowohl in einen Vorher-Nachher-Vergleich gestellt, sowie in einem Vergleich zwischen den drei Gruppen. Zusätzlich nahmen zwei Probanden an einer Fallstudie teil, deren Ziel es war die langfristi-gen Wirkungen auf die Beweglichkeit der Faszie zu analysieren.
Ergebnisse: Nach der Behandlung mit der Foam Roll verbesserte sich die Beweglichkeit der FTL durchschnittlich um 1,7915 mm (p<0,001). Dagegen konnte in der Placebogruppe (p=0,397) und in der Kontrollgruppe (p=0,861) keine signifikante Verbesserung der Beweg-lichkeit verzeichnet werden. Messungen mit weiteren Messinstrumenten zeigten keine Verbesserungen. Bei den zwei Probanden der Fallstudie konnte, über einen Zeitraum von vier Wochen, eine Verbesserung der Beweglichkeit der FTL festgestellt werden.
Schlussfolgerung: Durch die Anwendung der Foam Roll konnte die Beweglichkeit der Fascia Thoracolumbalis signifikant verbessert werden.

Hintergrund
Präventive Maßnahmen werden im Rahmen der Gesundheitsförderung immer bedeutsamer. Die regelmäßige Bewegung führt u. a. zu einer Stärkung des Herz-Kreislauf-Systems sowie zur Reduktion von muskuloskelettalen Beschwerden. Angestellte von Hochschulen sowie Studierende leiden häufig unter muskuloskelettalen Beschwerden.
Ziel
Ziel der Kundenbefragung ist es, die Zufriedenheit mit dem hochschulinternen Fitnessstudio INMOVE abzufragen, das rein physiotherapeutisch konzipiert ist. Mithilfe des Fragebogens sollen v. a. die Bereiche der Zufriedenheit, der physiotherapeutischen Betreuung sowie die subjektive Lebensqualität beurteilt werden.
Methode
Es wurde eine Kundenbefragung mit 26 Fragen, welche in vier Abschnitte eingeteilt sind, durchgeführt. Die geschlossenen Fragen wurden deskriptiv ausgewertet und die acht offenen Fragen wurden mit der Kernsatzmethode nach Leithäuser und Volmerg (1988) ausgewertet.
Ergebnisse
An der Befragung nahmen insgesamt 70 Trainierende (weiblich: 22/31,2 %, männlich: 48/68,8 %) teil. Anhand der Ergebnisse zeigt sich, dass die Teilnehmenden die Frage „Inwieweit sind die Kunden des hier genannten INMOVE mit dem Fitnessstudio zufrieden?“ mit „ziemlich zutreffend“ und „zutreffend“ beantworteten. Die zweite Forschungsfrage „Ist die physiotherapeutische Betreuung adäquat für das Fitnessstudio?“ kann mit „ziemlich zutreffend“ beantwortet werden. Die dritte Forschungsfrage bezog sich auf die Verbesserung der subjektiven Lebensqualität. Die Antwortenverteilung spiegelt, dass die Teilnehmenden diesen Themenbereich mit überwiegend „trifft genau zu“ oder „trifft ziemlich zu“ beantworteten.
Schlussfolgerung
Anhand der Kundenbefragung konnte die Zufriedenheit sowie die Auswirkungen auf die subjektive Lebensqualität beurteilt werden. Es lässt sich aus den Ergebnissen ableiten, dass ein hochschulinternes Fitnessstudio dabei helfen kann, Alltagsstress abzubauen und somit präventiv bei muskuloskelettalen Beschwerden nützlich ist.
Greenwashing, defined by the Oxford Dictionary as “disinformation disseminated by an organization so as to present an environmentally responsible public image” can cause multifarious problems for companies. The phenomenon of greenwashing has, however, not attracted much attention in the event marketing literature to date. The purpose of this paper is twofold. It first describes and analyses the specific characteristics and features of greenwashing in event marketing. It then seeks to identify the current fundamental approaches of how to avoid greenwashing in event marketing and to assess their potential. A two-step literature analysis with complementary search approaches served as a methodical framework. First, journals related to event marketing were screened for the keywords “greenwashing” and “greenwash”. Next, the general literature was consulted for the same keywords. The results clearly demonstrate that the subject of greenwashing has been widely neglected in the event literature. There appears to be no overall concept or approach that allows event actors to avoid greenwashing, albeit various individual initiatives exist. However, it also became clear that initiatives against greenwashing in event marketing can be developed and implemented in the short and long term, for example by integrating different stakeholders. Additional political and juridical efforts based on specific guidelines are also necessary to prevent greenwashing in the future. The study is the first one to provide a systematic approach to the topic of greenwashing in the context of event marketing, including relevant approaches for its avoidance. It can thus help practitioners to better detect and avoid greenwashing in the event industry and to guide similar research in the future.
