Mit der Ausgestaltung von Ganztagsschulen korrespondieren organisatorische und pädagogische Merkmale, die einen Rahmen schaffen können, um eine neue individualisierende sowie unterrichtliche Bildung und außerunterrichtliche Erziehung zusammenführende Praxis zu etablieren. Insbesondere erweiterte Kompetenzanforderungen sowie neue Formen der intra- und interprofessionellen Kooperation zeigen sich hierbei als Merkmal professioneller Förderung. Im Beitrag interessiert uns daher die Frage, inwieweit die fachliche Maxime der individuellen Förderung im Schulalltag anschlussfähig an das pädagogische Handeln der in den Ganztag eingebunden Akteure ist. Hier zeigen die berichteten Ergebnisse unserer Studie eine kaum operationalisierbare und differenzierbare Matrix von solchen Handlungsprozessen, die auf einer systematischen methodischen Grundlage basieren. Es dominiert ein eher situatives, den alltäglichen Ausprägungen von Förderbedarfen gerecht werdendes Handeln, das jedoch kaum in Bezug zu einem Meta-Wissen, einem Verweisungs- und Analysewissen gesetzt wird. Gegenwärtig zeigen sich auf Seiten der Ganztagsakteure unterschiedliche Interventionssituationen und Rollenerwartungen im Kontext der Förderung, die es nicht anzugleichen, aber dennoch professionstheoretisch und -praktisch in ihrer Komplementarität zu verorten gilt.

Begleitend zu den Beratungen eines Beirats des Bundesministeriums für Gesundheit zur Überprüfung des Pflegebedürftigkeitsbegriffs im SGB XI wurde ein neues Begutachtungsassessment (NBA) zur Feststellung der Pflegebedürftigkeit entwickelt, das auf einem pflegewissenschaftlich begründeten Verständnis von Pflegebedürftigkeit aufbaut. Darin werden gleichermaßen körperliche wie kognitive / psychische Beeinträchtigungen und Verhaltensauffälligkeiten berücksichtigt und die derzeit geltende Engführung auf wenige körperbezogene Alltagsverrichtungen bei der Bestimmung von Pflegebedürftigkeit wird überwunden. Als Maßstab zur Einschätzung von Pflegebedürftigkeit wird im NBA nicht die Pflegezeit, sondern der Grad der Selbständigkeit bei der Durchführung von Aktivitäten oder der Gestaltung von Lebensbereichen verwendet. Das NBA ist auch für die Einschätzung der Pflegebedürftigkeit bei Kindern geeignet und hat sich bei einer bundesweiten praktischen Erprobung gut bewährt. Es wurde vom Beirat zur Überprüfung des Pflegebedürftigkeitsbegriffs als zukünftiges Instrument zur Begutachtung der Pflegebedürftigkeit im Rahmen der Pflegeversicherung empfohlen.
Die Entlassung aus dem Krankenhaus bleibt, trotz mittlerweile jahrzehntelanger Diskussion, ein wichtiges Problem der Gesundheitsversorgung. Die Schnittstelle zwischen der intensiven Krankenhausbehandlung und einer poststationären Versorgung im häuslichen Umfeld, in einer Pflegeeinrichtung oder im Rahmen von Rehabilitationsmaßnahmen weist nach wie vor ein erhöhtes Potenzial für Informationsverluste, Diskontinuitäten in der Versorgung und andere Probleme auf, deren Vermeidung zu einer besseren Versorgungsqualität beitragen könnte. Das Deutsche Netzwerk für Qualitätsentwicklung in der Pflege (DNQP) hat bereits im Jahr 2003 einen Expertenstandard „Entlassungsmanagement in der Pflege“ entwickelt, modellhaft implementiert und veröffentlicht. Auf Basis dieses Expertenstandards lässt sich ein Entlassungsmanagement im Krankenhaus systematisch gestalten. Der folgende Beitrag greift die Herausforderungen des Entlassungsmanagements auf und stellt vor diesem Hintergrund die Intention und wesentlichen Inhalte des Expertenstandards dar.
Sozialpädagogik vollzieht sich immer in Räumen, in denen Menschen leben und in denen Institutionen ihren Ort haben. Theorie, konzeptionelle Grundsätze, Handlungsprinzipien und -felder einer kommunalen Sozialpädagogik werden in diesem Band begründet.
Die Kommunen, in diesem Band sind es im Schwerpunkt Städte mit ihren Stadtteilen, vermitteln die gesellschaftlichen Integrationsweisen von System und Lebenswelt. Kommunale Sozialpädagogik steht für die pädagogische angeleitete Ermöglichung von Zusammenhalt in der Stadtgesellschaft. Sie wird in diesem Band als intermediäre Instanz zwischen institutionell-professionellen und zivilgesellschaftlichen Öffentlichkeiten begründet. Ein gesellschaftstheoretischer Rahmen für die konzeptionelle Ausrichtung sozialpädagogischen Handelns in Stadtteilen ist ebenso Thema wie die daraus resultierenden Handlungsprinzipien und -felder.
Schule und Kinder- und Jugendhilfe sind in der Kommune gleichermaßen Gegenstand von Fachplanungen. Gerade inklusive Bildung verlangt ein Zusammenwirken beider Felder nicht nur in der (sozial-)pädagogischen Praxis, sondern auch in der kommunalen Steuerung sowie Planung entsprechender Angebotsstrukturen. Wie gelingen derartig abgestimmte Prozesse? Welches Verständnis von Inklusion leitet dabei das Planungsgeschehen und welche Maßstäbe lenken Steuerungsentscheidungen?
Diesen Fragen geht das Buch nach, indem empirische Befunde aus Untersuchungen in drei niedersächsischen Kommunen dargestellt und Konsequenzen für Theorie, Forschung und Praxis in diesem Themenkomplex abgeleitet werden.
Ein erklärtes Ziel der Pflegeversicherung besteht in der Priorität der häuslichen gegenüber der stationären Pflege. Um dieses Ziel zu erreichen, sieht die Pflegeversicherung eine Reihe von Leistungen vor, die in Ergänzung der Pflege durch Angehörige den Verbleib des pflegebedürftigen Menschen in der häuslichen Umgebung ermöglichen sollen. Die Betrachtung einiger Charakteristiken häuslicher Pflegearrangements verdeutlicht, dass die Bedarfslagen oftmals komplexer sind als die vorgesehenen Leistungen und diese nur für Teile davon angemessen erscheinen. Veränderungen und Erweiterungen des Leistungsspektrums der Pflegeversicherung im Laufe der Jahre verdeutlichen das Bemühen die Leistungen weiterzuentwickeln. Die Bedarfslagen in häuslichen Pflegearrangements erfordern jedoch weitere Anpassungen, auch durch die Einführung des neuen Pflegebedürftigkeitsbegriffs, die abschließend in diesem Beitrag skizziert werden.
