Pflegerische Organisationskonzepte in der ambulanten Pflege zur Prävention von vermeidbaren Infektionen am Beispiel von Menschen mit einer PEG-Anlage : Eine Fallanalyse

  • Hintergrund: In Deutschland gibt es 4,1 Millionen pflegebedürftige Menschen. Davon werden 24 Prozent von ambulanten Pflegeeinrichtungen versorgt. Ein Großteil der pflegebedürftigen Menschen leidet an chronischen Erkrankungen. Mehr als die Hälfte der über 65-Jährigen sind von einer chronischen Erkrankung betroffen. Eine Folge chronischer Erkrankungen kann eine PEG-Anlage sein. In der pflegerischen Praxis bestehen jedoch noch einige Probleme bezüglich der der evidenzbasierten Pflege der PEG-Anlage. Es bedarf die Förderung von Evidence-based Nursing durch die Umsetzung von pflegerischen Organisationskonzepten, um Komplikationen, wie die Infektion der PEG-Eintrittsstelle, präventiv entgegenzuwirken. Methodik/Zielsetzung: Die methodische Grundlage dieser Bachelorarbeit bildet das hermeneutische Fallverstehen. Im Rahmen eines Praxiseinsatzes in der ambulanten Pflege wurde ein Patient mit einer PEG-Anlage ausgewählt und analysiert. Das Ziel dieser Arbeit ist es durch die Anwendung pflegerischer Organisationskonzepte die Prävention von vermeidbaren Infektionen von einem Menschen mit einer PEG-Anlage in der ambulanten Pflege zu erreichen und diesen Vorgang darzustellen. Im Zuge dessen wurde eine ausführliche Literaturrecherche durchgeführt. Ergebnisse: Die Bearbeitung dieser Thematik anhand des Fallbeispiels hat ergeben, dass die Durchführung einer Pflegevisite, welche primär auf die Qualitätssicherung- und Entwicklung abzielt, die Prävention von vermeidbaren Infektionen bei Menschen mit einer PEG-Anlage in der ambulanten Pflege erreichen kann. Dafür bedarf es die individuelle Planung, Durchführung und Nachbereitung der Pflegevisite. Das wichtigste Element der Pflegevisite stellt die gemeinsame Formulierung von Handlungsaufträgen der Teilnehmer*innen durch die evidenzbasierte pflegerische Entscheidungsfindung dar, um die Prävention von vermeidbaren Infektionen zu erreichen. Schlussfolgerungen: Pflegerische Organisationskonzepte, wie die Pflegevisite, sollten in der ambulanten Pflege systematisch angewendet werden, um die Prävention von vermeidbaren Infektionen aber auch weiteren gesundheitlichen Problemen zu erreichen. Dies benötigt eine strukturierte Einführung der Pflegevisite in den ambulanten Pflegeeinrichtungen. Außerdem sollten akademisierte Pflegefachkräfte in Zukunft nachhaltig in die Pflegepraxis implementiert werden, da sie die Pflegequalität erhöhen und somit die Prävention und Gesundheitsförderung fördern.
  • Background: There are 4.1 million people in need of care in Germany. 24 percent of these people are cared by outpatient care services. A large proportion of people in need of care suffer from chronic diseases. More than half of the over-65s are affected by a chronic disease. A PEG system can be one consequence of chronic diseases. However, there are still some problems regarding the evidence-based nursing of PEG systems in the nursing practice. There is a need to promote evidence-based nursing through the implementation of nursing organisational concepts in order to prevent complications such as infection of the PEG insertion site. Methodology/Objectives: The methodological basis of this bachelor thesis is the hermeneutic case understanding. Within the framework of a practical assignment in an outpatient care facility, a patient with a PEG system was selected and analysed. The aim of this thesis is to achieve the prevention of avoidable infections of a person with a PEG system in outpatient care by applying nursing organisational concepts and to present this process. In the course of this, an extensive literature research was conducted. Results: The analysis of this topic on the basis of the case study has shown that the implementation of a nursing round, which primarily aims at quality assurance and development, can achieve the prevention of avoidable infections from people with a PEG system in outpatient care. This requires individual planning, implementation and follow-up of the nursing rounds. The most important element of the nursing rounds is the joint formulation of interventions by the participants through evidence-based nursing decision-making in order to achieve the prevention of avoidable infections. Conclusions: Nursing organisation concepts, such as nursing rounds, should be systematically applied in outpatient care in order to achieve the prevention of avoidable infections and other health problems. This requires a structured introduction of nursing rounds in outpatient care facilities. In addition, academic nursing staff should be sustainably implemented in nursing practice in the future, as they increase the quality of care and thus promote prevention and health promotion.