The study addresses staffing and workforce issues for home‐ and community‐based long‐term care in Germany. It is based on a study aimed at developing staffing recommendations for home‐care provider organisations. The study was commissioned within the regulation of the German long‐term care act. Following an exploratory literature search on staffing issues in home‐ and community‐based care qualitative interviews with 30 experts in home care were conducted. In addition, time needed for different interventions in homes of people in need of care (n = 129) was measured. Ethical approval for the study was obtained. The literature on the topic is limited. In Germany, no fixed staff‐to‐client ratio exists, but staffing is determined primarily by reimbursement policies, not by care recipients’ needs. The results of the interviews indicated that staffing ratios are not the main concern of home‐care providers. Experts stressed that general availability of staff with different qualification levels and the problems of existing regulation on services and their reimbursement are of higher concern. The measurement of time needed for selected interventions reveals the huge heterogeneity of home‐care service delivery and the difficulty of using a task‐based approach to determine staffing levels. Overall, the study shows that currently demand for home‐care exceeds supply. Staff shortage puts a risk to home care in Germany. Existing approaches of reimbursement‐driven determination of staffing levels have not been sufficient. A new balance between staffing, needs and reimbursement policies needs to be developed.
Der vorliegende Beitrag hat zum Ziel, zunächst die Bedingungen herauszuarbeiten, die zum Gelingen der Anrechnung im Kontext der wissenschaftlichen Weiterbildung erforderlich sind. Im Absprung hiervon soll herausgestellt werden, nach welchen Kriterien eine sinnvolle Verbindung von verschiedenen wissenschaftlichen Weiterbildungsangeboten untereinander bzw. mit Studiengängen geschehen sollte. Zur Untersuchung steht, unter welchen Bedingungen die Vergleichbarkeit von Zertifikaten der wissenschaftlichen Weiterbildung gewährleistet und eine einheitliche, transparente sowie weitestgehend verbindliche Zertifikatssystematik für die Bezeichnung von Abschlüssen im Kontext der wissenschaftlichen Weiterbildung implementiert werden kann.
Access to digital technologies depends on the availability of technical infrastructure, but this access is unequally distributed among social groups and newly summarized under the term digital divide. The aim is to analyze the perception of a tracing app to contain Covid-19 in Germany. The results showed that participants with the highest level of formal education rate the app as beneficial and were the most likely to use the app.
Background
Digital health technologies enable patients to make a personal contribution to the improvement of their health by enabling them to manage their health. In order to exploit the potential of digital health technologies, Internet-based networking between patients and health care providers is required. However, this networking and access to digital health technologies are less prevalent in sociodemographically deprived cohorts. The paper explores how the use of digital health technologies, which connect patients with health care providers and health insurers has changed during the COVID-19 pandemic.
Methods
The data from a German-based cross-sectional online study conducted between April 29 and May 8, 2020, were used for this purpose. A total of 1.570 participants were included in the study. Accordingly, the influence of sociodemographic determinants, subjective perceptions, and personal competencies will affect the use of online booking of medical appointments and medications, video consultations with providers, and the data transmission to health insurers via an app.
Results
The highest level of education (OR 1.806) and the presence of a chronic illness (OR 1.706) particularly increased the likelihood of using online booking. With regard to data transmission via an app to a health insurance company, the strongest increase in the probability of use was shown by belonging to the highest subjective social status (OR 1.757) and generation Y (OR 2.303). Furthermore, the results show that the higher the subjectively perceived restriction of the subjects' life situation was due to the COVID-19 pandemic, the higher the relative probability of using online booking (OR 1.103) as well as data transmission via an app to a health insurance company (OR 1.113). In addition, higher digital literacy contributes to the use of online booking (OR 1.033) and data transmission via an app to the health insurer (OR 1.034).
Conclusions
Socially determined differences can be identified for the likelihood of using digital technologies in health care, which persist even under restrictive conditions during the COVID-19 pandemic. Thus, the results indicate a digital divide with regard to the technologies investigated in this study.
Hebammenteams in Deutschland – Lösungsansätze für Herausforderungen im freiberuflichen Arbeitsfeld

Hintergrund: Ein zunehmender Mangel an berufstätigen Hebammen in Deutschland ist zum Teil auf die Unzufriedenheit vieler Hebammen mit den Arbeitsbedingungen zurückzuführen. Dies führt vielfach zu Reduzierung der Arbeitszeit und Berufsaufgabe. Angeregt durch Teamstrukturen der Pflegeorganisation Buurtzorg in den Niederlanden, deren Mitarbeiter eine sehr hohe Zufriedenheit auf-weisen, wurde die Frage gestellt, inwieweit auch Hebammen im Team innerhalb des bestehenden Systems in Deutschland ihre Arbeitsbedingungen verbessern können.