Dieser Band leistet einen Beitrag zur Klärung von Interprofessionalität in der Kinder- und Jugendhilfe, im Speziellen im Feld der Tagesbetreuung bzw. Familienzentren, beschreibt Netzwerkarbeit als unverzichtbare Grundlage einer erfolgreichen Betreuungspraxis der Zukunft und konturiert das komplexe Anliegen „Interprofessionalität“ in sozialen Handlungsfeldern. Den Schwerpunkt machen Ausführungen zur Konzeption und die Vorstellung von Modulen der Qualifizierung von Interprofessionalität in der Praxis aus: „INPRO“ wird den LeserInnen als systematische Arbeitsgrundlage für die Konzept-, Netzwerk-, Team und Organisationsarbeit präsentiert. Zusätzlich bieten Beiträge internationaler Partner interessante Einblicke in die interprofessionelle Praxis anderer europäischer Länder und regen die Reflexion der hiesigen Praxis an. Im Ergebnis werden notwendige Bedingungen für das anspruchsvolle Vorhaben der Interprofessionalität systematisiert.

Kooperation als Kontinuum : Erweiterte Perspektive einer schulbezogenen Kinder- und Jugendhilfe

In der jüngeren Fachdebatte zur Kinder- und Jugendhilfe wird immer wieder die Kooperation von Jugendhilfe und Schule thematisiert und die Bedeutung dieses Arbeitsbereichs betont. Vor diesem Hintergrund nimmt dieses Buch eine Standortbestimmung zur theoretischen und empirischen Betrachtung der Kooperation von Jugendhilfe und Schule vor. Dabei werden aktuelle Anforderungen des Kooperationsfeldes und dessen Veränderungen aufgezeigt und die empirische Ausprägung von Kooperationsstrukturen und -erfahrungen bewertet. Im Zentrum steht vor allem die Frage: Entstehen neuartige Vernetzungen der Kinder- und Jugendhilfe mit Schule, die Veränderungen in den Arbeitsweisen und Strukturen der Kinder- und Jugendhilfe zur Folge haben?
Die Kinder- und Jugendhilfe ist mit einer Vielzahl von fachlichen und organisatorischen Anforderungen konfrontiert. Stete Überprüfung und Weiterentwicklung ihrer Infrastruktur und ihrer Konzepte ist notwendig. Um vor diesem Hintergrund bedarfsgerechte Jugendhilfeleistungen für junge Menschen und ihre Familien vorhalten zu können, sind Informationen über die Entstehung von Jugendhilfebedarfen und Vorstellungen über die Entwicklung und Gestaltung von Jugendhilfeangeboten wichtig.
Wie lassen sich die aktuellen Entwicklungen in der Kinder- und Jugendhilfe charakterisieren und welche Relevanz haben sie für die Planung? Was sind aktuelle, veränderte Anforderungen an die Organisation von Planungsprozessen? Was sind die neuen und erkennbar nachhaltigen Anforderungen an die Praxis der kommunalen Jugendhilfeplanung? Diesen und anderen Fragen wird in der vollständig überarbeiteten und aktualisierten Neuauflage nachgegangen.
Die zunehmende Schnittstellenposition der Kinder- und Jugendhilfe, ihre Vernetzung mit dem Bildungs- und Gesundheitswesen etwa, verlangt die stetige Klärung ihrer Rollen und Funktionen. Praxisforschung als Kontext- und Prozessforschung kann dabei im begleitenden Kontakt zwischen Wissenschaftler(inne)n und Praktiker(inne)n eine wesentliche Funktion übernehmen. Welche neuen Fragestellungen und Erwartungen, zugeschriebenen Ziele und Funktionen ergeben sich für die Praxisforschung im Kontext einer sich ändernden Praxis der Kinder- und Jugendhilfe? Welchen aktiven Beitrag leistet andererseits Praxisforschung für die Entwicklung der Kinder- und Jugendhilfe? Anhand theoretischer und methodischer Überlegungen sowie praktischer Beispiele wird in diesem Band untersucht, ob das für die Praxisforschung konstituierende Verhältnis zur Praxis - der wechselseitige Transfer von Erkenntnissen - eine neue Qualität bekommt und welche Entwicklungsoptionen sich für die Praxisforschung und die dort tätigen Institutionen ergeben.
Der Einsatz paralleler Hardware-Architekturen betrifft alle Software-Entwickler und -Entwicklerinnen: vom Supercomputer bis zum eingebetteten System werden Multi- und Manycore-Systeme inzwischen eingesetzt. Die Herausforderungen an das Software Engineering sind vielfältig. Zum einen ist (wieder) ein stärkeres Verständnis für die Hardware notwendig. Ohne eine skalierbare Partitionierung der Software und parallele Algorithmen bleibt die Rechenleistung ungenutzt. Zum anderen stehen neue Programmiersprachen im Vordergrund, die die Ausführung von parallelen Anweisungen ermöglichen.
Dieses Buch betrachtet unterschiedliche Aspekte bei der Entwicklung paralleler Systeme und berücksichtigt dabei auch eingebettete Systeme. Es verbindet Theorie und praktische Anwendung und ist somit für Studierende und Anwender in der Praxis gleichermaßen geeignet. Durch die programmiersprachenunabhängige Darstellung der Algorithmen können sie leicht für die eigene Anwendung angepasst werden. Viele praktische Projekte erleichtern das Selbststudium und vertiefen das Gelernte.
Dieses Arbeitsbuch enthält zahlreiche Aufgaben großer thematischer Breite und unterschiedlicher Schwierigkeitsgrade – einschließlich vollständiger und ausführlicher Lösungen.
Das Buch greift als optimale Ergänzung zum Lehrbuch Lineare Algebra im algebraischen Kontext mit derselben Kapitelstruktur die dort genannten Aufgaben auf. Darüber hinaus ergänzt es weitere Übungen – zur Vertiefung oder um in neue Themen einzuführen. Es ist aber auch völlig unabhängig vom Lehrbuch nutzbar.
Neben Aufgaben zu linearen Gleichungssystemen, deren Lösungsverfahren und -theorie, Matrizenrechnung und Determinanten finden sich auch Übungen zur Eigenwerttheorie, Matrixdiagonalisierung und -trigonalisierung, zu Vektorräumen und Homomorphismen sowie zu Bilinearformen. Darüber hinaus wird mit Aufgaben zu Markov-Ketten und linearen Differenzialgleichungssystemen die Anwendung der linearen Algebra auf andere Bereiche der Mathematik angesprochen. Ein separates Kapitel mit Minitests dient der schnellen Überprüfung des eigenen Wissens zu besonders zentralen Themen.
Viele Aufgaben lassen sich ohne weitere Hilfsmittel lösen, während andere, vor allem praxisorientierte Problemstellungen, den Einsatz mathematischer Software nahelegen. Daher wird bei diesen Aufgaben der Lösungsweg durch den Einsatz von MATLAB® ergänzt.