Ziel: Da freiberufliche Hebammen mehr Gestaltungsmöglichkeiten haben als in der Klinik, soll her-ausgearbeitet werden, welche Lösungen für Herausforderungen im freiberuflichen Arbeitsfeld von bestehenden Hebammenteams entwickelt wurden, in welcher Weise diese die Zufriedenheit im Beruf erhöhen können, und welche Anregungen von Buurtzorg zusätzlich zu einer Verbesserung der Arbeitsbedingungen beitragen könnten.
Methode: In einer qualitativen Querschnittsstudie wurden fünf leitfadengestützte Interviews mit Ver-tretern von Hebammenteams in Deutschland und eines mit Mitarbeiterinnen der niederländischen Wochenpflegeorganisation Buurtzorgkraam, einer Tochter von Buurtzorg, geführt. Die Auswertung der transkribierten Interviews erfolgte angelehnt an die inhaltlich strukturierende Inhaltsanalyse nach Kuckartz.
Ergebnisse: Die Organisation im Team kann u.a. in folgenden Bereichen eine Verbesserung der Arbeitsbedingungen zeitigen: geregelte Freizeit, Rationalisierung mit Freisetzung von mehr Zeit für die Betreuung sowie höhere Vergütung. Durch die Teamarbeit selbst kommen weitere Herausforderungen auf, insbesondere in den Bereichen Entscheidungsfindung, Kommunikation, Fluktuation und Wachstum.
Diskussion: Besondere Vorteile von Teamarbeit sind die Unterstützung durch Teamkolleginnen und die Sicherstellung der Betreuungskontinuität bei erforderlichem Wechsel der Hebamme. Vielen potenziell belastenden Faktoren kann im Team besser begegnet werden. In Bezug auf Entscheidungsfindung und Wachstum bzw. Teilung von Teams kann der Blick auf Buurtzorg zusätzliche Anregungen geben. Die Frage, ob die erhöhte Arbeitszufriedenheit tatsächlich zu einer Verbesserung der Versorgunglage führt, bleibt offen und sollte Gegenstand weiterer Forschungen sein.
In Germany, a lot of young children at risk of language difficulties still go undetected or are not assessed before preschool-age. For children where parents may suspect a disorder, this practice causes a lot of emotional distress alongside lost time for intervention. Thus, what contribution can parents and nursery staff make for the earlier detection of language difficulties? 34 children from four German kindergartens were tested with a standardized preschool screening for language problems by an SLT. Parents and nursery staff completed a questionnaire (FEE 3-4) that was designed to collect potential risk-factors and included the rating of children’s abilities across the main language domains. Outcomes from the FEE 3-4 were compared between parents and nursery staff as well as triangulated with results from the standardized screening. Agreement between parents and nursery staff re. individual children’s potential language difficulties was moderate (Kappa = 0.44, p = .050). Overall, nursery staff rated children’s language abilities more strictly and precisely than parents. Especially their rating of ‘word order’ (p = .022) and ‘verb endings’ contributed significantly to the identification of potential language difficulties similar to the standardized screening. The screening identified two children at risk without caregiver's concern, but not two others who were at risk of language disorder and for whom caregivers expressed concern. Caregiver’s awareness of early language difficulties appears to be rather intuitive. Young children at risk are most reliably detected if standardized instruments are used in combination with caregiver questionnaires. Ideally, this process includes data from parents and nursery staff to be interpreted by an experienced SLT, as the use of a standardized screening alone may lead to missed or mistaken identification where essential information about the child’s environment (e.g. risk factors) is not provided. If parents are concerned about children’s language, full assessment is clearly justified.
The increasing complexity of caseloads in SLT practice, e.g. due to higher comorbidity, lacking information or experience in the
treatment of complicated cases, calls for support from experienced as well as specialist practitioners from within the field - especially
for novice therapists. One way to tackle these challenges may be peer coaching and how it can be employed within the educational
and professional SLT setting.
Peer coaching was implemented across five semesters of a successive SLT study programme at a University of Applied Sciences in
Germany. The approach was embedded in a clinical reasoning seminar with 25 SLT students who each presented a challenging case
study from their current workload. All participants completed a short online survey to evaluate the feasibility of the team approach
within this setting as well as their personal benefit and development re. the discussed case studies.
Students felt encouraged by being able to share their experience and tackle actual challenges. They particularly valued receiving
answers from a broad range of other SLTs but also contributing to other students’ queries and providing practical solutions for
them. All participants felt that peer coaching was an appropriate approach for clinical reasoning to support their professional as well
as personal development. Other outcomes were a perceived increased ability to employ metacognitive reflection to be used with
their whole caseload but also a prospective need for further training. Some students suggested the employment of peer coaching
within their work setting.
In the educational as well as professional SLT setting, peer coaching can be successfully employed, triggering metacognitive
reflection re. practitioner’s thinking and acting, resulting in an increased awareness of needs and skills as part of the clinical
reasoning process.
Background: Multilingual children with suspected SLCN are often overlooked or their needs not accurately differentiated regarding the necessity of language support or therapy. The purpose of the study was to conceptualize, carry out and evaluate a local language support (LS) project within linguistically and culturally diverse (LCD) families and its effects on all collaborating participants.
Methods: Eight SLT students and one lecturer took part in the LS-project, alongside equivalent numbers of family liaison personnel. Students visited more than 10 young children aged between 2-6 years, and for each child 10 weekly home visits were carried out. Language enhancement was documented, several case studies with children and interviews with five liaison personnel conducted.
Results: All SLT students perceived changes in the behaviour and communication of participating children. Children in the case studies developed from pre-verbal to verbal means of communication and family liaison personnel reported positive changes alongside parental wishes to continue the support.
Conclusion: Local language support projects with LCD families can lead to positive differences regarding their children's communication development and better inclusion in mainstream society. SLT students benefit from working with LCD families and their collaborative support together with family liaison personnel, and vice versa.
Learning Outcomes: To differentiate the influence of language enhancement vs. formalized SLT therapy. To enhance the relationship with LCD clientele and collaboration with liaison personnel in SLCN settings. To incorporate life-long learning and intercultural sensitization.
Die Realisierung von Klimaschutzvorhaben ist für die Menschheit essenziell. Nach der deutschen Bundesumweltministerin Svenja Schulze handelt es sich beim Klimaschutz sogar um die zentrale Existenzfrage der Menschheit, da die Folgen des Klimawandels das gesellschaftliche Leben gravierend verändern werden. Als unterste Stufe des exekutiven Verwaltungsaufbaus sind die Kommunen mit ihren Mitarbeitern dafür zuständig, dass Klimaschutzkonzepte zur Abmilderung des Klimawandels auf der lokalen Ebene realisiert werden. Vorangehende Forschungen rechtfertigen jedoch die Vermutung, dass mitarbeiterbezogene Akzeptanzprobleme innerhalb der Kommunalverwaltung eine adäquate Realisation von Klimaschutzkonzepten behindern.
An diese Vermutung anknüpfend deckt diese Bachelorarbeit potenzielle mitarbeiterbezogene Akzeptanzprobleme bei der Realisierung von Klimaschutzkonzepten in der Kommunalverwaltung auf, analysiert die Ursachen und bietet einen Lösungsansatz zur Abmilderung typischer Akzeptanzprobleme.
Um diese Forschungsziele zu erreichen, sind Experteninterviews mit verschiedenen Akteuren durchgeführt worden, die entweder selbst in der Kommunalverwaltung tätig sind oder im Bereich des Klimaschutzes mit kommunalen Akteuren zusammenarbeiten. Zentraler Gegenstand der Interviews waren die Akzeptanzprädikatoren einer angepassten Version der Unified Theory of Acceptance and Use of Technology von Venkatesh, Morris, Davis und Davis.
Die Antworten der Interviews zeigen dabei, dass häufig der Aufwand bei Klimaschutzbestrebungen von der Belegschaft als zu groß eingeschätzt wird. Klimaschutz wird als ein Übel ohne Nutzen gesehen. Ebenso spielen Versagensängste, fehlende Kenntnisse beim Wie der Realisation und mangelnder vorgelebter Aktionismus eine Rolle. Ursächlich dafür sind interne kulturelle Differenzen, Hemmnisse durch die Politik und Managementschwächen der Verwaltungsführung.
Als Lösungsansatz für die Abmilderung der typischen mitarbeiterbezogenen Akzeptanzprobleme hat sich die Mitarbeitersensibilisierung durch ein starkes integriertes Klimaschutzmanagement herauskristallisiert.
Die Nutzung des sozialen Online-Netzwerkes „Facebook“ ist ein wichtiger Bestandteil in der
Lebenswelt von Jugendlichen und jungen Erwachsenen, welcher viele Vorteile, jedoch auch
Nachteile mit sich bringt. Der theoretische Teil dieser Arbeit setzt sich mit dem Begriff der
Jugend auseinander, stellt besondere Merkmale dieser Lebensphase dar und erläutert das
Konzept der Entwicklungsaufgaben, um in diesem Zusammenhang den Bezug zum sozialen
Online-Netzwerk „Facebook“ herzustellen. Anschließend wird der Begriff des sozialen Online
Netzwerkes bestimmt, die Geschichte beziehungsweise die Erfolge von Facebook genauer
thematisiert und eine Auswahl an Funktionen vorgestellt, um den Leser in das Themenfeld
einzuführen. Der zweite Teil der Arbeit beinhaltet die empirische Untersuchung, um
herauszufinden in welcher Alterskategorie der Hilfebedarf zur Unterstützung und Begleitung
seitens der Sozialen Arbeit am größten ist. Anhand der empirischen Methode des
Fragebogens wurden Daten erhoben, um die Forschungsfrage zu beantworten, wie
Jugendliche und junge Erwachsene im Alter von 13 bis 25 Jahren Facebook nutzen.