Seit 1. Januar 2017 gelten ein neuer Pflegebedürftigkeitsbegriff und ein neues Begutachtungsinstrument im Recht der Pflegeversicherung und in der Hilfe zur Pflege. Der neue Pflegebedürftig-keitsbegriff ist eine maßgebliche Grundlage für die Beschreibung des Personenkreises, der Zugang zu Leistungen hat. Er bezieht körperliche, kognitive und psychische Beeinträchtigungen gleichberechtigt ein. Wesentliches Element des neuen Pflegebedürftigkeitsbegriffs ist die Abkehr vom Verrichtungsbezug und vom Faktor Zeit als einzigem Maßstab für die Schwere der Betroffenheit. Neuer Maßstab für Pflegebedürftigkeit ist der Grad der Selbständigkeit der Pflegebedürftigen.
Aus dem neuen Pflegebedürftigkeitsbegriff leitet sich auch das Verständnis von Pflege in der Pflegeversicherung und der Hilfe zur Pflege ab. Dieses Verständnis von Pflege prägt alle Bereiche der Pflege: Von den Inhalten der Leistungen über die Pflegedokumentation bis hin zum Qualitätsverständnis. Es ist daher auch Bezugspunkt für die aktuellen Weiterentwicklungen und Prozesse in der Pflegeversicherung (z.B. Qualitätsentwicklung, Personalbemessungsverfahren, Pflegeberatung). Dabei folgt dieses Verständnis von Pflege der pflegefachlichen und pflegewissenschaftlichen Perspektive. Daher wurden und werden bereits viele Bestandteile davon in der Pflegeausbildung ge-lehrt und in der Praxis der Pflege umgesetzt.
Im Auftrag des Bundesministeriums für Gesundheit haben Dr. Klaus Wingenfeld von der Universität Bielefeld und Prof. Dr. Andreas Büscher von der Hochschule Osnabrück im Oktober 2017 eine Expertise vorgelegt, mit der die pflegerischen Aufgaben auf der Grundlage des neuen Pflegebedürftigkeitsbegriffs strukturiert und beschrieben werden.
Der Beirat zur Einführung des neuen Pflegebedürftigkeitsbegriffs (Begleitgremium nach § 18c SGB XI) sieht diese Strukturierung und Beschreibung als geeignete fachliche Grundlage für ein gemeinsames Verständnis von Pflege an und empfiehlt, sie für die Anpassung und Weiterentwicklung von fachlichen Konzepten und Vereinbarungen in der Pflege zu nutzen.
Die Entwicklung und Sicherstellung pflegerischer Qualität wird nicht erst seit der Einführung der Pflegeversicherung intensiv diskutiert. Im Zuge dieser Auseinandersetzung zeigt sich, dass insbesondere in der ambulanten Versorgung unterschiedliche Ansichten darüber bestehen, wie Pflegequalität zu definieren, zu entwickeln und zu sichern ist. Dieser Beitrag stellt daher zentrale Merkmale sowie Rahmenbedingungen ambulanter Pflege dar, zeigt auf, welche unterschiedlichen Formen der Pflege und Unterstützung in der häuslichen Versorgung zum Tragen kommen und wie die Diskussion um die Qualität der ambulanten Pflege durch die unterschiedlichen Perspektiven der beteiligten Akteure bestimmt wird. Daran anschließend werden Ansatzpunkte für eine weitere Diskussion um die Qualität in der ambulanten Pflege aufgezeigt.
Das Deutsche Netzwerk für Qualitätsentwicklung in der Pflege (DNQP) entwickelt seit etwa 20 Jahren Expertenstandards für die Pflege. Expertenstandards sind innerhalb der Berufsgruppe abgestimmte Leistungsniveaus für die professionelle Pflege und Instrumente zur internen Qualitätsentwicklung in unterschiedlichen Settings der pflegerischen Versorgung. Durch die Aufbereitung verfügbarer Evidenz und die Orientierung am Pflegeprozess leisten Expertenstandards einen wichtigen Beitrag zum Theorie/Praxis-Transfer. Für die Einführung und Verstetigung neuer Vorgehensweisen im Praxisalltag wurde ein Implementierungskonzept entwickelt und es wird ein Audit-Instrument zur Verfügung gestellt. Erfahrungen aus den Praxisprojekten der letzten 15 Jahre verdeutlichen, dass nachhaltige Qualitätsentwicklung in erster Linie durch verbesserte Prozesse innerhalb der Krankenhäuser, Pflegeheime und ambulanten Pflegedienste erfolgt.
Seit Einführung der Pflegeversicherung wird intensiv zu Fragen der Qualitätssicherung der Pflege diskutiert und es hat vielfältige gesetzgeberische Aktivitäten gegeben, um Verfahren zu entwickeln, durch die ein Schutz pflegebedürftiger Menschen vor unsachgemäßer Pflege gewährleistet und eine Verbesserung der Pflegequalität erreicht werden kann. Der folgende Beitrag gibt einen Überblick über diese Aktivitäten und ihre Hintergründe. Im Fokus stehen dabei insbesondere die durch das Pflege-Weiterentwicklungsgesetz (PflWEG) und die Pflegestärkungsgesetze (PSG I und II) angestoßenen Entwicklungen.

Hintergrund:
Das zentrale Instrument in der Gesundheitsversorgung zur Messung von Qualität sind Qualitätsindikatoren. Die Qualität der Messung von Versorgungsqualität hängt von der Güte der Indikatoren ab. Mit dem QUALIFY Instrument kann diese Güte auf wissenschaftlicher Grundlage bestimmt werden. Die vorliegende Arbeit evaluiert QUALIFY und gibt Hinweise für eine Weiterentwicklung.
Methode:
Die Evaluation des QUALIFY Instruments erfolgte in Form von strukturierten qualitativen Interviews (Fokusgruppen-Interviews). Die Teilnehmer der ersten Fokusgruppe waren sowohl an der Entwicklung des Instruments beteiligt als auch in der Anwendung erfahren. In der zweiten Fokusgruppe Beteiligte waren ausschließlich QUALIFY Anwender.
Ergebnisse:
Zwischen beiden Gruppen traten keine wesentlichen Unterschiede in der Beurteilung auf. QUALIFY wurde bislang für die Bestimmung von Qualitätsindikatoren des Qualitätsberichts, für eine Vorauswahl von Indikatoren für die nationalen Versorgungsleitlinien und für ein Projekt Arzneimittelsicherheit des Aktionsbündnisses Patientensicherheit angewendet. Als Hemmschwelle für eine weitere Verbreitung wurde gesehen, dass das Instrument in seiner derzeitigen Anwendungsform eine nicht unerhebliche Komplexität aufweist und damit in der Anwendung auch entsprechend ressourcenaufwendig ist. Dennoch wurde die Kosten-Effektivität als angemessen eingestuft, da sich aus dem Einsatz nicht angemessener Qualitätsindikatoren erhebliche Folgekosten ableiten können. Ziel könnte es daher sein, QUALIFY für verschiedene Einsatzzwecke in der Komplexität abzustufen und zudem auch international stärker zu verankern.
Diskussion:
Mit QUALIFY und damit der Bewertung von Qualitätsindikatoren wurde weitgehend Neuland betreten. Die zukünftige Bedeutung von Qualitätsmessung macht die weitere Evaluation und Entwicklung entsprechender Instrumente notwendig. Fokusgruppeninterviews können hierbei ein geeigneter Ansatz sein, um die Akzeptanz und Umsetzungsprobleme in der Praxis zu evaluieren.
Hintergrund:
Prävention und Gesundheitsförderung bieten Chancen für eine Entlastung des Gesundheitssystems. Dennoch gleicht die Präventionslandschaft in Deutschland bisher einem Flickenteppich mit unklaren Zielen und Verantwortlichkeiten.
Ziel:
Anhand der für Deutschland geltenden Erkenntnisse zu Qualität, Wirksamkeit und Zielgruppen werden Kriterien für eine rationale Präventionsstrategie hergeleitet und Hindernisse für die Umsetzung einer solchen Strategie analysiert.
Ergebnis:
Empirisch nachgewiesene gesundheitliche Ungleichheiten geben Hinweise auf Zielparameter für Interventionen. Die Wirksamkeit verschiedener Maßnahmen sollte in einem wettbewerblichen Kontext untersucht und verglichen werden.
Schlussfolgerung:
Als institutioneller Rahmen ist eine Präventionsstiftung empfehlenswert, in der, in Form eines evidenzbasierten Versorgungsplans, Versorgungssituation und -bedarf analysiert und Planung, Steuerung und Finanzierung der Maßnahmen geregelt werden.
Bewertung von Krankenhäusern

Hintergrund:
Die Wahl der Versorgungsform, stationär oder ambulant, unterliegt im deutschen Gesundheitswesen derzeit einem tiefgreifenden Wandel. Eine neue Qualität erhält die Öffnung der Krankenhäuser mit der Neufassung des § 116b SGB V. Die Studie untersucht die Frage, welches ambulante Potenzial sich aus bisher vollstationär behandelten Fällen in der Rheumatologie ergibt.
Methode:
Die Auswertung basiert auf einem Datensatz für die Jahre 2004 bis 2008. Dieser enthält anonymisiert die Abrechnungsdaten von rund 23,6 Mio. GKV-Versicherten. Die Auswahl von Patienten mit rheumatologischen Erkrankungen erfolgte anhand der in § 116b SGB V angegebenen ICD-10-Diagnosen.
Ergebnisse:
Im untersuchten Zeitraum wurde ein Anstieg der rheumatologischen Fälle um 13,9% beobachtet bei einem Rückgang der durchschnittlichen Verweildauer von 9,46 Tagen auf 8,08 Tage und der behandelnden Krankenhäuser um 3,1%. Der Anteil der rheumatologischen Fälle mit kurzer Verweildauer (≤2 Tage) nahm um 32,3% zu. Als „ambulantes Potenzial“ definieren wir den Anteil dieser Kurzlieger an der Gesamtzahl der vollstationären rheumatologischen Fälle, er stieg von 25,7% auf 29,9%
Diskussion:
Nicht alle Kurzlieger können problemlos in eine ambulante Versorgung überführt werden; diese erfordert spezialisierte Strukturen und Personal. Eine Zentrenbildung findet bisher nicht statt. Die Studie erlaubt keine Aussagen zur Qualität der Versorgung in den betrachteten Krankenhäusern. Eine Verknüpfung von Versorgungsdaten mit Qualitätsdaten wäre sinnvoll.
Für das Haushaltswesen deutscher Verwaltungen könnte das gerade begonnene Jahrzehnt erhebliche Änderungen bringen. Es „droht“ – vor dem Hintergrund der Finanz- und Staatsschuldenkrise in Europa – die Einführung von „European Public Sector Accounting Standards“ – kurz EPSAS – auf der Grundlage der bereits vorhandenen „International Public Sector Accounting Standards“ – kurz IPSAS. Die EU drängt verstärkt auf deren Entwicklung. Diese verständlich nachzuzeichnen, inhaltlich vergleichend zu beschreiben und zu analysieren sowie auch kritisch vor dem Hintergrund der Ziele von (vermeintlichen) EPSAS zu würdigen, ist Gegenstand dieser Schrift.
The well-documented social gradient in health in the developed world will not just disappear on its own. Tackling health inequalities by introducing a universal coverage healthcare system recently became an important notion in the U.S. Using cross-sectional data from Germany which has maintained its compulsory egalitarian healthcare system for more than 50 years now, we apply logistic and negative binomial regression to uncover utilisation behaviour patterns under universal coverage. We find that lower education and unemployment raise the risks for all diseases under consideration. Unemployment increases the chance of contacting a physician, while income and education do not apparently affect the healthcare utilisation behaviour. Those diseases concentrated among unemployed and less educated, however, are associated with intensified healthcare utilisation. We conclude that universal coverage may make access to health care easier for those facing the worst health; the unemployed and lower educated.

Zentrum-Peripherie-Modelle, demographischer Wandel, Folgen der Digitalisierung, Finanzierungsaspekte, Innovationsaspekte, Regionalförderung – Europa der Regionen, Regionalfonds, führen immer auch zur konkreten Frage nach dem Erhalt der Leistungsfähigkeit der Verwaltungen in allen Regionen Europas und Deutschlands.
Welche Herausforderungen für die Gestaltungs- und Leistungsfähigkeit öffentlicher Verwaltung und Daseinsvorsorge sich im Kontext einer zunehmenden sozialen, ökonomischen und räumlichen Segregation stellen und welche praktischen Antworten möglich sind, war Thema der 3. Tagung des Praxis- und Forschungsnetzwerks der Hochschulen für den Öffentlichen Dienst, die am 6. und 7. Februar 2020 an der Hochschule Osnabrück stattfand.

Selecting items for Big Five questionnaires : At what sample size do factorloadings stabilize?

Researchers often use exploratory factor analysis (EFA) to develop and refine questionnaires assessing theBig Five personality traits. We use sequential sampling and bootstrapping to determine the number ofparticipants needed to yield stable loading patterns for the Big Five Inventory (BFI) and the InternationalPersonality Item Pool Big Five measure (IPIP). Overall 21,350 participants (BFI = 10,285; IPIP = 11,065)participated. In two studies primary factor loadings are highly variable in smaller samples (n< 500)and some primary loadings are not stable with 10,000 participants. Most studies will not have adequatesample size to yield stable loading patterns for Big Five measures such as the BFI and IPIP. Researchersshould assess and report the variability of loading patterns.
Background:
Children with severe psychomotor impairment (SPMI) often experience sleep disturbances that severely distress both the child and his or her parents. Validated questionnaires for the assessment of parents’ distress related to their child’s sleep disturbances are lacking.