Bezüglich der Konkretisierung der Fragestellung wurden sechs Hypothesen hergeleitet, um
verschiedene Teilbereiche der Facebok-Nutzung, wie zum Beispiel das Datenschutzthema
oder die täglich verbrachte Zeit, genauer untersuchen zu können. Die Stichprobe von N=
108, die im Schneeballsystem erhoben wurde, setzt sich aus Teilnehmern jeglichen
Geschlechts, Alters oder sozialer Schicht zusammen. Damit die Teilnehmer miteinander
verglichen werden können, wurden sie in drei verschiedene Kategorien von 13 bis 15
Jahren, 16 bis 18 Jahren und 19 bis 25 Jahren, unterteilt.
Als Resultat der empirischen Untersuchung wird deutlich, dass sich die drei verschiedenen
Alterskategorien nur in einigen Themenbereichen unterschieden und ansonsten ähnliche
Ansichten teilen. Lediglich die Alterskategorie der 16-18jährigen nimmt eine besondere Rolle
ein, da sie sich am stärksten von den anderen beiden Kategorien unterscheidet, da zum
Beispiel alle Teilnehmer dieser Kategorie bei Facebook einen Account besitzen und täglich
auch am längsten online sind.
Dadurch ergibt sich für die Soziale Arbeit ein Handlungsbedarf, der an kein bestimmtes Alter
gebunden ist, sondern sich an alle Alterskategorien richten sollte.
Land cover change is a dynamic phenomenon driven by synergetic biophysical and socioeconomic effects. It involves massive transitions from natural to less natural habitats and thereby threatens ecosystems and the services they provide. To retain intact ecosystems and reduce land cover change to a minimum of natural transition processes, a dense network of protected areas has been established across Europe. However, even protected areas and in particular the zones around protected areas have been shown to undergo land cover changes. The aim of our study was to compare land cover changes in protected areas, non-protected areas, and 1 km buffer zones around protected areas and analyse their relationship to climatic and socioeconomic factors across Europe between 2000 and 2012 based on earth observation data. We investigated land cover flows describing major change processes: urbanisation, afforestation, deforestation, intensification of agriculture, extensification of agriculture, and formation of water bodies. Based on boosted regression trees, we modelled correlations between land cover flows and climatic and socioeconomic factors. The results show that land cover changes were most frequent in 1 km buffer zones around protected areas (3.0% of all buffer areas affected). Overall, land cover changes within protected areas were less frequent than outside, although they still amounted to 18,800 km2 (1.5% of all protected areas) from 2000 to 2012. In some parts of Europe, urbanisation and intensification of agriculture still accounted for up to 25% of land cover changes within protected areas. Modelling revealed meaningful relationships between land cover changes and a combination of influencing factors. Demographic factors (accessibility to cities and population density) were most important for coarse-scale patterns of land cover changes, whereas fine-scale patterns were most related to longitude (representing the general east/west economic gradient) and latitude (representing the north/south climatic gradient).
The Corona pandemic confronted societies with several unexpected constraints that had the effect of making certain goods much scarcer than before. Withdrawal from Russian oil and gas supplies has a similar effect. Carbon abatement can also be seen as a deliberate choice to make certain goods scarc-er than they actually are. These parallels suggests that it may be worthwhile to take a close look at societies’ responses to all three challenges. This paper makes an attempt to synthetize empirical and theoretical insights regarding these scarcity shocks from a well-being perspective, i.e. replacing the prevalent welfare economic focus on production and consumption with a focus on sustainable well-being.
Taking the case of Germany, it will be argued that the observed responses to all three challenges reflect a focus on maintaining incomes and production and that therefore these responses risk being detrimental to sustainable wellbeing and even to economic stability. This is particularly relevant if carbon abatement requires not only transient material sacrifices but lasting and significant reduc-tions of consumption.
It will be argued that the impact of these new scarcities will be much less problematic in a society that acknowledges the priority of sustainable wellbeing over production and consumption measures. Such a society would still need to incur material sacrifices, but these need not translate into a loss of wellbeing if economic conditions and social norms adapt. This will also be more sustainable not only in terms of ecological impact, but also in terms of debt, inflation and inequality.