Methods:
We developed and validated a new questionnaire, the HOST (holistic assessment of sleep and daily troubles in parents of children with SPMI) to assess the effect of the sleep disturbances in children with SPMI on their parents. The questionnaire was developed based on published data and expert opinion, and it was refined via direct consultation with affected parents. Its psychometric characteristics were assessed in a sample of parents of 214 children with SPMI. It was retested using a random subsample of the participants.
Results:
Explorative factor analysis revealed that the HOST was composed of four scales. Fit indices, item analysis, and convergent validity (coherence with preexisting instruments of sleep disturbances and health status) were adequate. Retest analysis (n = 62) revealed high stability of the HOST questionnaire and adequate replication validity.
Conclusion:
Sleep-related difficulties significantly impact the sociomedical characteristics of the parents of children with complex neurologic diseases. Typically, parents are severely affected in various aspects of daily life (i.e., medical health, social life, professional life). The HOST proved to be a valid, reliable and economical assessment tool of sleep-related difficulties in parents and relatives of children with SPMI. The HOST is capable of identifying individuals and specific areas requiring intervention.
Purpose:
To analyse the willingness for postmortem cornea donation in Germany.
Methods:
Employees in two cities (UKM, UKS), and university hospitals (STM, STE), members of the German Ophthalmological Society (DOG), and employees of an automobile company (BO) participated in a questionnaire about postmortem cornea donation attitudes. The questionnaire consisted of demographic items, motives concerning postmortem cornea donation, general attitudes toward donation, and questions concerning the perceived needs for information about donation. The statistical analyses included logistic regression with the target parameter of 'willingness to donate cornea postmortem'.
Results:
Of the participants, 67.7 % (UKM, UKS), 70.9 % (STM, STE), 70.8 % (BO), and 79.4 % (DOG) declared their intention to donate their corneas postmortem. Younger age (p < 0.001), poorer general health (p < 0.05), faith in an eternal life (p < 0.05), disagreement with brain death diagnostics (p < 0.001), fear of receiving worse medical treatment (p < 0.001), and fear of the commercialization of organs (p < 0.001) were found to be risk factors for a negative attitude toward postmortem cornea. The majority of participants (57.4 %) indicated that additional information about donation would be appreciated, and the internet (69.9 %) was considered the most appropriate means for conveying this information.
Conclusions:
Emotional items were revealed to be the most relevant factors influencing the willingness to donate cornea postmortem, which may be counteracted by means of public education. The relatively low willingness among the medical staff contrasts with previous observations in a professional ophthalmologic society.
Aims:
To assess the diagnostic utility of a novel abbreviated monofilament test in comparison with the tuning fork test to detect diabetic peripheral neuropathy in children.
Methods:
A total of 88 children with Type 1 diabetes mellitus were screened for diabetic peripheral neuropathy using the monofilament test and the tuning fork. Nerve conduction studies were performed according to the ‘gold standard’ for neuropathy. We assessed the diagnostic utility and inter-rater agreement of the two screening methods.
Results:
A total of 43 (49%) children (aged 6–18 years) had at least one abnormal nerve conduction study result. Diagnostic utility and inter-rater agreement were very low for both screening methods. The monofilament test yielded a sensitivity of 18% and a specificity of 80%. The tuning fork yielded a sensitivity of 0% and a specificity of 98%.
Conclusion:
The present study found that an abbreviated monofilament test has low diagnostic utility for the detection of early diabetic peripheral neuropathy because of its low reliability. The problem of reliability needs to be more thoroughly addressed in order to improve the screening procedures in diabetes management in childhood and adolescence.
Clinically Significant Differences in Acute Pain Measured on Self-report Pain Scales in Children

Objectives
The objective was to determine the minimum and ideal clinically significant differences (MCSD, ICSD) in pain intensity in children for the Faces Pain Scale–Revised (FPS-R) and the Color Analog Scale (CAS) and to identify any differences in these estimates based on patient characteristics.
Methods
This was a prospective study of children aged 4 to 17 years with acute pain presenting to two urban pediatric emergency departments. Participants self-reported their pain intensity using the FPS−R and CAS and qualitatively described their changes in pain. Changes in pain score reported using the FPS-R and CAS that were associated with “a little less” and “much less” pain (MCSD and ICSD, respectively) were identified using a receiver operating characteristic–based method and expressed as raw change score and percent reductions. Estimates of MCSD and ICSD were determined for each category of initial pain intensity (mild, moderate, and severe) and patient characteristics (age, sex, and ethnicity). Post hoc exploratory analyses evaluated categories of race, primary language, and etiology of pain.
Results
A total of 314 children with acute pain were enrolled; mean (±SD) age was 9.8 (±3.8) years. The FPS-R raw change score and percent reduction MCSD estimates were 2/10 and 25%, with ICSD estimates of 3/10 and 60%. For the CAS, raw change score and percent reduction MCSD estimates were 1/10 and 15%, with ICSD estimates of 2.75/10 and 52%. For both scales, raw change score and percent reduction estimates of the MCSD remained unchanged in children with either moderate or severe pain. For both scales, estimates of ICSD were not stable across categories of initial pain intensity. There was no difference in MCSD or ICSD based on age, sex, ethnicity, race, primary language, or etiology of pain.
Conclusions
The MCSD estimates can be expressed as raw change score and percent reductions for the FPS-R and CAS. These estimates appear stable for children with moderate to severe pain, irrespective of age, sex, and ethnicity. Estimates of ICSD were not stable across different categories of initial pain intensity, therefore limiting their potential generalizability.
This review aimed to synthesize the current evidence on the effectiveness of invasive treatments for complex regional pain syndrome in children and adolescents. Studies on children and adolescents with complex regional pain syndrome that evaluated the effects of invasive treatment were identified in PubMed (search March 2013). Thirty-six studies met the inclusion criteria. Articles reported on a total of 173 children and adolescents with complex regional pain syndrome. Generally, many studies lack methodological quality. The invasive treatments applied most often were singular sympathetic blocks, followed by epidural catheters and continuous sympathetic blocks. Rarely, spinal cord stimulation and pain-directed surgeries were reported. An individual patient frequently received more than one invasive procedure. Concerning outcome, for approximately all patients, an improvement in pain and functional disability was reported. However, these outcomes were seldom assessed with validated tools. In conclusion, the evidence level for invasive therapies in the treatment of complex regional pain syndrome in children and adolescents is weak.
Objectives: The aims of this study were to define the Faces Pain Scale-Revised (FPS-R) and Color Analog Scale (CAS) scores associated with no pain, mild pain, moderate pain, and severe pain in children with acute pain, and to identify differences based on age, sex, and ethnicity.
Methods: We conducted a prospective observational study in 2 pediatric emergency departments of children aged 4 to 17 years with painful and nonpainful conditions. We assessed their pain intensity using the FPS-R, CAS, and qualitative measures. Pain score cut points that best differentiated adjacent categories of pain were identified using a receiver operating characteristic-based method. Cut points were compared within subgroups based on age, sex, and ethnicity.
Results: We enrolled 620 patients, of whom 314 had painful conditions. The mean age was 9.2 years; 315 (50.8%) were in the younger age group (aged 4-7 years); 291 (46.8%) were female; and 341 (55%) were Hispanic. The scores best representing categories of pain for the FPS-R were as follows: no pain, 0 and 2; mild pain, 4; moderate pain, 6; and severe pain, 8 and 10. For the CAS, these were 0 to 1, 1.25 to 2.75, 3 to 5.75, and 6 to 10, respectively. Children with no pain frequently reported nonzero pain scores. There was considerable overlap of scores associated with mild and moderate pain. There were no clinically meaningful differences of scores representing each category of pain based on age, ethnicity, and race.
Conclusions: We defined pain scores for the FPS-R and CAS associated with categories of pain intensity in children with acute pain that are generalizable across subgroups based on patient characteristics. There were minor but potentially important differences in pain scores used to delineate categories of pain intensity compared to prior convention.

Zur Stabilität dieser Effekte ist jedoch wenig bekannt. Fragestellung: Wie entwickeln sich die Veränderungsmotivation, die Essstörungspsychopathologie und das Selbstwertgefühl 8 Wochen nach Abschluss eines internetbasierten Motivationsprogramms?
Methode: Neunzig Frauen bearbeiteten den Stages of Change Questionnaire for Eating Disorders, den Eating Disorder Examination-Questionnaire und die Rosenberg Self-Esteem-Scale unmittelbar (Post) sowie 8 Wochen nach Abschluss der Intervention (Katamnese).
Ergebnisse: Es zeigten sich stabile Effekte in der Veränderungsmotivation sowie im Selbstwertgefühl. Zusätzlich zeigten sich in der Essstörungspsychopathologie signifikante Verbesserungen.
Schlussfolgerungen: Die Studie belegt die längerfristige Wirksamkeit eines internetbasierten Motivationsprogramms für Frauen mit Essstörungen.
Research into positive aspects of the psyche is growing as psychologists learn more about the protective role of positive processes in the development and course of mental disorders, and about their substantial role in promoting mental health. With increasing globalization, there is strong interest in studies examining positive constructs across cultures. To obtain valid cross-cultural comparisons, measurement invariance for the scales assessing positive constructs has to be established. The current study aims to assess the cross-cultural measurement invariance of questionnaires for 6 positive constructs: Social Support (Fydrich, Sommer, Tydecks, & Brähler, 2009), Happiness (Subjective Happiness Scale; Lyubomirsky & Lepper, 1999), Life Satisfaction (Diener, Emmons, Larsen, & Griffin, 1985), Positive Mental Health Scale (Lukat, Margraf, Lutz, van der Veld, & Becker, 2016), Optimism (revised Life Orientation Test [LOT-R]; Scheier, Carver, & Bridges, 1994) and Resilience (Schumacher, Leppert, Gunzelmann, Strauss, & Brähler, 2004). Participants included German (n = 4,453), Russian (n = 3,806), and Chinese (n = 12,524) university students. Confirmatory factor analyses and measurement invariance testing demonstrated at least partial strong measurement invariance for all scales except the LOT-R and Subjective Happiness Scale. The latent mean comparisons of the constructs indicated differences between national groups. Potential methodological and cultural explanations for the intergroup differences are discussed. Suicide is a major cause of death in adulthood and specifically in patients suffering from mental illnesses. The Depressive Symptom Inventory Suicidality Subscale (DSI-SS) is widely used to detect and prevent suicidal ideation. The aim of the present study was to determine optimal cut points for the DSI-SS in different populations.
We analysed the data of one population-based sample (n = 532), one outpatient sample (n = 180) and one inpatient sample (n = 244). Internal consistency, convergent validity and optimal cut points according to receiver operating characteristics were calculated.
In all samples, we found excellent item-total correlations and internal consistencies for the DSI-SS. Zero-order correlations between the DSI-SS and theoretically related constructs showed positive correlation coefficients, ranging from 0.50 to 0.67. The DSI-SS differentiated well between patients with and without suicide attempts in the population-based sample, but less so in the inpatient sample and only marginally in the outpatient sample. A bootstrapping analysis showed some variability in the cut points that emerged as optimal, but there was no overlap between the different samples.
The specific cut points that we identified may be used to improve the diagnostic utility of the DSI-SS and the chance to detect suicidal ideation.
Recent theoretical models suggest that repetitive negative thinking might be a key mechanism explaining the negative effects of maternal psychopathology on mother–infant relations. While an emerging body of research largely supports this idea, the relative importance of differences in the trajectory of repetitive negative thinking during and after pregnancy for mother–infant bonding as well as maternal depressive symptoms is currently unknown. Therefore, we investigated associations between the course of maternal repetitive negative thinking during pregnancy and after birth and mother–infant-bonding and maternal depressive symptoms in a longitudinal study. The overall level of repetitive negative thinking was a significant predictor of mother–infant bonding, maternal anxiety and rejection in dealing with her infant four months after birth. Furthermore, differences in the trajectory of repetitive negative thinking predicted bonding, but not anxiety or rejection. The overall levels of repetitive negative thinking as well as the differences in the trajectory of repetitive negative thinking were significant predictors of maternal depressive symptoms. These findings indicate that changes of repetitive negative thinking during and after pregnancy can increase the risk of postpartum depressive symptoms.

Content is of primary importance in the World Wide Web. In particular, subjective perceptions of content are known to influence a variety of user evaluations, thereby altering attitudes and behavioral outcomes. Thus, it is essential that individually experienced facets of content can be adequately assessed. In a series of seven studies, we create, validate, and benchmark a measure for users' subjective view on web content. In the first six studies, a total of 3106 participants evaluated a sum of 60 websites. The resulting Web-CLIC questionnaire is a 12-item measure based on prior research on web content. It encloses four main facets of users' content experience: clarity, likeability, informativeness, and credibility – jointly representing a general factor subjective content perception. Very high internal consistencies and high short- to medium-term retest reliabilities are demonstrated. Strong evidence for construct validity in terms of factorial, convergent, divergent, discriminative, concurrent, experimental, and predictive validity is found. In a seventh study, encompassing 7379 ratings on 120 websites, benchmarks for 10 different content domains and optimal cut points are provided. Overall, the present research suggests that the Web-CLIC is a sound measure of subjective content perception of both practical and theoretical benefit.
The Liebowitz Social Anxiety Scale (LSAS) is the most frequently used instrument to assess social anxiety disorder (SAD) in clinical research and practice. Both a self‐reported (LSAS‐SR) and a clinician‐administered (LSAS‐CA) version are available. The aim of the present study was to define optimal cut‐off (OC) scores for remission and response to treatment for the LSAS in a German sample.