The assessment of somatosensory function is a cornerstone of research and clinical practice in neurology. Recent initiatives have developed novel protocols for quantitative sensory testing (QST). Application of these methods led to intriguing findings, such as the presence lower pain-thresholds in healthy children compared to healthy adolescents. In this article, we (re-) introduce the basic concepts of signal detection theory (SDT) as a method to investigate such differences in somatosensory function in detail. SDT describes participants’ responses according to two parameters, sensitivity and response-bias. Sensitivity refers to individuals’ ability to discriminate between painful and non-painful stimulations. Response-bias refers to individuals’ criterion for giving a “painful” response. We describe how multilevel models can be used to estimate these parameters and to overcome central critiques of these methods. To provide an example we apply these methods to data from the mechanical pain sensitivity test of the QST protocol. The results show that adolescents are more sensitive to mechanical pain and contradict the idea that younger children simply use more lenient criteria to report pain. Overall, we hope that the wider use of multilevel modeling to describe somatosensory functioning may advance neurology research and practice.
Multiple-group confirmatory factor analysis (MG-CFA) is among the most productive extensions of.structural equation modeling. Many researchers conducting cross-cultural or longitudinal studies are interested in testing for measurement and structural invariance. The aim of the present paper is to provide a tutorial in MG-CFA using the freely available R-packages lavaan, semTools, and semPlot. The combination of these packages enable a highly efficient analysis of the measurement models both for normally distributed as well as ordinal data. Data from two freely available datasets – the first with continuous the second with ordered indicators - will be used to provide a walk-through the individual steps.
Background:
Recurrent pain is a common experience in childhood, but only few children with recurrent pain attend a physician. Previous studies yielded conflicting findings with regard to predictors of health care utilization in children with recurrent pain.
Methods:
The present study analyzes data from the German Health Interview and Examination Survey for Children and Adolescents (KiGGS) study comprising n = 2,149 children (3–10 years old) with recurrent pain to find robust predictors.We used multiple logistic regressions to investigate age, gender, socio-economic status(SES), migration background, pain intensity, pain frequency, pain-related disability, mental health problems, and health-related quality of life (HRQL) as predictors for visiting a doctor due to pain.
Results:
Overall, young girls with high pain-related disability, intensity, frequency, and migration background were more likely to attend a physician. Pain-related disability had the largest impact. Socioeconomic status, health-related quality of life anmental health problems were not systematically related to health care utilization. An analysis of the variability of these results indicated that several hundred participants
are needed until the results stabilize.
Conclusions:
Our findings highlight the importance of pain-related disability and frequency in assessing the severity of recurrent pain. Generic predictors and demographic variables are of lesser relevance to children with recurrent pain. On a methodological level, our results show that large-scale studies are need to reliably
identify predictors of health care utilization.
Guided by cultural labor economics, the paper analyzes the career paths of former actors from popular television soap operas, and addresses in particular, if and under which conditions such serial engagements may function as a stepping stone for a subsequent professional acting career. A novel database of 396 German artists with detailed and long-term biographical information is used for the quantitative empirical analyses. The results indicate that soaps, contrary to popular opinion, function as a stepping stone, especially for younger actors. However, soap engagements should be rather short but long enough to allow artists to play multiple roles in other shows or films besides being in the cast of a soap. While formal acting education does not influence soap actors’ future filmographies, there is evidence that it helps artists to find jobs in arts-related occupations such as voice acting. Finally, publicity and media presence foster a later acting career. Practical implications for artists and their managers are outlined, along with a discussion on the meaning of serials for the creation and commercialization of stars.
Over the past decade, the proliferation of entrepreneurial ventures and small firms with sustainable business models have pushed the sustainability agenda in the fashion industry. Despite prominent success stories, these companies often encounter significant operative challenges that hinder growth or even cause business failure. This paper adopts a qualitative research design and uses data gathered during 18 expert interviews with entrepreneurs and executive managers of 18 firms from 13 countries to identify and explore the dominant challenges that micro- and small-sized sustainable fashion companies struggle with. The results indicate that communicating with consumers is perceived as surprisingly difficult, especially because sustainable brands need to justify higher prices and explain the added benefits of their products. Second, resource constraints lead to operative friction and overburdening of the decision makers and their small management teams. Third, sustainable sourcing and production processes are economically difficult in an industry that continues to occupy a mass and low-cost logic. Against the background of these insights, a range of managerial recommendations are developed and contextualized in the framework of the Business Model Canvas. This novel approach makes the suggestions which are rooted in the resource-based view actionable and supports sustainable businesses to better manage their operations and achieve growth. For example, collaborative approaches with different stakeholders may mitigate constraints across all three identified problem areas.