Data of N = 311 patients with SAD were used who had completed psychotherapeutic treatment within a multicentre randomized controlled trial. Diagnosis of SAD and reduction in symptom severity according to the Structured Clinical Interview for Diagnostic and Statistical Manual of Mental Disorders, 4th edition, served as gold standard. OCs yielding the best balance between sensitivity and specificity were determined using receiver operating characteristics. The variability of the resulting OCs was estimated by nonparametric bootstrapping.
Using diagnosis of SAD (present vs. absent) as a criterion, results for remission indicated cut‐off values of 35 for the LSAS‐SR and 30 for the LSAS‐CA, with acceptable sensitivity (LSAS‐SR: .83, LSAS‐CA: .88) and specificity (LSAS‐SR: .82, LSAS‐CA: .87). For detection of response to treatment, assessed by a 1‐point reduction in the Structured Clinical Interview for Diagnostic and Statistical Manual of Mental Disorders, 4th edition, rating, a reduction of 28% for the LSAS‐SR and 29% for the LSAS‐CA yielded the best balance between sensitivity (LSAS‐SR: .75, LSAS‐CA: .83) and specificity (LSAS‐SR: .76, LSAS‐CA: .80).
To our knowledge, we are the first to define cut points for the LSAS in a German sample. Overall, the cut points for remission and response corroborate previously reported cut points, now building on a broader data basis.
The Brief Symptom Inventory (BSI)-18 is a widely-used tool to assess changes in general distress in patients despite an ongoing debate about its factorial structure and lack of evidence for longitudinal measurement invariance (LMI). We investigated BSI-18 scores from 1,081 patients from an outpatient clinic collected after the 2nd, 6th, 10th, 18th, and 26th therapy session. Confirmatory factor analysis (CFA) was used to compare models comprising one, three, and four latent dimensions that were proposed in the literature. LMI was investigated using a series of model comparisons, based on chi-square tests, effect sizes, and changes in comparative fit index (CFI). Psychological distress diminished over the course of therapy. A four-factor structure (depression, somatic symptoms, generalized anxiety, and panic) showed the best fit to the data at all measurement occasions. The series of model comparisons showed that constraining parameters to be equal across time resulted in very small decreases in model fit that did not exceed the cutoff for the assumption of measurement in variance. Our results show that the BSI-18 is best conceptualized as a four-dimensional tool that exhibits strict longitudinal measurement invariance. Clinicians and applied researchers do not have to be concerned about the interpretation of mean differences over time.
Positive factors are increasingly recognized in the field of psychology, however, few studies have investigated the longitudinal measurement invariance (LMI) and reciprocal associations of positive core constructs, such as happiness, life satisfaction and positive mental health. This study evaluated the LMI of these constructs over four years in a Chinese Student Sample (n = 4400) using the Subjective Happiness Scale (SHS), the Satisfaction with Life Scale (SWLS) and the Positive Mental Health Scale (PMH-scale). The longitudinal reciprocal associations of the constructs were examined within a random intercept cross-lagged panel model (RI-CLPM). The results show that the SHS, SWLS and PMH-scale are measurement invariant over time and that the constructs are positively inter-related, but show different reciprocal patterns over time.

Wenn Kinder Schmerzen haben, reagieren Eltern kognitiv-affektiv und verhaltensbezogen. Dysfunktionale Reaktionen wie elterliches Katastrophisieren können zur Chronifizierung kindlicher Schmerzen beitragen.
Ziel der Arbeit:
Ziele der Online-Studie sind (1) die erstmalige psychometrische Überprüfung existierender Fragebögen zu kognitiv-affektiven (Pain Catastrophizing Scale for Parents; PCS-P) und verhaltensbezogenen Reaktionen (Inventar zum schmerzbezogenen Elternverhalten; ISEV-E) an 105 schmerzfreien Eltern, (2) der Vergleich der Reaktionen mit existierenden (inter-)nationalen klinischen und schmerzfreien Samples sowie mit 80 Eltern mit selbstberichteten chronischen Schmerzen.
Methode:
Die teilnehmenden Eltern machten per Online-Fragebogen Angaben zu ihren elterlichen Reaktionen auf kindlichen Schmerz.
Ergebnisse:
Die Faktoren des ISEV-E konnten nicht bestätigt werden; die Faktorenstruktur des PCS-P war replizierbar. Das elterliche Katastrophisieren der schmerzfreien Eltern war geringer als in klinischen Samples. Unterschiede zwischen schmerzfreien Eltern und Eltern mit selbstberichteten chronischen Schmerzen aus der Allgemeinbevölkerung zeigten sich nicht.
Diskussion:
Die Ergebnisse bieten eine Basis zur Einordnung des elterlichen Katastrophisierens, um Risikogruppen mit starker Katastrophisierungsneigung frühzeitig zu identifizieren.
Small-fiber neuropathy and pain sensitization in survivors of pediatric acute lymphoblastic leukemia

Background:
Chemotherapy-induced Peripheral Neuropathy (CIPN) of large-fibers affects up to 20% of survivors of pediatric acute lymphoblastic leukemia (ALL). We aimed to describe small-fiber toxicity and pain sensitization in this group.
Methods:
In a cross-sectional, bicentric study we assessed 46 survivors of pediatric ALL (Mean age: 5.7 ± 3.5 years at diagnosis, median 2.5 years after therapy; males: 28). Inclusion criteria: ≥6 years of age, ≥3 months after last administration of Vincristine, and cumulative dose of Vincristine 12 mg/m2. We used a reduced version of the Pediatric-modified Total Neuropathy Score (Ped-mTNS) as bedside test and Quantitative Sensory Testing (QST) for assessment of small- and large-fiber neuropathy as well as pain sensitization. We employed Nerve Conduction Studies (NCS) as the most accurate tool for detecting large-fiber neuropathy.
Results:
Fifteen survivors (33%) had abnormal rPed-mTNS values (≥4 points) and 5 survivors (11%) reported pain. In QST, the survivor group showed significant (p < 0.001) inferior large-fiber function and pain sensitization when compared to healthy matched peers. We identified deficits of vibration in 33 (72%) and tactile hypoesthesia in 29 (63%), hyperalgesia to blunt pressure in 19 (41%), increased mechanical pain sensitivity in 12 (26%) and allodynia in 16 (35%) of 46 survivors. Only 7 survivors (15%) had pathologic NCS.
Conclusion:
QST is a sensitive tool that revealed signs of large-fiber neuropathy in two thirds, small-fiber neuropathy and pain sensitization in one third of survivors. Prospective studies using QST in pediatric oncology may help to elucidate the pathophysiology of small-fiber neuropathy and pain sensitization as well as their relevance for quality of survival.
Objective:
The German version of the Social Phobia and Anxiety Inventory (SPAI-G) is avalidated measure for the detection of social anxiety disorder (SAD). The aim of the presentstudy was to develop optimal cut points (OC) for remission and response to treatment for theSPAI-G.