Das Ziel der vorliegenden Ausarbeitung ist es darzustellen, welchen psychosozialen Belastungen Väter frühgeborener Kinder während der Zeit der kindlichen Hospitalisierung auf der Neonatologie ausgesetzt sind und welcher Auftrag sich dadurch für die Soziale Arbeit in diesem Praxisfeld ergibt. Dazu wird die Forschungsfrage gestellt: „Welchen psychosozialen Belastungen sind Väter von frühgeborenen Kindern während des kindlichen Aufenthaltes auf der Neonatologie ausgesetzt?“. Für das Forschungsvorhaben wurde eine quantitative Studie im Querschnittdesign mit der Zielgruppe von Vätern frühgeborener Kinder durchgeführt. Die Ergebnisse der Erhebung zeigen, dass besonders die Aufteilung zwischen innerklinischen sowie außerklinischen Verantwortungen und die fehlende Ausübung von Väterrollen die größten Belastungen darstellen. Die Väter zeigen zudem mehrheitlich Symptome einer Belastungsstörung. Gesprächen mit Partner*innen oder Familienangehörigen werden am häufigsten eine entlastende Wirkung zugeschrieben. Insgesamt zeigt sich, dass die Mehrheit der befragten Väter an keiner Elternberatung teilgenommen hat. Jedoch wünscht sich die breite Mehrheit aller befragten Väter explizite Angebote der Elternberatung für Väter frühgeborener Kinder. Aus den Ergebnissen ergibt sich für die Soziale Arbeit ein Auftrag, für die Belange und Bedürfnisse von Vätern frühgeborener Kinder auf der gesellschaftspolitischen Ebene und im Netzwerk einzustehen sowie die Elternberatung in der Neonatologie durch qualifizierte Weiterbildung und organisatorische Umstrukturierung zielgruppenfreundlich auszurichten.
Wirtschaftsinformatik und Medizinische Informatik gehören zu den sogenannten Bindestrich-Informatik-Fächern, die sich mit der Anwendung der Methoden und Erkenntnisse der Informatik, aber auch mit der Weiterentwicklung solcher Methoden und Erkenntnisse für gewisse Anwendungsgebiete befassen. Auf einer Podiumsdiskussion der Jahrestagung 2018 der Deutschen Gesellschaft für Medizinische Informatik, Biometrie und Epidemiologie (GMDS) wurde für Wirtschaftsinformatik, Medizinische Informatik und Informatik analysiert wie sie zueinander stehen. Die Analyse erfolgte anhand von fünf Fragen:
1. Welche grundlegenden Ziele bestimmen die jeweilige wissenschaftliche Arbeit?
2. Wie ist der Praxisbezug ausgeprägt?
3. Inwieweit sind Besonderheiten von Medizin bzw. Ökonomie prägend für die jeweilige wissenschaftliche Arbeit?
4. Welche Rolle spielen Theoriefundierung und Evidenz?
5. Was können Wirtschaftsinformatik und Informatik von Medizinischer Informatik und Medizin lernen – und umgekehrt?
Die Analyse zeigt, dass die drei Disziplinen von einem systematischen wechselseitigen Austausch profitieren können. Das „Lernende Gesundheitssystem“ bietet Ansätze für einen entsprechenden Rahmen.
Background: For more than 30 years, there has been close cooperation between Japanese and German scientists with regard to information systems in health care. Collaboration has been formalized by an agreement between the respective scientific associations. Following this agreement, two joint workshops took place to explore the similarities and differences of electronic health record systems (EHRS) against the background of the two national healthcare systems that share many commonalities.
Objectives: To establish a framework and requirements for the quality of EHRS that may also serve as a basis for comparing different EHRS.
Methods: Donabedian's three dimensions of quality of medical care were adapted to the outcome, process, and structural quality of EHRS and their management. These quality dimensions were proposed before the first workshop of EHRS experts and enriched during the discussions.
Results: The Quality Requirements Framework of EHRS (QRF-EHRS) was defined and complemented by requirements for high quality EHRS. The framework integrates three quality dimensions (outcome, process, and structural quality), three layers of information systems (processes and data, applications, and physical tools) and three dimensions of information management (strategic, tactical, and operational information management).
Conclusions: Describing and comparing the quality of EHRS is in fact a multidimensional problem as given by the QRF-EHRS framework. This framework will be utilized to compare Japanese and German EHRS, notably those that were presented at the second workshop.
Objective: To pilot benchmark measures of health information and communication technology (ICT) availability and use to facilitate cross-country learning.
Materials and Methods: A prior Organization for Economic Cooperation and Development–led effort involving 30 countries selected and defined functionality-based measures for availability and use of electronic health records, health information exchange, personal health records, and telehealth. In this pilot, an Organization for Economic Cooperation and Development Working Group compiled results for 38 countries for a subset of measures with broad coverage using new and/or adapted country-specific or multinational surveys and other sources from 2012 to 2015. We also synthesized country learnings to inform future benchmarking.
Results: While electronic records are widely used to store and manage patient information at the point of care—all but 2 pilot countries reported use by at least half of primary care physicians; many had rates above 75%—patient information exchange across organizations/settings is less common. Large variations in the availability and use of telehealth and personal health records also exist.
Discussion: Pilot participation demonstrated interest in cross-national benchmarking. Using the most comparable measures available to date, it showed substantial diversity in health ICT availability and use in all domains. The project also identified methodological considerations (e.g., structural and health systems issues that can affect measurement) important for future comparisons.
Conclusion: While health policies and priorities differ, many nations aim to increase access, quality, and/or efficiency of care through effective ICT use. By identifying variations and describing key contextual factors, benchmarking offers the potential to facilitate cross-national learning and accelerate the progress of individual countries.
This workshop will review the history of the TIGER initiative in order to set the framework for an understanding of international informatics competencies. We will include a description of clinical nursing informatics programs in 37 countries as well as the results of a recent survey of nursing competencies in order to further discussions of internationally agreed-upon competency definitions. These two surveys will provide the basis for developing a consensus regarding the integration of core competencies into informatics curriculum developments. Expected outcomes include building consensus on core competencies and developing plans toward implementing intra- and inter-professional informatics competencies across disciplines globally.
In September 2022, the interprofessional European Summer School on the topic “Information in Healthcare – From Data to Knowledge” was held at the University of Porto. This Summer School included the topics Interoperability, Data Protection and Security and Data Analytics and consisted of an online preparation phase and an attendance phase in Porto. The didactic concept involved problem-based learning using a case study. A variety of course materials were developed and used to achieve the learning objectives. There are plans to continue the Summer School concept at participating institutions in the future, starting with a Spring School 2023 in Osnabrück.
Interoperability, Data Protection and Security and Data Analytics are of high relevance for the future of eHealth and interprofessional care. Three online courses were therefore designed and delivered for these topics, all of which followed the same structure. A variety of materials were developed and different tools for knowledge transfer, communication and collaboration were used.
Der zunehmende Einsatz von Informations- und Kommunikationstechnologie im Gesundheitswesen verlangt auch von Angehörigen der Pflegeberufe Kompetenzen zur Nutzung der entsprechenden Systeme und Verfahren. Vor diesem Hintergrund haben sich die AG „Informationsverarbeitung in der Pflege“ der Deutschen Gesellschaft für Medizinische Informatik, Biometrie und Epidemiologie (GMDS), die Österreichische Gesellschaft für Pflegeinformatik (ÖGPI) und die Schweizerische Interessensgruppe Pflegeinformatik (IGPI) innerhalb des Schweizer Berufsverband der Pflegefachfrauen und Pflegefachmänner (SBK) entschlossen, gemeinsame Empfehlungen für benötigte Kernkompetenzfelder in Pflegeinformatik zu erarbeiten. Auf Basis einer iterativen multimethodischen Vorgehensweise unter Einbeziehung von einer großen Anzahl von Fachexperten aus Deutschland, Österreich und der Schweiz (D-A-CH) wurden 24 notwendige Felder von Kernkompetenzen definiert und deren Relevanz für fünf typische Berufsfelder in der Pflege bewertet. Damit liegt erstmalig eine wissenschaftlich fundierte Empfehlung für zu vermittelnde Kernkompetenzfelder in der Pflegeinformatik für verschiedene pflegerische Berufsfelder vor. Sie richtet sich an alle Personen mit Verantwortung für die Planung von Studium, Lehre, Aus- und Weiterbildung in der Pflege.
Background: IT adoption is a process that is influenced by different external and internal factors. This study aimed
1. to identify similarities and differences in the prevalence of medical and nursing IT systems in Austrian and German hospitals, and
2. to match these findings with characteristics of the two countries, in particular their healthcare system, and with features of the hospitals.
Methods: In 2007, all acute care hospitals in both countries received questionnaires with identical questions. 12.4% in Germany and 34.6% in Austria responded.
Results: The surveys revealed a consistent higher usage of nearly all clinical IT systems, especially nursing systems, but also PACS and electronic archiving systems, in Austrian than in German hospitals. These findings correspond with a significantly wider use of standardised nursing terminologies and a higher number of PC workstations on the wards (average 2.1 PCs in Germany, 3.2 PCs in Austria). Despite these differences, Austrian and German hospitals both reported a similar IT budget of 2.6% in Austria and 2.0% in Germany (median).
Conclusions: Despite the many similarities of the Austrian and German healthcare system there are distinct differences which may have led to a wider use of IT systems in Austrian hospitals. In nursing, the specific legal requirement to document nursing diagnoses in Austria may have stimulated the use of standardised terminologies for nursing diagnoses and the implementation of electronic nursing documentation systems. Other factors which correspond with the wider use of clinical IT systems in Austria are: good infrastructure of medical-technical devices, rigorous organisational changes which had led to leaner processes and to a lower length of stay, and finally a more IT friendly climate. As country size is the most pronounced difference between Germany and Austria it could be that smaller countries, such as Austria, are more ready to translate innovation into practice.