Methods:
We used Receiver Operating Characteristic methods and bootstrapping to analysethe data of 359 patients after psychotherapeutic treatment. OCs where defined as the cut pointswith the highest sensitivity and specificity after bootstrapping.
Results:
For remission, an OC of 2.79 was found, and for response, a change in score frompre- to posttreatment by 11% yielded best results.
Conclusions:
The OC we identified for remission may be used to improve the diagnostic utilityof the SPAI-G. However, the cut point for response achieved only borderline-acceptable levelsof sensitivity and specificity, calling into doubt their utility in clinical and research setting.
Introduction:
Many patients with cerebral palsy (CP) suffer chronic pain as one of the most limiting factors in their quality of life. In CP patients, pain mechanisms are not well understood, and pain therapy remains a challenge. Quantitative sensory testing (QST) might provide unique information about the functional status of the somatosensory system and therefore better guide pain treatment.
Objectives:
To understand better the underlying pain mechanisms in pediatric CP patients, we aimed to assess clinical and pain parameters, as well as QST profiles, which were matched to the patients' cerebral imaging pathology.
Patients and methods:
Thirty CP patients aged 6–20 years old (mean age 12 years) without intellectual impairment underwent standardized assessments of QST. Cerebral imaging was reassessed. QST results were compared to age- and sex-matched controls (multiple linear regression; Fisher's exact test; linear correlation analysis).
Results:
CP patients were less sensitive to all mechanical and thermal stimuli than healthy controls but more sensitive to all mechanical pain stimuli (each p < 0.001). Fifty percent of CP patients showed a combination of mechanical hypoesthesia, thermal hypoesthesia and mechanical hyperalgesia; 67% of CP patients had periventricular leukomalacia (PVL), which was correlated with mechanic (r = 0.661; p < 0.001) and thermal (r = 0.624; p = 0.001) hypoesthesia.
Conclusion:
The combination of mechanical hypoesthesia, thermal hypoesthesia and mechanical hyperalgesia in our CP patients implicates lemniscal and extralemniscal neuron dysfunction in the thalamus region, likely due to PVL. We suspect that extralemniscal tracts are involved in the original of pain in our CP patients, as in adults.
Background and purpose:
Clinical information logistics is a construct that aims to describe and explain various phenomena of information provision to drive clinical processes. It can be measured by the workflow composite score, an aggregated indicator of the degree of IT support in clinical processes. This study primarily aimed to investigate the yet unknown empirical patterns constituting this construct. The second goal was to derive a data-driven weighting scheme for the constituents of the workflow composite score and to contrast this scheme with a literature based, top-down procedure. This approach should finally test the validity and robustness of the workflow composite score.
Methods:
Based on secondary data from 183 German hospitals, a tiered factor analytic approach (confirmatory and subsequent exploratory factor analysis) was pursued. A weighting scheme, which was based on factor loadings obtained in the analyses, was put into practice.
Results:
We were able to identify five statistically significant factors of clinical information logistics that accounted for 63% of the overall variance. These factors were “flow of data and information”, “mobility”, “clinical decision support and patient safety”, “electronic patient record” and “integration and distribution”. The system of weights derived from the factor loadings resulted in values for the workflow composite score that differed only slightly from the score values that had been previously published based on a top-down approach.
Conclusion:
Our findings give insight into the internal composition of clinical information logistics both in terms of factors and weights. They also allowed us to propose a coherent model of clinical information logistics from a technical perspective that joins empirical findings with theoretical knowledge. Despite the new scheme of weights applied to the calculation of the workflow composite score, the score behaved robustly, which is yet another hint of its validity and therefore its usefulness.
Information Technology (IT) continues to evolve and develop with electronic devices and systems becoming integral to healthcare in every country. This has led to an urgent need for all professions working in healthcare to be knowledgeable and skilled in informatics. The Technology Informatics Guiding Education Reform (TIGER) Initiative was established in 2006 in the United States to develop key areas of informatics in nursing. One of these was to integrate informatics competencies into nursing curricula and life-long learning. In 2009, TIGER developed an informatics competency framework which outlines numerous IT competencies required for professional practice and this work helped increase the emphasis of informatics in nursing education standards in the United States. In 2012, TIGER expanded to the international community to help synthesise informatics competencies for nurses and pool educational resources in health IT. This transition led to a new interprofessional, interdisciplinary approach, as health informatics education needs to expand to other clinical fields and beyond.
In tandem, a European Union (EU) - United States (US) Collaboration on eHealth began a strand of work which focuses on developing the IT skills of the health workforce to ensure technology can be adopted and applied in healthcare. One initiative within this is the EU*US eHealth Work Project, which started in 2016 and is mapping the current structure and gaps in health IT skills and training needs globally. It aims to increase educational opportunities by developing a model for open and scalable access to eHealth training programmes. With this renewed initiative to incorporate informatics into the education and training of nurses and other health professionals globally, it is time for educators, researchers, practitioners and policy makers to join in and ROAR with TIGER.
Background:
While aiming for the same goal of building a national eHealth Infrastructure, Germany and the United States pursued different strategic approaches – particularly regarding the role of promoting the adoption and usage of hospital Electronic Health Records (EHR).
Objective:
To measure and model the diffusion dynamics of EHRs in German hospital care and to contrast the results with the developments in the US.
Materials and methods:
All acute care hospitals that were members of the German statutory health system were surveyed during the period 2007–2017 for EHR adoption. Bass models were computed based on the German data and the corresponding data of the American Hospital Association (AHA) from non-federal hospitals in order to model and explain the diffusion of innovation.
Results:
While the diffusion dynamics observed in the US resembled the typical s-shaped curve with high imitation effects (q = 0.583) but with a relatively low innovation effect (p = 0.025), EHR diffusion in Germany stagnated with adoption rates of approx. 50% (imitation effect q = -0.544) despite a higher innovation effect (p = 0.303).
Discussion:
These findings correlate with different governmental strategies in the US and Germany of financially supporting EHR adoption. Imitation only seems to work if there are financial incentives, e.g. those of the HITECH Act in the US. They are lacking in Germany, where the government left health IT adoption strategies solely to the free market and the consensus among all of the stakeholders.
Conclusion:
Bass diffusion models proved to be useful for distinguishing the diffusion dynamics in German and US non-federal hospitals. When applying the Bass model, the imitation parameter needs a broader interpretation beyond the network effects, including driving forces such as incentives and regulations, as was demonstrated by this study.

Dabei spielen jedoch viele Faktoren zusammen, die einen einfachen Vergleich von IT-Kosten bei Weitem übersteigen. Eine Forschungsgruppe der Hochschule Osnabrück hat mit dem IT-Benchmark Gesundheitswesen ein Analysetool vorgelegt, das auch einen Länder- vergleich ermöglicht.
